[Trichamber pacing in dilated myocardiopathy. TTDR pacing?].

This article describes the first case in Mexico city that received a three chamber pacing system. A 40 year-old man with dilated cardiomyopathy with variant cardiac rhythm and bradycardia. The three leads were introduced by right subclavian approaches. The right chamber leads were placed in atrial's appendage and in the right ventricular outflow tract and the last one was placed in the great cardiac vein. The two ventricular lead were connected a Y-connector to the ventricular channel of a standard bipolar DDDR pacemaker. The right ventricular lead was connected to the distal pole (anode) and

Leadless pacing: a comprehensive review.

Over the past decade, leadless pacing has undergone a rapid evolution, resulting in multiple leadless pacemaker (LPM) devices that offer advancements such as atrioventricular synchronized pacing in VDD mode, atrial stimulation, dual-chamber pacing, and longer battery longevity. Studies comparing LPMs with transvenous pacemakers (TVPMs) show a lower rate of device-related complications with LPMs. In the near future, LPMs could be combined with other devices such as non-transvenous implantable cardioverter-defibrillators to provide anti-tachycardia pacing or bradycardia pacing. Future prospectiv

[Leadless pacemakers : current evidence and future perspectives].

Leadless pacemakers represent a major advance in cardiac pacing. Unlike conventional systems, they are entirely implanted within the heart, without transvenous leads or a subcutaneous generator, thereby substantially reducing the risks of infection, lead fracture, and device displacement. First introduced in 2012 with the Nanostim™ (St. Jude Medical®), this technology subsequently evolved with the market introduction of the Micra™ (Medtronic®), which is easy to implant and has a safety profile comparable to that of conventional pacemakers. More recently, the AVEIR™ system (Abbott's®) has marke

Atrioventricular synchronous leadless pacemaker: state of art and broadened indications.

Leadless pacemakers (LLPMs) have become a major breakthrough in the management of bradyarrhythmia as an attractive alternative to the standard transvenous pacemakers (TV-PMs). Recently, the introduction of a second-generation LLPMs (Micra AV-MC1AVR1) has expanded pacing modes to obtain atrioventricular (AV) synchronous pacing, providing an interesting alternative in the actual scenario of leadless pacing. Nevertheless, actual reports have highlighted some concerns regarding those devices. In this review, we sought to provide an overview of this technology based on its approval studies and majo

[Analysis of programmed pacing rate levels in patients with the carotid sinus hypersensitivity syndrome and implanted Clarity pacemakers].

New Clarity DDDR pacemaker system (Vitatron Medical B.V.) Clarity DDDR, provides an option for recognizing sudden rate drop and responding by intervention pacing until it detects the recovering. In patients in whom syncopal episodes are mainly caused by occasional drops in heart rate, Sudden Rate Drop Intervention feature intends to provide high rate intervention pacing. We have implanted 10 of these devices in our Centre, 2 of which in patients with hypersensitive carotid sinus syndrome. In patients with carotid sinus syndrome it is possible to provoke this situation by sinus caroticus massag

Echocardiographic methods for selecting patients suitable for biventricular pacing therapy.

The large outcome studies of biventricular pacing to date have selected patients using electrocardiogram criteria (prolonged QRS and left bundle branch block morphology). However, 20-30% of patients do not appear to respond clinically, and as a result there has been much interest in developing more specific methods of detecting mechanical dyssynchrony. A number of different echocardiographic techniques have been developed which appear to offer greater sensitivity and specificity than ECG in selecting these patients. This paper reviews the most promising of the echocardiographic techniques and

[Electrophysiological study].

The purpose of an electrophysiological study (EPS) is to determine the mechanism of arrhythmias and to decide the means of therapy for that arrhythmia. The electrode catheters are introduced percutaneously into the atria or ventricle to record the intracardiac electrogram or to stimulate those areas by electrical current. In bradycardia, atrioventricular block and sick sinus syndrome are the good indications of EPS. To determine the indication of pacemaker implantation, recording the block site of atrioventricular block is useful, and sinus node recovery time and sinoatrial conduction time in

Alignment of P waves for signal averaging.

Signal averaged atrial activity recorded from the body surface is finding increased use in assessing atrial arrhythmias. The process of signal averaging is critically dependent upon precise alignment of the waveform of interest. The alignment accuracy of wide (0.01-300 Hz) and narrow (10-300 Hz) bandpass filtered P waves recorded from the body surface were compared to atrial electrograms recorded from temporary pacing wires. The locations of the filtered P waves relative to the atrial electrogram were calculated beat-by-beat for 100 beats in 15 patients aged 1 month to 71 years. The standard d

Leadless pacemakers at 5-year follow-up: the Micra transcatheter pacing system post-approval registry.

BACKGROUND AND AIMS: Prior reports have demonstrated a favourable safety and efficacy profile of the Micra leadless pacemaker over mid-term follow-up; however, long-term outcomes in real-world clinical practice remain unknown. Updated performance of the Micra VR leadless pacemaker through five years from the worldwide post-approval registry (PAR) was assessed. METHODS: All Micra PAR patients undergoing implant attempts were included. Endpoints included system- or procedure-related major complications and system revision rate for any cause through 60 months post-implant. Rates were compared th

[Cardiac memory following pacemaker implantation].

BACKGROUND: Although T wave inversions due to cardiac memory were described already 50 years ago, little is known about the prevalence and about clinical predictors of this phenomenon. METHODS: After exclusion of 238 patients due to bundle branch block or pacemaker dependency, a total of 325 consecutive patients were enrolled in this study during routine outpatient control of their pacemaker. A 12-lead standard ECG was obtained in all patients during transient inhibition of pacing therapy. RESULTS: Cardiac memory could be documented in 115 of 325 patients (35%) and showed a strong associatio

Cardiac pacing: the state of the art.

Permanent cardiac pacing remains the only effective treatment for chronic, symptomatic bradycardia. In recent years, the role of implantable pacing devices has expanded substantially. At the beginning of the 21st century, exciting developments in technology seem to happen at an exponential rate. Major advances have extended the use of pacing beyond the arrhythmia horizon. Such developments include dual-chamber pacers, rate-response algorithms, improved functionality of implantable cardioverter defibrillators, combinations of sensors for optimum physiological response, and advances in lead plac
