Pancreatic-Type Acinar Cell Carcinoma of the Duodenum: A Case Report and Literature Review.

INTRODUCTION: Acinar cell carcinoma (ACC) is a rare malignant tumor arising from pancreatic exocrine cells. While it typically originates in the pancreas, ectopic occurrences, especially in the duodenum, are extremely uncommon. Few reports exist of duodenal ACC, particularly those presenting as large tumors. CASE PRESENTATION: An 83-year-old man presented with melena. Endoscopy and imaging revealed a large mass, approximately 7 cm in size, extending from the duodenal bulb to the descending portion of the duodenum. Contrast-enhanced CT showed no evidence of extramural invasion or distant metas

Proteomic basis for pancreatic acinar cell carcinoma and pancreatoblastoma as similar yet distinct entities.

Acinar cell carcinoma (ACC) and pancreatoblastoma (PBL) are rare pancreatic malignancies with acinar differentiation. Proteogenomic profiling of ACC and PBL revealed distinct protein expression patterns compared to pancreatic ductal adenocarcinoma (PDAC) and benign pancreas. ACC and PBL exhibited similarities, with enrichment in proteins related to RNA processing, chromosome organization, and the mitoribosome, while PDACs overexpressed proteins associated with actin-based processes, extracellular matrix, and immune-active stroma. Pathway activity differences in metabolic adaptation, epithelial

Paraneoplastic Cast Nephropathy Associated With Pancreatic Acinar Cell Carcinoma: A Kidney Biopsy Teaching Case.

Paraneoplastic cast nephropathy, a rare cause of acute kidney injury, is most commonly observed in cases of multiple myeloma and is characterized by the formation of intratubular casts composed of monoclonal light chains. Nonmonoclonal paraneoplastic cast nephropathy has also been reported in patients with pancreatic acinar cell carcinoma or prolactinoma. In this case report, we present a case of polyclonal cast nephropathy in a patient with metastatic acinar cell carcinoma. We aim to emphasize the significance of recognizing this uncommon complication in patients with solid tumors and to disc

Germline BRCA1-Mutated Synchronous and Metachronous Pancreatic Acinar Cell Carcinoma With Long-Term Survival.

BACKGROUND: Pancreatic acinar cell carcinoma (PACC) is a rare pancreatic neoplasm. Recently, molecular analysis revealed that PACC shows a high frequency of the BRCA1/2 mutation and is likely to be considered a cancer associated with hereditary breast and ovarian cancer (HBOC). Hereditary cancers, including HBOC, are characterized by multifocal and/or metachronous tumors. However, no case reports exist of germline BRCA1-mutated synchronous and metachronous PACC. CASE: A 58-year-old man was diagnosed with synchronous and metachronous PACC at the age of 56 and underwent two surgeries. Ten month

Pancreatic acinar cell carcinoma: A multi-center series on clinical characteristics and treatment outcomes.

BACKGROUND: Acinar cell carcinoma (ACC) is a very rare tumor of the exocrine pancreas, representing less than 1% of all pancreatic malignancies. The majority of data regarding ACC are limited to small case series. METHODS: This is a retrospective study conducted at a large healthcare system from 1996 to 2019. Patients with pathologically confirmed ACC were included, and demographic data, tumor characteristics, and treatment outcomes were abstracted by chart review. Survival curves were obtained by using the Kaplan-Meier method and compared using the log-rank test. RESULTS: Sixty-six patients

A primary pure pancreatic-type acinar cell carcinoma of the stomach: a case report.

BACKGROUND: Acinar cell carcinoma represents only 1-2% of exocrine pancreatic neoplasms. On exceptionally rare occasions, primary acinar cell carcinoma can occur in ectopic locations. Herein, we report a case of pure pancreatic-type acinar cell carcinoma arising in the stomach. CASE PRESENTATION: A 54-year-old male presented with a gastric submucosal mass detected by endoscopic examination. Laparoscopic wedge resection was performed. Macroscopically, the 2.7 cm yellowish mass was located in the submucosa of the stomach. Microscopically, the tumor was well circumscribed and had a homogeneous a

Severe panniculitis and polyarthritis caused by acinar cell carcinoma arising from an ectopic pancreas.

The pancreatitis, panniculitis and polyarthritis (PPP) syndrome is a rare condition caused by pancreatic diseases, such as acute or chronic pancreatitis or pancreatic carcinoma. We report the first case of PPP syndrome caused by metastatic acinar cell carcinoma from an ectopic pancreas. The symptoms were successfully managed by the treatment of the metastatic carcinoma. Pancreatic cytosteatonecrosis should be always considered in a patient who is showing symptoms of panniculitis and polyarthritis.

Parotid acinar cell carcinoma in a long-term survivor of childhood acute lymphoblastic leukemia.

Secondary malignancies are an important cause of morbidity and mortality in childhood cancer survivors. Salivary gland tumors account for about 6% of the second cancers. The majority of these are mucoepidermoid carcinomas (MEC) of the parotid gland. We report the clinical and pathological features of a rarer histological type, acinic cell carcinoma (ACC), in a childhood acute lymphoblastic leukemia (ALL) survivor. The behavior of secondary ACC appears similar to primary tumor and similar treatment may be adopted. Early recognition and complete resection is important for achieving a good outcom

Brush cytology of acinar cell carcinoma of the pancreas with intraductal growth: a case report.

We describe a rare case of pancreatic acinar cell carcinoma (ACC) with intraductal growth in a 77-year-old man, which was diagnosed by endoscopic brush cytology. Preoperative imaging revealed an ill-defined mass involving the main pancreatic duct of the body, which was suspected to be an invasive ductal carcinoma. Endoscopic brush cytology showed several thick, small to large clusters of tumor cells. However, a loosely cohesive or individual cell arrangement was more prominent. Singly dispersed naked nuclei, occasionally with crush artifact, were frequently observed. The nuclear contour was sm

Pancreatic ducts as an important route of tumor extension for acinar cell carcinoma of the pancreas.

Acinar cell carcinoma (ACC) of the pancreas is very rare, which usually grows expansively. Recently, a variant of ACC with predominant growth in the pancreatic ducts has been proposed, and is speculated to have potentially less aggressive behavior. The aim of this study was to investigate how the pancreatic duct system is related to the growth and extension of ACC. We reviewed the detailed gross and histologic features of 13 cases of ACC, of which 7 (54%) showed intraductal polypoid growth (IPG) of the tumor in the large pancreatic ducts with a mean IPG length of 24.8 mm. Tumors with IPG were

Imaging phenotypes of pancreatic acinar cell carcinoma: a multicenter study of morphologic subtypes and recurrence.

OBJECTIVES: To identify the imaging features of pancreatic acinar cell carcinoma (PACC) and to classify them according to morphological subtypes. MATERIALS AND METHODS: This multicenter retrospective study included 36 patients with pathologically confirmed PACC who underwent CT/MRI from 2008 to 2024. Tumors were classified into three morphologic subtypes based on imaging features identified in this study: Type 1, well-demarcated solid mass; Type 2, predominantly intraductal mass; Type 3, indistinct infiltrative mass. Maximal tumor size, marked diffusion restriction, lobulated internal archite

Pancreatic acinar cell carcinoma with predominant extension into the main pancreatic duct: A case report.

A 34-year-old male was referred to our hospital for a possible pancreatic mass detected by computed tomography (CT) that was performed to find the cause of acute pancreatitis. Multiple imaging tests, including contrast-enhanced CT scan, magnetic resonance imaging, contrast-enhanced endoscopic ultrasonography, and endoscopic retrograde pancreatography, revealed a solid mass occupying the head of the main pancreatic duct (MDP), and pancreaticoduodenectomy was performed. In the resected specimen, the tumor showed expansive growth from the pancreatic parenchyma to the MDP and formed a tumor plug.

Hepatobiliary and Pancreatic: Primary acinar cell carcinoma of the liver showing good response to chemotherapy.

A 48-year old man with no history of liver disease or alcohol addiction developed jaundice, itching and dark urine without any symptom of abdominal pain. A CT examination revealed the presence of a liver mass extending between segments 4, 5 and 8 that was found to be an acinar cell carcinoma at biopsy. The primary hepatic origin was confirmed mainly excluding a primary pancreatic tumor by diagnostic work-up. Since the lesion was considered unresectable at the time of the diagnosis, the patient underwent to Xeloda and oxaliplatin neoadjuvant chemotherapy, with progressive reduction in size of t

Genome-based classification of pancreatic acinar cell carcinoma reveals similarities to KRAS wild-type PDAC.

BACKGROUND: Pancreatic acinar cell carcinoma (ACC) is a rare primary neoplasm of acinar cell origin. Histologically, it can present as pure ACC or mixed with neuroendocrine or ductal differentiation. We studied whether ACC shared genomic similarities with pancreatic ductal adenocarcinoma (PDAC) or well-differentiated pancreatic neuroendocrine tumors (PNET) using the hidden genome classifier methodology. METHODS: The hidden genome classifier was trained using next-generation sequencing of KRAS wild-type (KRAS-WT) PDAC (n = 251), KRAS-mutated (KRAS-mut) PDAC (n = 1872) and PNET (n = 127). We app
