We observed a case of lung infection with Paragonimus for a 4 year old child in Gabon. The treatment with bithionol was partially uneffective. Indeed, after the end of this treatment we observed hypereosinophilia, hyper IgG and X Rays pulmonary opacities but without eggs in stools of sputum.

Of the numerous infectious diseases afflicting humans, anthrax disease, caused by Bacillus anthracis, poses a major threat in its virulence and lack of effective treatment. Thirty-nine cases of pulmonary paragonimiasis due to Paragonimus westermani were identified in Manipur, India. Recurrent haemoptysis, often increased on exertion, was the most common symptom. The clinical features and radiological appearances simulated pulmonary tuberculosis and 23 cases were treated for tuberculosis for varying periods before the final diagnosis. Three cases of hepatic fascioliasis diagnosed in the Department of Internal Medicine are presented. All of the patients live in Madrid, all were fed watercress in the last month and the diagnosis was established during the invasive phase and confirmed by serological testing. We emphasized the importance of the TC scan in the correct diagnosis, the ectopic location of the parasite presentation like subcutaneous thoracic nodule and the efficacy of bithionol in the treatment.

A Laotian immigrant with persistent cough and hemoptysis was found to have multiple small cavities on chest roentgenogram. Ova of the lung fluke Paragonimus westermani were found in the sputum, and the patient responded to bithionol therapy. Pulmonary paragonimiasis should be considered in the differential diagnosis of cavitary lung disease in Indochinese refugees entering the United States.

We present a case report of invasive hepatic fascioliasis. Ramified hypodense lesions with peripheral distribution were found in the CT-scan. These lesions disappeared after treatment with Bithionol. CT-scan plays an important role in the diagnosis of invasive fascioliasis whereas ultrasound is useful in the chronic phase. Both are helpful to evaluate response to treatment.

The ideal type of an anthelmintic drug should be effective against the parasite and not toxic to the host. In early studies of this disease in Japan, a large number of different drugs was tested, none of which was promising enough. A 2-year-old Manipuri girl had haemoptysis and a chest roentgenogram disclosed diffuse-infiltrates in the right lung. Microscopic examination of sputum and morning gastric aspirate showed numerous golden-brown, operculated ova, and microscopic examination of stool specimens confirmed these ova to be those of the lung fluke, Paragonimus westermani. She responded favourably to bithionol therapy and was asymptomatic and growing normally during follow up for one and half years.

Fasciola hepatica, a zoonotic liver fluke, can cause disease in humans. Fascioliasis, while common in some tropical and developing countries, is uncommon in the United States. We report two cases of fascioliasis that illustrate both the hepatic and biliary stages of the disease. Clinical features and diagnostic aspects including serologic, radiographic, and histopathologic studies are emphasized. Praziquantel was ineffective in treatment of both patients. Bithionol appears to be an effective treatment for fascioliasis.

Five patients with fascioliasis presented with malaise, pruritus, and pain in the right hypochondrium. Eosinophilia, increase in serum gamma globulin, and pyrexia were other features. Four patients treated with oral bithionol recovered completely and it is suggested that this is an effective form of therapy. The double diffusion precipitin test, being speedy and simple, was used as a screening procedure for diagnosis. The source of infection was wild watercress growing in a farm ditch.

Two patients with hepatic fascioliasis are presented. Computed tomography demonstrated multiple low density areas located in the center and periphery of the liver. In one patient a spontaneous decrease in the size of the low-density areas occurred but the abdominal symptoms were persistent before bithionol treatment. After the treatment both patients became asymptomatic. The computed tomography scan can be a useful tool for the diagnosis and follow-up of the patients with hepatic fascioliasis.

A 57-year-old man with a history of having consumed fresh-water crab 8 months previously presented with a solitary mass lesion in the left lung without any symptoms. Since neoplastic disease could not be ruled out, an open biopsy was performed, and histological examination confirmed a definite diagnosis of paragonimiasis. A high level of the anti-Paragonimus westermani antibody was detected in the sera of the patient, but this level decreased substantially 3 months after chemotherapy with bithionol.

A case of fascioliasis is described in a 56 year-old male with symptoms of biliary colic and biochemical cholestasis. Paragonimiasis is a common parasitic zoonosis in China. Its clinical manifestations may be classified into two clinical entities, eg, paragonimiasis westermani type and paragonimiasis szechuanensis type. The latter mainly manifests as trematode larva migrans. The immunodiagnostic methods frequently used in the diagnosis of paragonimiasis are intradermal test, CFT and ELISA. The latex agglutination test (LAT) was compared with the complement fixation test (CFT), double diffusion test (DDT) and enzyme-linked immunosorbent assay (ELISA) for the serodiagnosis of South American paragonimiasis.
