Cognitive and balance functions of astronauts after spaceflight are comparable to those of individuals with bilateral vestibulopathy.

INTRODUCTION: This study compares the balance control and cognitive responses of subjects with bilateral vestibulopathy (BVP) to those of astronauts immediately after they return from long-duration spaceflight on board the International Space Station. METHODS: Twenty-eight astronauts and thirty subjects with BVP performed five tests using the same procedures: sit-to-stand, walk-and-turn, tandem walk, duration judgment, and reaction time. RESULTS: Compared to the astronauts' preflight responses, the BVP subjects' responses were impaired in all five tests. However, the BVP subjects' performanc

Associations of Bilateral Vestibulopathy With Cognition in Older Adults Matched With Healthy Controls for Hearing Status.

IMPORTANCE: Recent literature suggests there may be a significant effect of the vestibular system on cognition and visuospatial processing. Given the increasing prevalence of dementia and individuals at risk for it, exploring possible modifiable risk factors, including vestibular dysfunction, is vital. OBJECTIVES: To explore the association of bilateral vestibulopathy (BV) with cognitive function in older adults, taking hearing status into account, and to explore multiple vestibular characteristics and their potential associations with cognition in patients with BV. DESIGN, SETTING, AND PART

Novel subtype of idiopathic bilateral vestibulopathy: bilateral absence of vestibular evoked myogenic potentials in the presence of normal caloric responses.

To characterize clinical features of those patients who showed an absence of vestibular evoked myogenic potential (VEMP) responses in the presence of normal caloric responses bilaterally, we reviewed clinical records of 1,887 consecutive outpatients who complained of balance problems, and identified three patients, who showed absent VEMPs in the presence of normal caloric responses bilaterally with unknown causes. All three patients had episodes of recurrent vertigo without spontaneous, gaze-evoked, or positional nystagmus at the time of examination. They complained of oscillopsia while moving

Assessing gait variability concurrently with dynamic visual acuity on a treadmill in people with bilateral vestibulopathy.

BackgroundGait variability is increased in people with bilateral vestibulopathy (BVP). Since dedicated gait analysis can be resource-intensive, concurrent assessment with another vestibular function test, dynamic visual acuity (DVA), is worth consideration.ObjectiveTo assess comparability of results from a combined gait and DVA assessment with results from a previous dedicated gait analysis.Methods15 participants (4 women) with BVP were analysed. The DVA test assessed visual acuity during stance and during treadmill walking at 2, 4 and 6 km/h. An 8-camera motion capture system measured spatiot

Semicircular canal, saccular and utricular function in patients with bilateral vestibulopathy: analysis based on etiology.

The diagnosis of bilateral vestibulopathy (BV) is typically established based on bilateral semicircular canal dysfunction. The degree to which both otolith organs-the saccule and utricle-are also impaired in BV is not well-established, particularly with respect to the etiology and severity of BV. The aim of this study was to evaluate semicircular canal, saccular and utricular function in patients with BV due to aminoglycoside ototoxicity and bilateral Menière's disease, and with different severities of BV. Caloric and head impulse testing were used as measures of canal function. Cervical vesti

Decoding the bilateral vestibulopathy spectrum: etiology-based phenotypes, clinical profiles and pathways to implant candidacy.

INTRODUCTION: Bilateral vestibulopathy (BVP) is a chronic, disabling disorder characterized by bilateral loss of vestibular function, leading to imbalance, oscillopsia, and falls. Despite established diagnostic criteria, clinical expression and rehabilitation potential vary widely across etiologies. Understanding these functional signatures is crucial for tailoring future neurostimulation strategies. The main objective of this study is to decode the etiological and functional heterogeneity of BVP and define clinical-functional profiles guiding vestibular or cochleo-vestibular implant candidacy

Characterization of head movement patterns in patients with bilateral and unilateral vestibulopathy during functional mobility tasks.

INTRODUCTION: Bilateral and unilateral vestibulopathies mainly cause chronic imbalance/unsteadiness and oscillopsia, significantly impacting quality of life. Traditional clinical tests fail to assess functional impact on daily activities. This study aimed to characterize head movement patterns in bilateral vestibulopathy (BV) and unilateral vestibulopathy (UV) patients during functional mobility tasks in a semi-standardized environment. This study could provide useful information for rehabilitation and optimization of vestibular implant stimulation. METHODS: Fifty-nine participants (19 BV, 20

Paving the Way Toward Distinguishing Fallers From Non-fallers in Bilateral Vestibulopathy: A Wide Pilot Observation.

Patients with bilateral vestibulopathy (BVP) present with unsteadiness during standing and walking, limiting their activities of daily life and, more importantly, resulting in an increased risk of falling. In BVP patients, falls are considered as one of the major complications, with patients having a 31-fold increased risk of falling compared to healthy subjects. Thus, highlighting objective measures that can easily and accurately assess the risk of falling in BVP patients is an important step in reducing the incidence of falls and the accompanying burdens. Therefore, this study investigated t

Intronic FGF14 GAA repeat expansions are a common cause of ataxia syndromes with neuropathy and bilateral vestibulopathy.

BACKGROUND: Intronic GAA repeat expansions in the fibroblast growth factor 14 gene (FGF14) have recently been identified as a common cause of ataxia with potential phenotypic overlap with RFC1-related cerebellar ataxia, neuropathy and vestibular areflexia syndrome (CANVAS). Our objective was to report on the frequency of intronic FGF14 GAA repeat expansions in patients with an unexplained CANVAS-like phenotype. METHODS: We recruited 45 patients negative for biallelic RFC1 repeat expansions with a combination of cerebellar ataxia plus peripheral neuropathy and/or bilateral vestibulopathy (BVP)

Mechanisms underlying treatment effects of vestibular noise stimulation on postural instability in patients with bilateral vestibulopathy.

BACKGROUND: Previous studies indicate that imbalance in patients with bilateral vestibulopathy (BVP) may be reduced by treatment with low-intensity noisy galvanic vestibular stimulation (nGVS). OBJECTIVE: To elucidate the potential mechanisms underlying this therapeutic effect. In particular, we determined whether nGVS-induced balance improvements in patients are compatible with stochastic resonance (SR)-a mechanism by which weak noise stimulation can paradoxically enhance sensory signal processing. METHODS: Effects of nGVS of varying intensities (0-0.7 mA) on body sway were examined in 19 p

Cognitive Function in Acquired Bilateral Vestibulopathy: A Cross-Sectional Study on Cognition, Hearing, and Vestibular Loss.

BACKGROUND: Several studies have demonstrated cognitive deficits in patients with bilateral vestibulopathy (BVP). So far, little attention has been paid to the hearing status of vestibular patients when evaluating their cognition. Given the well-established link between sensorineural hearing loss (SNHL) and cognitive decline and the high prevalence of SNHL in BVP patients, it is therefore uncertain if the cognitive deficits in BVP patients are solely due to their vestibular loss or might be, partially, explained by a concomitant SNHL. OBJECTIVE: To evaluate the link between cognition, hearing

Acute vestibulopathy with simultaneous bilateral involvement of the vestibulo-ocular reflex limited to the low-frequency range.

We report a novel type of idiopathic bilateral vestibulopathy with acute simultaneous involvement of the vestibulo-ocular reflex limited to the low-frequency range. A 64-year-old female presented with dizziness, oscillopsia, and difficulty walking. She did not experience rotatory vertigo and did not show any nystagmus. Vestibular function tests showed absent caloric responses in both ears, while vestibulo-ocular reflex (VOR) gains in the video head impulse test (vHIT) were preserved in all six semicircular canals. Cervical and ocular vestibular evoked myogenic potentials in response to air-con

Full-body kinematics and head stabilisation strategies during walking in patients with chronic unilateral and bilateral vestibulopathy.

Chronic imbalance is a frequent and limiting symptom of patients with chronic unilateral and bilateral vestibulopathy. A full-body kinematic analysis of the movement of patients with vestibulopathy would provide a better understanding of the impact of the pathology on dynamic tasks such as walking. Therefore, this study aimed to investigate the global body movement during walking, its variability (assessed with the GaitSD), and the strategies to stabilise the head (assessed with the head Anchoring Index). The full-body motion capture data of 10 patients with bilateral vestibulopathy (BV), 10 p

Preserved vestibular evoked myogenic potentials (VEMP) in some patients with walking-induced oscillopsia due to bilateral vestibulopathy.

Bilateral vestibulopathy, i.e. decreased peripheral vestibular function affecting both ears, is characterized by unsteadiness of gait, particularly in darkness and by motion-induced oscillopsia. We have recently seen a few patients with severely impaired semicircular canal function albeit with rather normal vestibular evoked myogenic potentials (VEMP) suggesting normal saccular function. The five young patients, mean age 27 years (range 15-45), 4 males and 1 female, had severely impaired balance in darkness and they all reported walking-induced vertical oscillopsia. Hence, these patients with

Chronic Electrical Stimulation of the Otolith Organ: Preliminary Results in Humans with Bilateral Vestibulopathy and Sensorineural Hearing Loss.

INTRODUCTION: Bilateral vestibulopathy is an important cause of imbalance that is misdiagnosed. The clinical management of patients with bilateral vestibular loss remains difficult as there is no clear evidence for an effective treatment. In this paper, we try to analyze the effect of chronic electrical stimulation and adaptation to electrical stimulation of the vestibular system in humans when stimulating the otolith organ with a constant pulse train to mitigate imbalance due to bilateral vestibular dysfunction (BVD). METHODS: We included 2 patients in our study with BVD according to Criteri

Chronic subjective dizziness: Fewer symptoms in the early morning--a comparison with bilateral vestibulopathy and downbeat nystagmus syndrome.

OBJECTIVE: Chronic subjective dizziness (CSD) is a form of chronic dizziness. Many patients report fewer symptoms when they get up with an aggravation over of the day. Therefore, the change in the severity of the symptoms during daytime was evaluated and compared with other chronic vestibular diseases: bilateral vestibulopathy (BVP) and downbeat nystagmus (DBN). DESIGN/METHODS: In a prospective study 131 patients with CSD, 108 patients with BVP, and 38 patients with DBN filled out a questionnaire asking about the symptom intensity (0 = no symptoms, 1 = light, 2 = moderate, 3 = strong) at 6 ti
