Delayed Tracheoesophageal Fistula and Refractory Respiratory Failure Caused by Inhalation Burns and Emergency Venovenous ECMO to Facilitate Its Management: A Case Report.

Acquired tracheoesophageal fistulae are uncommon in burn patients but can occur as a complication of inhalation injury. We report a case of a 30-yr-old male patient presenting after suffering from inhalation and 25% total body surface area burns. On postburns day 14, he developed a massive tracheoesophageal fistula causing refractory acute respiratory failure. Veno-venous extracorporeal membrane (VV ECMO) oxygenation was initiated without systemic anticoagulation via bi-femoral cannulation under transthoracic echocardiography guidance. He underwent successful 5-hr apnoeic ventilation-assisted

Extracorporeal Life Support for Severely Burned Patients with Concurrent Inhalation Injury and Acute Respiratory Distress Syndrome: Experience from a Military Medical Burn Center.

BACKGROUND: Both inhalation injury and acute respiratory distress syndrome (ARDS) are risk factors that predict mortality in severely burned patients. Extracorporeal life support (ECLS) is widely used to rescue these patients; however, its efficacy and safety in this critical population have not been well defined. We report our experience of using ECLS for the treatment of severely burned patients with concurrent inhalation injury and ARDS. METHODS: This was a retrospective analysis of 14 patients collected from a single medical burn center from 2012 to 2019. All patients suffered from major

Correlation of Ultrasound Examination with FOB for Airway Assessment in Burn Patients with Inhalational Injury: A Prospective Observational Study.

AIM/BACKGROUND: Fiberoptic bronchoscopy (FOB) is the gold standard for assessing airway involvement in burn patients but is invasive. Ultrasound (USG) has not been previously used to evaluate the airway in burn patients. Our study evaluated the feasibility of using USG to assess airway involvement in inhalational burn injury and correlated its efficacy with FOB. MATERIALS AND METHODS: This prospective observational study was conducted in the burns intensive care unit (ICU) of a tertiary care hospital. Bedside airway USG was performed to evaluate vocal cord (VC) width for edema and other airwa

A Newborn Case of Esophagus and Vocal Cord Burn After CS (O-Chlorobenzylindene Malononitrile) Gas Inhalation.

Among the agents used for self-defense, O-chlorobenzylidene malononitrile (CS) is the most commonly used substance. It has been suggested that CS use is characterized by rapid onset of action, short duration of action, and minimal side effects. When the clinical situations and case series resulting from exposure to CS were examined, we saw that no cases of exposure during the neonatal period were identified. A male baby born at 40 weeks, weighing 3260 grams, was brought to the emergency room on the 24th postnatal day with bruising and respiratory distress due to a stranger spraying CS into his

Nebulization with γ-tocopherol ameliorates acute lung injury after burn and smoke inhalation in the ovine model.

We hypothesize that the nebulization of γ-tocopherol (g-T) in the airway of our ovine model of acute respiratory distress syndrome will effectively improve pulmonary function following burn and smoke inhalation after 96 h. Adult ewes (n = 14) were subjected to 40% total body surface area burn and were insufflated with 48 breaths of cotton smoke under deep anesthesia, in a double-blind comparative study. A customized aerosolization device continuously delivered g-T in ethanol with each breath from 3 to 48 h after the injury (g-T group, n = 6), whereas the control group (n = 5) was nebulized wit

Multiple costal cartilage graft reconstruction for the treatment of a full-length laryngotracheal stenosis after an inhalation burn.

After suffering an inhalation burn, a 22-year-old male was intubated for seven days. Full-length massive scar formation in the upper airway necessitated tracheostomy five months later. After this, the stenosis became complete in the cricoid region, and a long cannula was needed to maintain the severely damaged middle-distal trachea. After unsuccessful laser dilatation, the more stenotic 3 cm distal tracheal segment was resected, but two months later the stenosis recurred. As resection was ineffective, tracheoplasty was performed via a right-sided thoracotomy; the re-stenotized trachea was inci

Cardiopulmonary Hemodynamic Alterations During Early Resuscitation in Burn Patients with Inhalation Injury: Do They Impact Prognosis?

Inhalation injury remains a major contributor to morbidity and mortality in critically ill burn patients, although its hemodynamic impact during early resuscitation is incompletely understood. Patients with inhalation injury could exhibit distinct hemodynamic alterations compared with those without inhalation injury, potentially contributing to worse outcomes. We conducted a prospective, single-center observational study over four years including adult patients with major burns. Hemodynamic variables, fluid administration, and analytical biomarkers during initial resuscitation were analyzed ac

Survival and analysis of prognostic factors for severe burn patients with inhalation injury: based on the respiratory SOFA score.

BACKGROUND: It is important to determine the severity of inhalation injury in severely burned patients. The oxygenation index PaO2/FiO2(PF) ratio is a key clinical indicator of inhalation injury. Sequential organ failure assessment (SOFA) is developed to assess the acute incidence of critical illness in the population. We hope to provide an assessment of survival or prognostic factor for severely burned patients with inhalation injury based on the respiratory SOFA score. METHODS: This is a retrospective cohort study of all admissions to Department of Burn and Plastic Surgery at West China Hos

Inhaled methoxyflurane for pain and anxiety relief during burn wound care procedures: an Australian case series.

Pain is a common and significant feature of burn injury. The use of intravenous opioids forms the mainstay of procedural burn pain management, but in an outpatient setting, the demand for novel agents that do not require parenteral access, are easy to administer and have a rapid onset are urgently needed. One such agent is the inhaled anaesthetic agent, methoxyflurane (MF). The aim of this study was to conduct a pilot investigation into the clinical effectiveness of MF inhaler on pain and anxiety scores in patients undergoing burn wound care procedures in an outpatient setting. Beneficial effects of concomitant neuronal and inducible nitric oxide synthase inhibition in ovine burn and inhalation injury.

Different isoforms of nitric oxide (NO) synthase are critically involved in the development of pulmonary failure secondary to acute lung injury. Here we tested the hypothesis that simultaneous blockade of inducible and neuronal NO synthase effectively prevents the pulmonary lesions in an ovine model of acute respiratory distress syndrome induced by combined burn and smoke inhalation injury. Chronically instrumented sheep were allocated to a sham-injured group (n = 6), an injured and untreated group (n = 6), or an injured group treated with simultaneous infusion of selective inducible and neuro

Dim light at night modulates neuroimmune and clock gene responses to burn pit emission inhalation exposure in mice.

Many U.S. soldiers and support personnel stationed in the Middle East and Southwest Asia are exposed to hazardous combustion byproducts from military burn pits, which may produce chronic health effects. In addition, prolonged and irregular work schedules increase susceptibility to circadian rhythm disruption, worsening adverse health outcomes. This study aimed to examine the interaction between light at night and burn pit exposure and their effects on neuroinflammatory and behavioral outcomes in mice. Thirty-two male Swiss Webster mice were housed either in dark nights (LD) or dim light at nig

Effect of ablated bronchial blood flow on survival rate and pulmonary function after burn and smoke inhalation in sheep.

The bronchial circulation plays a significant role in the pathophysiological changes of burn and smoke-inhalation injury. Bronchial blood flow markedly increases immediately after inhalational injury. This study examines whether the ablation of the bronchial artery attenuates pathophysiological changes and improves survival after burn and smoke-inhalational injury in an ovine model. Acute lung injury was induced by 40% total body surface-area third-degree cutaneous burn and cotton smoke inhalation (48 breaths of cotton smoke, <40 degrees C) under deep anaesthesia. Twelve adult female sheep wer

Aerosolized alpha-tocopherol ameliorates acute lung injury following combined burn and smoke inhalation injury in sheep.

Victims of fire accidents who sustain both thermal injury to the skin and smoke inhalation have gross evidence of oxidant injury. Therefore, we hypothesized that delivery of vitamin E, an oxygen superoxide scavenger, directly into the airway would attenuate acute lung injury postburn and smoke inhalation. Sheep (N = 17 female, 35 +/- 5 kg) were divided into 3 groups: (1) injured, then nebulized with vitamin E (B&S, Vitamin E, n = 6); (2) injured, nebulized with saline (B&S, Saline, n = 6); and (3) not injured, not treated (Sham, n = 5). While under deep anesthesia with isoflurane, the sheep we

Superior Effects of Nebulized Epinephrine to Nebulized Albuterol and Phenylephrine in Burn and Smoke Inhalation-Induced Acute Lung Injury.

The severity of burn and smoke inhalation-induced acute lung injury (BSI-ALI) is associated with alveolar and interstitial edema, bronchospasm, and airway mucosal hyperemia. Previously, we have reported beneficial effects of epinephrine nebulization on BSI-ALI. However, the underlying mechanisms of salutary effects of nebulized epinephrine remain unclear. The present study compared the effects of epinephrine, phenylephrine, and albuterol on a model of BSI-ALI. We tested the hypothesis that both α1- and β2-agonist effects are required for ameliorating more efficiently the BSI-ALI. Forty percent
