Metabolic abnormalities and cardiovascular risk factors in children with myositis.

OBJECTIVE: To characterize the metabolic abnormalities and risk factors for future cardiovascular disease in children with myositis. STUDY DESIGN: Seventeen patients with severe juvenile myositis, primarily referred because of refractory disease, were examined with standardized disease activity and damage measures. Body mass index, fasting insulin and lipid levels, 2-hour oral glucose tolerance test results, and cytokine levels were obtained. RESULTS: Most patients (71%) had blood pressures >75th percentile; 23.5% of patients had hypertension; and body mass index was >85th percentile in 47%.

Autonomic mechanisms and therapeutic implications of postural diabetic cardiovascular abnormalities.

BACKGROUND: Cardiovascular autonomic neuropathy (CAN) is a disorder of progressive autonomic dysfunction (AD) associated with diabetes and other chronic diseases. Orthostatic hypotension (OH) is one of the most incapacitating symptoms of CAN and AD. AD in OH can include sympathetic withdrawal (SW). To detect and diagnose SW, parasympathetic and sympathetic changes must be clearly differentiated from each other. This is accomplished by means of the novel autonomic nervous system (ANS) method based on the simultaneous spectral analyses of respiratory activity (RA) and heart rate variability (HRV

Retinoic acid reverses ethanol-induced cardiovascular abnormalities in quail embryos.

The effect of ethanol on early avian cardiovascular development was investigated in stage 8 quail embryos grown in culture for 24 hr. When the culture medium contained 1% ethanol, 50% of the embryos developed abnormalities of the cardiovascular system, some of which resembled vitamin A deficiency. Only 15% of the embryos grown in control media developed abnormalities attributed to the manipulation of the embryo. When all-trans-retinoic acid, the active form of vitamin A, was added at 10(-8) M to the ethanol-containing medium, the cardiovascular development was similar to that of untreated cont

Immunohistochemical abnormalities of fibrillin in cardiovascular tissues in Marfan's syndrome.

BACKGROUND: Molecular defects in the glycoprotein fibrillin are believed to be responsible for impaired structural integrity of cardiovascular, skeletal, and ocular tissues in Marfan's syndrome (MFS). Traditionally, excellent results have been achieved with the Bentall composite graft repair of aneurysms of the ascending aorta in MFS. However, because of the potential complications associated with prosthetic valves, there is growing interest in techniques that preserve the native aortic valve. METHODS: Between May 1994 and February 1995, 15 patients with a history of concomitant or remote aor

Cardiovascular abnormalities in children with autosomal dominant polycystic kidney disease.

It is known that adults with autosomal dominant polycystic kidney disease (ADPKD) have an increased incidence of cardiovascular abnormalities, including mitral valve prolapse. The cardiac manifestations of ADPKD in the pediatric population have not been well established. To determine the cardiac manifestations of children with ADPKD, echocardiography was performed in 154 children of 66 families in which one parent has ADPKD. Eighty-six affected children and 68 unaffected children were evaluated in a prospective, single-blinded manner by echocardiography. Affected children were defined as those

[Cardiovascular abnormalities in Ehlers-Danlos syndrome. Report of a case].

This is the case of a 34-year-old woman with Ehlers-Danlos syndrome whose cardiopulmonary manifestations are the following: Prolapse of mitral and tricuspid valves. Aneurysmal dilatation of main arteries without aortic or pulmonary insufficiency. Disturbances in pulmonary function tests and pulmonary arterial hypertension. The diagnosis was verified by skin biopsy and an electron microscopic study. Due to the clinical and histopathological characteristics, we have considered this case to be a non-specified type of the 10 varieties described up to now, and have decided to report it also because

Cholesterolemia and cardiovascular abnormalities in rats caused by copper deficiency.

The association of copper with cardiovascular disease and a possible involvement of copper in the metabolism of cholesterol prompted the study on hypercholesterolemia mediated by copper deficiency. Copper deficient rats were found to exhibit a highly significant cholesterolemia (P less than 0.001), and plasma cholesterol showed a significant correlation with hepatic copper concentration (P less than 0.03). Two copper deficient rats died with hemothorax. The hearts of copper deficient rats were hypertrophied with large areas of hemorrhage, inflammation and focal necrosis. Prominent subendocardi

Abnormal kynurenine pathway of tryptophan catabolism in cardiovascular diseases.

Kynurenine pathway (KP) is the primary path of tryptophan (Trp) catabolism in most mammalian cells. The KP generates several bioactive catabolites, such as kynurenine (Kyn), kynurenic acid (KA), 3-hydroxykynurenine (3-HK), xanthurenic acid (XA), and 3-hydroxyanthranilic acid (3-HAA). Increased catabolite concentrations in serum are associated with several cardiovascular diseases (CVD), including heart disease, atherosclerosis, and endothelial dysfunction, as well as their risk factors, including hypertension, diabetes, obesity, and aging. The first catabolic step in KP is primarily controlled

Abnormalities of kidney function as a cause and a consequence of cardiovascular disease.

Hypertension, left ventricular hypertrophy (LVH), hypercreatininemia, and microalbuminuria (MA) are independent risk factors for cardiovascular disease (CVD). Hypertension increases the risk of CVD by two- to three-fold and LVH (especially concentric) is a risk factor for coronary heart disease, heart failure, stroke, and peripheral arterial disease. In people with hypertension, a serum creatinine level of 1.7 mg/dL or more may be an even stronger CVD risk factor than diabetes, smoking, LVH, or systolic blood pressure. Similarly, MA is a strong and independent predictor of CVD morbidity and mo

Cardiovascular abnormalities in progeria. Case report and review of the literature.

The cardiovascular findings in cases of progeria are similar in many respects to the changes associated with aging. These changes include prominent atherosclerosis and calcification of the aortic and mitral valves, coronary arteries, and aorta. The coronary artery disease leads to ischemic changes in the myocardium, including well-defined infarcts. In addition, the present case demonstrates narrowing of the small intramural arteries, which may have contributed to myocardial fibrosis.

Contributions of color Doppler to the evaluation of cardiovascular abnormalities in the fetus.

The applications of color Doppler sonography are reviewed in this article. Color Doppler is an efficient and time-saving means of assessing normal and abnormal flow in the fetus, making it an essential part of the echocardiographic examination for fetal congenital heart disease. In survey mode, color Doppler can be used to quickly evaluate whether flow channels are normal and to detect abnormal flow patterns. These abnormal flow patterns can then be examined further by pulsed or continuous wave Doppler, if needed. Color Doppler, which is subject to less rib shadowing than other Doppler techniq

Abnormal cardiovascular regulation in the mitral valve prolapse syndrome.

Studies of patients with mitral valve prolapse syndrome have suggested autonomic nervous system dysfunction, but a precise definition of mechanisms is lacking. We measured supine and standing heart rate, blood pressure, cardiac output, oxygen consumption, plasma catecholamines, and blood volume in 23 symptomatic women with both echocardiographic and phonographic signs of MVP and in 17 normal control subjects. An analysis of the results revealed 2 distinct subgroups of patients: those with normal heart rates but increased vasoconstriction (Group I, n = 10) and those with orthostatic tachycardia

Abnormalities of the urinary tract in association with congenital cardiovascular disease.

The incidence of urinary tract abnormalities in a group of infants and children with congenital cardiovascular disease was determined by assessing roentgenograms of the abdomen taken following angiocardiographic studies. The urinary tracts of 400 of these patients were so outlined, but in 50 others the tracts could not be outlined. In 21 of the latter the urinary tracts were assessed at autopsy.Of these 421 children, nine (approximately 2%) had serious urinary tract disease, and 15 (approximately 3.5%) had anomalies which were of no clinical significance. The incidence of four renal anomalies-b

Antihypertensive Effects of Diosmetin in Hypertension-Induced Cardiovascular Abnormalities in Rats.

INTRODUCTION: Hypertension is associated with cardiovascular dysfunction and remodeling. Diosmetin, a flavonoid isolated from citrus seeds, has various biological properties. This work investigated the effects of diosmetin on cardiovascular parameters and classical and non-classical renin-angiotensin systems (RAS) in two-kidney one-clip (2K-1C) rats. METHODS: 2K-1C hypertension was induced by placing a silver clip on the left renal artery. Three weeks after induction, rats were orally gavage with either vehicle, diosmetin (20 or 40 mg/kg), or telmisartan (5 mg/kg) for four weeks. Blood pressu

Long-term outcomes of patients with cardiovascular abnormalities and williams syndrome.

Williams syndrome (WS) is a congenital disorder affecting the vascular, connective tissue, and central nervous systems of 1 in 8,000 live births. Previous reports have reported high frequencies of cardiovascular abnormalities (CVAs) in small numbers of patients with WS. A retrospective review was undertaken of patients with WS evaluated at our institution from January 1, 1980 through December 31, 2007. WS was diagnosed by an experienced medical geneticist and/or by fluorescence in situ hybridization. CVAs were diagnosed using echocardiography, cardiac catheterization, or computed tomographic a

Acid-base and electrolyte abnormalities observed in patients receiving cardiovascular drugs.

Cardiovascular drugs can cause a variety of acid-base and electrolyte abnormalities that need to be considered when clinicians manage the large number of patients who receive these agents. Diuretic-induced metabolic alkalosis is the most common acid-base disorder observed and is associated with hypokalemia. Drug-induced hyperkalemia is the most important cause of increased potassium levels in everyday clinical practice. Multifactorial-origin diuretic-induced hyponatremia is mostly due to thiazides and should be carefully managed. This review focuses on the pathogenetic mechanisms as well as on
