[A clinical study of lobular carcinoma of the breast].

In a review of 549 cases of breast cancer in the last 16 years, 7 cases with lobular carcinoma have been studied. All of the patients appeared well and were without recurrence. Two cases had been lobular carcinomas in situ, one of which had arisen in a small fibroadenoma associated with malignant phyllodes tumor in the opposite breast, and the other had shown multicentric foci of a lobular neoplasm. The remaining 5 cases had been invasive lobular carcinomas. The incidence of lobular carcinoma represented 1.3% (7/549) of all primary breast carcinomas, and had been increasing chronologically. Es

Lobular carcinoma of the breast in situ and infiltrating.

Lobular carcinoma in situ, while histologically delineated almost 35 years ago, is still being diagnosed and reported with differing qualitative and quantitative criteria. It is important to recognize this lesion because of its frequent bilaterality and risk of developing infiltrating carcinoma in the affected breast. Because of the apparent morphologic identity of the cell of ALH and the cell of LCIS, because both are associated with some increased risk of developing carcinoma, and because both have an increased propensity to be found simultaneously with carcinoma--especially lobular carcinom

Long-term follow-up of lobular neoplasia (atypical lobular hyperplasia/lobular carcinoma in situ) diagnosed on core needle biopsy.

BACKGROUND: Lobular neoplasia (LN) includes atypical lobular hyperplasia (ALH) and lobular carcinoma in situ (LCIS). LN often is an incidental finding on breast core needle biopsy (CNBx) and management remains controversial. Our objective was to define the incidence of malignancy in women diagnosed with pure LN on CNBx, and identify a subset of patients that may be observed. METHODS: Patients diagnosed with LN on CNB between January 1993 and December 2010 were identified. Patients with an associated high-risk lesion or ipsilateral malignancy at time of diagnosis were excluded. All cases were

Histiocytic appearance of metastatic lobular breast carcinoma.

Autopsy findings in a case of widely disseminated lobular breast carcinoma showed an unusual histiocytoid appearance in metastases to the uterus and parathyroid gland. This morphology for breast carcinoma has been previously reported in eyelid metastases. Recognition of this pattern in other metastatic sites has important diagnostic and therapeutic implications. Problems might have arisen if the polypoid uterine mass had been the presenting lesion during life. We also review the occurrence of metastatic breast carcinoma in the uterus and parathyroid gland.

Impact of concurrent lobular carcinoma in situ on recurrence outcomes in patients with classic and pleomorphic invasive lobular carcinoma of the breast.

PURPOSE: We investigated outcomes of invasive lobular carcinoma (ILC) with or without concurrent lobular carcinoma in situ (LCIS) in patients with classic or pleomorphic ILC. METHODS: We retrospectively analyzed a single-institution database of patients with stage I-III ILC. We compared tumor features, treatment, and recurrence free survival (RFS) in patients with ILC-alone versus ILC + LCIS stratified by ILC tumor subtype. Multivariable Cox proportional hazards models were used for multivariate analysis. RESULTS: Of the 786 cases of ILC, 542 were classic and 92 were pleomorphic, with 70.6%

[Invasive lobular carcinoma: Clinicopathologic and biological insights].

Invasive lobular carcinoma (ILC), the second most common histologic subtype of breast cancer, is a clinicopathologic entity distinct from invasive carcinoma of no special type. It is defined by loss of E-cadherin (inactivation of CDH1 gene), which underlies the characteristic discohesive tumor cells, and is associated with ill-defined growth pattern, frequent multifocality and bilaterality, and a distinctive pattern of metastases (peritoneum, meninges, ovaries, gastrointestinal tract). Recent work has delineated several lobular-specific biological features, supporting the development of ILC-di

Epithelioid mammary myofibroblastoma mimicking invasive lobular carcinoma.

A case of a 53-year-old woman with the epithelioid variant of mammary myofibroblastoma, which was initially misinterpreted as invasive lobular carcinoma, is presented. A needle core biopsy of the 1.6 cm mass showed interlacing bundles of epithelioid myofibroblasts amid dense fibrous tissue associated with lobular carcinoma in situ of the classical type. Most epithelioid cells showed nuclear atypia, and a few exhibited signet-ring cytology. Immunoreactivity for estrogen and progesterone receptors further compounded the deception, and the neoplasm was misinterpreted as invasive lobular carcinoma

Histiocytoid breast carcinoma: an enigmatic lobular entity.

Histiocytoid breast carcinoma is an uncommon entity that is mostly regarded as a variant of lobular carcinoma. Its occurrence with apocrine lobular carcinoma in situ and consistent expression of gross cystic disease fluid protein 15 suggest apocrine differentiation. Its recognition is often challenging, particularly when histiocytoid tumour cells occur in a metastatic site before the primary diagnosis of breast carcinoma, or in limited core biopsy or cytology material. In the breast, its bland histological appearances can lead to a benign diagnosis. Clues to the correct conclusion include find

Lobular carcinoma in ectopic axillary breast tissue.

Accessory mammary tissue is reported to occur in 2 per cent to 6 per cent of women and results from failure of regression of a portion of the galactic band, which runs from the axilla to the groin during embryogenesis. The differential diagnosis of a mass located along the course of the primitive milk line must include accessory breast tissue. This tissue is subject to the same physiologic and pathologic changes as in the breast proper. There are numerous case reports of masses arising in accessory breast tissue ranging from fibroadenomas and phyllodes tumors to invasive mucinous adenocarcinom

Automated image morphometry of lobular breast carcinoma.

OBJECTIVE: To analyze the role of automated image morphometry (AIM) in distinguishing infiltrating lobular carcinoma (ILC) of the breast from benign, borderline and infiltrating ductal carcinoma (IDC). STUDY DESIGN: Only histopathologically proven lobular carcinoma, ductal carcinoma, borderline lesions and benign breast lesions were selected for the study. There were 19 cases of ILC and 30 cases of IDC, 20 cases of benign lesions (fibroadenoma, 18; fibrocystic disease, 1; and fibroadenosis, 1); 10 cases were borderline lesions (mild epithelial hyperplasia, 3; moderate epithelial hyperplasia,

[Lobular carcinoma in situ (LCIS): risk factor and precursor of invasive lobular breast cancer].

Lobular carcinoma in situ (LCIS) of the breast typically is an incidental finding in breast biopsies performed for a variety of reasons. As patients with LCIS have a bilaterally increased risk of developing invasive ductal or lobular breast cancer, the lesion is presently considered an indicator rather than a direct precursor of breast cancer. However, this view is challenged by the finding that LCIS often accompanies invasive lobular carcinomas (ILC) and that the frequency of ILC following LCIS is far higher compared to unselected patients. To further examine the role of LCIS in the developme

Surgical outcome of biopsy-proven lobular neoplasia: is there any difference between lobular carcinoma in situ and atypical lobular hyperplasia?

OBJECTIVE: The aims of our study were to determine the frequency of malignancy after surgical excision of biopsy-proven lobular carcinoma in situ (LCIS) or atypical lobular hyperplasia (ALH) lesions, to assess any difference between pure LCIS and pure ALH lesions regarding their radiologic presentation and the malignancy upgrade rate after surgical excision, and to evaluate the outcome of lesions that were not excised surgically but were followed up. MATERIALS AND METHODS: Radiologic and pathologic records of 14,435 imaging-guided needle biopsies of the breast performed between 2004 and 2008

Lobular carcinoma of the breast: a special variant of mucin-secreting carcinoma.

There is currently much speculation over the precise nature of lobular carcinoma in-situ of the breast and its significance. Histochemical study shows that it is rich in sialomucin. This has a characteristic intracellular distribution which distinghishes lobular carcinoma in-situ from cancerization of lobules. The same histochemical features are seen in the infiltrative phase of the tumour. These findings invalidate the concept that it is a myoepithelial-cell tumour. Practical applications of the distinctive pattern of mucin secretion are discussed.

Evolving concepts in breast lobular neoplasia and invasive lobular carcinoma, and their impact on imaging methods.

Invasive lobular carcinoma (ILC) and lobular neoplasia (LN) are two distinct conditions that still pose challenges regarding to their classification, diagnosis and management. Although they share similar cellular characteristics, such as discohesive neoplastic cells and absence of e-cadherin staining, they represent completely different conditions. LN encompasses atypical lobular hyperplasia (ALH) and lobular carcinoma in situ (LCIS), which are currently considered risk factors and non-obligatory precursors of breast neoplasia. These lesions are diagnosed as incidental findings in percutaneous

Contemporary management of ductal carcinoma in situ and lobular carcinoma in situ.

The management of in situ lesions ductal carcinoma in situ (DCIS) and lobular carcinoma in situ (LCIS) continues to evolve. These diagnoses now comprise a large burden of mammographically diagnosed cancers, and with a global trend towards more population-based screening, the incidence of these lesions will continue to rise. Because outcomes following treatment for DCIS and LCIS are excellent, there is emerging controversy about what extent of treatment is optimal for both diseases. Here we review the current approaches to the diagnosis and treatment of both DCIS and LCIS. In addition, we will
