Disseminated brain Nocardia abscess with bronchial and esophageal fistulas.

Disseminated nocardiosis is a rare but life-threatening infection, with limited documented cases. Moreover, there have been no reports of Nocardia mediastinal abscess leading to bronchial or esophageal fistulae. We present a 62-year-old immunocompetent male with cerebellar and mediastinal abscesses caused by Nocardia beijingensis, complicated by both bronchial-mediastinal and esophageal fistulas. He received treatment including surgical intervention, endoscopic procedures, and antibiotic therapy. This case highlights the critical role of multidisciplinary collaboration in diagnosing and managi

[Bronchial fistulae closure by hydatid lung cysts in children].

The comparative analysis by bronchial fistulae closure by hydatid lung cysts in children using two surgical techniques was conducted. All patients were divided in two groups: the first group (n = 98) included cases of fistula closure using the traditional solitary Z-suture; the second group (n = 54) had fistula closure with the use of the original method. The least was thoroughly described in the paper. The were no cases of postoperative complications among patients of the 2nd group, whereas 6 patients of the 1st group had bronchial fistula recurrence after the operation.

Aorto-bronchial fistula after implantation of a self-expanding bronchial stent in a patient with aortic dissection.

We report a case of aorto-bronchial fistula after implantation of a self-expanding stent into the left main bronchus compressed by a dissected descending aorta. A 66-year-old female, who underwent Stanford type-B aortic dissection two years previously, was admitted to our hospital for the treatment of a newly developed false lumen that originated from the ascending aorta and extended to the aortic bifurcation. She was unable to be weaned from the respirator after the graft replacement of the ascending aorta. Fiberoptic bronchoscopic examination revealed complete obstruction of the left main br

Successful Rescue of Massive Hemoptysis Caused by Vascular-Bronchial Fistula.

The case reported in this paper is a vascular-bronchial fistula associated with fatal massive hemoptysis. The patient was rescued successfully by experts from a multidisciplinary team. The bronchoscopy physician cleared the accumulated blood in the airway timely and maintained the airway patency, and the anesthesiologist established artificial ventilation quickly; eventually, the thoracic surgeon performed an emergency thoracotomy to control the bleeding. It reflected the significance of multidisciplinary collaborative treatment.

Pneumothorax due to perforated pulmonary hydatic cyst with bronchial fistula.

Cystic echinococcosis (CE) is a zoonotic disease caused by Echinococcus granulosus, with a high prevalence in Iran, particularly in Mazandaran province. This case report presents a 24-year-old male with pneumothorax resulting from ruptured pulmonary hydatid cysts with bronchial fistulization. The patient presented with persisted dyspnea, productive cough, fever, and mild right thoracoabdominal pleuritic pain, alongside a history of occupational exposure as a sheep keeper. Imaging revealed large cystic masses in lungs and a severe right pneumothorax. Despite initial stabilization with chest tub

Large bronchial artery-pulmonary artery fistula due to cavitating tuberculosis.

Bronchial artery-pulmonary artery fistulae are rare vascular malformations most commonly caused by infection. Our case presents a 57-year-old male who presented to the Emergency Department with a symptomatic bronchial artery-pulmonary artery fistula due to cavitating pulmonary tuberculosis (TB). The diagnosis was made with multiphase CT angiography of the thorax (including pulmonary and systemic arterial phases). The patient was brought to interventional radiology for further investigation and management. The left upper lobe bronchial artery-pulmonary artery fistula was successfully identified

[Successful Conservative Management of Bronchopleural Fistula after Intraoperative Bronchial Injury].

A 64-year-old woman diagnosed as primary lung cancer was admitted for surgery. Right lower lobectomy and ND2a-1 nodal dissection was performed under video-assisted thoracic surgery( VATS). The membranous portion of intermediate bronchus was injured about length of 5 mm while dissecting subcarinal lymph nodes. The fistula was closed by knotted suture using 4-0 polydioxanone (PDS) and covered with pericardial fat pad. Although the postoperative course was uneventful and discharged at postoperative day (POD) nine, bloody sputum appeared and right pneumothorax developed at POD 11. Bronchoscopy rev

Combined Management of a Bronchial Artery Fistula After Lung Transplantation.

A 46-year-old female patient exhibited massive endobronchial bleeding after dilation of a left bronchial anastomotic stenosis after lung transplantation, consistent with a bronchopulmonary artery fistula (BPAF). The BPAF was treated with a bronchial covered self-expandable metallic stent and percutaneous transcatheter pulmonary artery stent placement. BPAF is rare and leads to death in most cases because of massive hemoptysis. We describe a case of successful combined management of BPAF using both bronchial and pulmonary stent placement.

Acquired coronary-to-bronchial artery fistula after two valve surgeries.

A 62-year-old woman with Takayasu arteritis and 2 prior aortic valve replacements, was admitted with unstable angina. Computed tomographic angiography showed a newly developed right coronary artery-to-right bronchial artery fistula. Because of dense aortic calcification, catheter embolization was undertaken, which was successful. We believe the acquired coronary artery fistula developed after her uneventful second aortic valve surgery, due to inevitable trauma to the mediastinum and ascending aorta, and partly due to her underlying Takayasu arteritis.

[Reoperation for left main bronchial stump fistula developing after pneumonectomy].

We report the case of a 71-year-old male patient who underwent reoperation for bronchial stump fistula developing after left pneumonectomy for adenocarcinoma of the left lung (clinical stage IIB). After surgery, he developed persistent, severe cough and chest X-ray films taken on the 23rd postoperative day showed a drop in the air-fluid level in the left lung field, which, along with bronchoscopic findings, strongly suggested the bronchial stump fistula and subsequent reoperation was performed. Both superior pulmonary vein and main pulmonary artery were dissected again proximally in pericardiu

[A case report--congenital esophago-bronchial fistula with esophageal diverticulum].

Congenital esophago-bronchial fistula with diverticulum is rare, and in Japan only 29 cases have been reported. A 59-year-old man had suffered from recurrent pulmonary infections and often complained of coughing while drinking water during the previous two years. An UGI series for follow-up of gastric ulcer revealed an esophago-bronchial fistula, located between the mid-esophagus and B10 of the right lung. We resected the diverticulum and the fistulous tract with a partial excision of S10. Histological examination revealed that the squamous epithelium with the muscularis mucosa of the esophagu

[Bronchial fistula caused by Pott's abscesses. Report of 3 cases].

The authors report 3 cases of intrabronchial fistulisation of paravertebral abscesses due to tuberculous spondylitis of the thoracic spine. This complication is relatively rare (these 3 cases were in a series of 178 cases of Pott's disease) and is easy to diagnose due to its characteristic radio-clinical features. As mentioned in old publications, this type of fistulisation does not seem to have serious consequences. There are no particular aspects of treatment.

Surgical management of hydatid Bilio-bronchial fistula by exclusive thoracotomy.

In the industrialized countries, most of Bilio-bronchial fistula are secondary to hepatobiliary trauma, hepatic resection surgery or in the case of congenital malformation of the biliary tract, Bilio-bronchial fistula is recognized as the complication of a number of infectious pathologies such as hydatidosis and hepatic amoebiasis. Among the causes, the Bilio-bronchial fistula of hydatic origin is by far the most frequent especially in the zones of hydatid endemic as Morocco. It is a serious complication of liver hydatid cysts. The surgical management has long been believed to be difficult, an
