INTRODUCTION: Carotid webs (CaWs) are increasingly recognized as a cause of cryptogenic acute ischemic stroke (AIS). Due to the risk of recurrent ischemic stroke associated with CaW, it is important to identify them. Although several studies report digital subtraction angiography and computerized tomography angiography (CTA) to be better at diagnosing CaW than Doppler ultrasound (DUS), it remains to be defined which is the best radiological method to diagnose CaW. Our aim was to evaluate sensitivity and accuracy of DUS compared to CTA for CaW diagnosis.
METHODS: We searched PUBMED and EMBASE

A 48-year-old man with no history of diagnosis with any abnormality was admitted to our hospital 43 min after onset of stroke. He had a right conjugate deviation and severe left hemiparesis, and his initial National Institutes of Health Stroke Scale (NIHSS) score was 13. Computed tomography (CT) of the head revealed no early ischemic changes, while CT angiography showed occlusion of the M1 proximal segment of the right middle cerebral artery. Intravenous thrombolysis was administered within 27 min of admission, and mechanical thrombectomy was performed. Effective reperfusion was achieved 55 mi

BACKGROUND AND IMPORTANCE: A carotid web (CaW) is an intraluminal membrane-like filling defect typically located in the posterior wall of the proximal internal carotid artery and is increasingly recognized as a potential cause of embolic stroke. We herein reported a case of a CaW that has an unusual location at the CCA; furthermore, an elongated transverse process of the cervical spine was adjacent to the CaW at the CCA.
CLINICAL PRESENTATION: An 87-year-old woman with a history of minor stroke underwent thorough radiological examinations of her CCA lesion. Radiological examinations, includin

INTRODUCTION: Carotid web (CaW), a focal intimal dysplasia, is a potential cause of embolic ischemic stroke (IS). The optimal therapeutic approach for symptomatic CaW remains unclear.
METHODOLOGY: Since January 2016, patients hospitalized for IS with an ipsilateral CaW, identified during etiological workup, are prospectively included in this ongoing single-center observational cohort study.
RESULTS: Until April 2024, 32 patients (59% men; mean age 48±7.5 years; mean NIHSS score 8±6) were included. Seven patients (22%) had a history of prior IS. Thrombolysis and/or thrombectomy were performed

A carotid web is an under-recognized vascular abnormality increasingly identified as a cause of embolic ischemic stroke in younger patients without conventional vascular risk factors. Delayed diagnosis may lead to recurrent cerebrovascular events despite otherwise unrevealing investigations. We report the case of a 35-year-old woman who experienced three ischemic strokes over a three-year period. Initial investigations, including carotid Doppler ultrasonography, cardiac evaluation, CT angiography, and a young stroke laboratory screen, did not identify a clear etiology. Ongoing clinical suspic

Carotid web, a form of fibromuscular dysplasia, involves a thin, membrane-like tissue in the carotid bulb that can cause thrombus formation and is linked to cryptogenic ischemic stroke. Diagnosis typically relies on detecting a shelf-like filling defect in digital subtraction angiography or 3D-CT angiography. We report a case of the symptomatic carotid web that could not be diagnosed using DSA or 3D-CT angiography due to a lodged thrombus but was successfully identified through carotid ultrasonography. A 44-year-old woman with a history of subarachnoid hemorrhage developed left hemiparesis and

BACKGROUND: Carotid web (CaW) is non-atheromatous, shelf-like intraluminal projection, generally affecting the posterolateral wall of the proximal internal carotid artery, and associated with embolic stroke, particularly in younger patients without traditional stroke risk factors. Treatment options for symptomatic CaWs include interventional therapy with carotid endarterectomy or carotid stenting versus medical therapy with antiplatelet or anticoagulants. As safety and efficacy of these approaches have been incompletely delineated in small-to-moderate case series, we performed a systematic rev

BACKGROUND AND PURPOSE: Carotid web (CaW) is an intimal variant of fibromuscular dysplasia responsible for ipsilateral cerebral ischemic events (CIE). Symptomatic CaW likely has a high risk of recurrent CIE, but no salient prospective data are available. We aimed to assess recurrence rate and its predictors after a first-ever CIE.
METHODS: Consecutive Afro-Caribbean patients who had cryptogenic first-ever CIEs (ischemic stroke [IS] or transient ischemic attack [TIA]) associated with ipsilateral CaW were included in this multicenter observational cohort study. The follow-up (January 2008 to Ma

Background and Purpose- Carotid web (CW) is a rare form of focal fibromuscular dysplasia defined as an abnormal shelf-like projection of intimal fibrous tissue into the carotid bulb. It is theorized that CW leads to ischemic stroke secondary to blood flow stasis and subsequent embolization. The natural history and optimal management of CW are unclear. To address this knowledge gap, we performed a systematic literature review (SLR) of CW. Methods- Our librarians performed a SLR for CW and related terminology. Patient-level demographics, stroke risk factors, neuroimaging findings, stroke recurre

A 48-year-old man presented with impaired consciousness and aphasia. Magnetic resonance imaging (MRI) of the head revealed acute cerebral infarction in the left temporal and parietal lobes. Carotid ultrasonography revealed a mobile structure at the origin of the left internal carotid artery. Cervical MRI using fat-saturation T1 imaging showed an isointense structure at the origin, with a high-signal structure extending toward the head. Digital subtraction angiography revealed contrast stagnation, suggesting the presence of a carotid web (CaW). The mobile structure was released, indicating thro

Carotid web (CW) is a non-atheromatous and non-inflammatory linear filling defect which is an under-recognised aetiology of embolic stroke of undetermined source, particularly in the younger population under 60 years of age. Due to limited epidemiological data, imaging techniques and clinical experiences, CW may be underdiagnosed or misdiagnosed as other vascular pathologies such as atherosclerotic plaque. There are no established diagnostic or management guidelines for treating CW and preventing secondary stroke. We present the case of a male in his late 40s who experienced an ischaemic stroke

Background: Carotid web (CaW) usually presents as a shelf-like intimal flap at the beginning of the internal carotid artery. It has been proven that CaW is associated with ischemic stroke, particularly in young patients without other risk factors. This case report aimed to describe the carotid web that causes ischemic stroke due to embolic complications. Moreover, both pathologies were successfully treated with endovascular techniques in the presented case study. Methods: A 59-year-old male presented to the neurological department with motor aphasia, right-sided weakness, and hypoesthesia. Com

Acute ischemic stroke, a medical emergency caused by reduced cerebral blood flow, results in brain cell damage. While commonly associated with older individuals, strokes can also occur in young and middle-aged adults, posing significant socio-economic and health challenges due to the long-term impact of the condition. This poses significant socio-economic and health challenges because stroke is a leading cause of disability and mortality. A carotid web (CaW), a rare form of fibromuscular dysplasia, is increasingly recognized as a cause of cryptogenic stroke in young adults. Characterized by a

Carotid artery web (CaW) is a rare focal fibromuscular dysplasia, which is an underappreciated risk factor for transient ischemic attack. This case illustrates the dynamic evolution of secondary thrombus and plaque in CaW, and the importance of carotid doppler ultrasound in early detection and follow-up.

OBJECTIVE: Elongated styloid process (ESP) and carotid web are rare etiologies of ischemic stroke. We report a rare case of ESP concomitant with carotid web as the cause of recurrent stroke.
CASE PRESENTATION: A 59-year-old man was admitted to our hospital with recurrent numbness and weakness in the right upper extremity. The patient had a long-standing history of lightheadedness and left-side amaurosis with neck flexion. Magnetic resonance imaging (MRI) confirmed scattered infarctions in the left frontal and parietal lobes. After multi-modal imaging we determined that embolic cerebral infarc

Carotid atherosclerosis is the main cause of fatal cerebral ischemic events, thereby posing a major burden for public health and state economies. We propose a web-based platform named CAROTID to address the need for optimal management of patients with carotid atherosclerosis in a twofold sense: (a) objective selection of patients who need carotid-revascularization (i.e., high-risk patients), using a multifaceted description of the disease consisting of ultrasound imaging, biochemical and clinical markers, and (b) effective storage and retrieval of patient data to facilitate frequent follow-ups

PurposeRobotic-assisted carotid artery stenting (CAS) procedures are on the rise and have demonstrated benefits compared to manual intervention. The R-OneTM robotic platform has demonstrated safety and efficacy for percutaneous coronary interventions, but its utility for procedures in the common carotid artery has not been thoroughly studied. This study aims to assess the feasibility and initial safety of the R-OneTM robotic platform for treating patients with carotid artery disease (CAD) presenting with carotid web-diaphragm lesions.MethodsThe prospective CArotid Robotic procedure Evaluation

OBJECTIVES: Carotid artery web is an underrecognized cause of ischemic stroke and is associated with a high risk of recurrent events. It is uncertain whether medical management or carotid revascularization is beneficial for patients with ischemic stroke and ipsilateral carotid web. In the absence of large randomized clinical trials and observational studies, we performed a systematic review and meta-analysis comparing medical management and carotid revascularization in this population.
METHODS: The systematic review was registered in PROSPERO (CRD42024485069). We searched five databases: Emba
