We report a rare case of capitate osteonecrosis in a 51-year-old woman with thalassemia presenting with persistent wrist pain. Imaging findings were consistent with osteonecrosis of the capitate, and the patient underwent surgical treatment with a vascularized bone graft from the second metacarpal and cancellous bone graft from the iliac crest. Histopathological examination confirmed ischemic osteonecrosis. At 15 months after surgery, clinical improvement was limited; however, magnetic resonance imaging demonstrated preservation of capitate morphology and reduced signal abnormalities. Capitate

Isolated fractures of the capitate, though rare, are probably often missed entirely. As in scaphoid fractures, repeated radiographs, tomography, or bone scanning are necessary for the diagnosis. Reviewing the literature, we found that most capitate fractures reported were transverse or combined with other carpal injuries, including the scapho-capitate fracture syndrome. Our patient presented an isolated oblique intraarticular fracture of the capitate. An old fracture of the styloid process of the ulna was seen at the primary examination. This special type of capitate fracture has not been prev

The scapho-capitate fracture syndrome is a rare but severe injury of the carpus. It is characterized by a rotation of the proximal fragment of the capitate bone of 90 or 180 degrees and a fracture of the navicular bone in the intermediate third. This injury is caused by a fall on the outstretched and dorsiflected hand. The scapho-capitate fracture syndrome was firstly described in 1956 by Fenton. There are 31 cases described in literature but none of these involves both hands. Here we describe the first case of scapho-capitate fracture syndrome in both hands, in a 21 year old woman who jumped

Capitate and hamate fractures are infrequent injuries and are uncommon in isolation. A capitate fracture is usually associated with a scaphoid fracture. The primary mechanism of injury is a fall with the wrist in hyperextension. Other possible ways for capitate fractures are axial down trauma of the third metacarpal and direct trauma. Hamate fractures have a 2% incidence among carpal bone fractures, probably due to underreporting. They can occur on the hamate body or the hamulus or hook. Combined capitate and hamate fractures are uncommon and relevant studies, especially case reports, are scar

Avascular necrosis (AVN) of the capitate bone is a rare condition and it can be related to major trauma or idiopathy. Different treatments are available including soft tissue interposition and intercarpal arthrodesis including lunocapitate, scaphocapitate, four corner, and carpometacarpal fusions. Other surgical options are resection of the proximal pole and revascularization procedures. The main purpose of this article is to present two cases of AVN of the capitate treated with a revascularization procedure using the 4th-5th extensor compartment artery (4th-5th ECA). Two female patients with

PATIENT: Male, 13 FINAL DIAGNOSIS: Isolated capitate bone fracture Symptoms: -
MEDICATION: - Clinical Procedure: - Specialty: -
OBJECTIVE: Unusual clinical course.
BACKGROUND: Carpal fractures often appear in men under the age of 40 years. Isolated fracture of the capitate without dislocation is very rare and comprises 1% of all carpal fractures. Nonunion of capitate mostly resulted from delay in diagnosis and lack of initial treatment.
CASE REPORT: We reported the case of a 13-year-old boy who had a late-diagnosed capitate fracture. We put the wrist in a short-arm cast for 3 months. After

PURPOSE: We quantified the morphology and angulation of the third metacarpal (MC3) relative to the capitate using three-dimensional computed tomography data to inform surgical procedures such as total wrist arthroplasty and wrist arthrodesis. Specifically, we report the three-dimensional location of the intersections of the long axis of MC3 axis with the capitate cortical surface, the sagittal and coronal angles between the MC3 and capitate axes, and the MC3 shaft angle in the sagittal plane. We tested the hypothesis that these metrics did not differ between women and men.
METHODS: Three-dime

PURPOSE: We examined morphologic similarities of the medial lateral femoral trochlea (MFT) and lateral femoral trochlea (LFT) osteochondral flaps for reconstruction of the proximal capitate.
METHODS: Magnetic resonance imaging scans of the wrists and ipsilateral knees of 10 young healthy volunteers were obtained. Three morphologic parameters were investigated, comparing the MFT and LFT harvest sites to the capitate proximal pole. The correspondingly relevant surgical planes were compared. The coronal plane radius of curvature (ROC) of the capitate was compared with the sagittal planes of the

Capitate nonunion is an uncommon cause of occult wrist pain. History of wrist injury, tenderness in the distal carpal row, capitate-view x-rays, and special radiographic studies such as bone scan, CT scan, or magnetic resonance imaging can confirm the diagnosis.

Objective Kienbock's disease is an unusual disorder caused by osteonecrosis and the collapse of lunate bone which leads to pain and a chronic decrease in wrist function. The treatments in this disease aim to relieve pain and maintain wrist function and movement. Various surgical procedures have been recommended for the subjects with Kienbock's disease; however, the main question posed here is which of the selected procedures are more successful in relief of the pressure applied on lunate. Methods and Materials Computed tomography (CT) scan images of a normal subject were used to create a thr

Based on anatomic study, the vascularized capitate transposition to replace excised necrotic lunate was designed and applied in 40 cases of advanced Kienböck disease. It includes excision of the necrotic lunate and proximal shift of the vascularized capitate. The blood supply of the transposed capitate is provided by the dorsal branch of the anterior interosseous artery. Bone union occurred radiographically, and no postoperative capitate necrosis occurred in all cases after 6 weeks. Twenty-three cases were followed up for 1 year. No residual wrist pain existed in the range of motion, but limit

INTRODUCTION: Kienböck's disease is rare in patients with a neutral or positive ulnar variance. In these situations, treatment is challenging and controversial. Various intracarpal shortening osteotomy (ICSO) procedures have been proposed.
OBJECTIVE: Study the effect of the type of ICSO (isolated capitate osteotomy or combined with hamate osteotomy) on the clinical and radiological outcomes in a retrospective series.
METHODS: Patients with Kienböck's disease were treated with ICSO. A dorsal approach centered over the capitate was used. The transverse osteotomy was located 5mm below the capit

We report a unique case of carpal reconstruction for capitate deficiency in a child. The patient is a 15-year-old girl who had a destructive giant cell tumor involving the capitate and hamate. To the author's knowledge, this is the first case in which Achilles tendon interposition allograft was utilized to fill the bony void left behind by capitate and partial hamate excision. Arthrodesis was used as an adjunct for stabilizing the second and third proximal metacarpals. This resulted in a great functional and radiographic outcome with 7-year follow-up. In conclusion, when treating a carpal defe

BACKGROUND: Hemi-hamate osteochondral grafting is a surgical technique that is utilized to reconstruct the proximal interphalangeal (PIP) joint in cases of unstable dorsal fracture-dislocation with >50% articular surface involvement. However, hemi-hamate osteochondral grafting can be technically challenging, has been reported to have various technical modifications, and can lead to complications such as overstuffing of the joint. This surgical technique article describes successful PIP joint reconstruction with use of a hemi-capitate osteochondral graft, which may offer a viable alternative to

We report the treatment of three patients with stage 3 Kienböck disease that had advanced after radial shortening (n = 2) or lateral wedge osteotomy (n = 1). We shortened the capitate and fused the capitate and hamate, and applied a vascularised bone graft taken from the distal dorsal radius to the lunate. This was successful in all three patients.

Isolated capitate fractures that result in the development of aseptic necrosis of the proximal pole are uncommon. Five patients were treated who developed aseptic necrosis of the proximal pole. Four had a history of trauma with radiographic evidence of nonunion. In vitro arterial injection studies were done to correlate clinical problem with intraosseous capitate vascularity. Palmar vessels were found to contribute the majority of the blood supply to the capitate. Three patterns of intraosseous blood supply were seen. In each pattern, the proximal pole received its blood supply exclusively in

Proximal row carpectomy (PRC) is a long-time, well-accepted, easy-to-reproduce procedure for the treatment of several painful degenerative conditions of the wrist, when capitate pole and radius lunate fossa are preserved. It has been reported to relieve pain and preserve a substantial wrist range of motion, although a partial loss of strength has to be expected because of the decreased length of the carpus. Since 2010, a new technique has been described in the literature using the resurfacing capitate pyrocarbon implant, combined with PRC. This implant has been designed to perform PRC even in

We reviewed retrospectively seven patients with Apert acrosyndactyly and measured the size of the capitate ossification centre relative to that of the hamate and determined the relative position of the middle finger metacarpal relative to the ring finger metacarpal. We then compared those parameters in 197 normal children. In all patients, the middle finger metacarpal bone had migrated proximally relative to the ring finger metacarpal and the size of the capitate ossification centre was smaller than that of the hamate. After surgical release of the middle finger, relative proximal migration of

BACKGROUND: Capitate osteonecrosis is a rare condition that mainly presents in young patients with chronic wrist pain. Treatment aims to address pain, arthritic changes, or carpal instability.
METHODS: We present the surgical technique and outcomes of using a vascularized pedicled second metacarpal base transferred on the second dorsal metacarpal artery to treat capitate osteonecrosis in a 20-year-old female secretary, former gymnast, and a 25-year-old female student with acute lymphoblastic leukemia. These patients presented with idiopathic chronic wrist pain with MRI showing capitate osteon
