Sacrificing Internal Carotid Artery-External Carotid Artery-Radial Artery-Middle Cerebral Artery Bypass for the Treatment of Complex Intracranial Artery Cavernous Sinus Aneurysms.

The treatment of complex intracranial aneurysms has always been a great challenge in neurosurgery. Craniotomy has a high risk of clipping, there is a risk of rupture at any time, endovascular embolization is relatively low risk, but expensive and easy to relapse, the best treatment needs to be further discussed. Cavernous sinus aneurysms with pituitary adenomas are rare. This case reports a case of complex intracranial aneurysms. Chief complaints are: (1) blepharoptosis with blurred vision for 1 year, (2) headache for 3 days, and (3) digital subtraction angiography showed right internal caroti

External Carotid Artery Entrapment by the Hyoid Bone Associated with an Atherosclerotic Stenosis of the Internal Carotid Artery.

The mechanical compression of an external carotid artery (ECA) is a rare pathology. The compression of the carotid bifurcation can be positional, induced by anatomical elements, or provoked by volumetric formation in the neck area. In this study, we describe a rare case of an entrapment of the ECA. A 67-year-old man who had two episodes of transient ischemic attack (TIA) demonstrated by loss of consciousness was transferred to our hospital. Ultrasonography and computed tomography revealed the atherosclerotic stenosis (80%) of a right internal carotid artery (ICA) and, at the same time, entrapm

Common carotid artery occlusion with patent internal and external carotid arteries: diagnosis and surgical management.

PURPOSE: Occlusion of the common carotid artery (CCA) is generally associated with occlusion of the ipsilateral internal carotid artery (ICA) and external carotid artery (ECA). Occasionally, however, collateral circulation to the ECA may preserve patency of the ICA via retrograde perfusion through the bulb. These patients may suffer ongoing transient ischemic attacks and risk for stroke. Recognition of this pathologic variant may allow for effective surgical intervention. METHODS: We have performed seven operations in six patients with occluded CCAs and patent ECA and ICAs. The occluded CCA w

Spontaneous bilateral internal carotid and vertebral artery dissections with dominant-hemisphere circulation maintained by external carotid artery-ophthalmic artery anastomoses.

Spontaneous cervical artery dissection (sCAD) is a major cause of stroke in young adults. Multiple sCAD is a rarer, more poorly understood presentation of sCAD that has been increasingly attributed to cervical trauma such as spinal manipulation or genetic polymorphisms in extracellular matrix components. The authors present the case of a 49-year-old, otherwise healthy woman, who over the course of 2 weeks developed progressive, hemodynamically significant, bilateral internal carotid artery and vertebral artery dissections. Collateral response involved extensive external carotid artery-internal

Riles type 1A common carotid artery occlusion diagnosed by specific external carotid artery Doppler waveform pattern in carotid ultrasonography. Case report.

A 67-year-old man was admitted for evaluation of left homonymous hemianopsia. Carotid ultrasonography showed that the right common carotid artery (CCA) was occluded up to just proximal to the carotid bifurcation, and the patent external carotid artery showed retrograde flow to the patent internal carotid artery via the carotid bifurcation. The Doppler waveform pattern of the external carotid artery showed high end-diastolic flow velocity and low pulsatility index. The diagnosis was Riles type 1A CCA occlusion. Digital subtraction angiography and iodine-123 N-isopropyl-p-iodoamphetamine single

External carotid artery ligation in the management of carotid body tumors: A case report.

INTRODUCTION AND IMPORTANCE: Carotid body tumors (CBTs) are rare paragangliomas located at the bifurcation of the carotid artery. These tumors present significant clinical challenges due to their potential for local extension, high vascularity, and proximity to critical neurovascular structures. The rarity of complete resections requiring external carotid artery ligation (ECAL) underscores the clinical significance of this case, highlighting both the technical challenge and the functional outcome. CASE PRESENTATION: We report a case of a CBT in a middle-aged patient presenting with a progress

The natural history of the external carotid artery after carotid endarterectomy: implications for management.

PURPOSE: Most surgeons perform some type of endarterectomy of the external carotid artery (ECA) routinely during standard carotid endarterectomy (CEA). This approach has been shown to result in a small percentage of ECA occlusions, the clinical significance of which remains poorly understood. We have modified our approach to the management of the ECA during standard CEA by averting any attempt at external CEA. To evaluate the natural history of the untreated ECA after CEA, we reviewed the preoperative, postoperative, and follow-up duplex scans obtained from 232 CEAs over the past 4 years. MET

External carotid endarterectomy for amaurosis fugax in the presence of internal carotid artery occlusion.

Two patients with occlusion of the internal carotid artery who were experiencing repeated episodes of transient monocular blindness in the ipsilateral eye were successfully treated with external carotid endarterectomy. The mechanisms for the production of symptoms in the presence of an internal carotid occlusion are discussed, including the anatomical pathways for embolization through collaterals between the internal and external carotid arteries. The indications, technique and results of external carotid endarterectomy are reviewed.

[Pseudoaneurysm of Neck External Carotid Artery Related to Chiropractic Manipulation].

A 55-year-old man presented with progressive pain and expanding swelling in his right neck. He had no history of trauma or infectious disease. The patient had undergone chiropractic manipulations once in a month and the last manipulation was done one day before the admission to our hospital. On examination by laryngeal endoscopy, a swelling was found on the posterior wall of the pharynx on the right side. The right piriform fossa was invisible. CT revealed hematoma in the posterior wall of the right oropharynx compressing the airway tract. Aneurysm-like enhanced lesion was also seen near the r

External carotid artery branches involvement in reversible cerebral vasoconstriction syndrome.

Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by segmental vasoconstriction and dilatation of intracranial arteries, usually revealed by headaches, which spontaneously resolve in few weeks. We report a patient with RCVS, revealed by thunderclap headaches, involving both internal and external carotid artery (ECA). She received fluoxetin for depression and took a great amount of cannabis in the last months. While angio-MR, transcranial Doppler and CSF analysis were normal, cerebral angiography disclosed stenoses and dilatations of the middle cerebral artery. It also showe

Treatment of limb-shaking TIAs with external carotid artery stenting.

Limb-shaking transient ischemic attacks (TIAs) occur when perfusion is reduced to the cortical territory associated with the transient movements. We present a case in which a patient with preexisting left internal carotid artery (ICA) occlusion developed limb-shaking TIAs secondary to severe stenosis of her left external carotid artery (ECA). After angioplasty and stenting of her left ECA, her symptoms resolved.

The treatment of cerebral ischemia by external carotid artery revascularization.

Thirteen patients with internal carotid artery occlusion and symptomatic external carotid artery stenosis underwent external carotid artery revascularization by means of endarterectomy (n = 10) or subclavian artery--external carotid artery bypass (n = 3). All patients but one were followed up, for five to 46 months (mean, 20 months). There were no operative deaths or complications and no late strokes. One patient (7.7%) required early extracranial-intracranial (EC-IC) bypass for failure of the external carotid artery endarterectomy to relieve the initial symptoms. All remaining patients were c

Persistent hypoglossal artery arising from the external carotid artery diagnosed by MR angiography.

PURPOSE: Extremely rarely, a persistent hypoglossal artery arises from the external carotid artery; only three cases have been reported in the English-language literature. The purpose of this paper is to report a case of this variation diagnosed by magnetic resonance (MR) angiography. METHODS: A 75-year-old man with sudden-onset right hemiparesis and dysarthria underwent emergency cerebral MR imaging and cerebral MR angiography that included the cervical carotid bifurcation. A 1.5-T scanner was used and MR angiographic images were obtained using the standard three-dimensional time-of-flight t

Subclavian artery to external carotid artery bypass graft. Improvement of cerebral blood supply.

External carotid flow may contribute a substantial amount of cerebral flow via collaterals in cases of internal carotid artery occlusion. When common and internal carotid arteries are occluded, the distal portion of the external carotid artery is kept open by collateral flow. Revascularization of the external carotid by means of a bypass graft from the subclavian artery will improve the blood flow through collateral vessels connecting the external and internal carotid territories, and thus increase the cerebral blood supply from the branches of the external carotid artery.

Unusual branching pattern of the external carotid artery in a cadaver.

With the increasing use of invasive diagnostic and interventional procedures in cardiovascular disease, it is important to document and understand the types and frequencies of vascular variations. A sound knowledge of neurovascular variations is important for surgeons who remove cervical lymph nodes, for anesthesiologists, and for vascular surgeons. The external carotid arterial system is a complex vascular system which nourishes the terminal areas of the head, face and neck. The branches of the external carotid artery are the key landmarks for adequate exposure and appropriate placement of cr

Pseudo aneurysm of the external carotid artery: report of a case.

The authors report a case of left otorrhagia in a 1-year-old male infant in the presence of a mass involving the parotideal and upper cervical regions, which had appeared after an infection of the upper airways. Non-invasive techniques, such as echography and CT scan, provided useful but contradicting information. Surgery allowed us to define the diagnosis of mycotic aneurysm of the external carotid artery. The authors, after pointing out the extreme rarity of such a pathology, discuss the ethiopathogenetic theories, the clinical features, the diagnosis and the surgical and medical treatment o

Traumatic arteriovenous malformation of the external carotid arterial system.

Traumatic arteriovenous (AV) malformations of the face and scalp are rare lesions characterized by multiple endothelial-lined channels between the arterial and venous systems. If improperly managed they have a high propensity to recur, and may result in severe cosmetic deformity. Lesions should be delineated by arteriography unless small and localized. They are managed by complete excision and ligation of arterial feeding vessels. Five cases of traumatic AV malformation of the face and scalp and their management are reported.

External carotid endarterectomy in the treatment of symptomatic patients with internal carotid artery occlusion.

In patients with internal carotid artery (ICA) occlusion, the external carotid artery (ECA) can be both a source of collateral flow and a pathway for emboli. We identified 11 patients with ICA occlusion and ipsilateral ECA stenosis who underwent ECA endarterectomy to determine its role in treating extracranial cerebrovascular disease. Follow-up ranged from 1-65 months, with a mean of 27 months. Seven of eight patients with unilateral disease remained symptom free. The eighth patient had recurrent symptoms that were subsequently diagnosed as hemi-Parkinsonism. Two of three patients with bilater
