Percutaneous transphyseal screw induces varus proximal femoral growth modulation in a growing pig model-A three-dimensional analysis.

Bone and joint angular deformities are common pediatric orthopedic problems that are often corrected surgically. Previous studies successfully demonstrated proximal femoral growth modulation in animal models, however outcome measurements were limited by two-dimensional analysis. In this study, six Yorkshire cross male piglets were treated with unilateral percutaneous transphyseal screw placement across the medial aspect of the proximal femoral physis and analyzed using three-dimensional (3D) techniques. Three primary outcome measures were considered-physis angle, version, and femoral length. C

Chronic anterior dislocation of the shoulder.

The treatment of 17 chronic, unreduced anterior dislocations of the shoulder was reviewed. Eleven women and six men with on average age of 67 years (range 36 to 88 years) were studied. The duration of dislocation averaged 2.3 years (8 weeks to 8 years). Seven patients were treated without surgery despite severe functional deficits, for reasons of health or motivation. Ten were treated with surgery. One patient with preserved joint surfaces underwent open reduction and corticoid transfer to bone graft an eroded anterior glenoid. Nine patients with destroyed articular surfaces underwent unconstr

Primary stability of custom and anatomical uncemented femoral stems: a method for three-dimensional in vitro measurement of implant stability.

BACKGROUND: Lack of primary stability of cementless hip stems prevents bone ingrowth and may lead to loosening of the stem. Direct measures of the implant stability require drilled holes in the bone at the measuring site. These holes weaken the cortical bone, limit the number of possible measuring points and inhibit other biomechanical measurements. This in vitro study aimed to develop a method for indirect measurement of primary stability of femoral stems, leaving the specimen intact. The method was used to compare the primary stability of two uncemented femoral stems with different proximal

The relevance of the radiological signs of acetabular retroversion among patients with femoroacetabular impingement.

Orthopaedic surgeons have accepted various radiological signs to be representative of acetabular retroversion, which is the main characteristic of focal over-coverage in patients with femoroacetabular impingement (FAI). Using a validated method for radiological analysis, we assessed the relevance of these signs to predict intra-articular lesions in 93 patients undergoing surgery for FAI. A logistic regression model to predict chondral damage showed that an acetabular retroversion index (ARI) > 20%, a derivative of the well-known cross-over sign, was an independent predictor (p = 0.036). Howeve

A new device to treat intra-capsular fracture neck of femur non-union.

In adolescents and young adults, femoral neck fractures often result from high-velocity trauma. These fractures are usually of vertical shear pattern. There is an increased incidence of avascular necrosis and non-union, which is difficult to treat. Non-union of fractured neck of femur in young adults is a serious problem. There is growing evidence that these fractures should be treated with an angle-stable device to improve biomechanics at the fracture site. An ideal implant should prevent varus deformation and retroversion of the fracture in order to prevent failure of the osteosynthesis and

Paget-Schroetter syndrome in sports activities--case study and literature review.

The authors report 7 patients with thromboses in the upper extremity resembling Paget-Schroetter syndrome. According to their case histories, all patients had a temporal and causal relationship between partially unusual sports activities and the genesis of the thrombosis. The cause of this condition is a strain on the subclavian and axillary veins by retroversion or hyperabduction of the arm. This can entail microtraumatizations of the venous intima, consequently leading to a consecutive local activation of coagulation and to a possible thrombosis of the vessel. A mechanical compression of the

[Uncertainty in the radiological evaluation of deformity in proximal humerus fractures].

INTRODUCTION: The accurate measurement of the deformity of proximal humerus fractures is essential for a proper treatment, particularly in the growing bone. Due to the local pain the correct projection in standard X-rays is difficult to achieve and, in contrast to other joints, cannot be verified in the X-ray. Even with the correct projections a mismeasurement can occur when the rotation is 45° to both planes. The aim of this study was to objectify the error sources and reveal starting points for an improvement. MATERIAL AND METHODS: In a three-step study we initially developed a mathemathica

Southwick osteotomy stabilised with external fixator.

INTRODUCTION: Epiphysiolysis of the femoral head is the most common accident occurring towards the end of pre-puberty and puberty growth. CASE REPORT: The author describes the experience in the treatment of chronic epiphysiolysis in two patients treated by Southwick osteotomy. The site is accessed by way of a 15-cm long lateral skin incision and the trochanteric region is reached through the layers. The osteotomy angles prepared beforehand on a thin aluminium model are used to mark the Southwick osteotomy site on the anterior and lateral sides at the level of the lesser trochanter. Before per

[Reconstruction of an iatrogenic acromial pseudarthrosis: a case report].

Acromial fractures are rare but severe complications which can occur during subacromial decompression. We report a case of acromial pseudarthrosis which was discovered belatedly due to persistent pain after several operations. The pseudarthrosis was successfully treated by osteosynthesis with a distal radius plate and implantation of a monocortical bone graft from the iliac crest. Two years after surgery, the fracture has healed and the patient's pain improved significantly. In the constant score the patient achieved postoperatively 58 points compared to 25 points before surgery and 65 points

Glenoid bone loss in primary total shoulder arthroplasty: evaluation and management.

Glenohumeral osteoarthritis is the most common reason for shoulder replacement. Total shoulder arthroplasty provides reliable pain relief and restoration of function, with implant survivorship reported at 85% at 15 years. Glenoid component wear and aseptic loosening are among the most common reasons for revision. Glenoid wear characteristics have been correlated with, among other things, the degree of anatomic glenoid version correction. Anatomic glenoid reconstruction is particularly challenging in the presence of glenoid bone deficiency. Walch classified glenoid morphology into five types: t

Should 15° of valgus coronal-plane deformity be the upper limit for a total ankle arthroplasty?

AIMS: Preoperative talar valgus deformity ≥ 15° is considered a contraindication for total ankle arthroplasty (TAA). We compared operative procedures and clinical outcomes of TAA in patients with talar valgus deformity ≥ 15° and < 15°. METHODS: A matched cohort of patients similar for demographics and components used but differing in preoperative coronal-plane tibiotalar valgus deformity ≥ 15° (valgus, n = 50; 52% male, mean age 65.8 years (SD 10.3), mean body mass index (BMI) 29.4 (SD 5.2)) or < 15° (control, n = 50; 58% male, mean age 65.6 years (SD 9.8), mean BMI 28.7 (SD 4.2)), underwent

Reverse shoulder arthroplasty implant design and configuration has a significant effect on conjoint tendon impingement.

BACKGROUND: Anterior shoulder pain after reverse shoulder arthroplasty (RSA) is not uncommon and may be due to humeral impingement against the conjoint tendon during internal rotation (IR). It is unknown what effect different implant designs and configurations have on conjoint tendon impingement. The purpose of this study was to investigate the influence of several RSA implant design parameters on conjoint tendon impingement during IR. METHODS: Twelve upper extremity cadavers were dissected to visualize and digitize the path of the coracobrachialis using a tracking system. This data were tran

Preoperative planning to preserve glenoid subchondral bone in anatomical total shoulder replacement.

BACKGROUND: Glenoid loosening remains a concern in anatomical total shoulder replacement. Preoperative planning software allows optimization of the component positioning, but the target orientation remains unclear due to conflicting optimization priorities. Commonly, the component is aligned to the prescribed version and inclination that reflect the population's average anatomy. The freehand technique attempts to secure strong fixation by aiming to preserve the subchondral bone. This study compared the state of the subchondral plate after reaming and compared the results of these two technique

Arthroscopic Repair of Recurrent Posterior Shoulder Subluxation After Total Shoulder Arthroplasty: A Case Report.

CASE: A 53-year-old man presented with osteoarthritis (Walch biconcave [B2] glenoid retroversion, 22°; glenohumeral subluxation index, 65%) and a partial rupture of the supraspinatus tendon in the left shoulder. Following anatomic total joint replacement, he developed disabling recurrent posterior subluxation despite a stable prosthesis and a correctly centered glenoid head, as observed with postoperative radiography and computed tomography. In order to avoid bone loss and the complications associated with revision arthroplasty, we performed arthroscopic reefing of the posterior capsule as an

Hip Radiograph Findings in Patients Aged 40 Years and Under with Posterior Pelvic Pain.

BACKGROUND: Several sacroiliac joint (SIJ) provocative tests used to assess posterior pelvic pain involve moving and stressing the hip. It is unknown if there is a subgroup of patients with posterior pelvic pain who have underlying hip deformity that could potentially influence performance and interpretation of these tests. OBJECTIVE: To describe the prevalence of radiographic hip deformity and hip osteoarthritis in a group of adults 40 years old and under who met the clinical diagnostic criteria for treatment of posterior pelvic pain with an image guided intra-articular SIJ injection.
