The age- and sex-related distribution of positive patch test reactions was investigated in 234 children (0-7 years, n = 72 and 8-14 years, n = 162), 1200 adults (20 to 50 years) and 295 elderly patients (> or = 70 years) with suspected allergic contact dermatitis using a European standard series. The aim of our study is to identify the allergens in patients with leg ulcers who have contact dermatitis surrounding the skin of the ulcer and compare them with a control group consisting of patients with lower extremity contact dermatitis. Fragrance is the most common cause of cosmetic allergic contact dermatitis. Fragrance allergy detection is best accomplished by testing with the fragrance mixture, balsam of Peru, and either jasmine synthetic or absolute. It would be desirable to have common fragrance allergens listed on cosmetic labels so that patients could avoid the allergens to which they are allergic.

BACKGROUND: Sensitization to topical treatments used in leg ulcers is common. Questioning and patch testing are used to identify causative drugs or dressings.
OBJECTIVES: To study the current frequency of sensitization in our centre, to analyse previously published studies, and to compare sensitization in recent years with studies published before 1990.

112 patients with stasis dermatitis and chronic leg ulcers have been studied by patch testing. Positive reactions to one or more haptens were recorded in 75 patients (67%). The more frequent reactions were observed to lanolin, balsam of Perù, para group, glycols and benzoyl peroxide. The results are briefly discussed.

First mentioning the immunological processes of contact dermatoses, the authors study the six contact allergies encountered most frequently by them during the course of 133 cases of leg ulcers which they treated. They consider in detail the allergenic action of the following six substances: lanolin, neomycin, balsam of Peru, paraben, phenylenediamine and benzocaine. BACKGROUND: With the development of industry, the numbers of allergens are increasing, and the frequency of these allergens show variations from country to country. Fragrances are a common cause of contact dermatitis and account for a large percentage of reactions to cosmetic products. Novel fragrance compounds that may not be detected by the common fragrance screening agents (including balsam of Peru and fragrance mix) are continually being produced. Lyral is one of those allergens found in many cosmetic and household products. This review will discuss the recent literature and the significance of this allergen to allergic contact dermatitis.
