Dual Brachial Plexus Block for Distal Biceps Repair: A Case Report.

A 35-year-old male presented with a right distal biceps tear. The patient was scheduled for a distal biceps repair at an ambulatory surgical center. Given the patient's body mass index of 56.68 kg/m2, the surgery center's anesthesiologist recommended the use of a dual brachial plexus regional nerve block to avoid general anesthesia. While the use of dual brachial plexus blocks has been shown to be successful for shoulder procedures, their use in the distal humerus and elbow regions has not been fully established. Under ultrasound guidance, a combination of interscalene and supraclavicular nerv

Evaluating the spread of costoclavicular brachial plexus block: an anatomical study.

BACKGROUND AND OBJECTIVES: The costoclavicular brachial plexus block is performed deep and posterior to the midpoint of the clavicle. There are limited data evaluating the spread of the costoclavicular brachial plexus block. We performed a cadaveric study to evaluate the spread of injectate after a costoclavicular brachial plexus block. METHODS: Five ultrasound-guided costoclavicular block injections were performed with 20 mL of 0.1% methylene blue. The brachial plexus and its branches were dissected from the level of C4 to the lower axilla. The extent of dye spread was recorded including spr

Ultrasound-guided supraclavicular brachial plexus block in patient with halo device.

Ultrasound guided regional blocks are on the rise, many institutes are training their staff to master this technique of regional anesthesia. Regional anesthesia in case of an emergency surgery or elective surgery can be the best choice. The case described here is an example - patient with a halo fixation device after motor vehicle accident scheduled for surgery of the extremity. The main aim of management of this case is to achieve a safe anesthesia with minimal interference of the cervical fixation. Supraclavicular brachial plexus block is a good choice for surgeries of the arm and hand and u

[Brachial plexus block for minimally invasive parathyroidectomy: report of 3 cases].

The brachial plexus block, either deep or superficial, is one of the anesthetic techniques used in parathyroidectomy. The block is easy to perform and has few hemodynamic side effects. Surgery can be carried out in an awake patient. We describe 3 cases of patients with primary hyperparathyroidism (parathyroid adenoma) who underwent unilateral minimally invasive parathyroidectomy under regional anesthesia and sedation with 2 mg of midazolam plus remifentanil at dosages ranging from 0.6 to 0.1 microg kg(-1) min(-1). To provide a deep cervical block, we administered 15 mL of 0.75% ropivacaine. Fo

Ultrasound-guided brachial plexus block in a patient with multiple glomangiomatosis.

BACKGROUND AND OBJECTIVES: Glomangiomas are rare, vascular tumors consisting of an afferent artery, arteriovenous canal, neuro-reticular elements, collagen, and efferent veins, and are most often located in the soft tissue of the upper extremities. We describe how the use of ultrasound-guided nerve blockade altered the anesthetic management of a patient with multiple glomangiomatosis undergoing elective forearm surgery. ULTRASOUND FINDINGS: A 32-year-old man was scheduled for excision of painful glomangiomas from the ulnar aspect of his right wrist, with exploration of his ulnar nerve. The an

Ropivacaine-induced toxicity with overdose suspected after axillary brachial plexus block.

Ropivacaine has a high threshold for systemic toxicity. We report and highlight a rare case in which an overdose of ropivacaine was suspected of leading to a generalized convulsion following the injection of this agent for axillary brachial plexus block (ABPB). A 25-year-old woman (height, 153 cm; weight, 48 kg; American Society of Anesthesiologists physical status I) was scheduled for finger surgery with ABPB. The perivascular sheath was identified by fascial clicks. We administered 300 mg (6.25 mg x kg(-1)) ropivacaine, while confirming that no blood flow was observed in the injection line b

Infraclavicular coracoid approach brachial plexus block for radial club hand repair.

BACKGROUND: Radial club hand is a congenital deformity in which the radius is absent, the metacarpophylangeal and interphalayngeal joints are fused and muscles arising from the radius are absent. This can alter the motor response to peripheral nerve stimulation. The classical motor response to median nerve stimulation (contractions of the fingers) cannot be elicited, hence fine twitches in the hand, at the wrist or in the surgical area or pronation-supination at a current of 0.5 mA were taken as endpoints. The aim of this study was to assess the feasibility of achieving successful infraclavicu

Sympathectomy-like effects of brachial plexus block in arteriovenous access surgery.

We used color Doppler ultrasonography to prospectively study the effects of supraclavicular brachial plexus block (BPB) on blood flow and vein diameter in patients undergoing arteriovenous access surgery. BPB might produce a sympathectomy-like effect that could have a role in improving patencty. Thirty-one consecutive patients who underwent arteriovenous access surgery with BPB were studied prospectively, in addition to 5 patients with BPB having other operations and 5 patients undergoing general anesthesia for elective surgery. Vessel diameter and pulsatility index (PI) were among the paramet

[Emergency axillary brachial plexus block in a patient allergic to atropine].

Peripheral nerve blocks can provide a valid alternative for anesthetizing patients with allergy to drugs such as atropine that are often used for general anesthesia. We report the case of a 7-year-old boy (physical status ASA II, weight 28 Kg). Relevant medical history included generalized skin rash and breathing difficulties due to bronchospasm after administration of atropine eyedrops. The boy had never undergone skin testing for allergy. With a diagnosis of distal metaphyseal fracture of the right radius, he was proposed for urgent closed reduction and osteosynthesis of the lesion. After pr

Clinical efficacy of the brachial plexus block via the posterior approach.

BACKGROUND AND OBJECTIVES: The posterior approach to the brachial plexus remains underused. We assessed the clinical effectiveness of this technique for shoulder surgery. METHODS: One hundred eighty-seven consecutive patients scheduled to undergo shoulder surgery were assessed after a single-injection nerve-stimulation technique using ropivacaine 0.75%. Sensory and motor block was evaluated in the distribution area of each terminal branch of the brachial plexus every 10 minutes for 30 minutes. Postoperative analgesia was evaluated at regular time intervals at rest and with passive movement, u

Transient vascular insufficiency after axillary brachial plexus block in a child.

UNLABELLED: Axillary block is used in children for procedures on the hand and forearm. We report on a child with an amputation of the thumb in whom an axillary block was given, after which the limb became pale and pulseless. The pulses returned spontaneously in 15 min. The awareness of this possibility and chances of spontaneous recovery should be considered. IMPLICATIONS: Transient vascular insufficiency of the upper limb may happen as a rare complication after axillary block. Knowledge of this complication can help the anesthesiologist in the management of this problem.

Idiopathic brachial plexus lesion with conduction block of the ulnar nerve.

A 24-year-old male with idiopathic acute aching of the right wrist, weakness of ulnar innervated muscles of the forearm and hand, and numbness of the ulnar two digits, was examined electrophysiologically. Conduction studies showed almost complete conduction block of motor fibers of the ulnar nerve distally in the brachial plexus. Sensory fibers were less affected. The symptoms and pathophysiological findings improved over a period of 1-2 months and were normal at the final study performed 7 months after presentation. There was mild evidence of chronic partial denervation of the abductor digiti

Brachial plexus block. Unilateral thoraco-abdominal blockade following the supraclavicular approach.

A supraclavicular brachial plexus block was performed which resulted in unilateral sensory and motor blockade of the thoracic and abdominal walls. General anaesthesia was therefore used and postoperatively it was noticed that analgesia of the upper limb had developed. It is likely that the blockade resulted from an intrapleural injection of local anaesthetic.

Incidence of Phrenic Nerve Palsy in Pediatric Costoclavicular Brachial Plexus Block.

The costoclavicular brachial plexus block has gained relevance as a safe and effective regional anesthesia technique for upper limb orthopedic surgery in adults, but data in pediatric populations remain limited. This study aimed to evaluate the incidence of phrenic nerve palsy associated with CBPB in pediatric patients. We conducted a descriptive observational study in 30 children undergoing upper limb orthopedic surgery. Diaphragmatic excursion was assessed before and after the block using M-mode ultrasound. No cases of phrenic nerve palsy were observed. Postoperative analgesia was effective,

A novel technique of ultrasound-guided brachial plexus block in calves.

An interventional ultrasound technique to increase the safety of surgical treatment of the calf forelimb was tested. First, the brachial plexus was evaluated using ultrasonography and then 2% lidocaine was injected under ultrasound guidance. Ultrasonically, the brachial plexus appeared as multiple hypoechoic areas surrounded by a hyperechoic rim or a hyperechoic structure characterised by multiple discontinuous lines. It was located between the omotransverse muscle and axillary artery and vein. The sensitive effect in the forelimb was seen mainly in the area supplied by the musculocutaneous ne
