The Risk of Hemorrhage and Shunt Occlusion After Tonsillectomy in a Glenn Circulation Patient With Blalock-Taussig Shunt Dependence: A Case Report.

Tonsillectomy is among the most common procedures performed on children. Children with congenital heart disease sometimes undergo tonsillectomy to reduce pulmonary arterial pressure. These children can have low cardiac function and low oxygen saturation (SpO2) and pose particular challenges to the anesthesia team. In this article, we report our experience of anesthetic management of a child with post-Glenn procedure and Blalock-Taussig shunt dependence who underwent a tonsillectomy. To avoid postoperative hemorrhage, anti-coagulants/anti-platelets were discontinued five days before surgery. Af

Multiscale modeling of a modified Blalock-Taussig surgery in a patient-specific tetralogy of Fallot.

Tetralogy of Fallot (TOF) is a congenital heart anomaly that causes a drastic reduction in the oxygen level. In this study, we coupled a lumped-parameter model with a patient-specific three-dimensional (3D) model which included a modified Blalock-Taussig (MBT) shunt. By forming a closed loop, we investigated the effects of certain parameters on the flow rates and the pressures at different locations of the developed network. A local sensitivity analysis on an initial zero-dimensional (0D) closed-loop model was conducted. The 0D lumped parameter (LP) model was then refined based on the results

Patency of Blalock-Taussig shunt assessed by Doppler colour flow imaging.

A neonate with pulmonary atresia and intact ventricular septum underwent a modified right sided Blalock-Taussig shunt. Following the procedure, the patient developed a right sided haemothorax, which required drainage. A murmur was not audible subsequently. Echocardiography with colour flow imaging enabled identification of patency of the right sided shunt. It was then possible to discontinue intravenous prostaglandin medication.

Kawashima procedure after staged unifocalizations in asplenia with major aortopulmonary collateral arteries.

We report a Kawashima procedure (total cavopulmonary shunt) successfully carried out for asplenia syndrome, pulmonary atresia, and major aortopulmonary collateral arteries. At the age of 8, the patient underwent staged bilateral unifocalizations using confluent central pulmonary arteries concomitant with bilateral modified Blalock-Taussig shunts. As the result of an interrupted inferior vena cava with azygous continuation, the patient required a Kawashima procedure with augmentation of the central pulmonary arteries for definitive palliation 1 year later. Cyanosis, respiratory distress, and ve

Alternate Approach to Hypoplastic Left Heart Syndrome Stage 1 Surgery.

Hypoplastic left heart syndrome is almost always fatal in neonates unless it is palliated surgically. The conventional Norwood procedure with a Blalock-Taussig shunt or a Sano modification and a hybrid approach with branch pulmonary artery banding and ductal stenting are common surgical options. An alternate technique with a different strategy to restore pulmonary blood flow is described in this report.

PDA Stenting for Ductal-Dependent Cyanotic Congenital Heart Disease: History and View from 10,000 Feet.

This article provides a historical review and a current perspective on the procedures used to palliate cyanosis in ductal-dependent infants. Eighty years ago, Helen Taussig, Alfred Blalock, and Vivien Thomas developed the first effective treatment. The Blalock-Taussig-Thomas (BTT) shunt is the historical predecessor of both the contemporary-modified BTT shunt and interventional stenting of the Patent Ductus Arteriosus (PDA). The surgical shunt was firmly established therapy before catheterization was born, and PDA stenting was not possible until the technologies designed to address coronary he

Multicenter comparison of hybrid and Norwood procedures in patients with Fontan circulation.

OBJECTIVE: The hybrid stage 1 palliation (HS1P) procedure has been proposed as an alternative to the Norwood operation. We utilized a multicenter database to compare both strategies. METHODS: The Fontan Outcomes Registry using Clinical Examinations was queried for patients who underwent HS1P or Norwood. Propensity score matching was performed. Composite outcome (death, transplant listing, protein losing enteropathy, plastic bronchitis, atrial/ventricular tachyarrhythmia, or pulmonary artery reintervention) and cardiac magnetic resonance variables were compared. Percutaneous Atrioventricular Valve Repair With MitraClip in Failed Adult Fontan Circulation for Tricuspid Atresia.

A 60-year-old woman with tricuspid atresia had previously undergone palliation with a Glenn procedure and a Blalock-Taussig shunt, and finally correction with a Fontan procedure. Twenty-five years later, severe atrioventricular valve regurgitation with concomitant valvular prolapse and cleft leading to heart failure was successfully treated using 1 single MitraClip XTW, confirming its effectiveness as a lower-risk alternative to surgery. The patient showed marked improvement of heart failure-related symptoms, with residual mild to moderate regurgitation and improved liver function.

[Improvement of outcomes in the surgical treatment of hypoplastic left heart syndrome (HLHS) with staged Norwood operation].

On behalf of rapid progress of diagnostic technologies and new development of surgical technique or strategy, outcomes of surgical treatment of hypoplastic left heart syndrome (HLHS) has remarkably improved in the current practice. One of such approaches is the staged Norwood operation. We have reviewed our patients (n=54) between 2003 and 2013. A half of the patients with the staged group accomplished Norwood procedure and concomitant bidirectional cavopulmonary shunt at the mean age of 4 months old. An another half of the patients underwent secondary Norwood operation with Blalock-Taussig( B

Cardiothoracic Anesthesia and Critical Care in the United Kingdom (UK) Part 1: Some Insights Into the History and Development.

This review is intended to highlight some of the historic events that contributed to the development of thoracic and cardiac anesthesia and surgery in Great Britain and Northern Ireland (UK). The aim of this first of two parts is to concentrate on the development of techniques, facilities, and pharmacology that allowed progress and advancement in patient management that were developed primarily in the UK. However, progress usually requires input from a wide variety of sources of knowledge, and cardiothoracic practice is no exception. Reference is, thus, made to sources outside of the UK that g

Emergency transcatheter balloon recanalization of acutely thrombosed modified Blalock-Taussig shunts.

BACKGROUND: Modified Blalock-Taussig shunts are used to palliate a variety of cyanotic heart diseases associated with reduced pulmonary blood flow. Acute shunt thrombosis in patients with shunt-dependent pulmonary blood flow can result in life-threatening hypoxia. We describe our experience of emergency transcatheter recanalization in 5 severely hypoxic children with acute shunt occlusion. METHODS AND RESULTS: Five patients with ages ranging between 5 and 24 months (median 11 months) and weight ranging from 4 to 8 kg (median 5 kg) presented with severe hypoxia, acidosis and hypotension follow

Correlation of a novel noninvasive tissue oxygen saturation monitor to serum central venous oxygen saturation in pediatric patients with postoperative congenital cyanotic heart disease.

Using a novel noninvasive, visible-light optical diffusion oximeter (T-Stat VLS Tissue Oximeter; Spectros Corporation, Portola Valley, CA) to measure the tissue oxygen saturation (StO2) of the buccal mucosa, the correlation between StOz and central venous oxygen saturation (ScvO2) was examined in children with congenital cyanotic heart disease undergoing a cardiac surgical procedure. Paired StO2 and serum ScvO2 measurements were obtained postoperatively and statistically analyzed for agreement and association. Thirteen children (nine male) participated in the study (age range, 4 days to 18 mon

Vivien Theodore Thomas (1910-1985): An African-American laboratory technician who went on to become an innovator in cardiac surgery.

Vivien Theodore Thomas (1910-1985) was an African-American laboratory technician and instructor of surgery at Johns Hopkins University, Baltimore. He was born as the grandson of a slave in Lousiana, working as a carpenter and subsequently as a laboratory technician after the great depression and the loss of his savings derailed his plans to become a doctor. In his role as a laboratory technician, he overcame challenging personal circumstances to become an innovator in paediatric cardiac surgery, despite having no formal college education. He played an important role in assisting Alfred Blalock

[Ductus arteriosus patency with stenting in critical pulmonary stenosis and pulmonary atresia with intact interventricular septum].

We report our experience with stenting of the ductus arteriosus in three neonates. Two patients had pulmonary atresia with an intact interventricular septum and one had critical pulmonary stenosis. Radiofrequency ablation was used to open the atretic pulmonary valve in two patients. In all three patients implanting a stent avoided the need for surgical creation of a Blalock-Taussig shunt. In two patients the procedure was scheduled as elective surgery, and in one it was done as an emergency procedure. Ductus arteriosus stenting is an alternative to palliative cardiac surgery.

Intermediate-term mortality and cardiac transplantation in infants with single-ventricle lesions: risk factors and their interaction with shunt type.

OBJECTIVE: The study objective was to identify factors associated with death and cardiac transplantation in infants undergoing the Norwood procedure and to determine differences in associations that might favor the modified Blalock-Taussig shunt or a right ventricle-to-pulmonary artery shunt. METHODS: We used competing risks methodology to analyze death without transplantation, cardiac transplantation, and survival without transplantation. Parametric time-to-event modeling and bootstrapping were used to identify independent predictors. RESULTS: Data from 549 subjects (follow-up, 2.7 ± 0.9 ye
