Intravenous administration of xenogenic adipose-derived mesenchymal stem cells (ADMSC) and ADMSC-derived exosomes markedly reduced brain infarct volume and preserved neurological function in rat after acute ischemic stroke.

We tested the hypothesis that combined xenogenic (from mini-pig) adipose-derived mesenchymal stem cell (ADMSC) and ADMSC-derived exosome therapy could reduce brain-infarct zone (BIZ) and enhance neurological recovery in rat after acute ischemic stroke (AIS) induced by 50-min left middle cerebral artery occlusion. Adult-male Sprague-Dawley rats (n = 60) were divided equally into group 1 (sham-control), group 2 (AIS), group 3 [AIS-ADMSC (1.2×106 cells)], group 4 [AIS-exosome (100μg)], and group 5 (AIS-exosome-ADMSC). All therapies were provided intravenously at 3h after AIS procedure. BIZ determ

Magnetic resonance imaging in ischaemic accidents of the brain stem.

Seven cases of ischaemic accident affecting the brain stem are reported. There were two lacunar infarctions and five infarctions due to occlusion of major arteries (two lateral medullary infarctions, one pontine infarction and two thalamo-mesencephalic infarctions). In every case the abnormalities detected at MRI were concordant with the clinical findings. The authors underline the value of MRI in establishing clinico-topographic correlations in vivo and in differentiating lacunar infarctions from infarctions due to stenosis of major arteries in the vertebro-basilar system.

Autopsy correlations of computerized tomography: experience with 6,000 CT scans.

Seventy-nine autopsy correlations of CT scans showed (1) excellent correlations in normal brains, but the size of the lateral ventricles consistently larger during life than after death; (2) a distinctive pattern differentiating obstructive from nonobstructive hydrocephalus; (3) infarctions appearing as areas of decreased densities of parenchyma in vascular distributions; (4) distinctive high density appearances of hemorrhages that differentiated them from infarctions and, in general, all other pathologic processes; (5) supratentorial, intraventricular, and posterior fossa tumors appearing as

[Cerebellar infarct: clinical picture, diagnosis and treatment].

Cerebellar infarctions are a relatively rare disorder, the diagnosis of which even based on clinical, angiographical and ventriculographic studies is rather difficult. Of big service in such conditions are computer tomographies. In extensive isolated cerebellar infarctions with signs of developing occlusional hydrocephaly with brain stem compressions, there may be indications to surgical operations. The paper contains the literary review, where problems of diagnosis and treatment of cerebellar infarctions are considered. The paper also contains a description of one case where a surgical operat

Magnetic resonance imaging in cerebral malaria: a report of four cases.

OBJECTIVES: This is a retrospective institutional review of clinical data and radiological findings of cerebral malaria patients presenting to a tertiary centre in India, which is an known to be endemic for malarial disease. METHODS: The present series describes MRI in four cases all of which revealed bithalamic infarctions with or without haemorrhages in patients with cerebral malaria, and this review examines a subset of patients with this condition. In addition, acute haemorrhagic infarctions were also seen the in brain stem, cerebellum, cerebral white matter and insular cortex in two of t

[Tabular method of evaluation in the diagnosis of complicated brain infarct].

On the basis of a clinico-anatomical comparison in 70 cases of complicated infarctions and 77 observations of white infarctions the author elaborated computing tables. It was established that the following signs have certain diagnostic significance: the degree of changed consciousness, meningeal symptoms, symptoms indicating a secondary lesion of the brain stem and symptoms characterizing the degree of focal neurological disturbances. Among the supplementary methods of special significance is CSF analysis.

Subcortical cerebral infarctions in sickle cell trait.

At necropsy, two patients with sickle cell trait and progressive motor and visual deficits, lethargy and coma showed infarctions of the deep cerebral white matter and brain stem. The findings in these patients and another reported in the literature suggest that subcortical infarctions may be more common in sickle cell trait than has been recognised and should be suspected in any patient with sickle cell trait who presents with an unusual neurological illness.

An MRI analysis of brain-stem and cerebellar lesions and olivary hypertrophy.

We describe the relations between primary brain stem or cerebellar infarct or haemorrhage and secondary inferior olivary pseudo-hypertrophy (OPH). We identified 17 patients (43.6%) among 39 with brain stem or cerebellar vascular disease who had MRI follow-up more than 3 months after their ictus, with OPH. The primary lesions in the 22 cases without OPH were 11 haemorrhages, including 8 medial cerebellar and 3 brain stem lesions, and 11 infarcts: 4 brain stem lesions without accompanying cerebellar involvement, 2 cerebellar infarcts with brain stem extension, and 5 cerebellar lesions without a

Evaluation and graduation of brain stem auditory evoked potentials in patients with neurological diseases.

Based on normative data from 40 subjects, criteria for evaluating brain stem auditory evoked potentials have been elaborated. This system contained six grades from normal to diffusely abnormal, and was used in evaluating recordings from 142 patients, including 38 patients with definite and 48 with probable and possible multiple sclerosis, 18 patients with brain stem infarctions, six patients with other organic brain stem lesions, six patients with a postcommotional syndrome and 26 patients without any brain stem involvement. A statistically significant difference was found between normal perso

[Medial medullary infarct].

Medial medullary infarct is a rare type of brain stem infarction first described in 1908. It was only since the broad use of MRI that an accurate clinical topographical correlation could be documented in survivors. We observed two patients; one of them had an anteromedial unilateral infarction in the upper medulla, and the clinical picture was characterized by contralateral hemiparesis, facial weakness, dysarthria and palatal weakness. The outcome was good. The second patient however showed a severe tetraparesis with only minor brain stem signs, secondary to bilateral anteromedian infarction w

[Morphology of primary and secondary hemorrhages into the brain stem].

On the basis of morphological examinations of 5 cases of primary and 15 cases of secondary hemorrhages to the brain stem histological characteristics of those hemorrhages are presented. A high frequency of preceding disorders of the brain stem circulation in the forms of infarctions at various stages of development, perivascular hemorrhages and cysts due to changes (mostly of hypertensive origin) in the cerebral arterie is noted. These changes are regarded as factors promoting the spreading, and, possibly, the development of primary hemorrhages to the stem, as well as aggravating the degree of

[Morphologic verification in computer tomography of the brain].

A comparison of computer-aided tomograms of the brain with the results of morphological examinations of the latter in 51 autopsy cases was carried out. The causes of the death were brain infarctions, hemorrhages, and tumours. It was found that roentgenological diagnosis of cerebral infarctions entailed no difficulties, if the examinations were performed within 24 hours after the disease onset and the focus size was over 1 to 2 cm. There were some difficulties in diagnosing small foci in the brain stem and in the cortex of brain hemispheres as well as infarctions with hemorrhages irrespectively

Secondary brain stem hemorrhage in stroke.

The occurrence of secondary brain stem hemorrhage was studied in 435 autopsies from patients with recent cerebral hemorrhage, infarction or ruptured cerebral aneurysms. The frequency of secondary brain stem hemorrhage was found to be 45% in cerebral hemorrhage, 15% in cerebral infarction, and 36% in ruptured aneurysms. In the majority of cases the secondary brain stem hemorrhage occurred a few days after the onset of cerebral hemorrhage or infarction. Ruptured aneurysms showed a more widespread temporal distribution of secondary brain stem hemorrhage. The median survival time was 2 days in cas

Anatomic and MRI bases for medullary infarctions with patients' presentation.

OBJECTIVE: There is a low incidence of the medullary infarctions and sparse data about the vascular territories, as well as a correlation among the anatomic, magnetic resonance imaging (MRI) and neurologic signs. MATERIALS AND METHODS: Arteries of the 10 right and left sides of the brain stem were injected with India ink, fixed in formalin and microdissected. The enrolled 34 patients with medullary infarctions underwent a neurologic, MRI and Doppler examination. RESULTS: Four types of the infarctions were distinguished according to the involved vascular territories. The isolated medial medul

Magnetic resonance imaging in young adults with cerebral infarction due to moyamoya.

The number, size, and location of cerebral infarctions, and blood flow in the middle cerebral artery as seen on proton magnetic resonance imaging were assessed in six white adults with angiographically documented moyamoya. Findings were correlated with clinical presentation, computed tomography, and angiography. Large hemispheric infarctions were found in five hemispheres, predominantly in watershed regions. Subcortical infarctions (n = 56) were found in all hemispheres. They were predominantly located in the centrum semiovale, in the distal beds of supply of the penetrating branches of the an

[Repeated treatment of subtotal vertebrobasilar stenosis with tissue plasminogen activator (tPA)].

We report on the successful repeated treatment of a patient with rapidly progressive severe brain-stem stroke by application of I.V. tissue plasminogen activator (tPA). This treatment twice led to a prompt remission of severe brain-stem symptoms, although a permanent therapeutic success could not be achieved. Unfortunately, the patient died a few days later from pneumonia and sepsis. Necropsy revealed bilateral partial brain-stem infarctions and a subtotal stenosis of the vertebrobasilar junction with superimposed fresh thrombus. The clinically dramatic response to tPA indicates that this type

We report a neuropathological study of cerebellar infarctions involving the territory of the posterior inferior cerebellar artery (PICA) in 28 cases. Fifteen cases involved the PICA territory only. In 13 cases infarctions in the anterior inferior cerebellar artery (AICA) territory and/or in the superior cerebellar artery (SCA) territory were also present. A thorough post-mortem study of the arterial supply of the brain from the heart up to the cerebellar arteries, including the cervical spine segment of the vertebral arteries was performed in 27 cases. [Cerebellar infarction].

Cerebellar infarcts have long been underrecognized both clinically and neuroradiologically. In approximately a quarter of cases, they are oedematous and may require, unlike other cerebral infarcts, a sometimes life saving surgery when there are signs of brain stem compression. Benign infarcts are more frequent, but inaugural symptoms are identical: vertigo, headache, vomiting, unsteadiness, and dysarthria of sudden onset. Brain stem and occipital symptoms can be associated and are sometimes prominent. The infarct can be shown by CT but MRI is the method of choice for early recognition, precise
