Patterns of abnormality in cVEMP, oVEMP, and caloric tests may provide topological information about vestibular impairment.

BACKGROUND: The cervical vestibular evoked myogenic potential (cVEMP) is recorded from the sternocleidomastoid muscle (SCM) and represents a stimulus-evoked attenuation of electromyographic (EMG) activity following activation of the saccule and inferior vestibular nerve. In addition to the cVEMP, it is possible to record a biphasic response from the infraorbital region following stimulation that is identical to that used to record the cVEMP. This response is known as the ocular VEMP (oVEMP). The peripheral vestibular origins of the oVEMP elicited with air conduction remain controversial as som

Towards an optimized monothermal caloric screening test: comparing stimuli and thresholds.

OBJECTIVES: Monothermal caloric testing (MCT) shows potential as a screening method in vestibular assessment; however, the optimal stimulus modality and diagnostic threshold remain unclear. This study aimed to identify, within a single investigation, the most effective stimulus type (air or water; warm or cool) and the optimal cut-off threshold (15% or 25%) for maximising the diagnostic performance of MCT. DESIGN: Retrospective study. STUDY SAMPLE: Bithermal caloric test (BCT) results from 202 adults (103 water, 99 air) were analysed. MCT results were assessed at 15% and 25% thresholds based

Persistent Postural Perceptual Dizziness Induced by Caloric Testing: A Case Report.

BACKGROUND: The condition of persistent postural perceptual dizziness (PPPD) is a common chronic vestibular disorder that is thought to be due to a central vestibular maladaptation. The condition is frequently encountered in those with a history of migraine, anxiety, or panic attacks. Many cases of PPPD are preceded by vestibular disorders, with vestibular migraine thought to precede at least 20% of cases. PURPOSE: Those with a history of migraine have been shown to have difficulties with sensory integration, habituation, and intolerance to stimuli. This often leads to discomfort associated w

The effect of migraine and motion sickness on symptoms evoked by the caloric vestibular test.

BACKGROUND: The caloric vestibular test (CVT) may evoke headache and vestibular symptoms in susceptible people. Patients with migraines have higher susceptibility to motion sickness. In migraines, impaired habituation to repetitive stimuli is a well-known interictal abnormality. OBJECTIVE: This study is aimed at evaluating CVT-evoked headache, nausea, vomiting, and imbalance in patients with and without migraine and/or motion sickness. METHODS: A retrospective data analysis was performed on 554 patients with a complaint of dizziness who underwent bithermal CVT at a tertiary referral center.

Menière's Disease and Caloric Stimulation: Some News from an Old Test.

OBJECTIVES: The aim of the present study was to improve the instrumental diagnosis of assessing Menière's disease (MD) if the frequency and slow-phase velocity (SPV) of the thermally induced nystagmus analyzed through the caloric vestibular test (CVT) showed different alterations in relationship with an increasing severity of the cochlear involvement. MATERIALS AND METHODS: The study retrospectively analyzed the CVT results of 72 patients affected by unilateral "definite MD" according to the 2015 Barany Society Diagnostic Criteria and treated only conservatively. RESULTS: There were 7 (9.72%

Video head impulse in comparison to caloric testing in unilateral vestibular schwannoma.

CONCLUSIONS: Although there was a statistically significant relationship between the results of the vHIT and the caloric test, the limited strength of this relationship suggests that, for unilateral vestibular schwannoma (UVS), caloric testing and vHIT may provide complementary information on vestibular function. OBJECTIVE: There is limited information that can be used to determine which of the video head impulse test (vHIT) and caloric test might be better used in the diagnosis and management of UVS. In this study, a group of participants with un-operated UVS was studied using both methods.

Subclassification of vestibular disorders by means of statistical analysis in caloric labyrinth testing.

OBJECTIVE: In the past, various attempts were made to perform a quantitative analysis of nystagmographic findings but their diagnostic value was limited. Therefore, the authors present a multivariate analysis of nystagmus findings with the aim of increasing the precision of diagnostic differentiation in cases of vestibular dysfunction. MATERIAL AND METHODS: A group of 387 patients and 40 healthy volunteers were examined over a 14-month period using electronystagmography after stimulation by bithermal, bilateral irrigation of the labyrinth. Amplitude, slow-phase velocity, frequency and directi

Early vestibular dysfunction in Machado-Joseph disease detected by caloric test.

The selective vulnerability of distinct neuronal structures is a major feature of Machado-Joseph disease (MJD), also known as spinocerebellar ataxia 3 (SCA3). Vestibular dysfunction is known to be a symptom of MJD, but little is known about precisely when the vestibular system becomes impaired. Using a caloric test, we evaluated vestibular function in 2 MJD patients. One developed the initial symptom 1 year before evaluation, and the other 3 years before evaluation. Neither demonstrated a bilateral response in electronystagmography using ice-cold water irrigation, indicating severe vestibular

Relationship between video head impulse test (vHIT) and caloric test in patients with vestibular neuritis.

INTRODUCTION AND OBJECTIVES: The caloric test is the gold standard for the loss of vestibular function diagnosis. The Video Head Impulse Test (vHIT) assesses the same reflex by using a video- assisted examination of the impulsive maneuver. We intend to compare the variation of results of the vHIT and the caloric test in patients with vestibular neuritis with respect to their initial condition at two different moments of their evolution and to check the level of correlation between them and with that of the DHI test METHODS: We explored 20 patients with neuritis by using both vHIT and the calo

Oculomotor and Caloric Test Findings in 122 Patients With Unilateral Vestibular Schwannoma.

Introduction Vestibular schwannoma (VS), a benign neoplasm of the eighth cranial nerve, can disrupt both peripheral vestibular function and central oculomotor pathways. The caloric test evaluates the vestibulo-ocular reflex (VOR) pathway, while oculomotor testing assesses central neural mechanisms. In this study, caloric and oculomotor assessments were performed in patients with unilateral VS to examine peripheral and central pathway integrity. Materials and methods The study recruited 122 patients (aged 18-55 years) with unilateral VS. It encompassed 60 patients with right-sided VS (Rt-VS) an

Caloric and video head impulse test dissociated results in dizzy patients.

INTRODUCTION: We are now able to detect abnormalities for any semicircular canal with the use of the video head impulse test (vHIT). Prior to the vHIT, the gold standard for unilateral canal paresis of the lateral canal was considered the caloric test. Clinical cases where the caloric test and vHIT are discordant are not uncommon. METHODS: Retrospective study. All consecutive cases of dizziness seen from 11/2020 to 12/2021 for which the patient underwent both caloric and vHIT tests performed within 10 days, were reviewed. Patients with discordant results were included. We evaluated the calori

A head-tilt caloric test for evaluating the vertical semicircular canal function.

CONCLUSIONS: The caloric test with head-tilt can be used as a tool for assessing vertical canal function as an office procedure. OBJECTIVE: Evaluation of vertical canal function. PATIENTS AND METHODS: We provoked caloric response by cold water in the vertiginous patients in supine position. During the culmination of the response we rotated the head 45 degrees from the sagittal plane to place the posterior canal to earth-vertical. Thereafter we rotated the head 45 degrees to the opposite direction to place the anterior canal to earth-vertical. The eye movements were recorded by two-dimensiona

Analytic view to concordance between electrocochleography and caloric test in Meniere's disease.

We aimed to investigate if there is a concordance between summation potential (SP)/action potential (AP) ratio and unilateral weakness in patients with definite-Meniere's disease. There were two groups, a group of unilateral definite-Meniere patients who received no treatment and another group of control subjects. Twenty-six patients were identified as complying with the defined criteria. Fifteen healthy subjects with no hearing and balance disorders were assigned to the control group. Arithmetic mean of the four-tone average of thresholds at 0.5, 1, 2, and 3 or 4 kHz, SP/AP ratio and degree o

Influence of caloric and rotation testing upon blood pressure and pulse rate.

Vestibular stimulation is able to produce neurovegetative effects, which are disagreable and annoying. By an overflow of the vestibular stimulus to the neurovegetative centres changes in blood pressure and pulse rate are more frequently induced than are subjective neurovegetative symptoms as nausea, pallor etc. There is some correlation between both manifestations, but also some independency in occurrence.

Vestibulo-ocular reflexes in peripheral labyrinthine lesions: II. Caloric testing.

In a group of patients with unilateral labyrinthine dysfunction, the maximal velocity of the slow component response to caloric stimulation was compared with that to horizontal vestibular rotatory stimulation by a low-frequency stimulus. It was found that the larger the magnitude of the caloric response, the larger were the gain and sensitivity of the vestibulo-ocular reflex induced by physiologic stimuli. Rotary stimulation of patients with only one functioning inner ear provides information equivalent to that obtained from caloric stimulation and allows more rigorous quantification of the st

Effects of endolymphatic space volume on discrepancy of results between caloric test and video head impulse test.

OBJECTIVE: The caloric test (C-test) and video head impulse test (vHIT) are known to occasionally show contradictory results in patients with Meniere's disease (MD). The reasons underlying this discrepancy between the two tests are currently unclear. We aimed to reveal the mechanisms responsible for this discrepancy by performing volumetric evaluation of the endolymphatic space (ELS) by using endoluminal contrast-enhanced inner ear MRI (ieMRI). METHODS: We enrolled 136 patients (174 ears) who visited the vertigo/dizziness center of our university and underwent the C-test and vHIT between Febr

Comparison of Video Head Impulse Test and Caloric Reflex Test in advanced unilateral definite Menière's disease.

AIMS: There have been very few studies of the Video Head Impulse Test (VHIT) in patients with Menière's Disease (MD). Some reported 100% normal VHIT results, others not. These discrepancies may be due to differences in severity. The present study compared VHIT and caloric reflex test results in advanced unilateral definite MD. MATERIAL AND METHOD: A prospective study included 37 consecutive patients, with a mean age of 56±12 years. Mean hearing loss was 59±18dB HL; 12 patients were subject to Tumarkin's otolithic crises. Abnormal caloric reflex was defined as ≥20% deficit, and abnormal VHIT a
