Unsuccessful frontal balloon sinuplasty for recurrent sinus barotrauma.

The standard of care treatment for diffuse recurrent sinus barotrauma (RSB) is an endoscopic sphenoethmoidectomy with a complete frontal dissection. Successful healing leaves the RSB patient with no ethmoid sinuses and endoscopically patent frontal, sphenoid, and maxillary ostia. In persistent cases, patients with small frontal ostia will go on to require a frontal drillout. Patients presenting for surgical management of RSB generally have minimal sinus disease despite significant symptoms during flight and the prospect of extensive surgical management can be unappealing. With the advent of ba

Large lungs in divers: a risk for pulmonary barotrauma?

This retrospective study analysed a series of investigations on lung function in military divers and the importance of computed tomography (CT) scans concerning fitness to dive. We examined the incidence of blebs and bullae in a population of military divers with large lungs prompted by six cases of pulmonary barotrauma. All of these divers' medicals were normal apart from having large lungs (FVC > 120% predicted). A subsequent survey of the database of all divers and submariners of the Royal Netherlands Navy (RNLN) found another 72 divers/submariners with large lungs who were then evaluated b

Air travellers' awareness of the preventability of otic barotrauma.

OBJECTIVE: Otic barotrauma is common among air travellers and can cause severe otalgia, perforation of the tympanic membrane and hearing loss. Many prevention measures exist, with varying evidence to support their use. There are no data to establish if air travellers are aware of them or indeed use them. We aimed to establish air travellers' knowledge of such prevention measures. METHODS: We surveyed air travellers at two UK airports by means of a questionnaire. Answers to the questionnaire were collected over a two-week period. RESULTS: Overall, 179 air travellers with a mean age of 28 year

Tegmen dehiscence precipitated by barotrauma in idiopathic intracranial hypertension.

Attenuation of the tegmen has been associated with increased intracranial pressure (ICP) and increasingly observed in patients with idiopathic intracranial hypertension (IIH). Over time, the attenuated tegmen may be at increased risk for spontaneous dehiscence and result in cerebrospinal fluid (CSF) leakage. While spontaneous tegmen dehiscence has been reported in cases of elevated ICP, external etiologies (e.g., barotrauma) as potential precipitating factors remain understudied. We present the first two documented cases of tegmen dehiscence likely precipitated by barotrauma in patients with p

Postflight Pulmonary Barotrauma in a Pilot Without Underlying Disease.

BACKGROUND: Exposure to pressure changes can result in barotrauma in gas-filled cavities when volume expansion exceeds tissue strength, leading to local disruption. While few cases have been reported, we present a case of pulmonary barotrauma developed postflight in a pilot without underlying disease, and the most likely reason is improper pressure-breathing in flight. CASE REPORT: A 25-yr-old healthy male fighter pilot experienced slight chest pain 3 h after a 1-h flight training mission, during which the peak G was 6 G. The symptoms worsened during eating and were accompanied by chest tight

Prevention of middle ear barotrauma with oxymetazoline/fluticasone treatment.

Middle ear barotrauma (MEB) is a common complication of hyperbaric oxygen (HBO2) therapy. It has been reported in more than 40% of HBO2 treatments and can interrupt the sequence of HBO2. MEB may lead to pain, tympanic membrane rupture, and even hearing loss. The aim of this study was to determine if pretreatment with intranasal fluticasone and oxymetazoline affected the incidence of MEB. We conducted a retrospective chart review of subjects undergoing HBO2 at our institution between February 1, 2014, and May 31, 2019. Subjects in the fluticasone/oxymetazoline (FOT) treatment group used intrana

Colorectal barotrauma following compressed air spray to the perineum.

Colorectal injuries caused by high-pressure air compressors are rare and reported especially among industrial workers. They may appear because of intended or accidental injury. In the present paper, we report a case of colorectal injuries due to air insufflation from a distance towards the anus with the clothes on, as a means of a practical joke. The patient presented one day after the trauma to the Emergency Department with complaints of severe abdominal pain and vomiting. On examination, he had signs of peritonitis. A computed tomography (CT) scan did not show any perforation. Emergency lapa

Rare pulmonary barotrauma after explosive decompression: a case report.

BACKGROUND: Pneumothorax as a consequence of pulmonary barotrauma during explosive decompression military crew training in a hypobaric chamber is an extremely rare and sparsely diagnosed complication. Extensive bilateral tissue damage is even more unexpected. CASE PRESENTATION: A 26-year-old active duty Air Force pilot was performing an explosive decompression simulation from 8000 ft. (2438.4 m) to 25,000 ft. (7620 m) in a 1.5 s interval. The training was interrupted due to the pilot's apparent health complications. After transfer to the emergency department, a CT scan showed bilateral lung b

The prevalence of oro-facial barotrauma among scuba divers.

INTRODUCTION: Barotrauma is a physical injury that results from ambient pressure changes during flying, diving or hyperbaric oxygen therapy. The aim of this study was to assess the prevalence of oro-facial barotrauma among a sample of scuba divers in Jeddah, Saudi Arabia. MATERIALS AND METHODS: Data for the study were collected through a self-reported questionnaire that was distributed to 166 divers. The questionnaire was divided into two parts, in which the first part contained demographic data and the second part consists of multiple choice questions and a few open-ended questions discussin

Barotrauma developed during intra-hospital transfer -A case report-.

A 74-year-old male patient receiving ventilatory support due to aspiration pneumonia developed bilateral pneumothorax, pneumopericardium, pneumomediastinum, pneumo-retroperitoneum, and subcutaneous emphysema, after manual ventilation while being transferred from the intensive care unit (ICU) to the operating room (OR). These complications were assumed to be secondary to inappropriate manual ventilation of the intubated patient. In addition, it is likely that the possible migration of an already marginally acceptable endotracheal tube (ETT) position during transport was the cause of these compl

Pulmonary barotrauma in congenital diaphragmatic hernia: a clinicopathological correlation.

BACKGROUND/PURPOSE: The high mortality rate in congenital diaphragmatic hernia (CDH) has been ascribed to pulmonary hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). One of the principal treatment strategies has been the use of hyperventilation to reverse ductal shunting, but the wisdom of this approach is being questioned because of parenchymal lung injury from high inflation pressures. The authors hypothesize that the use of hyperventilation to reverse or prevent ductal shunting would result in ventilator-induced lung injury, which would be evident on postmortem examina

Middle ear barotrauma in patients undergoing hyperbaric oxygen therapy.

Hyperbaric oxygen therapy is associated with a risk of barotrauma to the middle ear. This prospective study of 82 patients undergoing long-term therapy for chronic conditions was designed to measure the incidence and severity of middle ear barotrauma. Twenty-four patients (29.3%) required the insertion of ventilation tubes for otalgia, significantly more of whom were suffering from radionecrosis of the head and neck region (P < 0.01). Thirty-two of the remaining 58 patients (55%) underwent specialist ENT assessment by otoscopy and tympanometry. Five ears (8%) showed the otoscopic changes of ba

Pulmonary barotrauma in mechanical ventilation. Patterns and risk factors.

The incidence of mediastinal emphysema (ME) and pneumothorax (PTX) was analyzed to determine the roentgenographic patterns and risk factors for the development of barotrauma in a population of mechanically ventilated patients. The roentgenograms of 139 intubated patients admitted to our medical intensive care unit over a ten-month period were evaluated for the presence of ME and PTX. Barotrauma was diagnosed in 34 of these patients, and ME was the initial manifestation in 24 patients. Of these patients with initial ME, ten subsequently developed PTX, a positive predictive value of 42 percent.
