Neglected diseases caused by bacterial infections.

Bacterial infections represent a major health problem, especially in third world countries. In endemic regions, large populations of people are greatly affected, but the medical care is very limited. In this review, the neglected diseases buruli ulcer and trachoma are elucidated. Buruli ulcer is caused by Mycobacterium ulcerans which produces an outstanding immunosuppressive toxin mycolactone that induces an ulcerative, necrotic skin disease. Until today, only the combination of rifampin/streptomycin is used to treat buruli ulcer. However, this therapy is ineffective and expensive. Here, we re

Severe bacterial infections in haemodialysis patients.

Infections are today by far the most common complication affecting patients with chronic kidney disease and particularly those undergoing chronic haemodialysis. Numerous antimicrobial-resistant pathogens have emerged among patients with chronic kidney disease. These patients are hospitalized with greater frequency than the general population and the number of hospitalizations has been shown to correlate with higher infection rates. This review will briefly discuss the most common infections in patients undergoing chronic haemodialysis, the clinical relevance of antimicrobial resistance in thes

Acute bacterial skin infections and cellulitis.

PURPOSE OF REVIEW: Acute bacterial skin infections are very common, with various presentations and severity. This review focuses on deep skin infections. We separate acute nonnecrotizing infections of the hypodermis (erysipelas), forms with abscesses or exudates and necrotizing fasciitis. These three types actually differ in risk factors, bacteriology, treatment and prognosis. RECENT FINDINGS: Leg erysipelas/cellulitis occurs in more than one person/1000/year. It remains mainly due to streptococci. Foot intertrigo is an important risk factor. Necrotizing fasciitis is much rarer and usually oc

Secondary bacterial infections complicating skin lesions.

Secondary bacterial infection in skin lesions is a common problem. This review summarises a series of studies of the microbiology of several of these infections: scabies, psoriasis, poison ivy, atopic dermatitis, eczema herpeticum and kerion. Staphylococcus aureus and group A beta-haemolytic streptococci were the most prevalent aerobes and were isolated from all body sites. In contrast, organisms that reside in the mucous membranes close to the lesions predominated in infections next to these membranes. In this fashion, enteric gram-negative bacilli and Bacteroides spp. were found most often i

[Bacterial infections of the abdominal aorta].

There is a high mortality of bacterial aortitis particularly as it is usually not recognized before the stage of rupture. Therefore the disease should early be considered in obscure febrile conditions. Problems of diagnosis and therapy are discussed on two own cases. A 63-year old man had a rupture of a small infrarenal aortic aneurysm in the course of salmonella sepsis, a 79-year old woman had a pyogenic osteomyelitis of a lumbar vertebra that spread to the aorta and caused its rupture. In both cases surgical treatment consisted of bleeding control followed by in-situ reconstruction.

[Intractable bacterial infections in surgical patients].

Although many new antibiotics became available for clinical use, intractable bacterial infections are still major cause of morbidity and mortality in surgical patients. The infections are attributable to multiple factors. Surgical stress such as extensive burns and major surgery results in the depressed host-defense function, which is mediated by cytokine responses. Necrotic tissue, ischemia, hematoma, cholelithiasis, foreign bodies, indwelling catheters, intra-tracheal tubes, and other medical devices are local factors making infection resistant to ordinary chemotherapy. Multi-resistant bacte

[Recent intractable bacterial infections in otolaryngology].

Antibiotic treatment has resulted in a significant change in the etiology and character of bacterial infections in otolaryngology as well as in another fields. Classic signs and symptoms may be masked. Administration of penicillin can be still effective in the treatment of usual bacterial infections, as it is effective against group A beta-hemolytic streptococci and most anaerobes except those that produce beta-lactamase. Administration of antibiotics, such as penicillin with clindamycin or chloramphenicol to combat both aerobic and anaerobic bacteria, coupled with surgical drainage is essenti

[Diagnostic possibility in neonatal bacterial infections].

Bacterial infections in neonates may be acquired in utero, as is the case in congenital syphilis, or after delivery, such as late onset B streptococcal meningitis. Although some infections remain localized and may even be self-limiting, the majority of neonatal bacterial infections require speedy diagnosis and immediate and appropriate treatment. Severe invasive bacterial infections are rare but catastrophic; they are more common in intensive care wards. Neonatal, maternal, and environmental factors favoring bacterial infections are discussed together with their main clinical features. Useful

Bacterial infections of oral soft tissues.

Bacterial infections of the oral soft tissues are not particularly common. When they occur, they can often be rapidly progressive and even fatal without prompt diagnosis, drainage, and antibiotic therapy. Most infections are caused by endogenous oral bacteria that have overcome the host immune system as a result of surgery or, more frequently, because of underlying defects in the innate or specific immune systems. The role of oral anaerobes in these infections is increasingly being realized, and empirical antibiotic therapy should always take account of the possibility of anaerobic involvement

Bacterial respiratory infections and otitis media.

Bacterial infections of the respiratory tract are an important potential problem for children who attend group day care. Immunization (for Hib) or chemoprophylaxis, when appropriate, should lessen the risk and help to control or prevent the problem in many instances.

Unsuspected bacterial infections in febrile convulsions.

In a 12-month prospective study in 1984, blood and urinary cultures were obtained as a routine from 307 children who presented with fever and convulsions to the Mater Misericordiae Children's Hospital, Brisbane, and the results were compared with data from 1981-1983 when cultures were not taken as a routine. In the prospective study, bacteraemia was found in 12 (4.3%) of 282 patients but was not suspected clinically in half of these; urinary-tract infection was found in seven (2.6%) of 272 patients and in six of these it was not suspected clinically. All 12 patients with unsuspected bacteraemi

New vaccines against bacterial enteric infections.

During the last 10 years, rapid progress in basic research and biotechnology related to enteric infection has now begun to have a substantial impact on vaccine development against these infections. Two new typhoid vaccines, one for oral administration and the other for injection, which have much fewer and milder side effects than previous whole-cell parenteral vaccines, have become available. An oral cholera vaccine has been developed which, without any adverse reactions at all, confers long-lasting protection against cholera. The new cholera vaccine also, through antitoxic immunologic cross-r

[Phlyctenular keratoconjunctivitis in bacterial epibulbar infections].

Phlyctenular keratoconjunctivitis is now a rare clinical entity. Histologic staging has been updated by infection-immunological findings: phlyctenules have been identified as delayed-type cellmediated immune responses involving bacterial, mycobacterial, chlamydial, fungal or parasitic pathogens. Establishment of a correct diagnosis should include conjunctival and systemic work-ups for pathogenic agents. Case reports of three patients are presented.

Bacterial infections in the compromised host.

Patients with defects in host defence mechanisms represent an important and increasing problem in medicine. While severe, primary immunodeficiency diseases are rare, patients with secondary or acquired immunodeficiency disorders are commonly encountered, particularly in hospital medicine. The cause of these secondary immunodeficiency states is either the underlying disease or iatrogenic factors associated with therapeutic regimens, often a combination. A particular pattern of infections and pathogens may to some extent be characteristical of the type of immunodeficiency present and may therefo
