Delusional misidentification syndromes (DMSs) are a fascinating group of disorders involving a fixed, false belief about the identity of persons, places, and objects in one's environment. DMSs include Capgras syndrome, Frégoli syndrome, intermetamorphosis, the syndrome of subjective doubles, and reduplicative paramnesia. Although DMSs have been described in patients with focal neurological lesions, they are more commonly associated with neurodegenerative diseases, specifically dementia with Lewy bodies. Lesion network mapping was used to identify brain regions functionally connected to the les

BACKGROUND: Capgras delusion is scientifically important but most commonly reported as single case studies. Studies analysing large clinical records databases focus on common disorders but none have investigated rare syndromes.
AIMS: Identify cases of Capgras delusion and associated psychopathology, demographics, cognitive function and neuropathology in light of existing models.
METHOD: Combined computational data extraction and qualitative classification using 250 000 case records from South London and Maudsley Clinical Record Interactive Search (CRIS) database.
RESULTS: We identified 84 i

ABSRACT Capgras syndrome is a distressing delusion found in a variety of neurological and psychiatric diseases where a patient believes that a family member, friend, or loved one has been replaced by an imposter. Patients recognize the physical resemblance of a familiar acquaintance but feel that the identity of that person is no longer the same. Here we describe a 73-year-old male with right posterior frontal and bilateral anterior-medial frontal damage from prior brain trauma with a similar delusion of an imposter replacing his pet cat. Misidentification syndromes for animals, as opposed to

BACKGROUND: Capgras' delusion has captured psychiatrists' imaginations, but the clinical features of the delusion have rarely been studied and presented systematically.
AIMS: The present study systematically reviews all case reports on Capgras' delusion in the English language in order to better understand differences between organic and functional aetiologies.
METHODS: All medical and psychiatric databases were searched, as were the bibliographies of published case reports, narrative reviews, and book chapters.
RESULTS: A total of 258 cases were identified from 175 papers. Functional Capgr

Electroencephalographic investigation of 21 patients with delusional misidentification syndromes (11 with the syndrome of Capgras, 7 with the syndrome of Frégoli, 2 with the syndrome of intermetamorphosis and 1 with the syndrome of subjective doubles) revealed a high abundance and severity of abnormalities. The findings are discussed within the framework of a possible organic (and more specifically dysrhythmic) contribution to the pathogenesis of these syndromes.

Since the historical contribution of Capgras and Reboul-Lachaux, several researchers began exploring various dimensions of the fascinating phenomena of delusional misidentification syndromes. Recently, Mulholland and O'Hara have reported a new variant of delusional misidentification syndromes named 'delusional hermaphroditism'. In this study, we report another case of this rare entity.

Six consecutive patients who had had temporal lobe surgery for epilepsy, and been referred for psychiatric assessment of psychotic symptoms, are reported. Their symptoms (a delusional depression, four schizophrenia-like illnesses, and a case of Capgras' syndrome) are discussed in relation to the possible role of their operations, all of which were on the right hemisphere.

A patient manifesting an acute psychosis after receiving an injection of procaine penicillin is reported. The psychosis began immediately after drug administration and gradually abated over a forty-eight-hour period. The clinical presentation was dominated by paranoid delusions and a Capgras-like syndrome. Sixty-six previously reported cases were identified and reviewed. Patients manifested combinations of fear, auditory hallucinations, somatic hallucinations, visual hallucinations, and paranoid or religious delusions. The syndrome may occur with inadvertent intravenous injection of procaine a

Vertebro-basilar insufficiency produces a rich spectrum of psychological and neurological symptoms. Where psychological symptoms dominate the picture, the patient may be presented first to a psychiatrist. The phenomenology of vertebro-basilar insufficiency is discussed with special reference to hallucinatory syndromes, memory disturbance, affective disorders, akinetic mutism, 'unusual reports', cortical blindness, agitated delirium, the Capgras syndrome and normal pressure hydrocephalus. Finally, the case of a 61-year-old man illustrating a variety of the neurological and psychological feature

Misidentification syndromes have been studied from a variety of perspectives, including phenomenological, biological, and nosological approaches. More recently, misidentification syndromes have been studied from a psychiatric-legal perspective, especially with regards to the problem of dangerousness. Capgras syndrome and other syndromes of misidentification can lead to hostile mood and subsequent physical violence. Little attention has so far been devoted to children as the objects of the psychotic person's misidentification delusion(s). We provide a review of cases from the anglophonic litera

Dementia with Lewy bodies (DLB) is the third most frequent type of dementia. The range of symptoms it causes is quite broad and includes parkinsonism, autonomic dysfunction, cognitive deficits, visual and auditory hallucinations, dream disturbances, and other unusual manifestations such as "sense of presence," Capgras syndrome, and fluctuations in neurologic status. The author, an experienced behavioral neurologist and previous Continuum author, has been diagnosed with DLB and assumes the dual role of physician and patient. Each role provides a unique perspective. He discusses the importance o

Recently, reduced platelet monoamine oxidase (MAO) activity was demonstrated in two cases of paranoid schizophrenia exhibiting Capgras' syndrome. The present authors report platelet MAO studies in an additional patient with the Delusion of Doubles. In this case, one of delusional hyperidentification (false recognition), decreased platelet MAO activity compared with normal controls was detected. The patient, while not schizophrenic, showed platelet MAO activity comparable with that of a chronic schizophrenic control group. The possible significance of the findings are discussed.

Syndromes of delusional misidentification consist of disordered familiarity and have been reported in diverse diagnoses, including Parkinson's disease. Although the most common delusional misidentification is Capgras syndrome, in which the sufferer believes a familiar person has been replaced by an identical imposter, other forms have been also described. The pathogenesis of delusions of misidentification appears to require dysfunction of or connection to a left cerebral cortical area involved in recognition of familiarity, and also right frontal cortex serving belief evaluation. Two cases of

Delusional misidentification syndromes (DMS) have been rarely reported in patients with conditions other than schizophrenia-related disorders, diffuse brain disease (dementia) and focal neurological illness. In this report, we describe DMS (i.e. Capgras and Fregoli syndromes) in two patients with severe and treatment resistant obsessive-compulsive disorder (OCD), one with paranoid personality disorder (PPD) and the other with a pervasive developmental disorder (PDD) not otherwise specified. While our findings highlight an interesting phenomenon (the occurrence of DMS in OCD), it is presently u

Psychotic symptoms and other neuropsychiatric manifestations in dementia are distressing conditions that deeply affect patients, carers, and staff. Low awareness and doubts about the competence of primary care services to intervene contribute to underdiagnosis and suboptimal management. We critically discuss what may and may not be done in primary care, building on the case of a 79-year-old man with dementia who developed Capgras delusion (a typical misidentification syndrome), believing his wife was replaced by impostors. We highlight diagnostic and therapeutic challenges, discussing practica

Misidentification syndromes or phenomena are found in a number of psychiatric situations that may become the subject of forensic science review. One of the most curious is misidentification of self in which the individual perceives himself or herself as another being while able to explain the loss of the original identity. Recognizing these phenomena may be helpful in accurate diagnosis, in considering such conditions as psychosis of whatever type, multiple personality disorder, and other amnesia and fugue states, and in understanding the person's psychopathology. Two cases are presented to il

In this case report, we present a young female patient exhibiting pathological misidentification symptoms, in which we employ differential diagnostic methods to distinguish between Capgras syndrome and Fregoli syndrome. The uniqueness of the case lies in the fact that the object of misidentification is not a human being but an animal - in this instance, a dog. Moreover, the underlying psychiatric disorder does not conform to the typical patterns commonly associated with such symptom formations. Following the presentation of findings from a comprehensive psychodiagnostic evaluation, we interpre

Dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD) have frequent psychological and behavioral symptoms (PBS). Treatment of these PBS is variably validated We carried out a review of the treatment of PBS in DLB and PDD from 2009 to 2023. Where the literature allowed, we made treatment recommendations. When the literature was insufficient, we used the Delphi method. The following proposals were made: for pervasive and criticized hallucinations, donepezil or rivastigmine; for delusion, and Capgras syndrome (Delphi), low doses of clozapine; for depression, sertraline, venlafaxi
