An excessive prominence of the bursal projection in the posterosuperior aspect of the calcaneous constitutes Haglund's deformity. Swelling in this area constitutes Haglund's disease and is associated with retrocalcaneal bursitis. Rigid and prominent heel counters with high heels impinge on the soft tissues overlying the prominence and give rise to symptoms of pain and swelling. Cavo varus deformities exacerbate this problem. The bursal projection can be demonstrated radiologically by a superior calcaneal angle of more than 75 degrees, a combination of calcaneal inclination and a posterior calc

A 47-year-old man with cirrhosis developed a case of previously unreported olecranon bursitis due to Cryptococcus neoformans. Most patients with disseminated cryptococcosis have deficiencies in cell mediated immunity. Cirrhosis may be an independent risk factor because of impaired chemotaxis and phagocytosis.

Trochanteric bursitis is one of the most common soft tissue abnormalities in the hip region, yet it is frequently underdiagnosed. There is a characteristic pattern of uptake seen on the bone scan in patients with this condition. Its recognition is important to avoid misinterpretation of increased trochanteric uptake, particularly in patients with malignant disease, and to aid in the diagnosis and effective management of this treatable condition.

INTRODUCTION: Phaeohyphomycosis is a spectrum of infections caused by dematiaceous pigmented fungi.
CASE REPORT: A 57 year old man with uncontrolled type 2 diabetes mellitus presented with a well circumscribed swelling of the posterior aspect of the left elbow. Clinical work up was done and a provisional diagnosis of tuberculous bursitis was made. Excision of the lesion was done. Histopathological examination showed a fungal abscess with features of subcutaneous phaeohyphomycosis.

Trochanteric bursitis has been used as a general term to describe pain around the greater trochanteric region of the hip. We hypothesised that trochanteric bursitis may not however have an inflammatory component and that accordingly, bursal inflammation has no role in lateral hip pain. This study was designed to test this hypothesis. Patients undergoing primary total hip replacement were enrolled in this prospective, case-controlled, blinded study. Twenty-five patients who met the criteria for diagnosis of trochanteric bursitis (group A) were matched with a control group of 25 patients (group

This case report describes a 36-year-old African American male with cerebral palsy and bilateral slowly enlarging knee masses. He has 90 degrees fixed flexion knee contractures bilaterally. Although he has poor communication skills, he does not have discomfort while ambulating. He has developed massive bilateral prepatellar bursitis from chronic and repetitive injury to the region bearing his body weight while ambulating. As the result of a protective response, the bursa provides a cushion for the underlying bone prominences of the tibial tubercle and patella. This compensatory mechanism has a

Peripatellar lesions causing knee pain include cellulitis, bursitis, synovitis, septic arthritis and patellar osteomyelitis. We present here two cases of patellar osteomyelitis which were misdiagnosed as prepatellar bursitis. Operative treatment was required for these lesions. Patellar osteomyelitis should always be considered when treating a peripatellar lesion, although it is very rare condition.

We report two rare cases of iliopsoas bursitis extending into the retroperitoneal space. The first lesion contained much gas, mimicking a retroperitoneal abscess, and the second was responsible for atypical inguinal pain. The diagnosis was made by contrast-enhanced CT in both cases and arthrography in the first case. Iliopsoas bursitis in these two patients, it is hypothesized, extended into the retroperitoneum, at least in part, by way of intraneural or perineural structures.

Four patients, three men aged 69, 55 and 45 years and one woman aged 72 years, complained of pain in and around the inguinal region without clear cause. The crippling symptoms were of a few days' to 8 years' standing. Passive hip movements were not restricted. Presence of pain after pressure on or stretching of the bursal wall during provocation tests and absence of other disorders resulted in a probability diagnosis of iliopectineal bursitis. This diagnosis was confirmed by marked abatement of the pain after an injection of lidocaine into the bursa. Inflammation of the iliopectineal bursa may

BACKGROUND: The cause of musculoskeletal symptoms in the proximal extremities of patients who have polymyalgia rheumatica is not completely understood. The diffuse and severe discomfort can only be partially explained by the mild joint synovitis that is observed in these patients.
OBJECTIVE: To determine the involvement of the synovial structures of the shoulder girdle of patients who have active symptoms of polymyalgia rheumatica.
DESIGN: Case-control study.
SETTING: 2 secondary referral centers of rheumatology.
PATIENTS: 13 case-patients who had active symptoms of polymyalgia rheumatica

We reviewed a single surgeon's experience with the surgical management of refractory trochanteric bursitis in an active population group. The surgical procedure consisted of simple longitudinal release of the iliotibial band over the greater trochanter and excision of the subgluteal bursa. To our knowledge, this is the first series to use this technique. Seven hips in five patients form the basis of the study, making it one of the largest surgical series reported. All patients had no improvement after a minimum of 1 year of nonoperative management consisting of nonsteroidal antiinflammatory me

Trochanteric bursitis, a common regional pain syndrome, is characterized by chronic, intermittent aching pain over the lateral aspect of the hip. The incidence of trochanteric bursitis peaks between the fourth and sixth decades of life, but cases have been reported in all age-groups. The diagnosis may be elusive, especially if symptoms are atypical. This condition can be associated with pain and limitation of function. Treatment includes physical therapy measures, analgesics, and local glucocorticoid injection. In this article, we review the pathogenesis, common initial symptoms, diagnostic ap

OBJECTIVE: To present a case of conservatively managed calcific trochanteric bursitis and discuss differential diagnosis and treatment alternatives.
CLINICAL FEATURES: A 53-yr-old obese female presented with acute severe lateral hip and posterolateral thigh pain. Examination findings were consistent with trochanteric bursitis and radiographs demonstrated calcific infiltration of the trochanteric bursa.
INTERVENTION AND OUTCOME: An intensive 2-wk course of pulsed ultrasound, ice massage, interferential current and chiropractic lumbopelvic manipulation resulted in symptomatic relief, abolishme

In 72 patients followed for two years, the diagnosis of trochanteric bursitis was based on precise clinical signs without significant contribution from laboratory tests or radiologic signs. In six of the patients (8.3%) the bursitis was an isolated condition of unknown origin. Six other patients had rheumatoid arthritis without involvement of the ipsilateral hip joint. The remaining 60 patients (83.4%) had pathologic conditions of the adjacent areas, such as symptomatic lumbar spine arthrosis or ipsilateral hip damage, conditions that often mask the actual source of pain and disability. Thirty-six cases of simple trochanteric bursitis were evaluated, particular in regard to corticosteroid injections. The syndrome was mostly chronic, prevalent in older females, interspersed with other diseases. Diagnostic criteria are purely clinical. One or two local corticosteroid injections gave excellent response in two-thirds, improvement in the remaining cases. One-fourth relapsed in 2 years. Trochanteric bursitis should always be considered in hip pain syndromes, as it is so easily relieved.

A previously healthy 65-yr-old man developed clinical signs of prepatellar bursitis. Aerobic cultures from fluid aspirated from the inflamed prepatellar bursa were negative. Direct microscopic examination of the fluid showed gram-positive cocci, and anaerobic cultures grew Streptococcus hemalyticus group A. The bacterial etiology of bursitis has recently been emphasized in the literature; in most of our cases Staphylococcus was responsible. Negative cultures were next in frequency. Other microorganisms may indeed be responsible for prepatellar bursitis and should be sought where aerobic cultur
