Vibrational spectroscopic and Hirshfeld surface analysis of carvedilol crystal forms.

The drug carvedilol, used to treat cardiovascular conditions, is known to exist in distinct crystalline forms. Polymorphs II and III and the hydrate are characterized by variations in their molecular packing and conformation. This study deals with the spectroscopic (supported by quantum chemistry calculations) characterization of carvedilol structures. Band assignments were performed considering the isolated molecules and periodic calculations. We discuss the correlation between the vibrational modes and the intermolecular forces in the crystalline structures. Towards a better understanding of

Is carvedilol better than other beta-blockers for heart failure?

There is wide consensus about the benefits of beta-blockers in systolic heart failure. However, it is not clear if one specific beta-blocker is superior to the others. Some guidelines favor three “evidence-based” beta-blockers (carvedilol, bisoprolol and metoprolol) that have proved to decrease mortality. Carvedilol might have different physiological properties, commonly referred as pleiotropic effects, but the clinical meaning of them is not clear. Searching in Epistemonikos database, which is maintained by screening 30 databases, we identified four systematic reviews including eight pertinen

Carvedilol ameliorates early diabetic nephropathy in streptozotocin-induced diabetic rats.

Diabetic nephropathy results in end-stage renal disease. On the other hand, carvedilol has been reported to have various pharmacological properties. The aim of this study therefore is to evaluate the possible protective effect of carvedilol on streptozotocin-induced early diabetic nephropathy and various mechanisms underlie this effect in rats. Single i.p. injection of streptozotocin (65 mg/kg) was administered to induce early diabetic nephropathy in Wistar rats. Oral administration of carvedilol at a dose level of 1 and 10 mg/kg daily for 4 weeks resulted in nephroprotective effect as evident

High-performance liquid chromatography determination of carvedilol in pig serum.

A simple and sensitive analytical method for quantification of carvedilol in pig serum was developed and validated. Carvedilol and internal standard (IS) were extracted into n-hexane-dichloromethane solvent system and separated using an isocratic mobile phase on a Phenomenex C(18) column. The eluent was monitored by spectroflourimetric detector at a flow rate of 1.0 mL/min. The linearity range of proposed method was 1-1000 ng/mL. The intra-day and inter-day coefficient of variation and percent error values of the assay method were less than 15%, and mean recovery was more than 89.95 and 94.27

[Preliminary study on the effect of carvedilol on children with primary endocardial fibroelastosis].

OBJECTIVE: Endocardial fibroelastosis (EFE), a common pediatric cardiovascular disease, often results in chronic heart failure (CHF) and death. Clinical trials have shown that the regimen of combining beta-adrenoreceptor blocker with traditional medicines against CHF can improve left ventricular function and prevent the ventricle from remodeling in patients with CHF. The present study aimed to observe the effect of carvedilol on concentration of plasma brain-type natriuretic peptide (BNP), and safety in children with EFE. METHODS: Twenty-one children with EFE were randomly divided into two gr

Protective effect of carvedilol on chenodeoxycholate induction of the permeability transition pore.

Intracellular accumulation of toxic, hydrophobic bile acids has been proposed as one of the putative final common pathways leading to cholestatic liver injury. Furthermore, bile acids have been proposed as a causative factor for hepatic cardiomyopathy. Hepatic tissue concentrations of chenodeoxycholic acid (CDCA) during cholestasis are greater than those of other toxic bile acids. In the presence of calcium and phosphate, CDCA induced the permeability transition pore (PTP) in freshly isolated rat liver mitochondria. In this study, we evaluated the effects of carvedilol, a multirole cardioprote

Carvedilol. A review of its pharmacodynamic and pharmacokinetic properties, and therapeutic efficacy.

Carvedilol is a beta-adrenoceptor antagonist which also causes peripheral vasodilation primarily via alpha 1-adrenergic blockade. Carvedilol produces its antihypertensive effect partly by reducing total peripheral resistance by blocking alpha 1-adrenoceptors and by preventing beta-adrenoceptor-mediated compensatory mechanisms. This combined action avoids many of the unwanted effects associated with traditional beta-blocker or vasodilator therapy. In clinical trials published to date, most of which enrolled small numbers of patients, the antihypertensive efficacy of carvedilol administered once

Comparative Effectiveness of Carvedilol Versus Other Nonselective β-Blockers in Cirrhosis.

BACKGROUND: In cirrhosis, nonselective β-blockers (NSBBs; carvedilol, nadolol, and propranolol) reduce hepatic portal pressure and have demonstrated benefit versus placebo for preventing decompensation. Although carvedilol has emerged as the preferred NSBB, direct evidence remains limited about its effectiveness for preventing decompensation and death versus other NSBBs. OBJECTIVE: To compare the effectiveness of carvedilol versus nadolol versus propranolol in cirrhosis. DESIGN: Database cohort study. SETTING: A U.S. administrative claims database, Optum Clinformatics Data Mart (2013 to 202

S- and R-Carvedilol Prevent Benzo(a)pyrene-Induced Lung Carcinogenesis.

Lung cancer is the most common and deadly type of cancer. Our previous study showed that carvedilol, a β-blocker, can prevent lung cancer induced by benzo(a)pyrene [B(a)P]. Carvedilol is a 1:1 racemic mixture of S- and R-carvedilol, with S-carvedilol acting as a β-adrenergic blocker, while R-carvedilol lacks β-blocking activity. Despite this, a previous study showed that R-carvedilol effectively prevents UV-induced skin cancer. This study aimed to determine whether carvedilol's lung cancer prevention relies on its β-blocking activity. We compared the effectiveness of S- and R-carvedilol in pre

Carvedilol Ameliorates Experimental Atherosclerosis by Regulating Cholesterol Efflux and Exosome Functions.

Carvedilol (Cav), a nonselective β-blocker with α1 adrenoceptor blocking effect, has been used as a standard therapy for coronary artery disease. This study investigated the effects of Cav on exosome expression and function, ATP-binding cassette transporter A1 (ABCA1) expression, and cholesterol efflux that are relevant to the process of atherosclerosis. Human monocytic (THP-1) cell line and human hepatic (Huh-7) cells were treated with Cav, and cholesterol efflux was measured. Exosomes from cell culture medium or mice serum were isolated using glycan-coated recognition beads. Low-density lipo

Beneficial effects of carvedilol modulating potassium channels on the control of glucose.

The increased prevalence of hypertensive patients with type 2 diabetes mellitus (T2DM) is evident worldwide, leading to a higher risk of cardiovascular disease onset, which is substantially associated with disabilities and mortality in the clinic. In order to achieve the satisfyingly clinical outcomes and prognosis, the comprehensive therapies have been conducted with a beneficial effect on both blood pressure and glucose homeostasis, and clinical trials reveal that some kind of antihypertensive drugs such as angiotensin converting enzyme inhibitors (ACE-I) may, at least in part, meet the dual

The effects of carvedilol, metoprolol and propranolol on cisplatin-induced kidney injury.

The β-adrenoceptor blockers may have anti-oxidant properties or induce β-arrestin recruitment beyond classical desensitization of receptor/G protein coupling, offering potential therapeutic benefits. Here, we investigated the effects of carvedilol, metoprolol and propranolol in an animal model of cisplatin-induced nephrotoxicity. Rats received the β-blockers (3 or 12 mg/kg/day) with or without cisplatin, and kidney function was investigated using renal scintigraphy, histopathology, and serum variables. Metoprolol and propranolol as well as low-dose carvedilol did not alter kidney function, per

Persistent wheezing caused by carvedilol overdose in a non-asthmatic man.

BACKGROUND: Cardiovascular dysfunction is the main manifestation of β-blocker intoxication; however, respiratory manifestations have rarely been reported. CASE PRESENTATION: A 41-year-old man, who had ingested 300 mg carvedilol in a suicide attempt, was transferred to our emergency department. The patient had wheezing on arrival; however, he had no known history of bronchial asthma. In the absence of signs of heart failure, we gave the patient inhaled procaterol, a short-acting β2 agonist. The wheezing disappeared approximately 60 h after carvedilol ingestion and did not recur thereafter. CO

Carvedilol and antioxidant proteins in a type I diabetes animal model.

BACKGROUND: Patients with diabetes are at a high risk of developing both micro- and macrovascular disease. Hyperglycaemia seems to be the main factor in the pathogenesis of diabetic cardiomyopathy, often based on increased oxidative stress. Carvedilol, a β-adrenergic blocker, has intrinsic antioxidant properties and was previously described to be effective in the protection of cardiac mitochondria against oxidative stress. The objective of this study was to evaluate the effect of carvedilol on hyperglycaemia-induced oxidative damage and mitochondrial abnormalities in cardiac and skeletal muscl
