Valve-sparing aortic root and partial aortic arch replacement with reimplantation of the brachiocephalic trunk for chronic type ІA aortic dissection.

A 47-year-old female with dyspnoea, poor blood pressure control and a family history of aortic disease presented with chronic type IA aortic dissection (with a maximal aortic diameter of 50 mm at the ascending aorta level). Anatomically, the patient presented with involvement of the brachiocephalic trunk and the left renal artery originated from the false lumen. The aortic valve was trileaflet but demonstrated severe eccentric insufficiency. Due to this constellation of findings, urgent surgical repair was indicated. Cardiopulmonary bypass was established centrally by direct true lumen cannula

Calcific emboli originating from the brachiocephalic trunk causing acute cerebral infarction and worm-like calcification in the right middle cerebral artery.

We report, to our knowledge, the first patient with brachiocephalic trunk calcified plaque causing the 'salted pretzel sign' and worm-like calcification. Arterial stenosis or occlusion caused by atherosclerosis and thrombosis are responsible for the majority of cases of cerebral infarction. However, there are a number of other causes. Interestingly, these findings were not observed on a CT scan 10 days before the stroke. Acute cerebral infarction with calcifications on unenhanced head CT scans should raise suspicion for disease in the carotid system. Identification of subtle findings of acute

Surgery for acute type A dissection using total arch replacement combined with stented elephant trunk implantation: Preservation of autologous brachiocephalic vessels.

OBJECTIVE: Various techniques have been introduced to treat acute type A dissection during aortic arch reconstruction. We retrospectively reviewed our experience of total arch replacement, with implantation of a stented elephant trunk, using preservation of autologous brachiocephalic vessels in patients with acute type A dissection. METHODS: Between August 2011 and April 2013, 20 patients (16 men, 4 women; mean age, 45 ± 10 years, range, 24-62 years) with acute type A dissection underwent total arch replacement combined with stented elephant trunk implantation, using preservation of autologou

Expanded polytetrafluoroethylene versus polyethylene terephthalate bridging stents for brachiocephalic trunk incorporation during arch branched endovascular repair: A comparative analysis and clinical implications.

OBJECTIVE: To compare postdeployment geometry, early clinical outcomes, and late durability of expanded polytetrafluoroethylene (ePTFE) vs polyethylene terephthalate (PET) bridging stents (BSs) used for brachiocephalic trunk (BCT) incorporation during arch branched endovascular aortic repair (A-BEVAR). METHODS: A retrospective single-center analysis was conducted of patients undergoing A-BEVAR with a triple branch and antegrade BCT incorporation between 2018 and 2025. Patients were stratified according to BCT BS material (ePTFE vs PET). Postoperative computed tomography imaging was used to as

Incidental Detection of Situs Inversus Totalis Following Identification of an Unexpected Left-Sided Brachiocephalic Trunk on Carotid Duplex Ultrasound: A Case Report.

Situs inversus totalis (SIT) is a challenging diagnosis, as failure to recognize the mirrored anatomy can result in complications during surgery, medical procedures, or emergency care. We present the case of a 60-year-old woman with a medical history of dyslipidemia who was referred for a carotid and vertebral artery duplex ultrasound ordered by the referring physician to evaluate subclinical atherosclerosis. The ultrasound unexpectedly showed that the brachiocephalic trunk was not identified on the right side, and a left-sided innominate artery was present, representing an unusual branching c

[Deep venous thrombosis on implantable infusion devices. A prospective study of 72 patients with Doppler ultrasonography of veins of the neck (jugular, subclavian and brachiocephalic trunk)].

Seventy-two consecutive patients with totally implanted catheters for administration of chemotherapy for solid tumours or lymphomas were studied prospectively to assess the prevalence of venous thrombosis. During the follow-up period of 343 (6-1,177) days, 11 cases of venous thrombosis (15.2%), of which 45% were partial and only 36% symptomatic were observed. Venous thrombosis was an early complication, 6/11 cases being observed within 1 month of implantation. No clinical or biological predisposing factor, apart from the presence of malignant disease, could be identified. Doppler ultrasonograp

Coexisting of aortic arch variation of the left common carotid artery arising from brachiocephalic trunk and absence of the main branches of right subclavian artery: a review of the literature.

The variations in the aortic arch and its branches are of surgical interest. Many anomalies of the aortic arch and great vessels are uncovered as serendipitous findings on imaging studies, in the anatomy laboratory, or at surgery. A 56-year-old woman had an arch angiogram as part of an evaluation for cardiovascular disease. A two-vessel left aortic arch was identified consisting of the brachiocephalic trunk and left subclavian artery. In this study, we present a case of the left common carotid (LCC) artery arising from the brachiocephalic trunk (BT) and absence of the main branches of right su

Takayasu's arteriopathy with associated occlusion of right coronary artery, brachiocephalic trunk and left subclavian artery and aortic regurgitation. Cardiovascular approach leading to a successful outcome - a case presentation.

In a 37 year-old woman with Takayasu's arteriopathy angiography revealed occlusion of right coronary artery (RCA), brachiocephalic trunk and left carotid artery (LCA), as well as aortic regurgitation. She underwent a complex cardiovascular surgery consisting of aortic valve implantation, RCA grafting and implantation of vascular bifurcated graft anastomosed between ascending aorta and brachiocephalic trunk and LCA. The multi-slice computed tomography performed two weeks after the operation revealed preserved grafts patency.

[The iatrogenic complications of tracheostomy].

The objective of the present study was to analyze the complications of tracheostomy associated with bleeding from the brachiocephalic trunk. A total of 13 protocols of the autopsy study of the patients who had died in the intensive care unit were available for the analysis. These patients had experienced heavy external bleeding from the tracheostomy defect shortly before death. The study has demonstrated that all the victims had the tracheostomy hole localized below the level of the standard dissection of the tracheal rings. Nine patients presented with a damage to the brachiocephalic trunk wh

Two Common Trunks Arising From Arch of Aorta: Case Report and Literature Review of A Very Rare Variation.

Arch of aorta normally gives off three branches, the brachiocephalic trunk, left common carotid artery and left subclavian artery. Due to its complex development, variations in the branching pattern are not infrequent and since many such variants remain asymptomatic, they are detected incidentally at diagnostic imaging, autopsy and surgery. The classical branching pattern is reported to be present in 63.5%-89.4% cases and the most common variant observed is the presence of common trunk of brachiocephalic and left common carotid arteries. Direct aortic arch origin of left vertebral artery is th

[Surgical management of an innominate artery aneurysm].

Aneurysms of the innominate artery are rare, representing only 3% of all aneurysms of the supra-aortic trunks. Early treatment of these aneurysms is recommended in order to prevent the occurrence of rupture and/or embolization to the brain or peripheral circulation. The authors report the case of an asymptomatic patient, with an aneurysm of the brachiocephalic trunk with 3.2 cm of greatest diameter, associated to ectasia of the right subclavian artery and common carotid double kinking. A bypass was performed from the ascending aorta to the carotid and subclavian arteries using a Dacron bifurca
