Factors influencing women's decisions regarding birth planning in a rural setting in Kenya and their implications for family planning programmes.

Evidence suggests that socio-cultural barriers that limit the ability of women in sub-Saharan Africa to make decisions regarding contraceptive use and childbearing contribute to the slow uptake of contraception in this region. This paper explores factors influencing women's decisions regarding contraceptive uptake, switching and discontinuation, and the implications of those decisions for family planning programmes. Data were from in-depth interviews that were conducted in 2018 with 42 women who participated in a longitudinal research project implemented in Homa Bay County of Kenya. Participan

Prevalence and risk factors for hyperbilirubinemia among newborns from a low-risk birth setting using delayed cord clamping in Japan.

AIM: Neonatal jaundice is a common problem among infants. Among the several risk factors are East Asian race and delayed cord clamping. Birth centers manage low-risk term deliveries using physiological management, which may include delayed cord clamping. This study aimed to investigate the occurrence of hyperbilirubinemia, a pathological process of jaundice, and its risk factors among neonates born at a Japanese birth center. METHODS: This was a retrospective cohort study. Data were collected from March 2006 to October 2014 from healthy mothers and neonates at a birth center in a metropolitan

The burden of inappropriate birth weight on neonatal survival in term newborns: a population-based study in a middle-income setting.

INTRODUCTION: Premature birth, perinatal asphyxia, and infections are the main causes of neonatal death. Growth deviations at birth also affect neonatal survival according to week of gestation at birth, particularly in developing countries. The purpose of this study was to verify the association between inappropriate birth weight and neonatal death in term live births. METHODS: This is an observational follow-up study with all term live births from 2004 to 2013 in Sao Paulo State, Brazil. Data were retrieved with the deterministic linkage of death and birth certificates. The definition of ver

Adult Externalizing and Suicidal Behavior in Children Who Set Fires: Analysis of a 40 Year Birth Cohort Study.

OBJECTIVE: Firesetting in children is thought to be an indicator of severe conduct problems in young people. However, no research has examined whether childhood firesetting is also associated with increased risk of externalizing and suicidal behaviors in adulthood. METHOD: Data were obtained from a longitudinal study (n = 1265). Childhood firesetting/conduct problems (7-10 years) were derived from an assessment of antisocial behavior. Externalizing/suicidal behavior was derived from the Composite International Diagnostic Interview and the Self-Report Delinquency Inventory. Generalized estimat

60 Million non-facility births: who can deliver in community settings to reduce intrapartum-related deaths?

BACKGROUND: For the world's 60 million non-facility births, addressing who is currently attending these births and what effect they have on birth outcomes is a key starting point toward improving care during childbirth. OBJECTIVE: We present a systematic review of evidence for the effect of community-based cadres-community-based skilled birth attendants (SBAs), trained traditional birth attendants (TBAs), and community health workers (CHWs)-in improving perinatal and intrapartum-related outcomes. RESULTS: The evidence for providing skilled birth attendance in the community is low quality, co

Predictors of mortality among low birth weight neonates after hospital discharge in a low-resource setting: A case study in Uganda.

BACKGROUND: Most neonatal deaths occur among low birth weight infants. However, in resource-limited settings, these infants are commonly discharged early which further exposes them to mortality. Previous studies on morbidity and mortality among low birth weight infants after early discharge mainly focused on very low birth weight infants, and none described post-discharge neonatal mortality. This study aimed to determine the proportion and predictors of mortality among low birth weight neonates discharged from the Special Care Baby Unit at Mulago National Referral Hospital in Uganda. METHODS:

'She convinced me'- partner involvement in choosing a high risk birth setting against medical advice in the Netherlands: A qualitative analysis.

Home births in high risk pregnancies and unassisted childbirth seem to be increasing in the Netherlands. There is a lack of qualitative data on women's partners' involvement in these choices in the Dutch maternity care system, where integrated midwifery care and home birth are regular options in low risk pregnancies. The majority of available literature focuses on the women's motivations, while the partner's influence on these decisions is much less well understood. We aimed to examine partners' involvement in the decision to birth outside the system, in order to provide medical professionals

Preterm birth, low birth weight, and their co-occurrence among women with preexisting chronic diseases prior to conception: a cross-sectional analysis of postpartum women in a low-resource setting in Ghana.

BACKGROUND: The incidence of chronic diseases, which are significant contributors to maternal deaths and adverse new-born outcomes, is increasing among women of reproductive age in northern Ghana. This emerging health issue raises serious concerns about the potential exacerbation of adverse birth outcomes in this setting, given that it is one of the regions in the country with a high incidence of such outcomes. We investigated the risks of preterm birth (PTB), low birth weight (LBW), and concurrent PTB and LBW among women with preexisting chronic conditions prior to conception in the Tamale Me

Is time of birth a predictor of adverse perinatal outcome? A hospital-based cross-sectional study in a low-resource setting, Tanzania.

BACKGROUND: Inconsistent evidence of a higher risk of adverse perinatal outcomes during off-hours compared to office hours necessitated a search for clear evidence of an association between time of birth and adverse perinatal outcomes. METHODS: A cross-sectional study conducted at a tertiary referral hospital compared perinatal outcomes across three working shifts over 24 h. A checklist and a questionnaire were used to record parturients' socio-demographic and obstetric characteristics, mode of delivery and perinatal outcomes, including 5th minute Apgar score, and early neonatal mortality. The lived experience of mothers who give birth after induction of labour in hospital settings in Southern Ethiopia: a phenomenological study.

BACKGROUND: Induction of labor (IOL) is a medical intervention used to initiate labor artificially and has become one of the most common procedures in modern obstetrics to improve maternal and neonatal outcomes. While extensive research has focused on the methods and outcomes of labor induction, limited evidence exists regarding mothers' lived experiences following IOL, particularly in the Ethiopian context. OBJECTIVE: To explore the lived experience of mothers who give birth after induction of labor at Wolaita Sodo Comprehensive Specialized Hospital, Southern Ethiopia, 2024. METHODS: A qual

Immediate skin-to-skin contact after birth ensures stable thermoregulation in very preterm infants in high-resource settings.

AIM: To investigate the impact of immediate skin-to-skin contact with a parent after birth on thermal regulation in very preterm infants. METHODS: This clinical trial was conducted in three neonatal intensive care units in Scandinavia from 2018 to 2021. Infants born between 28 + 0 and 32 + 6 weeks and days of gestation were randomised to immediate skin-to-skin contact or conventional care in an incubator during the first 6 postnatal hours. We report on a secondary outcome: serial measurements of axillary temperature. RESULTS: Ninety-one infants were randomised to skin-to-skin contact or conv

Perspective: L-arginine and L-citrulline Supplementation in Pregnancy: A Potential Strategy to Improve Birth Outcomes in Low-Resource Settings.

The available data support the hypothesis that L-arginine or L-citrulline supplementation would be suitable for implementation in resource-constrained settings and will enhance placental vascular development and improve birth outcomes. In resource-constrained settings, the rates of adverse birth outcomes, including fetal growth restriction, preterm birth, and low birth weight, are disproportionately high. Complications resulting from preterm birth are now the leading cause of mortality in children <5 y of age worldwide. Despite the global health burden of adverse birth outcomes, few effective

Domestic abuse disclosure in perinatal healthcare settings - A qualitative study into the experiences of mothers who had their baby removed at birth.

BackgroundIn the UK, numbers of newborn babies entering state care through child protection processes have significantly increased in the last decade. Their mothers often have experienced cumulative and intersecting disadvantages, including childhood abuse and domestic abuse (DA). DA will often be a key factor in child protection decision-making, with a persistent narrative of mothers 'failing to protect' their children. Given the profound consequences of DA disclosures and the failure-to-protect discourse that might impact care proceeding outcomes for these mothers, it is important to explore

Epidemiological trends of isolated and non-isolated central nervous system congenital malformations in live births in a middle-income setting.

OBJECTIVES: This study aimed to analyze, in the São Paulo state of Brazil, time trends in prevalence, neonatal mortality, and neonatal lethality of central nervous system congenital malformations (CNS-CM) between 2004 and 2015. METHODS: Population-based study of all live births with gestational age ≥22 weeks and/or birthweight ≥400 g from mothers living in São Paulo State, during 2004-2015. CNS-CM was defined by the presence of International Classification Disease 10th edition codes Q00-Q07 in the death and/or live birth certificates. CNS-CM was classified as isolated (only Q00-Q07 codes), an

Point-of-Care Quantification of Serum Alpha-Fetoprotein for Screening Birth Defects in Resource-Limited Settings: Proof-of-Concept Study.

BACKGROUND: Maternal serum alpha-fetoprotein (MSAFP) concentration typically increases during pregnancy and is routinely measured during the second trimester as a part of screening for fetal neural tube defects and Down syndrome. However, most pregnancy screening tests are not available in the settings they are needed the most. A mobile device-enabled technology based on MSAFP for screening birth defects could enable the rapid screening and triage of high-risk pregnancies, especially where maternal serum screening and fetal ultrasound scan facilities are not easily accessible. Meeting the challenge of providing neonatal nutritional care to very or extremely low birth weight infants in low-resource settings.

Most infant deaths (99%) occur in developing countries. The 14.9 million infants born prematurely (>11% of all live births) carry a particularly high mortality risk. This chapter discusses strategies to improve neonatal outcome under resource-restricted conditions, with a focus on nutritional interventions. Evidence-based interventions begin before conception with strategies to prevent and treat malnutrition among women of reproductive age, and micronutrient supplementation in pregnancy. As an example, a practically feasibly strategy of feeding very low birth weight infants in South Africa is
