African Mole-Rats May Have High Bone Conduction Sensitivity to Counterbalance Low Air Conduction Sensitivity.

Background/Objectives: Subterranean mole-rats live in an intricate system of underground tunnels, a unique acoustic environment that has led to adaptations to their hearing. Most experimenters have concluded that mole-rats have poor hearing thresholds, perhaps 20-40 dB less sensitive than rodents living on the surface. The potential problem identified here is that mole-rat thresholds have all been measured in air, whereas there is some evidence-theoretical and observational-to suggest that these animals may hear more sensitively via bone conduction. Methods: A wide-ranging review of the litera

Implantable bone conduction hearing device: Audiant bone conductor. Update on our experiences.

An implantable hearing device has been developed and has undergone testing by animal experimentation and clinical trials in humans. Recently, the Food and Drug Administration has approved the use of this device in adults who are within the criteria of its utilization, mainly patients with conductive hearing impairments who have good inner ear reception and who are unable to benefit from surgical correction. This update describes observations and experiences we have had during the device's development and defines its limitations as well as its unique thrust into the future relief of hearing imp

Interactions in the cochlea between air conduction and osseous and non-osseous bone conduction stimulation.

Since air-conducted (AC) and clinical (mastoid) bone-conducted (BC) sounds interact in the cochlea (e.g. pitch, cancellation, masking, beats), it has been thought that both AC and BC stimulations lead to a mechanical wave in the cochlea. However, there are also "non-osseous" forms of BC, i.e. auditory sensation produced when the clinical bone vibrator is applied to "non-osseous" soft tissue sites. In the present study, such "non-osseous" sites were identified (e.g. eye, cheek, neck) and they interacted with AC and osseous BC (pitch matching, beats, masking), indicating that all of these forms

Conductive hearing loss and bone conduction devices: restored binaural hearing?

An important aspect of binaural hearing is the proper detection of interaural sound level differences and interaural timing differences. Assessments of binaural hearing were made in patients with acquired unilateral conductive hearing loss (UCHL, n = 11) or congenital UCHL (n = 10) after unilateral application of a bone conduction device (BCD), and in patients with bilateral conductive or mixed hearing loss after bilateral BCD application. Benefit (bilateral versus unilateral listening) was assessed by measuring directional hearing, compensation of the acoustic head shadow, binaural summation

Vestibular activation by bone conducted sound.

OBJECTIVE: To examine the properties and potential clinical uses of myogenic potentials to bone conducted sound. METHODS: Myogenic potentials were recorded from normal volunteers, using bone conducted tone bursts of 7 ms duration and 250-2000 Hz frequencies delivered over the mastoid processes by a B 71 clinical bone vibrator. Biphasic positive-negative (p1n1) responses were recorded from both sternocleidomastoid (SCM) muscles using averaged unrectified EMG. The best location for stimulus delivery, optimum stimulus frequency, stimulus thresholds, and the effect of aging on evoked response amp

Preoperative bone-conducted electrocochleography in otosclerosis.

Prior to surgery, pure-tone audiometry and bone-conducted (BC) electrocochleography (ECOG) were performed on 17 patients with preoperatively diagnosed otosclerosis. Short tone bursts, approximately 5 ms in duration and presented by means of a bone conductor on the exposed surface of the mastoid, were used as stimuli in BC ECOG. The thresholds thus obtained were compared by regression analysis with conventional psychoacoustic thresholds. Thresholds recorded at BC ECOG showed a close correlation to preoperative pure-tone BC thresholds, and there was no difference in predictability of postoperati

[A hearing aid anchored in the cranial bone for amplification of bone conduction].

The purpose of this study was to compare hearing and understanding with a bone conduction hearing aid of a new type and with conventional models. The new instrument, BAHA (bone anchored hearing aid) is connected with the skull percutaneously rather than transcutaneously as with the conventional instruments. In the ENT department of Nijmegen University Hospital the understanding of speech with both types of hearing aid was evaluated thoroughly. The patients had a middle-ear loss with in addition an inner-ear loss of 60 dBHL at most. A conventional air conduction hearing aid (behind or inside th

Unilateral crosstalk cancellation via bone conduction: Methods and evaluation.

Bone conduction hearing aids (BCHAs) offer an alternative solution for individuals with outer or middle ear issues who cannot benefit from traditional air conduction hearing aids. However, the phenomenon of "crosstalk," where sound intended for one ear is mistakenly transmitted to the other ear through bone conduction, presents a challenge. This unintended transmission may limit the benefits of binaural hearing that can be achieved using two BCHAs, such as accurately detecting a sound source's direction. In this article, we present a method to suppress "crosstalk" within the human head using a

Three-Year Follow-Up with the Bone Conduction Implant.

BACKGROUND: The bone conduction implant (BCI) is an active transcutaneous bone conduction device where the transducer has direct contact to the bone, and the skin is intact. Sixteen patients have been implanted with the BCI with a planned follow-up of 5 years. This study reports on hearing, quality of life, and objective measures up to 36 months of follow-up in 10 patients. METHOD: Repeated measures were performed at fitting and after 1, 3, 6, 12, and 36 months including sound field warble tone thresholds, speech recognition thresholds in quiet, speech recognition score in noise, and speech-t

Influence of transducer types on bone conduction hearing thresholds.

OBJECTIVE: Different types of bone conduction transducers with different physical and electro-acoustic properties are available for audiometric hearing threshold measurements. The reference equivalent threshold vibratory force levels (RETVFL) specified in ISO 389-3 are based on measurements conducted with the B71 and KH70 transducers but apply to all types of transducers available for bone conduction audiometry. The objective of this study was to compare bone conduction hearing thresholds measured by different transducers. DESIGN: In a prospective study the hearing thresholds were measured ps

Determining fitting ranges of various bone conduction hearing aids.

OBJECTIVES: To define fitting ranges for nine bone conduction devices (BCDs) over different frequencies based on the device's maximum power output (MPO) and to validate the assessment of MPO of BCDs in the ear canal. BACKGROUND: Maximum power output (MPO) is an important characteristic when fitting BCDs. It is the highest output level a device can deliver and is one of the major determinants of a device's fitting range. A skull simulator can be used to verify MPO of percutaneous BCDs. No such simulator is available for active and passive transcutaneous devices. DESIGN: The MPO of nine differ

Behavioral bone-conduction thresholds for infants with normal hearing.

BACKGROUND: Bone-conduction thresholds have been used in audiologic assessments of both infants and adults to differentiate between conductive and sensorineural hearing losses. However, air- and bone-conduction thresholds estimated for infants with normal hearing using physiological measures have identified an "air-bone gap" in the low frequencies that does not result from conductive hearing impairment but, rather, from maturational differences in sensitivity. This maturational air-bone gap appears to be present up to at least 2 yr of age. Because most infants older than 6 mo of age are clinic
