Mapping the Cortical Representation of Paraspinal Muscles Using Transcranial Magnetic Stimulation Optimized in People With Chronic Back Pain.

INTRODUCTION: Chronic low back pain (CLBP) is a global burden with an unknown etiology. Reorganization of the cortical representation of paraspinal muscles in the primary motor cortex (M1) may be related to the pathology. Single-pulse transcranial magnetic stimulation (TMS), commonly used to map the functional organization of M1, is not potent enough to stimulate the cortical maps of paraspinal muscles in M1 in CLBP patients with reduced corticospinal excitability (CSE) with intensities even as high as maximum stimulator output (100% MSO). This makes TMS mapping impractical for these patients.

Stronger back muscles reduce the incidence of vertebral fractures: a prospective 10 year follow-up of postmenopausal women.

The long-term protective effect of stronger back muscles on the spine was determined in 50 healthy white postmenopausal women, aged 58-75 years, 8 years after they had completed a 2 year randomized, controlled trial. Twenty-seven subjects had performed progressive, resistive back-strengthening exercises for 2 years and 23 had served as controls. Bone mineral density, spine radiographs, back extensor strength, biochemical marker values, and level of physical activity were obtained for all subjects at baseline, 2 years, and 10 years. Mean back extensor strength (BES) in the back-exercise (BE) gr

Differences in the elastic modulus of the lumbar muscles between female athletes with and without low back pain.

BACKGROUND: Low back pain is the most prevalent musculoskeletal disorder affecting performance and sports participation among athletes and is more prevalent among female athletes. Evaluating the stiffness of the lumbar muscles is important for understanding the pathophysiology of low back pain. Therefore, this study examined the differences in stiffness of the lumbar multifidus and erector spinae muscles between female university athletes with and without low back pain. METHODS: This was a cross-sectional study. The study was conducted at a single centre, the university research laboratory. T

Low Back Pain in Patients with Lumbar Spinal Stenosis-Hemodynamic and electrophysiological study of the lumbar multifidus muscles.

INTRODUCTION: Several studies have demonstrated improvement in low back pain (LBP) after decompression surgery for lower extremity symptoms in lumbar spinal stenosis (LSS); however, the influence of neuropathic disorders on LBP is uncertain. Aim of this study is to identify the features of motion-induced and walking-induced LBP in patients with LSS and to assess whether neuropathic LBP develops. METHODS: In total, 234 patients with LSS including L4/5 lesion were asked to identify their LBP. Subjects were classified into three groups: walking-induced LBP that aggravated during walking (W group

Comparison of Isometric and Dynamic Bridging Exercises on Low Back Muscle Oxygenation.

Bridging exercises are commonly performed by people with low back pain (LBP). However, the effect of the contraction mode in a bridging exercise on the hemodynamics of the low back muscle has not been investigated in people with and without LBP. The objective of this study was to assess the effect of the mode of bridging exercise on oxygenation of the low back muscle. A near-infrared spectroscopy was used to measure hemodynamic responses of the erector spinae between isometric and dynamic bridging exercises in 16 healthy participants. The results demonstrated that during exercise, the isometri

Clinical trial of intensive muscle training for chronic low back pain.

105 patients who had chronic low back pain without clinical signs of lumbar nerve root compression or radiological evidence of spondylolysis or osteomalacia were randomised to three treatments: 30 sessions of intensive dynamic back extensor exercises over three months; a similar programme at one-fifth the exercise intensity; or one month of thermotherapy, massage, and mild exercises. The results consistently favoured intensive exercise, which had no adverse effects. Since these exercises can be conducted in groups, the intensive programme is no more costly than conventional strategies that req

[Muscle abnormalities on the back of the hand and their differentiation from soft-tissue tumors].

Anomalous muscles at the hand and forearm are described in several studies. They may be confused with ganglia or with other soft-tissue tumours. Two rare cases of atavistic muscles in the area of the back of the hand are described which caused intense pain of the wrist during some months. During the operation an enlongated muscle directly below the extensor tendon, was found which was covered by the extensor retinaculum. It is an extensor digiti brevis manus, which exists in amphibia and is replaced in man by the long tendonous part of the extensor digitorum.

A review and empirical findings of fasciae and muscle interactions in low back pain.

BACKGROUND: Low Back Pain (LBP) is a global musculoskeletal disorder affecting quality of life, with 90% of cases categorized as nonspecific, indicating that the underlying cause is unknown. One of the current treatment modalities that physiotherapists use are fascia tissue manipulations (FTMs), such as soft tissue mobilization, myofascial release, and elastic tape, to enhance joint mobility and muscle flexibility in LBP individuals. PURPOSE: This review and experimental research explore the hypothetical mechanisms of FTMs using Skin Displacement (SKD), either by hand or with elastic tape. M

A dental stool with chest support reduces lower back muscle activation.

Activation of back musculature during work tasks leads to fatigue and potential injury. This is especially prevalent in dentists who perform much of their work from a seated position. We examined the use of an ergonomic dental stool with mid-sternum chest support for reducing lower back muscle activation. Electromyography of lower back extensors was assessed from 30 dental students for 20 s during three conditions in random order: (a) sitting upright at 90° of hip flexion on a standard stool, (b) leaning forward at 80° of hip flexion on a standard stool, and (c) leaning forward at 80° of hip f

Use of antagonist muscle EMG in the assessment of neuromuscular health of the low back.

BACKGROUND: Non-specific low back pain (LBP) has been one of the most frequently occurring musculoskeletal problems. Impairment in the mechanical stability of the lumbar spine has been known to lower the safety margin of the spine musculature and can result in the occurrence of pain symptoms of the low back area. Previously, changes in spinal stability have been identified by investigating recruitment patterns of low back and abdominal muscles in laboratory experiments with controlled postures and physical activities that were hard to conduct in daily life. The main objective of this study was

Back pain and disability after lumbar laminectomy: is there a relationship to muscle retraction?

OBJECTIVE: Preliminary studies have suggested that prolonged retraction of the paraspinal muscle during spinal surgery may produce ischemic damage. We report the continuous measurement of intramuscular pressure (IMP) during decompressive lumbar laminectomy and its relationship to subsequent back pain and disability. METHODS: Twenty patients undergoing two-level decompressive lumbar laminectomy for lumbar canal stenosis were recruited. Back pain and disability were assessed by use of the Visual Analog Score (VAS), Oswestry Disability Index (ODI), and Short-Form 36 (SF-36) Health Survey. During

Hypothalamic effects on medullary reticular activation of deep back muscle EMG.

Medullary reticular stimulation can activate deep back muscle EMG in urethane-anesthetized female rats. Midbrain central gray stimulation can facilitate brainstem reticular control over deep back muscles. Since these deep back muscles lateral longissimus (LL) and medial longissimus (ML) execute the vertebral dorsiflexion of lordosis behavior, and since the motor control hierarchy sketched above parallels lordosis behavior circuitry, we tested the hypothesis that medial hypothalamic lesions (which, in behavioral experiments, decrease lordosis) can also reduce medullary reticular activation of d

Stretch syncope in humans: Evidence for symptomatic neural reflex hypotension triggered by stretching of shoulder and upper back muscles.

BACKGROUND: Stretch-induced syncope (SIS) is a poorly understood condition that we hypothesized may be due to a neural reflex hypotensive response triggered by stretching of shoulder/upper back muscles. OBJECTIVE: This study compared the impact of shoulder/upper back stretching on heart rate (HR) and blood pressure (BP) responses in patients with SIS, with the findings in controls evaluated for symptoms unrelated to stretching. METHODS: The study population comprised 33 individuals: 9 otherwise healthy patients with SIS and 24 healthy controls. Beat-to-beat HR and systolic BP (SBP) and mean

Individuals with chronic low back pain have reduced myofascial force transmission between latissimus dorsi and contralateral gluteus maximus muscles.

The thoracolumbar fascia is essential in lumbar stabilization and is considered a path of transmitting myofascial force. This study investigates whether there is a difference in the myofascial force transmission between latissimus dorsi and contralateral gluteus maximus in individuals with and without chronic low back pain (CLBP). Forty-eight individuals were divided into CLBP and control groups. Outcome variables were evaluated in two experimental conditions: relaxed and contracted latissimus dorsi. Lumbar stiffness was assessed using a non-invasive digital indentometer, and passive propertie
