Retention of carbon dioxide in tissue following carbonic anhydrase inhibition in dogs.

To evaluate the retention of carbon dioxide in tissue during the reduction of carbonic anhydrase activity following the administration of 5, 10, 20, or 30 mg/kg of acetazolamide in dogs, we measured carbon dioxide pressure (PCO2) in arterial blood, mixed venous blood, alveoli, and tissue. Respiration was maintained at a constant level. In the control (noninjected) group, PCO2 in tissue did not change for 3 hours under controlled respiration. Following the injection of 5 to 30 mg/kg of acetazolamide, PCO2 increased in arterial blood, mixed venous blood, and tissue in a dose-related manner, and

Effects of heavy hyperventilation on transcutaneous and arterial oxygen tensions in healthy adults.

Transcutaneous pO2 on thorax and the volar part of the forearm was measured in six healthy volunteers during heavy hyperventilation, and the values compared with simultaneously measured arterial blood pO2. We found a significant rise in arterial pO2 during hyperventilation with a lesser increase in thoracic transcutaneous pO2. When measured on arm the transcutaneous pO2 did not even rise significantly. After hyperventilation pO2 fell to values below the resting level. The transcutaneous/arterial pO2 index, fell significantly during hyperventilation with the greatest reduction in the arm index.

Validation of Continuous Transcutaneous Carbon Dioxide Monitoring in Neonates Undergoing Therapeutic Hypothermia for Neonatal Encephalopathy.

OBJECTIVE: To assess the accuracy of transcutaneous carbon dioxide (tcPCO2) monitoring compared with intermittently sampled blood gas PCO2 in neonates undergoing therapeutic hypothermia (TH). STUDY DESIGN: Neonates ≥34 weeks of gestation undergoing TH were enrolled and subsequently monitored with continuous tcPCO2. Agreement between tcPCO2 and both temperature-corrected and measured (uncorrected) blood gas PCO2 from arterial, venous, and capillary sources was assessed using Bland-Altman analyses that accounted for paired repeated measures. RESULTS: One hundred thirty paired tcPCO2-PCO2 measu

Why critical limb ischemia criteria are not applicable to diabetic foot and what the consequences are.

Neuropathy, peripheral arterial occlusive disease and microvascular disturbances are important factors contributing to foot problems in diabetic patients. In the diabetic foot with ischemia, the alterations in skin microvascular function are pronounced including severely reduced capillary circulation and abolished hyperaemic responses. These microvascular disturbances, which are superimposed on the already existing structural diabetic microangiopathy, are compatible with a state of "chronic capillary ischemia" and an increased shunting of blood through arteriovenous channels. This maldistribut

Increase in cerebrospinal fluid pressure in normocapnic volunteers in response to nalbuphine.

We have carried out a double-blind randomized study of the effect of nalbuphine (0.2 mg.kg-1 i.v.) or placebo on mean lumbar cerebrospinal fluid (CSF) pressure, mean cerebral perfusion pressure (CPP), transcutaneous PCO2 (tcPCO2), mean arterial blood pressure (MAP), and heart rate (HR) in 10 spontaneously breathing volunteers using invasive CSF pressure measurement. Nalbuphine increased CSF pressure from 9.2 mmHg to 16.4 mmHg and decreased CPP from 83.6 mmHg to 74.4 mmHg without significantly changing tcPCO2, MAP, or heart rate. In the placebo group there were no significant changes in CSF pre

The Precision Between Transcutaneous Carbon Dioxide Versus PaCO2 in Infants Undergoing Therapeutic Hypothermia.

BACKGROUND: Infants with hypoxic-ischemic encephalopathy are often treated with therapeutic hypothermia and high-frequency ventilation. Fluctuations in PaCO2 during therapeutic hypothermia are associated with poor neurodevelopmental outcomes. Transcutaneous CO2 monitors offer a noninvasive estimate of PaCO2 represented by transcutaneously measured partial pressure of carbon dioxide (PtcCO2 ). We aimed to assess the precision between PtcCO2 and PaCO2 values in neonates undergoing therapeutic hypothermia. METHODS: This was a retrospective chart review of 10 neonates who underwent therapeutic hy

The effect of topical nitroglycerin on transcutaneous oxygen.

The use of topical nitroglycerin has been reported to aid the healing of chronic skin ulcers. In this study, the effect of 2% nitroglycerin ointment on transcutaneous oxygen pressure (TcPO2) was investigated in skin adjacent to the area of ointment application. Topical nitroglycerin caused a two-fivefold decrease in TcPO2 in skin next to or up to 9 cm away from the site of application of the ointment.

Selection of lower limb amputation level not aided by transcutaneous pO2 measurements.

The transcutaneous oxygen pressure measurements were evaluated as supplementary ones for predicting stump healing in 58 below-the-knee and 16 above-the-knee amputations; the lower level was selected if the skin perfusion pressure was greater than 30 mm Hg below the knee. The failure rates in below-the-knee and above-the-knee amputations were 17 and 25 percent, respectively, and unrelated to the transcutaneous oxygen pressure measured at the amputation level. We conclude that no further information is acquired by measuring transcutaneous oxygen pressure.

Nasopharyngeal oxygen in children.

Hypoxia caused by pneumonia or bronchiolitis is a common cause of death in children in developing countries. Oxygen is very expensive in developing countries, and it is important that the limited supplies available be used as efficiently as possible. This study evaluated the administration of oxygen through an 8 FG catheter inserted into the nose to a depth equal to the distance from the side of the nose to the front of the ear, so that the tip of the catheter was just visible in the pharynx below the soft palate. A flow rate of 150 ml/kg/min gave an inspired oxygen concentration of about 50%

Validation of transcutaneous CO2 measurement in awake children undergoing sleep studies.

Despite the reliance on transcutaneous CO2 measurement (PtcCO2) for continuous alveolar ventilation monitoring in pediatric sleep studies, the agreement between PtcCO2 and arterial CO2 pressure in hemodynamically stable children has not been assessed. To address this gap, we conducted a retrospective study to compare PtcCO2 and arterialized capillary CO2 pressure (PcapCO2) values obtained upon awakening and to identify factors influencing any differences. We retrospectively analysed 335 pediatric records where each patient underwent both earlobe arterialized capillary blood gas measurement and

A 6-Month-Old Infant With Different Capnography Values in Polysomnography.

A 6-month-old infant with a past medical history of hypoxic ischemic encephalopathy was referred for evaluation of snoring. She was born at 41 weeks' gestational age to a 25-year-old gravida 1, para 1 mother via vacuum-assisted delivery due to cardiac decelerations. The infant's Apgar scores were 1, 4, and 6 with nuchal cord and meconium at delivery. She was started on positive-pressure ventilation but eventually required intubation at approximately 40 minutes of life. Brain MRI showed abnormal areas of restricted diffusion, involving the corpus callosum, bilateral posterior limb of the intern

Application of exercise transcutaneous oxygen pressure measurements for detection of proximal lower extremity arterial disease: a case report.

Proximal claudication is secondary to ischemia caused by peripheral artery disease (PAD), whereas proximal pseudo-claudication is secondary to other disease processes such as hip arthritis, spinal stenosis, neuropathy, and so forth. The differentiation between the two can be challenging. Exercise transcutaneous oxygen pressure measurement (exercise-TcPO2) allows noninvasive detection of flow-reducing lesions in the proximal arteries and tributaries of the lower extremity arterial tree. We present the first case report in the United States using an exercise-TcPO2 algorithm. A 71-year-old diabet

Effects of a single dose of terlipressin on transcutaneous oxygen pressures.

OBJECTIVE: Terlipressin (TP) is a potent vasoconstrictor, which is widely used in the treatment of bleeding esophageal varices and the hepatorenal syndrome. Side effects to TP are often related to skin hypoxaemia. The aim of the study was to investigate the transcutaneous oxygen pressures (TcPO(2) mmHg) after administration of 2 mg of TP. PATIENTS AND METHODS: Nineteen patients with cirrhosis and ascites were included. TcPO(2) mmHg were measured continuously measured at the chest, abdominal wall and at the lower extremity at baseline and after 2 mg TP in 15 patients and placebo in 4 patients.

Hyperventilation in patients with recurrent functional symptoms.

In view of the similarity between the reported effects of hyperventilation and recurrent functional symptom presented in primary care, a study was undertaken to establish whether such symptoms are attributable to hyperventilation. Twenty patients with two or more recurrent functional symptoms which their doctors found difficult to diagnose or treat, and 30 controls, were studied using symptom questionnaires and a series of hyperventilation provocation tests during which the partial pressure of carbon dioxide (PCO2) and symptoms were recorded. Sixteen cases (80%) had unexplained breathlessness

The use of transcutaneous oximetry in the noninvasive vascular laboratory.

Transcutaneous oximetry is gaining worldwide acceptance as a simple and effective means of evaluating the cutaneous circulation. Oximetry involves the use of Clark-type electrically heated oxygen-sensing electrodes that are attached to the skin; various protocols for the performance of studies (utilizing exercise, oxygen inhalation, leg elevation, and various other maneuvers) have been developed which may improve the accuracy when used in certain settings. Applications for transcutaneous oximetry are found in areas such as: (1) diagnosis of disease, (2) quantification of disease severity, (3)

Analysis of the tcPO2 response to blood interruption in infants and adults.

The transcutaneous PO2 (tcPO2) response to blood interruption (BIS test) was measured in 6 healthy adults and 28 infants, including premature infants. The response was similar in both infants and adults with a time lag at the beginning of blood interruption, a subsequent linear decrease, a time lag at the end of blood interruption, and an exponential recovery to the final steady state. The linear decrease of tcPO2 in the BIS test had a correlation coefficient of more than 0.98 in all subjects and indicated the cutaneous oxygen consumption rate independent of the oxygen pressure. There was a si

The use of infrared laser therapy in the treatment of venous ulceration.

Management of intractable venous ulceration remains an unrewarding task which is increasingly delegated to the realm of the vascular surgeon. The purpose of this pilot study was to assess the ulcer-healing effects of the newest form of biostimulation--the low power laser. Twelve patients with chronic venous ulcers unresponsive to conservative measures were treated with infrared laser irradiation for twelve weeks. Two ulcers healed completely and there was a 27% (p less than 0.01) reduction in size of the remaining ulcers. Treatment resulted in a 44% (p less than 0.01) increase in ulcer floor a

[Blood gas changes during treadmill exercise in convalescents from pulmonary tuberculosis. 1. Method of measurement].

With advances in treatment for tuberculosis, pulmonary tuberculosis has become curable. However, chronic respiratory failure has been more often observed in those who have ever had thoracic operation, and were cured with anti-tuberculous chemotherapy in spite of initially severe pulmonary tuberculosis with wide spread lesions in both lungs, particularly in the aged. Therefore it is now necessary to understand accurately the real physio-dynamic situation of respiro-circulatory impairment in these patients. For this purpose, an exercise test was newly designed, and changing curves of blood gas (

Prediction of arterial blood gases by transcutaneous O2 and CO2 in critically ill hyperdynamic trauma patients.

The management of severe adult respiratory distress syndrome in critically injured patients requires the frequent measurement of arterial blood gases for adjustment of cardiovascular and ventilatory support. Since these require blood withdrawal and laboratory determinations, a noninvasive method of assessment of arterial gas tensions would permit more frequent assessment of the patient as well as permitting rapid changes in the patient's ventilatory status to be detected earlier in the clinical course. The role of transcutaneous O2 and CO2 tension in providing these measurements was evaluated

Transcutaneous Carbon Dioxide Monitoring During Apnea Testing for Determination of Neurologic Death in Children: A Retrospective Case Series.

OBJECTIVES: Determination of neurologic death in children is a clinical diagnosis based on absence of neurologic function with irreversible coma and apnea. Apnea testing during determination of neurologic death assesses spontaneous respiration when PaCO2 increases to greater than or equal to 60 and greater than or equal to 20 mm Hg above pre-apneic baseline. The utility of transcutaneous carbon dioxide measurements during apnea testing in children is unknown. We seek to determine the degree of correlation between paired transcutaneous carbon dioxide and PaCO2 values during apnea testing for de
