Alport syndrome, basement membranes and collagen.

Alport syndrome, an inherited disorder of the kidney, eye and ear, has fascinated nephrologists, pathologists, and geneticists for nearly a century. With the recent application of molecular biochemical and genetic techniques, this mysterious disease has begun to yield some of its secrets. Alport syndrome can now be viewed as a generalized disorder of basement membranes that appears to result from mutations in an X-chromosome-encoded basement membrane collagen chain. This chain, along with two other novel collagen chains, is absent from Alport basement membranes, in contrast to the classical ch

[Structure and antigenicity of the glomerular basement membrane].

The glomerular basement membrane is a complex extracellular matrix formed of various molecules which build a supramolecular network. The major structural components are collagen IV, laminin, heparan sulfate proteoglycan, and nidogen/entactin. Cross-reacting antibodies against laminin, nidogen, and collagen IV may occur after several infectious diseases. They are however of doubtful pathogenetic significance. The pathogenetic relevant autoantibodies in Goodpasture's syndrome and rapidly progressive glomerulonephritis with linear immunofluorescence pattern are directed against epitopes which are

Inherited defects of renal basement membranes.

For 15 to 20 years, electron microscopy studies have shown that some hereditary renal diseases are characterized by various abnormalities of the basement membranes. In the nail-patella syndrome, in Alport's syndrome and variants, and in benign familial hematuria, the changes involve the glomerular basement membrane (GBM). In nephronophthisis, the tubular basement membrane (TBM) is affected. These histopathologic features are not uniform. The progress afforded by biochemistry, immunology, and molecular biology allows us to compare these various lesions, to analyze their biochemical characterist

[Basement membranes--structure, function, pathology].

Basement membranes are extracellular structures with a heterogeneous molecular composition. Several components have been identified and could be localized in specific morphological structures. Type IV collagen is found in the lamina densa, whereas laminin is the major component of the lamina lucida. Small amounts of heparan sulfate proteoglycan are also present in the lamina lucida. In some basement membranes, fibronectin and another glycoprotein, nidogen, could be identified. Epidermal basement membranes contain, in addition, the bullous pemphigoid antigen. Basement membranes are involved in

Structure, function, and pathology of basement membranes.

Basement membranes support epithelial and endothelial cells, prevent the passage of proteins, and generate histologically distinct compartments in the body. Basement membranes contain a number of components, only some of which have been isolated and characterized. These include type IV collagen, heparan sulfate proteoglycan, laminin, entactin, and fibronectin. Some components, such as bullous pemphigoid antigen and Goodpasture antigen, are present only in specific tissues, such as the skin or the kidneys. Alterations in basement membranes are associated with various diseases. For example, meta

Double Trouble in the Basement! A Case of PLA2R-Associated Membranous Nephropathy with Atypical Anti-Glomerular Basement Membrane Disease.

INTRODUCTION: Concurrent atypical anti-glomerular basement membrane (anti-GBM) disease with membranous nephropathy is a rare occurrence. Compared to typical anti-GBM disease, atypical anti-GBM disease is often seronegative, with a mild disease presentation and course. We present a case of concomitant of PLA2R-associated membranous nephropathy and atypical anti-GBM disease in a patient with cholangiocarcinoma. CASE PRESENTATION: A 66-year-old male with type 2 diabetes, hypertension, hyperlipidemia, hepatitis C, cirrhosis, and cholangiocarcinoma presented with nephrotic range proteinuria and wo

Basement membrane perforations guide anterior-posterior axis formation.

Establishment of the anterior-posterior (AP) axis is a critical symmetry-breaking event in mammalian development. In mice, this process involves the directed migration of the distal visceral endoderm (DVE). Here, we use targeted perturbations to demonstrate that asymmetric perforations in the basement membrane guide DVE migration. During implantation, matrix metalloproteinases in extra-embryonic tissues create uneven basement membrane perforations, establishing directional cues for cohesive DVE migration. Using light-sheet microscopy and tissue cartography, we show that migrating DVE deforms s

Oct4 controls basement membrane development during human embryogenesis.

Basement membranes (BMs) are sheet-like structures of extracellular matrix (ECM) that provide structural support for many tissues and play a central role in signaling. They are key regulators of cell behavior and tissue functions, and defects in their assembly or composition are involved in numerous human diseases. Due to the differences between human and animal embryogenesis, ethical concerns, legal constraints, the scarcity of human tissue material, and the inaccessibility of the in vivo condition, BM regulation during human embryo development has remained elusive. Using the post-implantatio

[Epithelial basement membrane dystrophy diagnosed following LASIK surgery].

A 48-year-old woman with no significant past history underwent bilateral simultaneous laser in situ keratomileusis for correction of her myopia. On the tenth postoperative day, the patient complained of visual decrease and photophobia. Slit lamp exam showed corneal epithelial irregularities. Confocal microscopy was performed and revealed a characteristic appearance of epithelial basement membrane dystrophy (EBMD). The patient was successfully treated with artificial tears and autologous serum eyedrops. EBMD may be missed before LASIK surgery, even after a careful pre-operative examination. Exa

The glomerular basement membrane: not gone, just forgotten.

The glomerular capillaries function as the filtration barrier that retains albumin and other plasma proteins in the circulation. The unresolved question that has been asked for more than 50 years is, Which structural component of these capillaries constitutes the main molecular sieve that normally retains albumin and allows its passage in diseases associated with proteinuria? There is considerable evidence implicating both the glomerular basement membrane (GBM) and the epithelial filtration slits as the barrier. However, the prevailing point of view at present is that the slit diaphragms bridg

Thin basement membrane nephropathy: diffuse and segmental types.

CONTEXT: Diffuse thinning of the glomerular basement membrane (GBM) is the ultrastructural diagnostic criterion of thin basement membrane nephropathy (TBMN). However, there is no consensus regarding what diagnosis should be made if the attenuation is segmental. OBJECTIVE: To develop a diagnostic approach to TBMN in cases with segmental GBM thinning. DESIGN: We compared the diagnostic sensitivities of 2 methods used for the quantitative expression of GBM width in a consecutive series of renal biopsies from 26 patients (median age, 36 years; range, 15 to 59 years) with dysmorphic hematuria (a

Galloway-Mowat syndrome: a glomerular basement membrane disorder?

We report a female infant with Galloway-Mowat syndrome. In addition to the characteristic dysmorphic appearance, neurological anomalies and early-onset nephrotic syndrome, she had arachnodactyly, an observation thus far reported uniquely in Taiwan. Also, her elder sister had the same condition. Renal pathology on light microscopy showed cystic dilatation of the renal tubules. Electron microscopy showed an irregular glomerular basement membrane and effacement of foot processes. This observation suggests that malformation of the glomerular basement membrane may cause the glomerulopathy in Gallow

Molecular pathology of the cutaneous basement membrane zone.

Recent research has provided considerable information concerning the biology of the cutaneous basement membrane zone (BMZ) in health and disease. In particular, identification of pathogenetic mutations in the genes encoding protein components at the BMZ has done much to increase our understanding of the inherited skin blistering disease, epidermolysis bullosa (EB). As the molecular pathology of different forms of EB is elucidated, correlations between genotype and phenotype become apparent. Determination of specific mutations in patients and families has not only clinical significance, but has

Molecular complexity of the cutaneous basement membrane zone.

Ultrastructural examination of the cutaneous basement membrane zone (BMZ) reveals the presence of several attachment structures, which are critical for integrity of the stable association of epidermis and dermis. These include hemidesmosomes which extend from the intracellular compartment of the basal keratinocyte to the underlying basement membrane where they complex with anchoring filaments, thread-like structures traversing the lamina lucida. At the lower portion of dermal-epidermal attachment zone, anchoring fibrils extend from the lamina densa to the papillary dermis, where they associate

Familial haematuria due to thin basement membrane nephropathy.

Persistent microscopic haematuria, especially when present in young individuals and associated with proteinuria, is generally of glomerular origin. Important causes include glomerulonephritis and Alport's syndrome. A new type of benign familial haematuria has now been reported characterised by little or no proteinuria, no deafness or hypertension and no deterioration of renal function. Examination by electron microscopy of renal biopsy material shows thinning or attenuation of the glomerular basement membrane. A family with thin basement membrane nephropathy is reported to illustrate this newl
