Electroencephalography-guided transcranial direct current stimulation improves picture-naming performance.

Transcranial direct current stimulation (tDCS) is a potential method for improving verbal function by stimulating Broca's area. Previous studies have shown the effectiveness of using functional magnetic resonance imaging (fMRI) to optimize the stimulation site, but it is unclear whether similar optimization can be achieved using scalp electroencephalography (EEG). Here, we investigated whether tDCS targeting a brain area identified by EEG can improve verbalization performance during a picture-naming task. In Experiment 1, EEG and fMRI data were acquired during a naming task with 21 participant

Deficit in complex sequence processing after a virtual lesion of left BA45.

Although the contribution of Broca's area to motor cognition is generally accepted, its exact role remains controversial. A previous functional imaging study has suggested that Broca's area implements hierarchically organised motor behaviours and, in particular, that its anterior (Brodmann area 45, BA45) and posterior (BA44) parts process, respectively, higher and lower-level hierarchical elements. This function of Broca's area could generalize to other cognitive functions, including language. However, because of the correlative nature of functional imaging data, the causal relationship betwee

Syntactic Impairment Associated with Hypoperfusion in the Left Middle and Inferior Frontal Gyri after Right Cerebellar Hemorrhage.

Cerebellar injuries can cause syntax impairments. Cortical dysfunction due to cerebello-cerebral diaschisis is assumed to play a role in this phenomenon. Functional magnetic resonance imaging studies have repeatedly shown the activation of Broca's area in response to syntactic tasks. However, there have been no reports of selective syntax impairment and hypoperfusion restricted to this area after cerebellar injury. We herein report a patient with right cerebellar hemorrhage that led to marked syntax impairment along with severe hypoperfusion confined to the Brodmann area (BA) 45 (anterior part

[Functional coupling of the frontal and parietal lobes in action].

Apraxia is the inability to perform actions or move different parts of the body in the intended manner, despite normal physical capability of movement. Based on his studies, Liepmann divided apraxia into three types: ideational apraxia, ideomotor apraxia, and limb-kinetic apraxia. Clinical findings such as ideomotor apraxia in Broca's area and apraxia in patients with Parkinson's disease (PD) and parietal ataxia suggest sensorimotor integration and action control in the parietofrontal circuits. Based on studies of the mirror neuron system, disorders in gesture production could be associated wi

The impact of brain lesions on tDCS-induced electric fields.

Transcranial direct current stimulation (tDCS) can enhance motor and language rehabilitation after stroke. Though brain lesions distort tDCS-induced electric field (E-field), systematic accounts remain limited. Using electric field modelling, we investigated the effect of 630 synthetic lesions on E-field magnitude in the region of interest (ROI). Models were conducted for two tDCS montages targeting either primary motor cortex (M1) or Broca's area (BA44). Absolute E-field magnitude in the ROI differed by up to 42% compared to the non-lesioned brain depending on lesion size, lesion-ROI distance

The error of Broca: From the traditional localizationist concept to a connectomal anatomy of human brain.

Neurology, especially aphasiology, was mainly built on behavioral-structural correlations ("lesion method"). In this setting, Broca's area has been considered as the "speech area"; moreover, this observation led to localizationism. However, advances in brain mapping techniques, as functional neuroimaging and direct electrical stimulation in patients undergoing awake surgery for gliomas, has resulted in a paradigmatic shift regarding models of neural architecture. In fact, the brain is organized in distributed complex networks underpinning sensorimotor, visuospatial, language, cognitive and emo

Bilateral Repetitive Transcranial Magnetic Stimulation for Auditory Hallucinations in Patients with Schizophrenia: A Randomized Controlled, Cross-over Study.

OBJECTIVE: A randomized double-blind cross-over trial was conducted in patients with persistent auditory hallucinations (AHs) to investigate whether bilateral repetitive transcranial magnetic stimulation (rTMS) at the temporoparietal area or Broca's area is more effective at high- or low-frequencies compared to a sham condition. METHODS: Twenty three patients with persistent AHs who remained stable on the same medication for 2 months were enrolled. They were randomized to one of four conditions: low-frequency (1 Hz)-rTMS to the temporoparietal area (L-TP), high-frequency (20 Hz)-rTMS to the t

The posterior middle temporal gyrus serves as a hub in syntactic comprehension: A model on the syntactic neural network.

Neuroimaging studies have revealed a distributed neural network involving multiple fronto-temporal regions that are active during syntactic processing. Here, we investigated how these regions work collaboratively to support syntactic comprehension by examining the behavioral relevance of the global functional integration of the syntax network (SN). We found that individuals with a stronger resting-state within-network integration in the left posterior middle temporal gyrus (lpMTG) were better at syntactic comprehension. Furthermore, the pair-wise functional connectivity between the lpMTG and t

Characteristics of oxyhemoglobin during the verbal fluency task in subthreshold depression: A multi-channel near-infrared spectroscopy study.

OBJECTIVE: Subthreshold depression is an essential precursor and risk factor for major depressive disorder, and its accurate identification and timely intervention are important for reducing the prevalence of major depressive disorder. Therefore, we used functional near-infrared spectroscopic imaging (fNIRS) to explore the characteristics of the brain neural activity of college students with subthreshold depression in the verbal fluency task. METHODS: A total of 72 subthreshold depressed college students (SDs) and 67 healthy college students (HCs) were recruited, and all subjects were subject

The Effects of Different Brain Regions on fNIRS-based Task-state Detection in Speech Imagery.

Brain-computer interface (BCI) based on speech imagery can decode users' verbal intent and help people with motor disabilities communicate naturally. Functional near-infrared spectroscopy (fNIRS) is a commonly used brain signal acquisition method. Asynchronous BCI can response to control commands at any time, which provides great convenience for users. Task state detection, defined as identifying whether user starts or continues covertly articulating, plays an important role in speech imagery BCIs. To better distinguish task state from idle state during speech imagery, this work used fNIRS sig

Magnetic resonance spectroscopy investigations of functionally defined language areas in schizophrenia patients with and without auditory hallucinations.

BACKGROUND: Cerebral dysfunction occurring in mental disorders can show metabolic disturbances which are limited to circumscribed brain areas. Auditory hallucinations have been shown to be related to defined cortical areas linked to specific language functions. Here, we investigated if the study of metabolic changes in auditory hallucinations requires a functional rather than an anatomical definition of their location and size to allow a reliable investigation by magnetic resonance spectroscopy (MRS). METHODS: Schizophrenia patients with (AH; n=12) and without hallucinations (NH; n=8) and hea

Application value of resting-state fMRI in preoperative lateralization of language areas in epilepsy with left-sided epileptogenic foci.

OBJECTIVE: Focusing on patients with epilepsy and left-sided epileptogenic foci, this study aimed to clarify language lateralization differences from healthy individuals, explore Broca's/Wernicke's area lateralization and dominant hemisphere shift, and construct a laterality index-activation map combined scheme. It provides a theoretical basis for optimizing epilepsy surgery and reducing postoperative language impairment risk. METHODS: We retrospectively studied 36 patients with left-sided epileptogenic foci and 45 healthy controls (2018-2023, Hebei Medical University Second Hospital). After

Diffuse cerebral language representation in tuberous sclerosis complex.

INTRODUCTION: Tuberous sclerosis complex (TSC) is a multisystem genetic disorder affecting multiple organs, including the brain, and very often associated with epileptic activity. Language acquisition and development seems to be altered in a significant proportion of patients with TSC. In the present study, we used magnetoencephalography (MEG) to investigate spatiotemporal cerebral language processing in subjects with TSC and epilepsy during a reading semantic decision task, compared to healthy control participants. METHODS: Fifteen patients with TSC and 31 healthy subjects performed a lexico

Altered functional connectivity in language and non-language brain networks in patients diagnosed with acute post-stroke aphasia.

OBJECTIVE: A resting-state functional magnetic resonance imaging (rs-fMRI) approach was used to explore functional connectivity (FC) in language and non-language brain networks in acute post-stroke aphasia (PSA) patients, with a specific focus on the relationship between these fMRI results and patient clinical presentation. METHODS: In total, 20 acute PSA patients and 30 age-, sex-, and education level-matched healthy control (HC) participants were recruited and subjected to rs-fMRI imaging. In addition, western aphasia battery analyses(WAB) were used to compute aphasia quotient (AQ) values f

The cortical organization of language: distilling human connectome insights for supratentorial neurosurgery.

Connectomics is the production and study of detailed "connection" maps within the nervous system. With unprecedented advances in imaging and high-performance computing, the construction of individualized connectomes for routine neurosurgical use is on the horizon. Multiple projects, including the Human Connectome Project (HCP), have unraveled new and exciting data describing the functional and structural connectivity of the brain. However, the abstraction from much of these data to clinical relevance remains elusive. In the context of preserving neurological function after supratentorial surge

Fos expression in gonadotropin-releasing hormone neurons by naloxone or bicuculline in intact male rats.

Double immunocytochemistry for Fos and GnRH was performed after the intravenous infusion of naloxone or bicuculline. Either naloxone or bicuculline significantly increased serum LH concentrations. After the naloxone infusion, some Fos-positive GnRH neurons were found from the preoptic area to the basal hypothalamic area, whereas those were found from the diagonal band of Broca to the preoptic area after the bicuculline infusion. These results suggest the distribution of GnRH neurons activated by naloxone and bicuculline in intact male rats.
