Application of the Blood Culture Train model based on procedural and behavioural interventions to improve the quality of blood culture collection.

OBJECTIVE: This study evaluated whether a multi-modal intervention based on the 'Blood Culture Train' (BCT) framework could enhance blood culture collection compliance and diagnostic yield in adult inpatients. METHODS: We conducted a single-centre, before-and-after quasi-experimental study that compared key collection indicators in 2023 (before the intervention) and 2024 (after the intervention). The intervention consisted of a Clinical and Laboratory Standards Institute M47-A-compliant standardized workflow, blockchain-enabled traceability, virtual reality-based simulation training and a clo

Does This Patient Need Blood Cultures? A Scoping Review of Indications for Blood Cultures in Adult Nonneutropenic Inpatients.

Guidance regarding indications for initial or follow-up blood cultures is limited. We conducted a scoping review of articles published between January 2004 and June 2019 that reported the yield of blood cultures and/or their impact in the clinical management of fever and common infectious syndromes in nonneutropenic adult inpatients. A total of 2893 articles were screened; 50 were included. Based on the reported incidence of bacteremia, syndromes were categorized into low, moderate, and high pretest probability of bacteremia. Routine blood cultures are recommended in syndromes with a high like

Comparison of conventional culture and automated blood culture system for microbiologic diagnosis of pleural infection.

Microbiological diagnosis of pleural infection is often hindered by the low sensitivity of conventional culture. The automated blood culture system (ABCS) has been shown to improve diagnostic sensitivity for the culture of non-blood specimens. This study examined the additional diagnostic benefit of ABCS against conventional bacterial culture in patients who underwent thoracentesis or percutaneous drainage. Non-duplicate patients whose pleural fluid samples were tested using both conventional culture and ABCS (BACT/ALERT 3D and VIRTUO, bioMerieux) from 2001 through 2021 were included. Cases we

The impact of direct inoculation of ascites into blood culture bottles on ascites culture positivity.

OBJECTIVE: Assess whether direct inoculation of ascites into blood culture bottles would improve ascites culture yield. DESIGN: Pre-post-study. SETTING: The study was performed at a quaternary academic medical center in Houston, Texas, including all inpatient and emergency department encounters. PATIENTS: Ascites cultures collected from November 2020 to December 2022 were reviewed and screened for spontaneous bacterial peritonitis. Patients were excluded if a prior ascites culture from the same patient was already included in the study or if there was evidence of secondary bacterial periton

Detection of bacteraemia in children seen in the outpatient department: a comparison of conventional blood culture methods and a Castaneda blood culture.

Modified Castaneda blood culture bottles were used to diagnose bacteraemia in children attending the out patient clinic. Bacterial growth was detected in twelve out of 147 patients (8%), in both the routine and Castaneda blood culture bottles. Streptococcus pneumoniae was recovered in nine patients (6%), and H. influenzae in three patients (2%). The average length of time required to identify the organisms utilizing the routine blood culture bottles was 2 days (range 1 to 4 days), while the average time utilizing Castaneda bottles was 3.5 days (range 1 to 6 days). Castaneda blood bottles were

Larger Blood Volume Increases Detection of Fastidious Mycobacteria and Fungi in Blood Culture.

Blood volume impacts the sensitivity of blood culture for diagnosis of bloodstream infections caused by nonfastidious organisms, but its effect on recovery of fastidious organisms is poorly understood. Collection of 2 Myco/F Lytic bottles, 5 mL of blood each, increased the recovery of fastidious mycobacteria and fungi by 24.1% (7 of 29).

Isolation of Legionella pneumophila serogroup 1 from blood with nonsupplemented blood culture bottles.

Legionella pneumophila serogroup 1 was isolated from antemortem aerobic blood culture bottles not supplemented with L-cysteine or ferric pyrophosphate and from the postmortem lung tissue of a 72-year-old man. We recommend that aerobic blood culture bottles (Johnston Laboratories, Cockeysville, Md.) of BACTEC be subcultured to an agar that supports the growth of L. pneumophila when growth indexes range from 30 to 60 but fail to increase further.

Blood cultures in the evaluation of uncomplicated cellulitis.

PURPOSE: The frequency of bacteremia and the array of microorganisms involved in cellulitis vary greatly among studies. Although current guidelines do not recommend routine blood culture in uncomplicated cellulitis, their implementation in clinical practice remains challenging. We therefore aimed to assess the frequency, determinants and microbiology of bacteremia in hospitalized patients with uncomplicated cellulitis. METHODS: We retrospectively reviewed the medical records of all adult patients admitted at a primary-care hospital with a diagnosis of community-acquired uncomplicated cellulit

Value of anaerobic blood cultures in pediatrics.

The aim of the study presented here was to evaluate the utility of anaerobically incubated blood cultures for detecting infections in pediatric patients. During a 2-year period 9,165 pediatric blood samples were processed, and significant microorganisms were recovered from 497 (5.4%) of them. Only two of the microorganisms isolated were strictly anaerobic. Of the total isolates, 13% were detected in anaerobic bottles solely. Considering that the quantity of blood available from pediatric patients for blood cultures is usually small, it may be reasonable to limit the use of anaerobic blood cult

Blood cultures: clinical aspects and controversies.

During the past two decades, there have been important advances in blood culture methodology. These advances have resulted in earlier detection and identification of pathogens causing bloodstream infections. However, there are many facets of the blood culture as a diagnostic test that are not affected by new culture methods or systems that continue to cause problems with interpretation of results. The objective of this review is to focus on those factors influencing the results of blood cultures that have clinical relevance. Such factors include skin preparation, timing, procurement techniques

[An evaluation of the blood culture in disseminated mycobacteriosis].

The objective of this study was to evaluate the usefulness of blood culture in the diagnosis of disseminated mycobacteria (DMB). This prospective study included all blood cultures done for patients with fever and under suspicion of having DMB between January 1991 and July 1992. Fifty-seven blood samples from 16 patients were cultured; 14 (87.5%) patients were HIV positive and all were diagnosed as having DMB. The cultures were processed by lysis-centrifugation and identification of mycobacteria was by hybridization with a DNA probe. Mycobacterial growth was detected in 5 cultures (8.7%) from 4

Blood Cultures Versus Respiratory Cultures: 2 Different Views of Pneumonia.

BACKGROUND: Choice of empiric therapy for pneumonia depends on risk for antimicrobial resistance. Models to predict resistance are derived from blood and respiratory culture results. We compared these results to understand if organisms and resistance patterns differed by site. We also compared characteristics and outcomes of patients with positive cultures by site. METHODS: We studied adult patients discharged from 177 US hospitals from July 2010 through June 2015, with principal diagnoses of pneumonia, or principal diagnoses of respiratory failure, acute respiratory distress syndrome, respir

Diagnosis of brucellosis by using blood cultures.

The performances of three blood culture systems, Hemoline performance diphasic medium (bioMérieux, Marcy l'Etoile, France), Bactec Plus Aerobic/F* (Becton Dickinson, Paramus, N.J.), and Vital Aer (bioMérieux), were compared for the diagnosis of 17 cases of brucellosis. By using a 5-day incubation protocol, positive results were 52.9, 82.4, and 11.8%, respectively. When the protocol was extended to 7 days, the results were 76.5, 94.1, and 47.1%, respectively. Bactec was the fastest system (P < 0.05).

Assessment of the yield of anaerobic blood cultures.

Because of the declining incidence of anaerobic bacteremia, the predictable sites of anaerobic infection and the increasing importance of aerobic isolates (eg; yeasts), the practice of routinely culturing half the volume of blood collected anaerobically has been questioned. We have assessed the yield of routine anaerobic blood cultures in our clinical setting. Blood culture isolates from November 1994 through October 1995 at Auckland (AKH) and Green Lane/National Women's Hospitals (GL/NWH) were recorded. The medical records of patients with anaerobic bacteremia were examined. For the three mon
