Presumed corneal argyrosis from occlusive soft contact lenses: a case report.

PURPOSE: To report a case of presumed corneal argyrosis associated with sliver nitrate-coated cosmetic soft contact lens wear. METHODS: A 67-year-old woman wore silver nitrate-coated occlusive soft lenses for 17 years for the management of intractable diplopia. Slit-lamp examination revealed a diffuse blue-gray deposit that was characteristic of corneal argyrosis just anterior to Descemet membrane. Confocal microscopy, anterior segment optical coherence tomography, and noncontact specular microscopy were performed. RESULTS: Confocal microscopy showed hyperreflective granules in Bowman layer,

[Ocular histoenzymatic research on an experimental viral attack].

We sought to investigate enzyme response appearing subsequent to sub-conjunctival administration of the Coxsackie B3 virus. This virus stimulates oxidising enzymes, diaphorase and leucin aminopeptidase, dihydrofolate reductase, and adenosinetriphosphatase. The most typical enzyme changes are been in the chorion of the conjunctival mucose and the corneal parenchyma there by showing that the virus, triggers local immune defence mechanisms. The appearance of highly active Langerhans cells around Bowman membrane and corneal tissue proves that the virus injected greatly stimulates the mobilisation

Electron microscopic studies in hereditary nephritis.

A characteristic electron microscopic lesion-longitudinal splitting of the glomerular basement membranes with accumulation of dark particles-was found in some cases of hereditary nephritis, especially in Alport syndrome. A somewhat similar but less specific alteration was present in the tubular basement membranes and in the Bowman capsule. The lesions tended to exhibit a familial segregation. The possible pathogenesis of the lesions and their relation to physiologic abnormalities are briefly discussed.

[Histological study of 4 lenses from epikeratoplasty. Anatomo-clinical correlation].

Four removed kerato-lens lenticles (1 for keratoconus, 1 for aphakia, and 2 for myopia) have been studied histologically. The reasons for removal of the lenticles were poor visual acuity in spite of a satisfactory anatomic result in three cases and epithelialization of the interface in one case. The epithelium presented modifications, particularly with exoserosis of basal cells and irregularities of thickness. The Bowman membrane was irregular and interrupted in some parts. The corneal stroma had few cells; the keratocytes were observed in the periphery of the lenticle and near the interface.

Histology, histochemistry and ultrastructure of cornea of domestic pigs (Sus scrofa domesticus).

A comprehensive light and ultrastructural examination of the cornea in Domestic Pigs (Sus scrofa domesticus) revealed four distinct layers: the anterior epithelium, corneal stroma, Descemet's membrane and endothelium. Although Bowman's layer was not distinctly identified through histology, histochemical analysis indicated the presence of a rudimentary Bowman's layer, possibly vestigial from evolution. Scanning electron microscopy of the outer corneal surface unveiled two cell types, characterized by micro-projections, with light cells exhibiting shorter, thicker projections compared to dark ce

Corneal responses to eye rubbing with spectral domain optical coherence tomography.

PURPOSE: Surprisingly, a significant reduction (18.4%) in epithelial thickness was found after 15 seconds of eye rubbing measured using Holden-Payor optical pachometer reported in the literature. Hence, we aimed at studying the effects of compressive and shearing pressure associated with eye rubbing on total corneal, epithelial and Bowman's membrane thickness using spectral domain OCT (SD-OCT). METHODS: SD-OCT (Spectralis; Heidelberg Engineering) was used to acquire cross-sectional images of the cornea. Central total-corneal thickness (TCT), epithelial thickness (ET) and Bowman's membrane thi

Three-dimensional mapping of the orientation of collagen corneal lamellae in healthy and keratoconic human corneas using SHG microscopy.

Keratoconus is an eye disorder that causes the cornea to take an abnormal conical shape, thus impairing its refractive functions and causing blindness. The late diagnosis of keratoconus is among the principal reasons for corneal surgical transplantation. This pathology is characterized by a reduced corneal stiffness in the region immediately below Bowman's membrane, probably due to a different lamellar organization, as suggested by previous studies. Here, the lamellar organization in this corneal region is characterized in three dimensions by means of second-harmonic generation (SHG) microscop

Abnormalities of the extracellular matrix in keratoconus corneas.

PURPOSE: To study alterations of the extracellular matrix (ECM) and basement membrane (BM) components in human keratoconus corneas. METHODS: Fifteen normal and 13 keratoconus corneas were characterized by immunofluorescence with antibodies to 23 ECM and BM components. RESULTS: Keratoconus staining patterns for posterior nonscarred regions and Descemet's membrane were normal. We focused on three areas of keratoconus corneas: (a) nonscarred anterior corneal regions, (b) scarred anterior and posterior corneal regions, and (c) gaps in Bowman's layer. In each of these areas, consistent ECM and BM

[Chromatic Swept-Source Laser Scanning - Concept for a Cell-Resolving Confocal Laser Slit Lamp?].

BACKGROUND: The in vivo characterisation of corneal epithelial tissue morphology is of considerable importance for diagnosis, disease prognosis, and the development of a treatment strategy for ocular surface diseases. In contrast to many alternative methods, in vivo corneal confocal microscopy (CCM) not only provides a macroscopic description of the corneal tissue but also allows its visualisation with cellular resolution. However, the translation of CCM from research to clinical practice is significantly limited by the complex and still largely manual operation of available CCM systems. In ad

Intrastromal Allogeneic Inclusions for the Management of Keratoconus: A Review of Current Literature.

A new therapeutic alternative has been developed in the past 6 years to treat severe keratoconus in young patients. Those patients had only corneal transplantation as an option, but now a variety of surgical alternatives in the form of allogeneic corneal inclusions have bloomed and are becoming more popular. Although Bowman layer transplantation is the most studied technique, recent studies have described different options with very promising preliminary results. Mostly all the techniques described improve corneal curvature, visual acuity, pachymetry, contact lens tolerance, and foremost, mana

[Intracorneal selective stromal transplantation].

UNLABELLED: Deep anterior lamellar keratoplasty or penetrating keratoplasty are currently considered the optimal methods of surgical treatment of stromal dystrophies and corneal degeneration. Despite certain advantages and benefits of these methods, they also have significant limitations: involvement of superficial corneal layers in the surgery, need for suturing, development of post-keratoplasty astigmatism etc. PURPOSE: This study aimed to test and describe the new method of closed sutureless keratoplasty (intracorneal selective stromal transplantation), which was indicated in isolated dyst

Corneal elastosis. Appearance of band-like keratopathy and spheroidal degeneration.

Clinical and pathologic studies of six corneas from patients with band-like keratopathy and spheroid degeneration were performed. These included cases of noncalcific band keratopathy; in one case very early changes in Bowman membrane were demonstrated by light and electron microscopy. Two other corneas displayed more advanced degeneration that manifested clinically as irregular golden yellow plaques. One case of corneal spheroid degeneration was associated with lattice dystrophy of the cornea. The corneal deposits may be best characterized as a form of elastotic degeneration and were associate

Histology and transmission electron microscopy of the cornea in xeroderma pigmentosum type C.

Xeroderma pigmentosum is a very rare precancerous skin disease that is triggered by sunlight. It is caused by a defect in the DNA repair system and causes benign and malignant transformations. Only eye tissues that come into contact with UV light are affected, such as the lids, conjunctiva and cornea. We describe a patient who suffered from xeroderma pigmentosum type C, showing the typical skin alterations but no sign of malignancy. A perforating keratoplasty was performed on both eyes because of the dense opacity of the corneas. The corneal buttons obtained were examined by light and transmis

The immunohistochemical composition of corneal basement membrane in keratoconus.

PURPOSE: Keratoconus is a gradually progressing disease of unknown cause, characterized by central thinning, increased curvature, and finally scarring of the cornea. This causes myopia and astigmatism and the ultimate treatment is keratoplasty. We studied the composition of basement membranes (BMs) in normal, scarred and keratoconus corneas to find out possible changes specific for keratoconus. METHODS: Frozen sections of normal, scarred and keratoconus corneas were immunostained with various antibodies against basement membrane (BM) proteins and integrin beta 4. RESULTS: In the keratoconus

Increased blood viscosity as the pathogenic agent in minimal change nephrosis: a new hypothesis.

It is proposed that minimal change nephrosis (MCN) is the result of impaired glomerular blood flow to slightly increased blood viscosity or to an abnormality of an autoregulatory mechanism controlling glomerular perfusion pressure or to unusually small efferent arterioles, or to combinations of these factors. The functional response to any of such changes would be an increase in intraglomerular pressure. If the pressure exceeded a critical level albumin would be able to pass into Bowman's space after deforming the pressure-dependent thixotropic basement membrane. In response to the presence of
