Cancer-related pain: A bidirectional modulator of cancer progression.

BACKGROUND: Cancer-related pain (CRP), with a high prevalence and prognostic value in malignancies, is not only a passive phenomenon but rather engages in a bidirectional crosstalk with cancer progression. MAIN TOPICS: This review focuses on the intricate reciprocal relationship between CRP and cancer progression, delineating how advanced cancers promote CRP directly through tissue destruction, invasion and inflammation, or indirectly via inflammatory, neural, stromal and metabolic components within the tumour microenvironment. However, controversies remain in the field, such as the ambiguous

Drezotomy and Myelotomy for Cancer Pain.

BACKGROUND: When conservative treatment, including intrathecal infusion of pharmacological agents, or even multimodal therapies are not sufficiently effective, if their respective benefits-risks balances are favorable, Drezotomy and myelotomy, ablative surgeries targeting sensory circuits in the spinal cord, may still have a place today for well-selected patients. SUMMARY: In this review article, the rationale and technical principles and their current potential indications, based on their results published, are described. Drezotomy have been reported effective for topographically limited can

Chronic Pain in the Cancer Survivor.

PURPOSE OF REVIEW: As cancer survivorship continues to rise, attention has increasingly turned to the long-term sequelae of oncologic treatment. While interventions such as chemotherapy, radiation therapy, and surgery have significantly improved survival outcomes, they are frequently associated with chronic pain syndromes that can profoundly impact quality of life. This review synthesizes current evidence and therapeutic approaches for three prevalent pain conditions in cancer survivors: chemotherapy-induced peripheral neuropathy, post-radiation pain syndromes, and post-surgical pain syndromes

Methadone for treatment of cancer pain.

Methadone is a unique mu opioid agonist, which also has delta receptor affinity and properties of N-methyl-D-aspartate receptor antagonism and monoamine reuptake inhibition. It is mainly used in the setting of uncontrolled pain or dose-limiting toxicity. Caution is advised when switching to methadone, especially from high doses of previous opioid, due to its variable conversion ratio and the potential for delayed toxicity due to its long half-life. Increasing evidence of risk also exists for a prolonged QT interval and torsades de pointes with very large doses of methadone. Methadone is likely

The pharmacologic management of cancer pain.

The management of cancer pain requires familiarity with a range of therapeutic strategies, including antineoplastic therapies, analgesic pharmacotherapy, and anesthetic, neurosurgical, psychological, and rehabilitation techniques. Successful pain management is characterized by implementation of the techniques with the most favorable therapeutic index for the prevailing circumstances, along with provision for repeated evaluations, so that a favorable balance between pain relief and adverse effects is maintained. For most patients, pain management involves the administration of specific analgesi

Neuropathic pain in patients with cancer.

We provide a detailed description of painful neural lesions in hospitalized patients with cancer. A total of 187 consecutive patients with cancer and pain, referred to the pain service of a cancer hospital, were evaluated within 24 hours by two neurologists and followed until discharge or death. Based on history, pain descriptors, physical examination, and radiological and electrophysiological studies, the pain was categorized as neuropathic in 103 patients. The most frequent sites of neurological injury were nerve roots, spinal cord and cauda equina, brachial and lumbosacral plexus, and perip

Ketamine in cancer pain: an update.

Ketamine has been shown to have potent analgesic properties at low dosages. Bioavailability is high when it is given parenterally, but low after oral or rectal administration. Active metabolites should account for part of the analgesic effect of ketamine during long-term oral administration. NMDA receptor inhibition by ketamine may reflect a wind-down phenomenon, and should alleviate NMDA-related neuropathic pain, reversing the rightward shift of the opioid-response curve. A synergistic effect between ketamine and opioids has been observed in cancer pain patients who have lost an analgesic res

[The psychological aspect of cancer pain.].

The perception and expression of pain are primarily psychological phenomena and are not directly correlated with the intensity of the nociceptive stimulus. They are often influenced by earlier experiences of pain, and also by current expectations and fears. The cancer patient interprets pain as a sign of the continued existence and of the progression of the malignant disease: if the pain can be controlled the patient will take this as an indication that the underlying disease can be cured. Inappropriately treated pain, on the other hand, can initiate a vicious circle leading to really excrucia

Analysis of cancer pain by the neurologist.

Pain intensity and the importance of pain for the cancer patient depend on a variety of individual factors and show considerable divergence (Bonica and Ventafridda 1979). They are greatly influenced by the enigma of the cancer disease as well as the consequential psychiatric problems (Frey 1980). An effective neurological analysis of the cancer pain, although demanding for the physician, is important for further specific diagnostic procedures and for the indication of appropriate therapy. Sometimes thorough neurological examination can lead to the first diagnosis of the carcinoma; when there i

Transdermal fentanyl for pain control in adults with chronic cancer pain.

The transdermal therapeutic system (TTS) for fentanyl is a drug-delivery system for use in patients with chronic pain who require an opioid analgesic. A multicentre, randomized, double-blind, placebo-controlled study was performed to evaluate the efficacy and safety of TTS-fentanyl as an analgesic for chronic cancer pain. One hundred and thirty-eight patients entered a 15-day dose-titration period, followed by a 9-day double-blind period (95 patients) with TTS-fentanyl or placebo. Fifty-five patients entered a follow-up period of indefinite duration. For the majority of patients, TTS-fentanyl

Intrathecal Infusion in Cancer Pain Surgery.

BACKGROUND: Cancer pain is a complex and multifactorial phenomenon that significantly impairs quality of life, particularly in advanced stages of malignancy. Although systemic pharmacologic therapies remain the cornerstone of management, a substantial proportion of patients experience refractory pain or intolerable side effects. Intrathecal drug delivery (ITDD) offers a targeted method of analgesia by administering medications directly into the cerebrospinal fluid, enabling effective pain control at lower doses while minimizing systemic toxicity. SUMMARY: This review explores the physiologica

Cancer pain: molecular mechanisms and management.

Cancer pain, a highly prevalent and distressing symptom among cancer patients, has a seriously harmful effect on their life and presents a complex challenge in clinical management. Despite extensive research efforts and the existence of clinical guidelines, significant controversies persist regarding the molecular mechanisms underpinning cancer pain as well as the most effective management strategies. This review systematically delves into the neurobiological underpinnings of cancer pain, centering on the interplay of peripheral and central sensitization, cellular stress and dysfunction, as we

Cancer Pain and Opioid Use Disorder.

Opioid use disorder (OUD) is increasingly recognized and co-present in patients with cancer. Unfortunately, OUD is not addressed or treated adequately in oncology settings. In addition, patients with cancer-related pain treated with narcotic pain medications are at risk for nonmedical opioid use (NMOU). More than two-thirds of patients with advanced cancer have pain. Both OUD and NMOU need to be concomitantly addressed alongside cancer-related pain management to avoid complications such as overdose. We review the approach to identifying and treating OUD and NMOU in patients with cancer and can

Methadone switching for refractory cancer pain.

BACKGROUND: Methadone is commonly considered an alternative opioid treatment for refractory cancer pain. This study aims to investigate the efficacy, safety, and cost of methadone in the treatment of refractory cancer pain. METHODS: A retrospective study was conducted in patients who used methadone for refractory cancer pain from April 2016 to December 2020 at a cancer specialized hospital. Pain control, evaluated via pain score and breakthrough pain frequency, and adverse events of methadone were compared with analgesic regimens prior to methadone administration. The factors potentially affe

Neurosurgical Management of Cancer Facial Pain.

Facial pain occurs in approximately 80% of patients with head and neck cancers. Pain in these settings may result directly from the tumor, or indirectly as a side effect of oncological treatment of the tumor. Optimizing treatment for cancer pain of the face, therefore, involves a variety of diagnostic and treatment considerations, with the development of a successful treatment algorithm dependent on accurate diagnosis of the anatomical location of the pain, its relationship to the facial pain pathway, the type of pain being treated and, finally, patient's prognosis and preference for treatment

Surgical/radiological interventions for cancer pain.

Pain is a major morbidity associated with cancer and up to 20% patients require invasive procedures for pain relief. Ablative techniques can be directed towards the spinal cord and brain to palliate pain or modify its perception. Anterolateral cordotomy, myelotomy, DREZotomy and cingulotomy are useful interventions for the management of refractory cancer pain. Advanced imaging modalities, including intraoperative computed tomography (CT) guidance, have increased safety and efficacy of these interventions. In this paper, authors review the recent literature regarding surgical interventions for

Acupuncture for cancer-induced bone pain?

Bone pain is the most common type of pain in cancer. Bony metastases are common in advanced cancers, particularly in multiple myeloma, breast, prostate or lung cancer. Current pain-relieving strategies include the use of opioid-based analgesia, bisphosphonates and radiotherapy. Although patients experience some pain relief, these interventions may produce unacceptable side-effects which inevitably affect the quality of life. Acupuncture may represent a potentially valuable adjunct to existing strategies for pain relief and it is known to be relatively free of harmful side-effects. Although acu

[Non-opioid analgesics in cancer pain].

Non-opioid analgesics (NSAIDs, acetaminophen) are the most basic drugs used from the first to third steps according to the WHO analgesic ladder. Even after the initiation of the use of opioids, improvement in pain is observed by the continuation of non-opioid analgesics. NSAIDs have anti-inflammatory effects and are particularly effective against somatic pain represented by that due to bone metastasis. On the other hand, NSAIDs should be prescribed with adequate attention being paid to side effects (gastrointestinal disorders and nephropathy) due to long-term administration. Acetaminophen with
