A decontamination and sterilization protocol employed during reuse of cardiac electrophysiology catheters inactivates human immunodeficiency virus.

OBJECTIVE: To assess the effect of a standard decontamination and sterilization protocol employed during reuse of cardiac electrophysiology (EP) catheters on human immunodeficiency virus (HIV). SETTING: Public health viral research laboratory. METHODS: Studies were performed on distal, electrode-containing segments of 40 EP catheters previously used in up to 10 clinical EP procedures. EP catheter segments were immersed for 1 hour in blood contaminated with a high titer of HIV. After air drying for 2 hours, subgroups of 8 EP catheters were subjected to either (1) no treatment, (2) washing in

Cardiac magnetic resonance imaging predicts cardiac catheter findings for great artery stenosis in children with congenital cardiac disease.

OBJECTIVE: To assess the cardiac catheterisation findings of all children in whom cardiac magnetic resonance imaging found great artery stenosis. METHODS: We conducted a retrospective analysis of all 45 consecutive children with congenital cardiac disease who were undergoing cardiac catheterisation for intervention on cardiac magnetic resonance-defined great vessel stenosis, between January, 2006 and August, 2008. RESULTS: Following cardiac magnetic resonance, 60 significant great vessel stenoses were identified and referred to cardiac catheterisation for intervention. All patients were cath

Incidence of cardiac dysrhythmias in patients during pulmonary artery catheter removal after cardiac surgery.

OBJECTIVE: No published studies focus on determining the frequency of dysrhythmias during pulmonary artery catheter removal by use of a standard technique. The objective was to assess the incidence and hemodynamic effect of dysrhythmias in patients who had recently undergone cardiac surgery (within 24 hours of when dysrhythmia was noted). METHODS: The study used an electrocardiogram recording by continuous rhythm strip during removal of the pulmonary artery catheter. Pulmonary artery catheters were removed by use of a standard procedure. Data were collected with respect to patients' serum pot

A device profile on the MAGiC ablation catheter for treatment of cardiac arrhythmia.

INTRODUCTION: Catheter ablation is standard of care for cardiac arrhythmias, but conventional catheters are still limited by single-plane steering, variable tissue contact, and high irrigation demands, particularly in complex anatomy. Robotic magnetic navigation (RMN) addresses several of these constraints, but it has long depended on a first-generation radiofrequency catheter. AREAS COVERED: This device profile reviews the design, mechanism of action, available clinical evidence, regulatory status and future development of the MAGiC magnetic ablation catheter, the first purpose-built cathete

Safety of Catheter Ablation Therapy for Atrial Fibrillation in Cardiac Amyloidosis.

Background Despite the high burden of atrial fibrillation in cardiac amyloidosis (CA), the safety of catheter ablation therapy in CA is not well established. We sought to examine short-term safety outcomes following atrial fibrillation ablation in patients with CA compared with matched patients with dilated cardiomyopathy (DCM). Methods and Results Using data from the National Inpatient Sample, we identified all hospitalizations for atrial fibrillation ablation from the fourth quarter of 2015 through 2019. Admissions for CA and DCM were matched in a 1:5 ratio using propensity scores based on t

Catheter Ablation in Arrhythmic Cardiac Diseases: Endocardial and Epicardial Ablation.

Arrhythmogenic cardiomyopathy (ACM) is a group of arrhythmogenic disorders of the myocardium that are not caused by ischemic, hypertensive, or valvular heart disease. The clinical manifestations of ACMs may overlap those of dilated cardiomyopathy, complicating the differential diagnosis. In several ACMs, ventricular tachycardia (VT) has been observed at an early stage, regardless of the severity of the disease. Therefore, preventing recurrences of VT can be a clinical challenge. There is a wide range of efficacy and side effects associated with the use of antiarrhythmic drugs (AADs) in the tre

Recent trends in cardiac electrophysiology and catheter ablation in New Zealand.

BACKGROUND: Catheter ablation has rapidly become an integral part of the management of many arrhythmias. AIMS: To provide a history of clinical cardiac electrophysiology (EP) in New Zealand (NZ) and analysis of recent trends in EP procedures and catheter ablations across NZ, which has not previously been reported. METHODS: EP case type and volume were obtained from the EP databases from each of the four public and four private EP centres in NZ from 1 January 2014 to 31 December 2018. Procedure rates were expressed as per million population. RESULTS: A total of 7695 EP cases was performed, i

Nonfatal cardiac perforation after central venous catheter insertion.

Cardiac tamponade caused by perforation of the cardiac wall is a rare complication related to central venous catheter (CVC) placement. A 71-year-old female with a previous history of moderate aortic stenosis and kidney transplantation was admitted to hospital due to global heart failure and worsening of allograft function. Intensified hemodialysis was commenced through a CVC placed in the right subclavian vein. Chest radiography revealed catheter tip in the right atrium and no signs of pneumothorax. Thorough diagnostics outruled immediate life-threatening conditions, such as myocardial infarct

Long-term peritoneal port-catheter in a patient with cardiac ascites.

A peritoneal port-catheter was inserted in a 70-year-old man because of repeated paracentesis due to cardiac ascites. Instead of frequent hospital admissions, the patient could drain his ascites at home, which dramatically improved his quality of life and enabled him to perform his daily activities.

Management of cardiac migration of a distal shunt catheter: the radiological pitfalls.

Cardiac migration is a rare complication of the ventriculoperitoneal shunt. We report a case of a late migration of the distal shunt into the pulmonary arteries. The authors underline the radiological features that suggested the presence of a knot. Preoperative planning and the assistance of a vascular surgeon were required due to the presence of a knot.

Cardiac tamponade after withdrawal of a peripheral access central catheter.

Central venous catheterization is a very common technique, although its complications can be multiple and sometimes fatal. A case is presented of cardiac tamponade by parenteral nutrition a few hours after moving a central venous catheter peripherally inserted a few days before. The diagnosis was made by echocardiography, and an emergency pericardiocentesis was performed, achieving complete recovery of the patient. Peripherally inserted central venous catheters are more likely to change their position secondary to the movements of the patient's arm, thus it is important to use soft catheters,

Frequency and outcomes of cardiac operations and catheter interventions in Turner syndrome.

Cardiac malformations occur commonly in Turner syndrome (TS), but the outcomes of cardiac operations and catheter-based procedures are unknown. The Pediatric Cardiac Care Consortium database was queried for individuals with TS and other female subjects without genetic abnormalities or syndromes (non-TS [NTS]). Procedures for left-sided heart lesions represented most TS procedures (95.2%). Three hundred ninety-eight patients with TS who underwent 637 of these procedures of interest were compared with 25,913 female NTS subjects who underwent 56,625 procedures. The numbers of procedures per admis

Cardiac injury after percutaneous catheter ablation for atrial fibrillation.

AIMS: Small elevations in troponin T levels have been shown with limited radiofrequency (RF) ablation procedures for supraventricular tachycardia, usually to levels below the threshold for ischaemia or infarction. Left atrial catheter ablation for atrial fibrillation (AF) requires far more RF energy, therefore could be expected to have greater elevation in troponin T. We determined troponin T levels before and after ablation in these patients to evaluate the amount of rise with this ablation. METHODS: All patients undergoing pulmonary vein isolation (PVI) from May 2004 to October 2004 had tro

Review of economic evaluations of radiofrequency catheter ablation for cardiac arrhythmias.

A systematic review of current studies on the cost effectiveness of catheter ablation for treatment of tachycardia in adults was undertaken. The results are summarized based on a predefined framework of principles for economic evaluation. Of 192 abstracts identified, only three cost effectiveness studies were identified. Each focused on a different and specific patient group with selected target disorders, and used decision analysis modelling to estimate cost effectiveness. Radiofrequency catheter ablation is likely to be economically attractive compared with drug therapy in adult patients wit

Timing of surgery/catheter intervention in common congenital cardiac defects.

A large load of children with congenital heart disease (CHD) exists in our country. Fortunately facilities for treatment of almost all types of defects are available, although in few centres only. Many children are however, referred late for correction making them either high risk for surgery or inoperable due to development of irreversible pulmonary vascular obstructive disease. Therefore, it is important to recognise the ideal time of intervention in different types of CHD. A child with large atrial septal defect should have its closure by 3 to 4 years of age; on the other hand, a large vent
