Investigating the Association Between Volatile Organic Compounds and Preterm Birth in Detroit, Michigan: Protocol for the Center for Leadership in Environmental Awareness and Research Birth Cohort Study.

BACKGROUND: Preterm birth (PTB), or birth before 37 weeks of gestation, remains a significant public health issue in the United States, particularly in Detroit, Michigan. Growing evidence suggests that volatile organic compounds (VOCs), aromatic or chlorinated organic compounds that vaporize readily, may influence PTB risk. However, much of this prior work is limited by indirect VOC exposure estimates (eg, assignment based on maternal residential address), single-point or cumulative exposure estimates during pregnancy, or limited consideration of potential mechanistic factors. OBJECTIVE: The

Integrating Neonatal Intensive Care Into a Family Birth Center: Describing the Integrated NICU (I-NIC).

Parent-infant separation resulting from admission to a neonatal intensive care unit (NICU) is often reported as the most challenging and distressing experience for parents. Aiming to mitigate the stress of parent-infant separation, a new neonatal care model was designed to integrate NIC with delivery and postpartum care. Yet, little is known about the model and its implementation. Therefore, using a qualitative descriptive design with field observations, we describe the characteristics of an integrated neonatal intensive care (I-NIC) model and examined perceptions of clinical staff ( n = 8) an

A Multi-Center Survey of Necrotizing Enterocolitis Prevention Strategies in Very Low Birth Weight Infants.

OBJECTIVE: To compare the prevention practices of necrotizing enterocolitis (NEC) across 17 neonatal intensive care units (NICUs) in China. METHODS: A web-based survey was sent to 17 level 3 NICUs in China on September 21, 2023, to evaluate the prevention strategies for NEC. RESULTS: All 17 Neonatal Intensive Care Units (NICUs) responded to the survey. There was significant variation in the initial empirical use of antibiotics for early-onset sepsis, late-onset sepsis, and NEC among different NICUs. Out of the 17 NICUs, only 5 (29.4%) used donor human milk. Additionally, 15 (88.2%) NICUs per

Neonatal mortality and survival of low-birth-weight infants at hospitals in indonesia, a multi center study.

The first month of life is the most vulnerable period for child survival. Newborns who are born too soon or too small, or sick, are at the greatest risk of death. Objective of the study was to delve into neonatal mortality and low birth weight (LBW) survival at hospitals in Indonesia. It was a retrospective cohort on the neonates cared at 12 hospitals in various islands of Indonesia during August-October 2021. Inclusion criteria were singleton and age 0-28 days, whereas exclusion criteria were incomplete medical records and gestational age ≥ 42 weeks. Of 2726 neonates, 2017 were eligible for t

Survival Rates of Extremely Low-Birth-Weight Infants in a Tertiary Care Center in Saudi Arabia.

Introduction Extremely low birth weight (ELBW) refers to the condition in which an infant is born with a weight of less than one thousand grams (2.2 pounds) at birth. ELBW infants face significant challenges and are at increased risk for various medical complications and developmental issues. ELBW poses unique challenges for infants, families, and healthcare providers. Understanding the causes, consequences, and appropriate management strategies for ELBW is crucial for improving the survival rates of these vulnerable infants. Aim This study aimed to measure the survival rates of ELBW infants i

Vaginal birth after cesarean section: 10 years of experience in a tertiary medical center in Taiwan.

OBJECTIVE: Because of the increased risk of uterine rupture and other morbidities, instances of trial of labor after cesarean (TOLAC) have decreased in number each year. Nevertheless, under careful assessment and advanced medical care, TOLAC is still a safe option for delivery. The objective of this study is to find the factors that impact the success rate for TOLAC and to compare the results with Taiwan national registry data. MATERIALS AND METHODS: A longitudinal cohort study that includes a total of 254 cases of women receiving TOLAC in a tertiary medical center over a period of 10 years.

Individual and center-level factors affecting mortality among extremely low birth weight infants.

OBJECTIVE: To examine factors affecting center differences in mortality for extremely low birth weight (ELBW) infants. METHODS: We analyzed data for 5418 ELBW infants born at 16 Neonatal Research Network centers during 2006-2009. The primary outcomes of early mortality (≤12 hours after birth) and in-hospital mortality were assessed by using multilevel hierarchical models. Models were developed to investigate associations of center rates of selected interventions with mortality while adjusting for patient-level risk factors. These analyses were performed for all gestational ages (GAs) and sepa

Outcomes of very low birth weight infants in a newborn tertiary center in Turkey, 1997-2000.

Our purpose was to determine mortality and morbidity rates and selected outcome variables for infants weighing less than 1500 g, who were admitted to the neonatal intensive care unit of our hospital from 1997 to 2000. The ultimate goal of the study was to define a model for developing a regional database. Information on all very low birth weight (VLBW) admissions to a tertiary level neonatal intensive care unit (NICU) in Ankara between January 1997 and December 2000 was prospectively collected by three neonatologists using a standard manual of operation and definitions. The data consisted of p

In-hospital care for low-risk childbirth. Comparison with results from the National Birth Center Study.

The largest prospective study of freestanding birth centers was reported in 1989. This article reports on data from a comparison group of over 2,000 low-risk women who were admitted to hospital-care settings during the same period. The data on the hospitalized women were collected using the research methodology and data collection instruments developed for the birth center study. Consequently, these data offer the opportunity to observe differences that can be associated with birth site. Both groups of women experienced similar rates of serious antepartum and intrapartum health problems and ma

The Case for History-Indicated Cerclage: A Patient-Centered Approach to Preventing Pregnancy Loss and Preterm Birth.

This review summarizes evidence supporting history-indicated cerclage and the evolution of commonly used eligibility thresholds. Randomized trials and meta-analyses demonstrate that cerclage reduces very early preterm birth in high-risk pregnancies without meaningful heterogeneity of treatment effect across risk strata. Although studies comparing cervical length surveillance versus history-indicated cerclage report similar population-level outcomes, differences in patient selection and study design limit interpretation. History-indicated cerclage, therefore, remains an important option for sel

The theory behind a successful upright breech birth: A single-center systematic video-based analysis.

OBJECTIVE: Safe vaginal breech birth requires timely recognition of stagnation and skilled execution of maneuvers, yet detailed descriptions of upright (all-fours) breech techniques remain scarce. Video-based approaches provide an opportunity to capture real-life practice and inform refinement of existing algorithms. The aim was to describe the management of vaginal breech birth on all fours using systematic video analysis, document maneuvers applied, and generate exploratory insights to refine the physiological breech birth algorithm. METHODS: Retrospective analysis of 66 video-recorded succ

Urogenital cultures and preterm birth in women with cervical cerclage: a single center retrospective cohort study.

BACKGROUND: The leading hypothesis of the pathogenesis of cervical insufficiency suggests a role of cervical inflammation. Urogenital tract infections could play a causative role in this process. To test this hypothesis in women with a cervical cerclage, we aimed to retrospectively examine the relationship between gestational age (GA) at delivery and positive urogenital cultures. METHODS: This single center retrospective study reviewed the records of all women with a singleton pregnancy that underwent cervical cerclage (n = 203) between 2010 and 2020 at the University Hospital of Leuven, Belg
