Case Report: Low cardiac output syndrome with multisystem complications following total repair of tetralogy of fallot.

BACKGROUND: Tetralogy of Fallot (ToF) repair in infants may be complicated by low cardiac output syndrome (LCOS), which can precipitate rare multisystem complications. Junctional ectopic tachycardia (JET), atrial thrombosis, and cortical blindness are underrecognized sequelae requiring multidisciplinary management. CASE PRESENTATION: We report a 23-month-old male with ToF who developed LCOS post-surgery, followed by refractory JET, left atrial thrombus, embolic occipital infarction, and transient cortical blindness. LCOS was defined by elevated lactate, low ScvO₂, hypotension, and oliguria. J

Post-operative cerebral oximetry for detection of low-cardiac output syndrome after coronary artery bypass surgery.

While cerebral near infrared spectroscopy (NIRS) is a valuable diagnostic tool to monitor brain oxygenation during cardiac surgery, its value in low cardiac output syndrome (LCOS) in adults has not been evaluated. This study was prospective and observational. Patients undergoing coronary artery bypass grafting (CABG) with reduced ejection fraction (LVEF < 35%) were included and received NIRS monitoring for up to 48 h after surgery with simultaneous continuous cardiac index (CI) monitoring. The primary endpoint was LCOS by a standard definition. From 2020 to 2023, 82 Patients with severely redu

Hemodynamic effects of levosimendan in low cardiac output syndrome post-CABG refractory to standard inotropic therapy.

BACKGROUND: Low cardiac output syndrome (LCOS) following coronary artery bypass grafting (CABG) remains a major cause of morbidity and mortality. Standard inotropes frequently provide only marginal hemodynamic benefit and are associated with increased myocardial oxygen consumption and arrhythmogenic risk. Levosimendan, a calcium sensitizer with inodilatory properties, may offer more favorable hemodynamics without these adverse effects. METHODS: In this retrospective study, we evaluated 41 patients with LCOS post-CABG who failed to respond to standard catecholamines, defined by < 20% improveme

Comparison of Cardiac Output Measurement by Electrical Velocimetry with Echocardiography in Extremely Low Birth Weight Neonates.

INTRODUCTION: Electrical velocimetry (EV) offers a noninvasive tool for continuous cardiac output (CO) measurements which might facilitate hemodynamic monitoring and targeted therapy in low birth neonates, in whom other methods of CO measurement are not practicably feasible. METHODS: This prospective observational study compared simultaneous cardiac output measurements by electrical velocimetry (COEV) with transthoracic echocardiography (COTTE) in extremely low birth weight (ELBW) neonates in the neonatal intensive care unit (NICU). Echocardiography was performed by 1 single examiner. Data we

Body mass index is associated with low postoperative cardiac output in patients undergoing aortic valve replacement.

BACKGROUND: Overweight and obesity (O/O) generate lipotoxicity of the cardiac fiber and increase the incidence and progression of aortic valve stenosis. The low cardiac output syndrome (LCOS) is a timing complication after to aortic valve replacement (AVR) surgery. OBJECTIVE: The objective of the study was to investigate if body mass index (BMI) kg/m2 is a risk factor associated with LCOS and mortality in the post-operative period of AVR. METHODS: A historic cohort study was designed, including patients with severe aortic stenosis (SAS), who were subjected to AVR. RESULTS: 152 patients were

Predictors of Progression to Low Cardiac Output or Cardiogenic Shock in Patients Hospitalized for Acute Heart Failure.

During hospitalization, patients with acute heart failure (AHF) may progress to low cardiac output syndrome (LCOS) or cardiogenic shock (CS). However, the predictive factors for this evolution are not clearly defined. To evaluate the frequency of progression to LCOS or CS in patients hospitalized for AHF and to identify predictive factors of this progression among clinical, echocardiographic, and laboratory parameters obtained at admission. A single-center cohort study was conducted, performing a retrospective analysis of prospectively collected data from consecutive patients admitted for AHF

The cardiovascular changes underlying a low cardiac output with exercise in patients with type 2 diabetes mellitus.

The significant morbidity and premature mortality of type 2 diabetes mellitus (T2DM) is largely associated with its cardiovascular consequences. Focus has long been on the arterial atheromatosis of DM giving rise to early stroke and myocardial infarctions, whereas less attention has been given to its non-ischemic cardiovascular consequences. Irrespective of ischemic changes, T2DM is associated with heart failure (HF) most commonly with preserved ejection fraction (HFpEF). Largely due to increasing population ages, hypertension, obesity and T2DM, HFpEF is becoming the most prevalent form of hea

A Randomized Porcine Study in Low Cardiac Output of Vasoactive and Inotropic Drug Effects on the Gastrointestinal Tract.

BACKGROUND: Splanchnic vasodilation by inodilators is an argument for their use in critical cardiac dysfunction. To isolate peripheral vasoactivity from inotropy, such drugs were investigated, and contrasted to vasopressors, in a fixed low cardiac output (CO) model resembling acute cardiac dysfunction effects on the gastrointestinal tract. We hypothesized that inodilators would vasodilate and preserve the aerobic metabolism in the splanchnic circulation in low CO. METHODS: In anesthetized pigs, CO was lowered to 60% of baseline by partial inferior caval vein balloon inflation. The animals wer

Extracorporeal membrane oxygenation as a support for TGA/IVS with low cardiac output syndrome and pulmonary hemorrhage.

A 15-day-old neonate with complete transposition of the great arteries/intact ventricular septum was admitted with life-threatening hypoxemia and heart arrest. After successful resuscitation, heart beat recovered but blood lactate began to arise and maintained above 15 mmol/L 6 hours later. Emergency arterial switch operation was done at 20 hours after resuscitation. Planned extracorporeal membrane oxygenation support was employed postoperatively. The baby experienced severe pulmonary hemorrhage and severe hypoxemia after weaning from cardiopulmonary bypass, which were treated with extracorpor

[Comparison of the hemodynamic effects of dobutamine and enoximone in the treatment of low cardiac output after valvular surgery].

Forty patients developed low cardiac output states after surgery for mitral valve disease or with associated cardiac disease and were randomly allocated to two treatment groups, one group to receive Dobutamine (D) and the other Enoximone (E), a phosphodiesterase inhibitor. Haemodynamic assessment covered a 24 hour period but treatment was continued for as long as was necessary. An improvement was observed from the 15th minute of treatment. At the second hour, the cardiac index had increased by 55% in Group E and by 59% in Group D whilst the heart rate increased by only 12% in Group E compared

Hemodynamic responses to dopamine infusion at controlled normal and low cardiac output states in the anesthetized dog.

In order to isolate the renal and pulmonary vascular effects of dopamine, we employed a dog preparation utilizing fixed venous return in order to control cardiac output at either a normal or low flow state. Dopamine infusion was carried out at doses of 8 and 16 microng/Kg./min. Heart rate, mean systemic arterial pressure, mean pulmonary artery pressure, and renal blood flow were measured. Changes in mean systemic arterial pressure, mean pulmonary artery pressure, and renal blood flow were not significant at either doses of dopamine. There was a dose-related increase in heart rate, most signifi

Influence of patent ductus arteriosus and ventilators on electrical velocimetry for measuring cardiac output in very-low/low birth weight infants.

OBJECTIVE: We evaluated electrical velocimetry, a noninvasive method for continuous cardiac output measurement, in very-low and low birth weight infants and the influence of patent ductus arteriosus (PDA) and ventilators on this method. STUDY DESIGN: This prospective study compared 81 pairs of simultaneous cardiac output measurements by electrical velocimetry and transthoracic echocardiography in 28 patients. Data were compared by correlation, Bland-Altman analysis and two-way analysis of variance. RESULTS: The two methods exhibited a high correlation (r=0.859, P<0.0001). The bias (mean diff

Congestion, but not low cardiac output, is independently associated with acute kidney injury following contrast agent exposure.

AIM: Contrast-associated acute kidney injury (CA-AKI) is frequently associated with adverse outcomes. Renal perfusion gradient is central to glomerular filtration. While renal vasoconstriction can partially compensate low-output-associated impairment of the renal perfusion gradient, venous congestion cannot be compensated. We hypothesized that congestion is a stronger predictor of CA-AKI than low output in cardiovascular diseases. METHODS AND RESULTS: An all-comer single-center cohort of 3,126 cases undergoing coronary angiography with simultaneous right heart catheterization was analyzed. Pa

Non-invasive cerebral pulse rate monitoring during two low-cardiac-output events after birth - A case report.

A female, term neonate required cardiopulmonary resuscitation (CPR) for 45 s after vaginal birth. A novel cerebral pulsatile near-infrared spectroscopy (pNIRS) mode guided chest compressions during CPR, as it showed a cerebral pulse rate of around 100 per minute, suggesting sufficient perfusion pressure. After initial stabilization and intubation due to minimal breathing efforts, she spontaneously developed broad complex tachycardia. During this instance, the pNIRS detected a regular cerebral pulse rate, indicating stable cerebral perfusion and reassuring that CPR and emergency defibrillation

Amplitude-Integrated/Continuous Electroencephalography for Early Detection of Low Cardiac Output After Chest Closure in an Infant.

BACKGROUND: Weaning from cardiopulmonary bypass in pediatric cardiac surgery is challenging, especially after prolonged procedures. Delayed chest closure may be necessary in cases of low cardiac output syndrome (LCOS) to support hemodynamic recovery. Although near-infrared spectroscopy is standard for neuromonitoring, amplitude-integrated electroencephalography (aEEG) and continuous electroencephalography (cEEG) are emerging tools. CASE SUMMARY: We report the case of a 6-month-old infant with transposition of the great arteries who required delayed chest closure after surgical repair. LCOS re

Correction of High Afterload Improves Low Cardiac Output in Patients Supported on Left Ventricular Assist Device Therapy.

There is a paucity of data describing the invasive assessment of afterload and influence on cardiac output in patients supported on left ventricular assist device (LVAD) therapy. From 2015 to 2018, patients on LVAD therapy were evaluated with simultaneous left/right heart catheterization ramp study for speed optimization. Hemodynamic parameters from 31 patients without significant aortic insufficiency were analyzed. Mean central aortic pressure (MAP) was elevated at 87 ± 13 mm Hg at baseline. No direct association between MAP and cardiac index (CI) was found (r = 0.11, p = 0.20). However, vari

Low Cardiac Output Syndrome: Incidence after Mitral Valve Replacement with or without Preservation of Mitral Valve Apparatus.

Standard mitral valve replacement in patients with chronic mitral valve regurgitation and mitral valve stenosis consistently results in a decrease in early postoperative left ventricular ejection performance. Some studies showed that preservation of mitral valve leaflet and subvalvular apparatus can reduce postoperative left ventricular dysfunction. On the basis of the concept, this randomized clinical trial comparing mitral valve replacement with preservation of mitral subvalvular apparatus and conventional mitral valve replacement performed in National Institute of Cardiovascular Diseases (N
