Clinical effect of ethanol co-use in patients with acute drug toxicity involving the use of central nervous system depressant recreational drugs.

BACKGROUND AND IMPORTANCE: Patients who use recreational drugs frequently co-ingest ethanol, which is considered a central nervous system (CNS) depressant. The clinical relevance of this in acute toxicity involving other CNS depressants is not well described. OBJECTIVE: To assess the clinical impact of ethanol co-use in patients presenting to the emergency department (ED) with acute toxicity involving the use of CNS depressant drugs. DESIGN, SETTINGS AND PARTICIPANTS: A retrospective multicentre study using data from the Euro-DEN Plus database from January 2014 to December 2019. OUTCOMES ME

Trends in Prescribed Central Nervous System Depressant Medications Among Adults Who Regularly Consume Alcohol: United States 1999 to 2014.

BACKGROUND: Although the risks of using central nervous system depressant (CNS-D) medications with alcohol are well documented, little is known about trends in prescribed use of these medications among individuals who regularly consume alcohol (i.e., trends in "concurrent use"). We examined changes in the prevalence of prescribed CNS-D medications among individuals who drank alcohol on 52 or more occasions in the past year ("regular drinking"). CNS-D medications included sedative-hypnotics (subclassified as anxiolytics or sleep medications) and opioids. METHODS: We used 8 cross-sectional cycl

Genetic association study using GWAS and Mendelian randomization to central nervous system comorbidities between multisite chronic pain and major depressive disorder.

BACKGROUND: Multisite chronic pain (MCP) and major depressive disorder (MDD) exhibit significant clinical relevance. However, the central nervous system comorbidity mechanism and the shared genetic risk mechanisms remain poorly elucidated. This study aims to systematically investigate the neurobiological and genetic correlation features between MCP and MDD. METHOD: Leveraging a multidimensional genetic framework, we integrated GWAS data for MCP (UK Biobank, UKB), MDD (Psychiatric Genomics Consortium, PGC), and ENIGMA neuroimaging data to investigate their genetic-neurobiological interplay. Randomized Crossover Trial to Compare Abuse Liability of Intravenous Remimazolam Versus Intravenous Midazolam and Placebo in Recreational Central Nervous System Depressant Users.

Remimazolam (RMZ) is a new and ultra-fast-acting, short-duration intravenous benzodiazepine, a drug class associated with abuse potential. This trial was designed to compare the abuse potential of remimazolam with placebo and midazolam (MDZ), a well-characterized member of the same pharmacological class in healthy, recreational drug users 18-55 years-of-age, who demonstrated good drug tolerance and were able to discriminate between midazolam and placebo. At equipotent intravenous doses selected to produce effects ranging from mild/moderate to relatively strong sedation without loss of consciou

3-Halo-5,7-dimethylpyrazolo [1,5-a]pyrimidines, a nonbenzodiazepinoid class of antianxiety agents devoid of potentiation of central nervous system depressant effects of ethanol or barbiturates.

Forty derivatives (1-40) of pyrazolo[1,5-a]pyrimidine were synthesized and evaluated for antianxiety properties via gross behavioral observations in rats. Five of these compounds, including 5,7-dimethylpyrazolo[1,5-a]pyrimidine (6) and the 3-fluoro (7), 3-chloro (8), 3-bromo (9), and 3-iodo (10) derivatives, were selected for advanced evaluation. Although 6 and 7 had marginal activity, 8-10 had an anxiolytic effect in animals comparable to the clinically useful benzodiazepines, diazepam, and chlorodiazepoxide. Comparison with chlorpromazine indicated that 6-10 are probably not antipsychotic ag

Polypharmacy involving opioid, psychotropic, and central nervous system depressant medications, period prevalence and association with suicidal ideation, active component, U.S. Armed Forces, 2016.

This report uses routinely collected data in the Defense Medical Surveillance System (DMSS) to explore the period prevalence of polypharmacy among the active component U.S. military in 2016. The period prevalence across the Department of Defense was 10.8% and was highest for the Army (14.5%) and lowest for the Marine Corps (7.4%). Furthermore, a case control study was conducted to explore the potential association between polypharmacy and incident suicidal ideation (SI). There was an increased adjusted odds of incident SI within 12 months following polypharmacy exposure, with adjusted odds rat

Prolonged Post-Electroconvulsive Therapy Delirium in Treatment-Resistant Depression With Central Nervous System Risk Factors Under Programmed Cell Death Protein 1 Inhibitor Therapy: A Case Report.

We report a diagnostically challenging case of treatment-resistant depression (TRD) in a 52-year-old woman with multiple factors potentially increasing susceptibility to central nervous system (CNS) adverse effects, including global cortical atrophy, concomitant lithium and antipsychotic treatment, and ongoing programmed cell death protein 1 (PD-1) inhibitor therapy. After failing multiple pharmacological and neuromodulatory treatments, electroconvulsive therapy (ECT) was initiated. While ECT led to marked clinical improvement, the patient developed a prolonged delirium after the 12th session,

Expecting medication misuse: a proactive approach to drug discovery to prevent fatal overdose.

Misuse of central nervous system (CNS) depressants (alprazolam, fentanyl, etc.) is a major cause of fatal overdose, with a high prevalence of deaths involving polydrug interactions from the victim's own prescriptions. Thus, there is an urgent need to improve the safety of CNS depressants to prevent fatalities. Pharmacological pursuits aiming to prevent harm through the design of non-addictive alternatives have either failed before clinical trials or produced mediocre treatment alternatives. Therefore, we propose a new perspective for medicinal chemists: rather than aiming to prevent misuse, we

Two occult causes of insomnia and their therapeutic problems.

Insomnia may be associated with a sleep-induced apnea syndrome in nonobese patients who snore. The "central" type of apnea appears to be predominant in this population, in opposition to Pickwickian and nonobese hypersomniacs. An abnormal "swallowing reflex syndrome," also induced by sleep, may be a differential diagnosis. Sleeping pills that are central nervous system depressants should be cautiously prescribed for patients with such syndromes.

Thermodynamic interpretation of synergism in barbiturate/ethanol poisoning.

Ethanol and barbiturate drugs behave as non-specific depressants of the central nervous system (Ferguson substances). Their interaction can therefore be understood quantitatively by the application of thermodynamic theory. It is unnecessary to postulate specific biochemical mechanisms for the observed synergism. A similar situation should exist for any pair of Ferguson substances.

Buprenorphine-related complications in elderly hospitalised patients: a case series.

We report a case series of buprenorphine-related respiratory and neurological depression in opioid-naïve elderly hospitalised patients who received buprenorphine for acute pain management at our institution over a 24-month period. All six patients had risk factors for respiratory depression such as advanced age, concurrent comorbidities, or the ingestion of other potential central nervous system depressants. All patients required escalation of management with additional monitoring, with some transferred to a high dependency or intensive care unit. Five patients had attempted naloxone reversal

The Physiology and Maintenance of Respiration: A Narrative Review.

Chronic pain is one of the most common reasons adults seek medical care and is often managed with opioid analgesics; however, opioids may cause respiratory depression by suppressing various components of respiration. Respiration is the physiological process that facilitates gas exchange and is mediated through the proper function of and communication among central neural control (respiratory drive), sensory input systems, the lungs, and the muscles involved in respiration. Normal respiratory function can be dampened with the use of central nervous system (CNS) depressants and/or underlying hea

Pentobarbital-like discriminative stimulus properties of halothane, 1,1,1-trichloroethane, isoamyl nitrite, flurothyl and oxazepam in mice.

Volatile inhalants represent a diverse group of chemicals which pose a public health problem because of their abuse potential and neurobehavioral toxicity. Although relatively little is known about their pharmacology, they share certain pharmacological properties with classic central nervous system depressants. Drug discrimination procedures were used to compare the effects produced by pentobarbital (PB), oxazepam and several inhalants. Mice were trained to discriminate PB from saline injections in a two-lever operant task. Stimulus generalization was examined after 20-min inhalation exposures

Abuse of skeletal muscle relaxants.

Centrally acting skeletal muscle relaxants are potential drugs of abuse. Although skeletal muscle relaxants are occasionally the primary drug of abuse, they are often used along with other central nervous system depressants, such as narcotics or alcohol. The major toxic effects are respiratory depression and coma. For the most part, the abuse problem is initiated and sustained iatrogenically. Physicians should be aware of the abuse potential of skeletal muscle relaxants.
