Cerebral revascularization by transverse cervical-common carotid artery anastomosis.

A surgical procedure to bypass occlusive lesions of the proximal common carotid or vertebral artery is described. The case of a patient with recurrent cerebral ischemic episodes associated with proximal left common carotid stenosis is reported. A left transverse cervical-common carotid artery anastomosis was performed. It is suggested that this simple procedure may be useful in selected cases.

[Subclavian artery to external carotid artery bypass with saphenous vein graft in the treatment of common carotid artery stenosis].

A case of TIAs due to proximal common carotid artery stenosis which was successfully treated with autogenous saphenous vein graft between the subclavian artery and the external carotid artery is presented. A 57-year-old, right handed female was admitted to our hospital for the treatment of left common carotid artery stenosis which was pointed out at a local hospital. She had a 7-years' history of repeated transient right hemiparesis and/or left amaurosis fugax. No neurological deficit was revealed on admission. Angiography showed an 80% irregular stenosis of the left common carotid artery at i

Carotid-carotid artery crossover bypass using the Saphenous Vein Graft for symptomatic common carotid artery occlusion: how I do it.

BACKGROUND: Common carotid artery occlusion (CCAO) is rare, where a revascularization procedure might be needed in symptomatic or recurrent ischemic events. In this study, we describe the carotid-carotid artery crossover bypass technique for Riles type 1 A CCAO. METHODS: The procedure was conducted via bilateral neck incisions utilizing the saphenous vein graft. The graft was patent after surgery, along with substantial improvement in cerebral perfusion, resulting in a stroke-free postoperative period. CONCLUSION: The carotid-carotid crossover bypass is effective for CCAO patients requiring

Common Carotid-Greater Saphenous-M2 Bypass for Myoepithelial Carcinoma Encasing the Carotid Artery.

Revascularization of the anterior circulation using a high-capacitance graft is sometimes necessary for treatment of cervical tumors encasing the internal carotid artery (ICA). In this surgical video, we aim to demonstrate the technical nuances of high-flow extra-to-intracranial bypass using a saphenous vein graft. The patient is a 23-year-old female who presented with a 4-month history of an enlarging left-sided neck mass, dysphagia, and 25-lb weight loss. Computed tomography and magnetic resonance imaging demonstrated an enhancing lesion encasing the cervical ICA. The patient underwent an op

Isolated anomalous origin of the vertebral artery from the common carotid artery.

Anomalous origins of the vertebral arteries are uncommon and typically associated with other abnormalities of the great vessels. We present a case of an isolated origin of the right vertebral artery from the ipsilateral common carotid artery detected using magnetic resonance angiography. Such variants can significantly affect endovascular and surgical planning.

Common Carotid Artery Occlusion Presenting with Recurrent Syncopal Episodes.

Symptomatic common carotid artery (CCA) occlusion is an uncommon occurrence that may require surgical intervention. We aim to describe a case of CCA occlusion that presented with the unusual symptom of recurrent syncope. A 69-year-old lady presented with a history of recurrent syncopal episodes and amaurosis fugax associated with left leg weakness. She was found to have a right CCA occlusion on duplex ultrasound and angiography. She underwent a right common carotid endarterectomy and intraoperative findings revealed a heavily calcified plaque in the CCA just proximal to the bifurcation with or

Proximal common carotid artery lesions: endovascular and open repair.

OBJECTIVES: Management of proximal common carotid artery (pCCA) lesions is infrequently reported. We described open and endovascular treatment with regard to the neurological outcome and patency in patients suffering from atherosclerotic pCCA lesions. METHODS: Data were collected prospectively and analysed in a retrospective manner. RESULTS: From November 1991 to January 2010, 52 procedures, 24 surgical (11 bypasses, 12 transpositions and retrograde endarterectomy) and 28 endovascular (13 open transcervical and 15 transfemoral stent implantations) were performed (40.4% female, mean age 62.3

Unilateral anomalous left common carotid artery; a case report.

An anomaly of the left common carotid artery was observed in a Japanese male cadaver during an anatomy class at the Saitama Medical School in 1995. The superior thyroid, lingual and facial arteries arose from the common carotid artery, and the posterior auricular, maxillary and superficial temporal arteries arose from the common carotid artery by a common trunk. The occipital and ascending pharyngeal arteries arose from the internal carotid artery. The left carotid body (glomus caroticum) was observed to be slightly below the lingual artery, behind the common carotid artery, and it was located

Spontaneous dissection in the common carotid artery--case report.

A 60-year-old female presented with a very rare spontaneous dissection in the common carotid artery manifesting as disturbance of consciousness and left dense hemiparesis immediately after swimming. Cerebral angiography showed complete obstruction of the right common carotid artery. Endarterectomy was performed to remove the subintimal thrombus and dissected intima. Blood flow was re-established 4 hours after onset of symptoms. Her disturbance of consciousness was resolved and left dense hemiparesis disappeared completely 12 hours after onset.

Extrathoracic bypass procedures for proximal common carotid artery lesions.

During a 10-year period ending in December 1991, 31 extrathoracic bypass procedures were performed in 29 patients for proximal common carotid artery atherosclerotic stenosis or occlusion. This included 16 men and 13 women, with a mean age of 63 years. Indications for surgery included transient ischemic attacks in 23 patients (79%), nonfocal symptoms in 4 patients (14%), and asymptomatic proximal common carotid artery stenosis associated with near-total occlusion of the internal carotid artery in 2 patients (7%). Severe proximal stenosis or complete occlusion of the common carotid artery was de

Thyrolinguofacial trunk arising laterally from the common carotid artery with superior laryngeal artery and high carotid bifurcation.

During routine dissection of a 59-year-old male cadaver, we identified a rare anatomical variation consisting of a thyrolinguofacial trunk (TLFT) arising from the lateral aspect of the right common carotid artery (CCA), which also gave rise to the superior laryngeal artery (SLA). The trunk measured 3 mm in diameter and branched successively into the superior thyroid artery, SLA, lingual artery, and facial artery. This variation coexisted with a very high carotid bifurcation at the C2 vertebral level (angle of the mandible). To the best of our knowledge, this specific combination, lateral CCA o

Common carotid artery end-diastolic velocity and acceleration time can predict degree of internal carotid artery stenosis.

OBJECTIVE: Whereas duplex ultrasound parameters for predicting internal carotid artery (ICA) stenosis are well defined, the use of common carotid artery (CCA) Doppler characteristics to predict ICA stenosis when the ICA cannot be insonated directly or accurately because of anatomy, calcification, or tortuosity has not been studied. The objective of this study was to identify CCA Doppler parameters that may predict ICA stenosis. METHODS: We reviewed all patients at our institution who underwent carotid duplex ultrasound (CDU) from 2008 to 2015 and also had a comparison computed tomography, mag

Aorto-left common carotid artery bypass with off-pump coronary artery bypass.

BACKGROUND: Although carotid artery disease can occur in patients who have coronary artery disease (CAD), ostial stenosis of a left common carotid artery (CCA) along with CAD is a rarely reported condition. CASE REPORT: We describe a rare case of severe stenosis of the left CCA ostium and CAD, which were successfully treated with an off-pump coronary artery bypass and aorto-left CCA bypass. This strategy minimizes the risk of perioperative strokes. Furthermore, an aorto-supra aortic trunk bypass provides a more physiological blood flow pattern than cerebral approaches. This approach would be

Management of Innominate Artery Occlusion With Severe Left Common Carotid Artery Stenosis.

Innominate artery occlusion is a rare entity, particularly when coupled with severe left common carotid artery stenosis. Innominate artery disease may present with varying degrees of symptomatology and can place patients at risk for both posterior fossa and hemispheric ischemic events. We present a symptomatic case of innominate artery occlusion with severe left common carotid disease. We reviewed the literature and current options for the treatment of innominate artery disease. The patient underwent successful hybrid repair with left carotid artery retrograde stenting and left carotid artery
