Home blood pressure monitoring: advantages and limitations.

Home blood pressure monitoring is a useful tool for clinical management of patients with hypertension. Its major advantages are the ease with which the techniques can be learned, reproducibility of values, sensitivity of measurement and availability of normotensive data. In spite of the ability to tell whether a subject has normal or abnormal values, because of the lack of prospective mortality/morbidity data, home blood pressure monitoring cannot be used to decide whether treatment is indicated. The treatment decision must be based on repeated clinic blood pressure readings. After that the ho

Ambulatory blood pressure monitoring in hypertensive adolescents.

The purpose of this study was to determine the ability of ambulatory blood pressure monitoring to identify youths with chronic blood pressure elevation. Nineteen adolescent boys were studied, ten had 5-year average systolic or diastolic pressures above the 95th percentile, nine had normal pressure. A Del Mar Avionics Pressurometer III system recorded an average of 121 readings on each subject. The coefficients of variation for pressure were similar for hypertensive and normotensive individuals. During classes, eight of the ten hypertensive youths had elevated pressures in over half of the meas

Clinical use of ambulatory blood pressure monitoring.

This paper describes the alerting reaction and blood pressure rise induced in the patient when blood pressure measurement is taken by the doctor, emphasizing that this may be responsible for important errors in estimating the real untreated or treated blood pressure in hypertension. It then examines the advantages offered by ambulatory blood pressure monitoring. It is shown that when performed intra-arterially this approach may be useful in the diagnosis of pheochromocytoma. However, its wider use as a means to better discriminate between normotensive and borderline or mild hypertensive subjec

Ambulatory blood pressure monitoring: practical considerations.

Ambulatory blood pressure monitoring (ABPM) allows one to evaluate the blood pressure (BP) profile over a 24-hour period in the patient's natural environment. Casual pressure measurements in the physician's office can be affected by alarm reactions, thus causing "white coat" hypertension. ABPM allows one to evaluate these reactions and determine the average pressure and variability of BP along with the effects of physical activity and emotional arousal on BP patterns while at work, at home, and during sleep. Average pressures determined by ABPM are more predictive of target organ involvement a

Indirect blood pressure monitoring in the postpartum patient.

Direct intra-arterial blood pressure (BP) measurements were compared with simultaneous standard cuff sphygmomanometry and automated inflatable cuff manometry in 12 postpartum patients. An adjustable arterial damping device was used to correct overestimations of systolic values due to distorted pressure waveforms. No significant differences in diastolic pressure with and without arterial damping were noted between noninvasive techniques and direct intra-arterial values. The systolic readings obtained with the standard cuff were significantly lower than the intra-arterial values with or without

Remote Blood Pressure Monitoring: A Comprehensive Review.

BACKGROUND: Remote blood pressure (BP) monitoring, or blood pressure measurement outside of a clinical setting, has been an evolving approach to monitoring BP outside of a clinical setting, allowing for a better understanding of day-to-day fluctuations in BP. This technology at home allows for the detection of instantaneous fluctuations in BP that could be predictive of cardiovascular disease events. It also holds the potential for a more accurate diagnosis of hypertension and improved titration of antihypertensive medications. METHODS: We performed a narrative review to describe the current

Patient Engagement With Home Blood Pressure Monitoring.

IMPORTANCE: Home blood pressure monitoring (HBPM) is essential and universally recommended for hypertension management, but patterns of real-world patient engagement with HBPM have not been studied and remain largely unknown. OBJECTIVE: To evaluate patient engagement with HBPM in a remote hypertension management program. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study analyzing prospectively collected program data was conducted within a remote hypertension management program at a large academic health care system, Mass General Brigham, in Boston, Massachusetts. Data were c

Blood pressure monitoring is key in aortic dissection.

Blood pressure (BP) control is essential for both the prevention and long-term management of aortic dissection. While office BP monitoring remains the most widely used method, its limitations in detecting BP variability, masked hypertension, and nocturnal hypertension highlight the need for alternative approaches. Ambulatory BP monitoring and home BP monitoring offer superior prognostic value, enabling more precise BP assessment and treatment optimization. However, challenges such as accessibility, patient compliance, and integration into clinical workflows persist. Digital health solutions, i

Home Blood Pressure Monitoring And Nocturia In Adults.

Although widely viewed as a urological condition, nocturia has been increasingly recognized to accompany various non-urological conditions such as hypertension and blood pressure (BP) elevation on office determination. Home BP monitoring (HBPM) has been shown superior to office-based readings and provides an opportunity to assess potential relationships between nocturia and novel indices derived from multiple BP recordings including BP load, BP variability, and arterial stiffness, which have prognostic significance. We retrospectively studied 103 home BP logs and nocturia frequencies provided

Cuffless Blood Pressure Monitoring: Promises and Challenges.

Current BP measurements are on the basis of traditional BP cuff approaches. Ambulatory BP monitoring, at 15- to 30-minute intervals usually over 24 hours, provides sufficiently continuous readings that are superior to the office-based snapshot, but this system is not suitable for frequent repeated use. A true continuous BP measurement that could collect BP passively and frequently would require a cuffless method that could be worn by the patient, with the data stored electronically much the same way that heart rate and heart rhythm are already done routinely. Ideally, BP should be measured con

Home blood pressure monitoring in the 21st century.

Home blood pressure monitoring provides multiple measurements in the usual environment of each individual, allows the detection of intermediate hypertension phenotypes (white-coat and masked hypertension), and appears to have superior prognostic value compared to the conventional office blood pressure measurements. Accumulating evidence suggests that home blood pressure monitoring improves long-term hypertension control rates. Moreover, it is widely available, relatively inexpensive, and well accepted by patients. Thus, current guidelines recommend home blood pressure monitoring as an essentia

Ambulatory blood pressure monitoring in primary care.

The accurate assessment and appropriate management of blood pressure (BP) is critical for primary care physicians. Traditional office-based BP measurement has limitations that can be addressed, in part, through the use of ambulatory BP monitoring (ABPM). Because BP readings are provided at specific time intervals throughout a 24-hour period, ABPM represents a better picture of the normal fluctuations in BP levels associated with daily activities, including sleep. Blood pressure values obtained by 24-hour ABPM are a better predictor of cardiovascular risk than office-based BP measurements, and

Ambulatory blood pressure monitoring in clinical practice.

Ambulatory blood pressure (ABP) monitoring has become increasingly more available in routine clinical practice in Canada. The ABP is more reliable and more reproducible than office readings, and is a better predictor of target organ damage. Normal values for ABP have been established using both cross-sectional and longitudinal outcome data. Abnormal mean 24 h and awake ABP values should exceed 135/85 mmHg and 140/90 mmHg, respectively. ABP recordings are useful in making a diagnosis of hypertension by identifying people with high office but normal ABP values. ABP monitoring can also be perform

Ambulatory blood pressure monitoring: which arm?

To determine the effects of routinely selecting the nondominant arm for ambulatory blood pressure monitoring (ABPM) on estimates of patients' blood pressure (BP) and to evaluate the practise of using manual BP from one arm and ambulatory BP from the other on the estimation of white coat effect (WCE), an observational study was conducted in 10 volunteers, exhibiting an interarm resting clinic systolic BP (SBP) difference > or =10 mm Hg. The main outcome measures were: (i) average ambulatory SBP measured on right and left arm simultaneously during 24 h, and (ii) estimate of WCE derived, by curre

Clinical value of ambulatory blood pressure monitoring.

UNLABELLED: Ambulatory blood pressure monitoring (ABPM) has now become an established clinical tool. It is appropriate to take stock and assess the situation of this technique. UPDATE ON EQUIPMENT: Important improvements in equipment have occurred, with reductions in weight, in awkwardness and in noisiness of the machines, better acceptability and tolerance by the patients, and better reliability. Validation programmes have been proposed and should be referred to. Limitations of the technique persist with intermittent recording in current practice. The reproducibility is limited in the short-t

Orthostatic hypotension and ambulatory blood pressure monitoring.

Orthostatic hypotension has a number of causes, many of which are easily reversible. However, characteristics such as standing hypotension in conjunction with supine hypertension are not always easily detected in the clinic or office setting. Ambulatory blood pressure monitoring may be a valuable technique for diagnosis and for the assessment of therapeutic effects in patients with orthostatic hypotension.

Clinical uses of ambulatory blood pressure monitoring.

In the past, the diagnosis and management of hypertension has been based on office blood pressure. However, office blood pressure is not always a true reflection of a patient's blood pressure profile. Since ambulatory blood pressure monitoring permits a large number of readings to be taken in the patient's usual environment, it may provide a more representative blood pressure profile. Indeed, ambulatory blood pressure has been better correlated than office blood pressure with the target-organ complications of hypertension. Office or white-coat hypertension (elevated blood pressure only when me

Self-monitoring of blood pressure during pregnancy.

Instruction in self-determination of blood pressure offers a means for managing a pregnant woman who has chronic hypertension. This approach offers patient participation and reinforces compliance to bed rest. If further identifies the anxiety and emotions involved in a visit to the physician's office, since the blood pressure readings are significantly lower when taken by the patient elsewhere. Furthermore, it offers an effective tool to monitor the effect of antihypertensive drugs on blood pressure in a more accurate manner.

Accuracy and 'range of uncertainty' of oscillometric blood pressure monitors around the upper arm and the wrist.

BACKGROUND: Although they have been marketed widely, few data about the diagnostic accuracy of blood pressure monitors are available. METHODS: Repeated measurements of blood pressures in 85 patients were performed in random sequence with two oscillometric blood pressure monitors around the upper arm (Visomat OZ2) and the wrist (Omron R3( and with a standard sphygmomanometer. The oscillometric blood pressure monitors were validated according to protocols of the British Hypertension Society (BHS) and the American Association for the Advancement of Medical Instrumentation (AAMI). Factors confounding assessment of ambulatory blood pressure monitors, studied during formal evaluation of the Tycos Quiet-Trak.

The credibility of studies assessing ambulatory blood pressure monitoring (ABPM) devices will be enhanced by minimizing opportunities to manipulate the outcome through biased selection of patient volunteers, and by insuring that participants are representative of the target patient population. While subjecting the Welch Allyn Quiet-Trak to the British Hypertension Society (BHS) protocol, we examined the extent to which certain candidate characteristics might influence device assessment. The Quiet-Trak achieved an A grade overall. During its field testing, daytime physical activity measured wit
