[His bundle electrography in children (author's transl)].

The His bundle electrography has confirmed many earlier hypotheses about the localization of atrioventricular block and the mechanism of tachyarrythmias; beyond this, it has added new information. In this paper, normal values and results of the method during childhood are reviewed. The indication for His bundle electrography as an invasive method is limited in children. An important indication consists of clarification of unclear partial atrioventricular block and of the state of the remaining third fasicle in postoperative bifascicular block.

A Simulation Study of the Effects of His Bundle Pacing in Left Bundle Branch Block.

His bundle pacing (HBP) has emerged as a feasible alternative to right (RVP) and biventricular pacing (BVP) for Cardiac Resynchronization Therapy (CRT). This study sought to assess, in ex-vivo experimental models, the optimal setup for HBP in terms of electrode placement and pacing protocol to achieve superior electrical synchrony in the case of complete His-Purkinje block and left bundle branch block (LBBB). We developed a 3D model of His bundle and bundle branches, embedded in a patient-specific biventricular heart model reconstructed from CT images. A monodomain reaction-diffusion model was

Permanent His bundle pacing: Recommendations from a Multicenter His Bundle Pacing Collaborative Working Group for standardization of definitions, implant measurements, and follow-up.

His bundle pacing (HBP) prevents ventricular dyssynchrony and its long-term consequences by preserving normal electrical activation of the ventricles. Since the original description of permanent HBP in 2000, the adoption of HBP has increased over the past several years. However, the reporting of procedural and clinical outcomes to date is not uniform. This article is a collaboration between several implanters with significant experience in HBP to establish a uniform set of definitions encompassing the different forms of HBP as well as define a standardized approach to gathering data end points

Comparison of Pacing Performance and Clinical Outcomes Between Left Bundle Branch and His Bundle Pacing.

BACKGROUND: Left bundle branch (LBBP) and His-bundle pacing (HBP) provide physiological ventricular activation. OBJECTIVES: This study investigated differences in feasibility, device performance, and clinical outcomes between LBBP and HBP. METHODS: Consecutive patients with LBBP and HBP from 2018 to 2021 in 2 centers were prospectively studied. The primary endpoint was optimal device performance during follow-up, defined as the presence of pacing thresholds <2.5 V, R-wave amplitude ≥5 V, and absence of conduction system pacing (CSP)-related complications. The secondary endpoint was the compo

Electrocardiographic characteristics of distal His bundle pacing in a patient with left bundle branch block.

This case demonstrates the feasibility of placing the pacing lead helix at the His bundle distal to the region of left bundle branch block and reveals three types of electrocardiographic characteristics of distal His bundle pacing during correction of left bundle branch block in a patient. As the pacing lead helix was placed distal to the block, left bundle branch block correction was achieved by pacing with a low and stable capture threshold.

How to Implant His Bundle and Left Bundle Pacing Leads: Tips and Pearls.

Cardiac pacing is the treatment of choice for the management of patients with bradycardia. Although right ventricular apical pacing is the standard therapy, it is associated with an increased risk of pacing-induced cardiomyopathy and heart failure. Physiological pacing using His bundle pacing and left bundle branch pacing has recently evolved as the preferred alternative pacing option. Both His bundle pacing and left bundle branch pacing have also demonstrated significant efficacy in correcting left bundle branch block and achieving cardiac resynchronisation therapy. In this article, the autho

Physiologic Pacing Targeting the His Bundle and Left Bundle Branch: a Review of the Literature.

PURPOSE OF REVIEW: Conduction system pacing (CSP) has emerged as a means to preserve or restore physiological ventricular activation via pacing at the His bundle or at more distal targets in the conduction system, including the left bundle branch area. This review examines strengths, weaknesses, and clinical applications of CSP performed via these approaches. RECENT FINDINGS: His bundle pacing (HBP) has been successfully utilized for standard bradyarrhythmia indications and for QRS correction among patients receiving devices for cardiac resynchronization therapy (CRT). Limitations of HBP paci

Physiological mechanisms of QRS narrowing in bundle branch block patients undergoing permanent His bundle pacing.

His bundle pacing is increasingly used to avoid chronic right ventricular pacing, and electrically resynchronize ventricular activation by narrowing or normalizing the QRS interval in left and right bundle branch block. The mechanisms by which this occurs remain poorly understood. In this review, the proposed mechanisms and evidence supporting them are discussed. Also discussed are aspects of mechanisms that are not completely supported by the evidence. We also review the differences and physiological bases for direct vs. indirect His bundle capture, and the physiological mechanisms for QRS na

Painful left bundle branch block syndrome treated with his bundle pacing.

This is a case report of a patient with painful LBBB Syndrome that responded favorably to His Bundle Pacing. This syndrome is largely under recognized despite 50 reports in the literature over the last 60 years. Both diagnosis and treatment are not well defined and represent a major challenge in patients with this entity. Right ventricular pacing has been attempted with inconsistent efficacy outcomes. We report for the first-time complete resolution of chest pain with His bundle pacing. HBP provides a promising alternative pacing option that might provide symptom resolution to patients with a

Discrimination between His-bundle and the right bundle branch during electrophysiologic studies.

BACKGROUND: The purpose of this study was to identify the His-bundle (HB) versus right bundle branch (RBB) during electrophysiologic studies, using the V3 phenomenon, and to compare the timing of HB versus RBB potentials of sinus cycles (His-ventricular [H-V] interval). METHODS: The study enrolled 16 patients without structural heart disease, who underwent electrophysiologic studies during which the H-V interval was within normal limits and the V3 phenomenon was induced during recordings of the HB and the RBB potentials by a multi-electrode catheter. The recording site of the earliest HB pote

Programming and follow-up of patients with His bundle pacing.

His bundle pacing (HBP) is being increasingly adopted worldwide, with the aim of providing more physiological stimulation of the heart as opposed to right ventricular pacing or as an alternative to cardiac resynchronization therapy (CRT). Current devices are not specifically designed for HBP, which gives rise to programming challenges. This article aims to provide practical recommendations for HBP programming and follow-up.

Transvenous catheter ablation of the His bundle in ventricular tachycardia.

The usefulness of transvenous catheter ablation of the His bundle in three patients with recurrent ventricular tachycardia (VT), in which the initiating mechanism was recognized during a rapid atrial rhythm, is reported. Tachycardia was refractory to conventional treatment and required transthoracic direct-current shocks in all patients. In patient No. 1 double tachycardia (atrial flutter and VT) was documented and VT was easily induced by rapid atrial pacing. In patients Nos. 2 and 3 initiation of VT during junctional reciprocating and atrial tachycardia, respectively, was observed. Interrupt

His electrogram alternans reveal dual-wavefront inputs into and longitudinal dissociation within the bundle of His.

BACKGROUND: His electrogram (HE) amplitude and morphology changes were observed in our previous studies during transition from "fast" to "slow" atrioventricular nodal (AVN) conduction. This phenomenon and its significance for the dual-AVN electrophysiology are not well recognized and have not been studied. METHODS AND RESULTS: Experiments were performed on 17 healthy rabbit atrial-AVN preparations during standard programmed electrical pacing. HEs were mapped along the His bundle with roving surface electrodes, along with recording of cellular action potentials (APs). HEs recorded from the sup

The use of His bundle pacing for the treatment of painful left bundle branch block syndrome.

Painful left bundle branch block syndrome is a rare disorder in which patients develop typical angina-like pain without identifiable ischemia. To date, there have been few published cases of effective treatment. In this case report, we describe successful implementation of His bundle pacing for durable symptom resolution in this disorder.

Effect of His bundle pacing on right bundle branch block located distal to site of pacing.

BACKGROUND: It is generally accepted that bundle branch block (BBB) may be corrected simply by selective His bundle pacing (S-HBP) distal to site of block. However, clinical HBP is often non-selective (NS), the site of block unknown and moving the HB lead is not always an option. Thus, while correction of right (R) BBB in NS-HBP has been reported, its mechanism is unknown. METHODS: Threshold testing was done during His bundle pacing in 39 patients with RBBB. We compared NS-HBP and S-HBP and analyzed the role of pacing voltage and capture threshold of the parallel pathway, in partially or comp

Cardiomyocyte apoptosis in cocaine-induced myocarditis with involvement of bundle of His and left bundle branch.

Cocaine abuse can cause degenerative and inflammatory alterations of the myocardium. Cocaine-induced myocarditis has been previously described, but mostly in chronic cocaine addicts. Among several pathogenetic mechanisms of cocaine-related myocardial damage, the myocardial adrenergic stress and cardiomyocytes apoptosis have been recently proposed. We report an unusual case of myocarditis involving the bundle of His and left bundle branch and cardiac myocytes apoptosis in otherwise healthy occasional cocaine abuser.
