Fifth metatarsal distal metaphyseal osteotomy represents a very common surgical procedure performed for tailor's bunion deformity. The authors advocate using a cortical bone screw for fixation device offers many advantages over conventional devices (i.e., Kirschner wire). The purpose of this paper is to offer a means to thoroughly evaluate tailor's bunion deformity clinically, biomechanically, and radiographically. This will allow the podiatric surgeon to choose a procedure of choice to correct the deformity. The technique of fifth metatarsal distal metaphyseal osteotomy with cortical bone scr

The direct approach to the fifth metatarsophalangeal joint for the treatment of metatarsus quintus valgus often has been unsuccessful, mainly because of technical difficulties in the osteotomy of the narrow fifth metatarsal neck and the fixation of the unstable head fragment. Complications such as delayed union, nonunion, or recurrence have been reported. This paper describes a surgical technique for optimal correction of this deformity. The technique has a biomechanically sound fixation and a shortened bone healing time with minimum complications. Results of 12 patients presenting severe defo

BACKGROUND: A dorsal bunion may occur in nonambulatory adolescents with cerebral palsy (CP) and a Gross Motor Function Classification System (GMFCS) level of IV or V. The deformity can cause pain, skin breakdown, and difficulty wearing shoes and braces. A consensus on the biomechanics and surgical management of dorsal bunions in persons with severe CP has not been established.
METHODS: This retrospective cohort study included 23 nonambulatory adolescents with CP, GMFCS level IV or V, and symptomatic dorsal bunions requiring surgery. The median age at surgery was 17 years, and the median follo

Minimally invasive surgery (MIS) is a growing technique that favors faster recovery and cosmetic results. We propose a recommended osteotomy position or "MIS Sweet Spot" to make the procedure more reproducible and lessen the learning curve to improve outcomes. A total of 211 patients from 3 surgeons between 2 different study centers were included in a retrospective radiograph measurement analysis and a cadaveric dissection to confirm the safety of nearby anatomical structures. An average distance of 2.2 cm proximal from the first metatarsophalangeal joint was found as the "Sweet Spot" to perfo

BACKGROUND: Fourth-generation minimally invasive bunion surgery (MIBS) has gained popularity given its minimal soft tissue disruption, small incisions, and potential for expedited recovery. Although early outcomes have been encouraging, limited data exists regarding complication rates during the initial learning curve phase of this most recent technique evolution.
PURPOSE: To evaluate postoperative complications associated with early adoption of fourth-generation MIBS.
STUDY DESIGN: Retrospective review.
METHODS: 72 fourth-generation MIBS procedures (67 patients) performed by three surgeons

BACKGROUND: The current approach to soft tissue release in bunion surgery is through an incision in the dorsum of the first web space between the first and second metatarsals. However, the soft tissue release through this approach can be excessive or inadequate for the correction required. In addition, it leaves a visible scar. A new technique described here provides a stepwise protocol to limit the extent of soft tissue release to only what is required for the correction. The purpose of this study was to evaluate functional outcomes of the new technique.

BACKGROUND: The purpose of this study was to evaluate the clinical results of distal chevron osteotomy performed in conjunction with selective lateral soft tissue release. The criterion for doing a lateral soft tissue release was assessed by determining the ease and completeness of passive hallux valgus correction at the time of surgery.
MATERIALS AND METHODS: Between August 2005 and November 2007, 48 feet in 43 patients classified as having mild to moderate hallux valgus were retrospectively studied. Distal chevron osteotomy without lateral soft tissue release was performed in 26 cases (Grou

Hallux valgus is a common foot problem affecting nearly one in every four adults. Generalized ligament laxity was proposed as the intrinsic cause or risk factor toward the development of the deformity which was difficult to be investigated by cohort clinical trials. Herein, we aimed to evaluate the isolated influence of generalized ligament laxity on the deterioration using computer simulation (finite element analysis). We reconstructed a computational foot model from a mild hallux valgus participant and conducted a gait analysis to drive the simulation of walking. Through parametric analysis,

AIM: The prevalence of severely symptomatic deformities of the first metatarsophalangeal (MTP) joint in adolescents with cerebral palsy (CP) requiring arthrodesis is unknown. Recent literature regarding these deformities is limited. We studied the presentation of severe, symptomatic deformities of the first ray in a large population of children and adolescents with CP and their association with gross motor function, CP subtype, and other musculoskeletal deformities.
METHOD: We identified 41 patients with CP and a symptomatic deformity of the first MTP joint, managed by arthrodesis, from a lar

Surgical correction of bunion deformity in adolescents is performed infrequently. Double osteotomy and longitudinal pin fixation of the first ray was performed on 15 feet. This procedure is technically easy, provides excellent correction and stability, and has a low rate of recurrence of deformity. Five numerical determinations were obtained from preoperative, intraoperative, and final follow-up radiographs. They included intermetatarsal angle, first-fifth metatarsal angle, and metatarsophalangeal angle, first-second metatarsal distance, and length of the first and second metatarsals. The mean

OBJECTIVE: To investigate the clinical effects of the minimal invasive osteotomy treatment for hallux valgus combined with tailor's bunion deformity.
METHODS: From May 2007 to May 2009, Forty-nine feet of 32 patients (a mean age of 57 years old) of hallux valgus combined with tailor's bunion deformity were reviewed retrospectively, including 2 males(2 feet) and 30 females (47 feet). All patients were treated by the minimal invasive osteotomy. The axial and lateral films of all feet with loading were taken before and after operation. The hallux abducto valgus angle(HAV),intermetatarsal angle(I

A long-term retrospective review of a long oblique distal osteotomy of the fifth metatarsal for correction of a tailor's bunion deformity is presented. The technique, results, and follow-up of this procedure are described. The long oblique distal osteotomy was performed on 25 feet on 16 patients. Follow-up ranged from 11 to 110 months with an average of 64.4 months. The average preoperative intermetatarsal 4-5 angle and lateral deviation angle improved from 10.88 degrees and 4.24 degrees, respectively, to 5.52 degrees and -4.24 degrees postoperatively, respectively. A modified scoring scale fo

OBJECTIVE: The first ray plays a role in the development of bunion foot deformity, but for unknown reasons. This prospective, cross-sectional study investigated first ray kinematics in women with rheumatoid arthritis (RA) and bunion.
METHODS: Nine participants having RA-bunion were analyzed in comparison to a control group (n = 10). Data were acquired using a magnetic resonance scanner. Conditions were standardized to simulate gait midstance, heel off, and terminal stance. Foot posture (hallux angle, intermetatarsal angle, arch angle, and calcaneus angle) and relative first ray position angle

Hallux valgus is a prevalent condition. Many open surgical methods of correction have been described. We performed a percutaneous, extra-articular distal metatarsal osteotomy for mild to moderate bunion deformity. The minimally invasive technique was used in 217 feet (180 patients; age 49 ± 4.7 years, mean ± standard deviation) at 4 centers in North America (Center 1: November 2012 to March 2017; Center 2: January 2010 to May 2016; Center 3: October 2013 to June 2016; Center 4: January 2015 to June 2017). The procedure was used in simultaneous bilateral cases in 28 patients (15.6%) and in nons

With the proper instruments, the minimal incision approach to osteotomies of the lesser metatarsals has many advantages. Less trauma, less disability, no need for internal fixation, and generally, less discomfort for the patient are some of them.

The authors have evaluated the long-term results of the Keller/Mayo bunionectomy with replacement of the first metatarsophalangeal joint with a Silastic double-stemmed implant (Swanson design). Examination of 50 preoperative and postoperative radiographs revealed a decrease in the hallux abductus angle of 21.46 +/- 2.66 degrees, and the intermetatarsal angle of 6.40 +/- 0.67 degrees; both of these figures have a statistically significant probability value of 0.05 attached. The mean patient age at time of surgery was 61.31 years; the mean follow-up time was 5.01 months. A literature review of i
