Effect of Different Sampling Schedules on Results of Bioavailability and Bioequivalence Studies: Evaluation by Means of Monte Carlo Simulations.

Bioavailability and bioequivalence study is one of the most frequently performed investigations in clinical trials. Bioequivalence testing is based on the assumption that 2 drug products will be therapeutically equivalent when they are equivalent in the rate and extent to which the active drug ingredient or therapeutic moiety is absorbed and becomes available at the site of drug action. In recent years there has been a significant growth in published papers that use in silico studies based on mathematical simulations to analyze pharmacokinetic and pharmacodynamic properties of drugs, including

Novel beta-lactam antibiotics synthesized by amination of catechols using fungal laccase.

Novel cephalosporins, penicillins, and carbacephems were synthesized by amination of catechols with amino-beta-lactams like cefadroxil, amoxicillin, ampicillin and the structurally related carbacephem loracarbef using laccase from Trametes sp. All isolated monoaminated products inhibited the growth of several Gram positive bacterial strains in the agar diffusion assay, among them methicillin-resistant Staphylococcus aureus strains and vancomycin-resistant Enterococci. Observed differences in the cytotoxicity and in vivo activity in a "Staphylococcus-infected, immune suppressed mouse" model are

An international survey of the antimicrobial susceptibility of pathogens from uncomplicated urinary tract infections: the ECO.SENS Project.

The ECO.SENS study is the first international survey to investigate the prevalence and susceptibility of pathogens causing community-acquired acute uncomplicated urinary tract infections (UTIs). Midstream urine samples were taken for culture and for testing for the presence of leucocytes from 4734 women not older than 65 years presenting with symptoms of acute UTI at 252 community health care centres in 17 countries. Recognized urinary tract pathogens were identified and the susceptibility to 12 antimicrobials determined. Pathogens were present in 3278 (69.2%) patients, Escherichia coli accoun

[Pneumonia caused by Streptococcus pyogenes. Report of one case].

We report a 58-year-old female presenting with fever and vomiting. The initial laboratory examination disclosed two blood cultures that were positive for Streptococcus Pyogenes. An abdominal CAT scan showed a right basal pneumonia. The patient was treated with antimicrobials and discharged with oral cefadroxil for 21 days. One month after discharge she was asymptomatic and with a normal C reactive protein. Pneumonia is an important differential diagnosis in unknown origin bacteremia caused by Streptococcus Pyogenes. It may have a fulminant evolution and may complicate with abscess and empyema.

Cross-reactivity and tolerability of aztreonam and cephalosporins in subjects with a T cell-mediated hypersensitivity to penicillins.

BACKGROUND: The few studies performed in adults with T cell-mediated hypersensitivity to penicillins have found a rate of cross-reactivity with cephalosporins ranging from 2.8% to 31.2% and an absence of cross-reactivity with aztreonam. OBJECTIVE: We sought to evaluate the possibility of using cephalosporins and aztreonam in subjects with documented delayed hypersensitivity to penicillins who especially require them. METHODS: We conducted a prospective study of 214 consecutive subjects who had 307 nonimmediate reactions to penicillins (almost exclusively aminopenicillins) and had positive pa

The fecal microflora of 1-3-month-old infants during treatment with eight oral antibiotics.

We examined the fecal microflora of 1-3-month-old infants during treatment with phenoxymethylpenicillin, amoxycillin, pivampicillin, cefaclor, cefadroxil, loracarbef, erythromycin or cotrimoxazole. Escherichia coli increased during treatment with penicillins or cephalosporins, but was not affected by erythromycin or cotrimoxazole. Other enterobacteria were acquired or increased during treatment with all agents except cotrimoxazole. Enterococci persisted or increased during phenoxymethylpenicillin, cephalosporin or cotrimoxazole treatment, whereas erythromycin and the other penicillins suppress

Hospitalization due to Pasteurella multocida-infected animal bite wounds: correlation with inadequate primary antibiotic medication.

Over a 10-y period, patients hospitalized with Pasteurella-induced cat or dog bite-associated wound infection were analysed retrospectively with regard to preceding antibiotic medication. In 10/14 cases, hospitalization was necessitated in spite of prophylactic or therapeutic administration of oral antibiotics. In 1 case, phenoxymethylpenicillin and flucloxacillin had been prescribed. The other patients received flucloxacillin (7 patients), erythromycin, or cefadroxil (1 patient each), agents that are not consistently active against Pasteurella. In conclusion, hospitalization due to Pasteurell

Antimicrobial Resistance Profile of Bacteria Causing Pediatric Infections at the University Teaching Hospital in Rwanda.

Bacterial infections pose a global threat, especially in the pediatric population. Antimicrobials that are used to treat such infections continuously show reduced efficacy, and empirical therapy is a major treatment option in Rwanda. This study aimed to determine the resistance rate of commonly used antibiotics in pediatric patients. The study was conducted from June 1, 2018 to May 30, 2019, and microbiological samples were collected from 712 children with suspected bacterial infections. Antimicrobial sensitivity testing was performed on 177 positive cultures (24%) that were considered for dat

Campylobacter, Salmonella, Shigella and Escherichia coli in live and dressed poultry from metropolitan accra.

This study on the microbiology of chicken assessed a total of 97 live birds from three selected farms and 87 carcasses/chicken parts from two supermarkets, two open markets and one wholesale outlet (cold store) in the Accra metropolis. Campylobacter spp. were isolated from 14 (14.4%) gut contents of live birds from three farms. None of the frozen birds were positive for Campylobacter spp. These isolates were sensitive to most common antibiotics but not to ampicillin and tetracyclines. Salmonella spp. were isolated from 7 (7.2%) gut contents and 13 (6.8%) carcasses and were resistant to erythro

Multifocal Abdominal Pyomyositis From Subcutaneous Dissemination in an Immunocompetent Patient.

A 61-year-old woman presented to the emergency ward complaining of low back pain for a month. She had undergone several spinal surgeries and a right radical nephrectomy 30 years before. A few days earlier she was injected with an intramuscular painkiller in her right buttock. An abdominal CT scan revealed multiple abscesses in the psoas muscle and the right posterior abdominal wall, including cellulitis in the adjacent subcutaneous tissue and the injection site. A diagnosis of pyomyositis from subcutaneous dissemination was made, and intravenous cefazolin was initiated. After five days of favo

Unraveling the Challenges: A Compelling Case of Staph Meningitis and Graft Infection in a Bovine Brain Graft Recipient.

Meningitis due to Staphylococcus aureus is extremely rare, with an annual incidence of 1-3%. In this report, we present a rare case involving meningitis, an infected graft, and an infected fluid collection with two forms of S. aureus in a patient who received a bovine brain graft status post-decompression and suboccipital craniectomy with C1 laminectomy and duraplasty for Chiari malformation. The treatment approach included surgical debridement and graft retention, followed by an extended course of antibiotic treatment with oxacillin and rifampin. The patient successfully completed 12 weeks of

Erysipelothrix rhusiopathiae Prosthetic Joint Infection of the Knee in an Immunocompromised Patient.

A male in his 70s with a remote history of a left total knee arthroplasty presented with acute left knee pain and swelling. He was a farmer with significant exposure to domestic and feral swine. CT of the knee showed a large complex left knee effusion and knee aspiration was concerning for infection. He underwent a debridement, antibiotics, and implant retention (DAIR) surgical procedure. Synovial fluid and operative cultures ultimately yielded Erysipelothrix rhusiopathiae, likely acquired from his animal exposures. He completed six weeks of intravenous daptomycin, followed by oral cefadroxil

Penicillin tolerance in beta-streptococci isolated from patients with tonsillitis.

Beta-streptococci isolated from patients with acute tonsillitis were tested for penicillin tolerance defined as an MBC/MIC ratio greater than or equal to 16. 11/18 strains recovered from patients with clinical treatment failure were tolerant to penicillin in comparison with 0/15 strains from successfully treated patients. The MBC/MIC ratio was less than 16 for all strains versus cefadroxil but above that ratio for many strains versus clindamycin, doxycycline and erythromycin. We suggest that penicillin tolerance may be one reason to treatment failures in individuals with streptococcal tonsilli

Prevention of postoperative infection in cesarean section after rupture of the membranes.

Patients having a cesarean section more than 6 h after rupture of the membranes constitute a high risk group for postoperative infections. Two such groups were studied. Patients were given either cefuroxime 1.5 g every 8th hour for 24 h followed by cefadroxil 0.5 g twice daily for 6 days or received no medication. The infection rate was significantly reduced in the treatment group as compared to the control group (15% vs. 48%). Non-infected patients had a significantly shorter stay in hospital (8 days vs. 12 days). Combined use of these drugs for prevention of post-cesarean infection has not p

[Computerized analysis of activity profiles of beta-lactams].

This study was designed to compare the in vitro activities of recent cephalosporins and other beta-lactams. One of our objectives was to determine whether the microbiologist could use a single cephalosporin for routine antibiotic sensitivity testing. All the strains studied were recovered from outpatients. Sensitivity and/or resistance were evaluated by MIC determination, according to a program developed in our laboratory, using a personal computer. Using special programs, patterns of activities of beta-lactams were determined quantitatively and graphically for each bacterial strain tested. Fo
