Associated Factors for Antegrade Continence Enemas for Refractory Constipation and Fecal Incontinence.

OBJECTIVE: Determine clinical and manometric parameters associated with success of antegrade continence enemas (ACEs) administered via cecostomy in the treatment of constipation and fecal overflow incontinence. METHODS: We performed a retrospective review of clinical symptoms and manometry (colonic and anorectal) before cecostomy in 40 pediatric patients (20 males, 20 females). The mean age at time of follow-up was 9.5 ± 4.4 years with a mean follow-up time of 12.2 ± 10.9 months. Clinical outcomes were defined as good, if subjects had >3 bowel movements per week, <2 episodes of soiling per we

Volvulus of the cecum: choice of operation.

Although it is clear that there is a significant frequency of recurrent volvulus after cecopexy, there is no evidence from either this series or previous reports that resection is associated with a lower recurrence rate than tube cecostomy. In the absence of data suggesting a clear superiority of colon resection, it appears that tube cecostomy should be the treatment of choice for cases of cecal volvulus that are not complicated by vascular compromise.

Diastatic perforation of the cecum without distal obstruction. Case report and review of the literature.

We present the first reported case to our knowledge of diastatic rupture of the normal cecum following cardiac surgery. All other reported cases of diastatic rupture of the cecum are reviewed. Nasotracheal intubation, hypoxemia, and enemas are thought to contribute to this complication. Prophylactic cecostomy for cecal diameters greater than 9 cm is recommended. Cecostomy and drainage are generally the treatment of choice should perforation occur.

Ten-year experience using antegrade enemas in children.

OBJECTIVE: To describe a single-center, 10-year experience with the use of antegrade enemas. STUDY DESIGN: Retrospective analysis of 99 patients treated with antegrade enemas at Nationwide Children's Hospital. RESULTS: Study subjects (median age 8 years) were followed for a mean time of 46 months (range 2-125 months) after cecostomy placement. Seventy-one patients had the cecostomy placed percutaneously and 28 by surgery. Thirty-five patients had functional constipation and 64 patients an organic disease (spinal abnormalities, cerebral palsy, imperforate anus, Hirschsprung's disease). While

[Cecal volvulus].

A retrospective study on 18 patients with cecal volvulus surgically treated was made. Demographics and clinical data, as well as treatment were determined from clinical reports. The operative procedures employed were cecostomy (56%), cecopexy (22%) and right colectomy (22%). The length of follow up averaged 63 months and there was one recurrence. The Authors suggest that cecostomy should be employed in patients with viable bowel, and resection should be limited to cases with gangrene.

Diagnostic yield of left-sided colonic manometry in identifying colonic dysmotility in pediatric patients.

BACKGROUND: Colonic manometry (CM) is a diagnostic procedure utilized in the evaluation of intractable constipation and involves endoscopic insertion of a manometry catheter with the tip placed in the cecum. Difficulty in advancing the colonic manometry catheter to the right colon and/or distal displacement of the catheter after appropriate placement can result in partial evaluation of the colon. Our study aimed to assess the value of limited left CM in identifying motility disorders. METHODS: We evaluated CM studies conducted at a tertiary pediatric center (2019-2022). Abnormal CM studies wi

Co-existence of cecal volvulus with situs inversus totalis: A case report.

Detorsion, cecopexy, cecostomy and tube cecostomy are the treatment options for acute cecal volvulus if there is no intestinal ischemia. Resection required if intestinal viability, necrosis, gangrene or perforation exists. After resection, primary anastomosis or ileostomy can be performed. First colonoscopic decompression testing may be appropriate in terms of saving time for elective surgery. The co-existance of situs inversus totalis with cecal volvulus may cause uncertainty of the definite diagnosis and delay of surgical procedure. This is a case report about cecal volvulus together with si

[Success of antegrade enemas in children with functional constipation].

AIM: To assess the benefit of antegrade enemas via cecostomy in children with functional constipation unresponsive to medical treatment. METHODS: A retrospective chart review identified 19 children (12 male, age 10.4 +/- 4.3 yrs) who from 1998-2001 underwent placement of a cecostomy for administration of antegrade enemas. All children were neurologically normal, suffered from functional constipation; all had a history of using multiple medications, having daily soiling, and 65% had prior hospitalizations due to fecal impaction. Sixteen of 19 children had undergone colonic manometry which rule

Significance of retrograde flow with antegrade continence enemas in children with fecal incontinence and constipation.

BACKGROUND: For children with constipation and fecal incontinence treated with antegrade continence enemas (ACE), a fluoroscopic study with contrast administered via appendicostomy/cecostomy can define the anatomy of the colon and simulate the flush to investigate associated symptoms or inadequate response. These studies can at times show retrograde flow into the small intestine. Our objective was to investigate the significance of this finding. METHODS: We reviewed studies at our institution with contrast administered via appendicostomy/cecostomy in children treated with ACE, identifying tho

Acute Colonic Pseudo-Obstruction.

A 33-year-old man with a history of chronic alcohol use, generalized anxiety disorder, and hypertension presented to the emergency department after a syncopal event. He was admitted to the medical intensive care unit for alcohol withdrawal, requiring intubation and sedation. On hospital day 7, abdominal x-ray image demonstrated a dilated cecum to 12 cm, transverse colon dilation to 7 cm, and decompressed distal colon ( Fig. 1 ). CT scan of the abdomen and pelvis confirmed dilation of the cecum and ascending and transverse colons ( Fig. 2 ). Colonoscopy showed no evidence of distal obstruction,

[The clinico-evolutionary and therapeutic aspects in diastatic perforation of the cecum].

We describe five cases of diastatic perforation of cecum due to colonic cancer obstruction with secondary diffuse peritonitis, all in elderly patients with many organics dysfunctions and, most important a late arrival for hospitalisation (over 48 h). The surgical treatment aimed for the perforation and the obstruction has permitted the realization--in emergency--of: cecostomy on site of perforation, with multiple peritoneal drainage; subtotal colectomy with ileosigmoid anastomosis; cecum perforation sutures, with left colostomy, opened per primam; cecostomy on site of perforation with left col

Colon atresia and stenosis in Zimbabwe: case reports and a review of the literature.

Two neonates with colon atresia and one with colon stenosis presented in Harare over a six month period. The first patient was treated with a local resection and primary anastomosis for a type II colon atresia. The second had an excision of obstructing septum, widening coloplasty and cecostomy for a type I colon atresia. The third patient had a sigmoido-rectoplasty and cecostomy for a sigmoid colon stenosis. The third patient also had a cystic duplication of the small bowel. No other anomalies were noted. All three patients survived and were developing normally at six months post-operatively.

Antegrade continence enemas improve quality of life in patients with medically-refractory encopresis.

PURPOSE: Fecal incontinence is a socially debilitating problem for many children. We hypothesized that in selected patients with medically-refractory encopresis, placement of an appendicostomy or cecostomy tube for administration of antegrade continence enemas (ACE) would improve quality of life (QOL). METHODS: We reviewed all patients with encopresis who underwent appendicostomy or cecostomy placement from 2003 to 2014 at our institution. We contacted subjects' parents by phone and administered 3 surveys: a survey reflecting current stooling habits, a disease-specific QOL survey, and the Ped

Pseudo-obstruction of the colon.

Psuedo-obstruction of the colon is a potentially lethal condition. The diagnosis should be suspected in a patient with derangement of a major extra-abdominal organ system in whom abdominal distention develops. Diagnosis is established by plain abdominal roentgenograms, signoidoscopy, and barium enema examination. The salient features of these examinations are as follows: (1) massive gaseous distention of the cecum and variable distances of the distal colon to a "cut-off-point"; (2) lack of fluid-filled colon; (3) normal sigmoidoscopy; and (4) barium enema examination, which rules out mechanica

Fournier's gangrene due to perioperative iatrogenic colon perforation in a renal transplant recipient.

Fournier's gangrene is not a common cause of morbidity in renal transplant recipients, but, if it occurs, it is difficult to treat because of the immunosuppression and associated increased mortality rate. We describe the case of a male patient who underwent renal transplantation with complicated post-operative course, resulting in cecum perforation (thermal injury due to cautery use during transplantation) requiring exploratory laparotomy and cecostomy. A few days later, he developed Fournier's gangrene and urgent radical surgical debridement of the scrotum was performed, along with aggressive

Antegrade Continence Enema Approaches: Outcomes, Lessons Learned and Overall Burden in a Mixed Urban-Rural Population.

Background: Antegrade continence enemas (ACEs) are administered by appendicostomy or cecostomy to manage bowel conditions. Cecostomies utilize buttons while appendicostomies utilize the appendix for colonic flushing. This study evaluates the outcomes and overall burden of these procedures in a mixed urban-rural population, highlighting unique social determinants of health (SDoH) and access factors. Methods: A retrospective cohort analysis of 31 pediatric patients was conducted at a tertiary academic hospital where 8 underwent a Malone-type ACE (MACE) and 23 underwent a laparoscopic cecostomy (

Management of intractable constipation with antegrade enemas in neurologically intact children.

OBJECTIVES: To assess the benefit of antegrade enemas in children with severe constipation who were referred to a tertiary care center. METHODS: From 1997 to 1999, 12 children (9 male, aged 8.7 +/- 4.4 years) underwent cecostomy placement. All children were neurologically normal and had been extensively examined to rule out organic causes of constipation. Follow-up included a questionnaire to interview caregivers 13.1 +/- 8.5 months after cecostomy placement. RESULTS: For all children, antegrade enemas led to improvement in the number of bowel movements / week (7.1 versus 1.4, P < 0.005), nu

Button device for antegrade enema in the treatment of incontinence and constipation.

Antegrade enema through an appendicostomy is effective in improving the quality of life for patients with incontinence or severe constipation. For patients without an appendix, a conventional gastrostomy button was used to create a cecostomy (or ileostomy) that served as a port for the administration of enema solutions.

Surgical management of chronic unremitting constipation and fecal incontinence associated with megarectum: A preliminary report.

BACKGROUND/PURPOSE: Functional constipation with associated fecal incontinence responds poorly to medical management once megarectum has developed. The authors describe resecting the dilated rectum and inserting a cecostomy button for antegrade enemas in this difficult condition. METHODS: Four children, ages 9 to 15 years, with a history of unremitting constipation and fecal incontinence were referred for evaluation after not responding to medical management. All patients had exhibited normal lumbosacral magnetic resonance images (MRI) and open rectal biopsies; however, all 4 patients had a m

Role of the antegrade continence enema in the management of the most debilitating childhood recto-urogenital anomalies.

PURPOSE: We evaluated the antegrade continence enema for managing the most severely compromised fecal continence mechanisms and the efficacy of nonappendiceal conduits in children when appendix is unavailable. MATERIALS AND METHODS: Ten patients 3 to 25 years old underwent continent cecostomy for the purpose of the antegrade continence enema. Diagnoses included the VATER association in 4 cases, myelomeningocele in 3, cloacal anomaly in 2 and isolated imperforate anus in 1. Eight patients received a tubularized ileal conduit, 2 received an appendiceal conduit and 2 underwent concomitant fecal
