Carbimazole-induced, ANCA-associated, crescentic glomerulonephritis: case report and literature review.

Anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitis is a rare complication of antithyroid drug use that was first described with propylthiouracil. We describe an ANCA-associated rapidly progressive glomerulonephritis in a patient treated with carbimazole during 6 months for Graves disease that resulted in end-stage renal disease. A 66-year-old man treated with carbimazole for Graves disease was admitted for macroscopic hematuria and edema of the lower extremities. Laboratory work-up showed elevated serum creatinine (435 μmol/L), mixed hematuria, nephrotic range proteinuria, and

Severe agranulocytosis in patient with Graves' disease on second exposure to carbimazole.

SUMMARY: For half a century, antithyroid drugs (ATDs) have been the first-line treatment for Graves' disease. ATDs, e.g. carbimazole, are associated with minor and serious adverse effects. These include rash, urticaria, arthralgia, hepatotoxicity, vasculitis, and agranulocytosis. Agranulocytosis is a potentially serious adverse effect characterised by a neutrophil count <0.5 × 109 cells/L, which may deteriorate further into sepsis and death. This is a case report of a 47-year-old female who had a 17-year history of Graves' disease. She received carbimazole for 2 years, which was stopped after

Severe Neutropenia Due to Carbimazole and the Importance of Early Recognition and Management.

Agranulocytosis is a rare but potentially life-threatening adverse effect of antithyroid medications, particularly carbimazole. This case report presents a well-structured and clinically relevant discussion of a rare but serious adverse effect of carbimazole therapy in a patient with Graves' disease. The case highlights the importance of early recognition and prompt discontinuation of the drug, which are crucial to preventing severe complications. We present the case of a 51-year-old female patient who was recently diagnosed with Graves' disease and developed agranulocytosis, leading to neutro

Feline hyperthyroidism: spectrum of clinical presentions and response to carbimazole therapy.

OBJECTIVE: To determine the spectrum of clinical presentations of hyperthyroidism in cats and response to carbimazole therapy by analysis of historical, clinical and laboratory data. DESIGN: A prospective clinical study involving client-owned cats presenting to a private veterinary practice in Australia. PROCEDURE: Twenty-five cats diagnosed as hyperthyroid during a 23-month period participated in the study with owner consent. Therapy with carbimazole was instituted and revisits were scheduled 2, 6 and 13 weeks after diagnosis. The cats were physically examined and underwent haematological a

Comparison of the efficacy of single and multiple regimens of carbimazole in the treatment of thyrotoxicosis.

Carbimazole, in 3 divided daily doses, is commonly prescribed for the treatment of thyrotoxicosis. However, based on its long intra-thyroid half-life, the drug may be effective when used as a single or twice daily dose. This study was undertaken to determine the effect of once, twice or thrice daily doses of carbimazole on thyroid function in patients with thyrotoxicosis. Seventy previously untreated thyrotoxic patients were randomly allocated to receive carbimazole 30 mg once (group 1), 15 mg twice (group 2) and 10 mg thrice (group 3) daily. All patients were also prescribed propranolol 20 mg

A case of carbimazole-induced intrahepatic cholestasis. An immune-mediated reaction?

A patient is described with cholestatic hepatitis following the use of carbimazole. A liver biopsy specimen showed intracanalicular cholestasis and some mononuclear cell infiltrate in the portal triades, consistent with drug toxicity; indications of an autoimmune or viral pathogenesis were absent. Rechallenge with the drug precipitated jaundice and disturbed liver function once more. Carbimazole induced a blastogenic response of patient lymphocytes in vitro. Both may suggest the involvement of an immune-mediated reaction, especially as it has been shown that sensitized lymphocytes may produce

Carbimazole-Resistant Graves' Disease Responding to Oral Prednisolone: A Case Report.

Conventional management of Graves' disease includes antithyroid drugs, radioactive iodine and thyroidectomy. Patients rarely may be resistant to antithyroid drugs and may require additional management, including corticosteroids. We treated an 18-year-old girl with Graves' thyrotoxicosis and resistance to Carbimazole. She was clinically and biochemically hyperthyroid despite getting 75mg Carbimazole and 120mg Propranolol daily. We added tablet Prednisolone (20mg for 15 days, then 10mg for the next 15 days) to Carbimazole (75mg/day). Four weeks later, she was free from thyrotoxic symptoms and he

Carbimazole induced agranulocytosis: rescue with human recombinant granulocyte colony stimulating factor.

We report on a 25 yr old woman with primary autoimmune hyperthyroidism who was treated with carbimazole. Forty-three days later she developed agranulocytosis (64 x 10(6)/l). With 4 days treatment with recombinant human Granulocyte Colony Stimulating factor the granulocyte count rose to normal--4,350 cells x 10(6)/l. This is considerably faster than the rate of spontaneous recovery which usually takes one to 2 weeks.

Thyrotoxicosis: relations between clinical state and biochemical changes during carbimazole treatment.

The relation between clinical and biochemical changes in thyrotoxicosis were studied in 12 patients with Graves's disease who were being treated with carbimazole. Clinical assessment (using the Crooks-Wayne index) was combined with the measurement of free thyroxine and triiodothyronine indices (FT4I and FT3I) and the assessment of two tissue markers of thyroid hormone action--sex-hormone-binding globulin (SHBG) levels and the thyrotrophin responses to TRH. In general the FT4I and FT3I fell rapidly once treatment was started, and returned to normal in one to four weeks, followed shortly by SHBG

Carbimazole-induced atypia - A mimicker of malignancy in fine-needle aspiration cytology.

A 20-year-old female presented with complaints of thyroid swelling and showed signs and symptoms of thyrotoxicosis and fine-needle aspiration cytology (FNAC) was requested by the surgeon. On examination of FNAC smear, it showed thyroid follicular cells with atypical features like bizarre giant cells, pseudo nuclear inclusions, and mitotic figure. Correlation between clinical history and cytomorphologic features was done and it was reported as atypical changes in thyroid probably due to carbimazole-induced changes. It helped the patient, as radical surgery and its untoward complications were av

Acute cholestatic hepatitis along with agranulocytosis: a rare side effect of carbimazole.

Antithyroid drugs have been used for more than 50 years for the management of hyperthyroidism. Most patients tolerate treatment well but some may develop life-threatening side effects such as agranulocytosis, aplastic anemia and cholestatic hepatitis. A 45-year-old female was diagnosed with severe hyperthyroidism. Treatment with Carbimazole 30 mg/day was initiated. Within six weeks following the start of therapy, patient developed potentially life-threatening acute cholestatic hepatitis and agranulocytosis as adverse effects to carbimazole. The patient's symptoms and laboratory abnormalities r

Treatment of carbimazole-induced agranulocytosis and sepsis with granulocyte colony stimulating factor.

We present the management of agranulocytosis and neutropenic sepsis secondary to carbimazole with recombinant human granulocyte colony stimulating factor (G-CSF). A 72-year-old woman with a history of thyrotoxicosis presented with sore throat and fever two weeks after starting carbimazole. Investigations confirmed a leucopenia and neutropenia. G-CSF was used as an adjunctive therapy with discontinuation of carbimazole, barrier nursing and a broad-spectrum antibiotic regimen to treat her neutropenic sepsis. Total white cell count and neutrophil count returned to normal and she made an uneventfu

Beneath the Cure Lies the Risk: Carbimazole-Induced Angioedema in Graves' Disease.

Carbimazole is a widely used antithyroid medication for the treatment of hyperthyroidism and is generally well tolerated. Most adverse effects are mild and limited to cutaneous reactions. However, angioedema is a rare but potentially serious hypersensitivity response that may occur shortly after drug initiation. We describe a patient who developed acute facial angioedema soon after starting carbimazole therapy. The patient presented with sudden periorbital and lip swelling accompanied by mild difficulty in swallowing, without any respiratory compromise. There was no history of allergies, autoi

Radioiodine therapy in patients with Graves' disease and the effects of prior carbimazole therapy.

UNLABELLED: The use of radioiodine as the first line of treatment in Graves' disease is restricted in India because of its limited availability and an unrealistic risk perception associated with it. Additionally, the effectiveness of radioiodine ablation in Graves' disease is influenced by many factors. Prior medical antithyroid therapy is one such important factor. AIMS: To analyze the efficacy of low dose radioiodine therapy (5 mCi) in treatment of naive patients of Graves' disease in comparison to that in which it was already primed with an antithyroid drug, carbimazole. SETTINGS AND DESI

[The effect of carbimazole on lactate dehydrogenase isoenzymes and thyroid hormone levels in sheep].

The oxidative processes in the organism are activated by hormones of the thyroid gland. The pyruvate-lactate component catalyzed by lactate dehydrogenase presents one of the important oxidic-reductive systems in the animal organism of biochemical importance (EC. 1.1.1.27). In certain cases of the thyroid gland disturbances antithyroid drugs, such as derivatives of thiouracil, mercaptoimidazole and some others may be used for the treatment of men and animals (Marchant et al., 1979; Negwer, 1987). In the present study, the activity changes of lactate dehydrogenase isoenzymes in the blood plasma

Preoperative treatment of thyrotoxicosis in developing countries: a comparative study of carbimazole and propranolol.

Preoperative management of treatment in thyrotoxicosis was investigated in a comparative study of carbimazole and propranolol in 41 consecutive patients collected during a 3-year period. The groups included 20 and 21 patients respectively, of equal sex and age distribution and of similar severity. The length of the preoperative treatment was significantly reduced in the propranolol pretreated patients (P greater than 0.001). Intraoperative blood loss, postoperative function with regard to the thyroid, parathyroids and vocal cords were similar in both groups. Three and two patients respectively
