Cerebellar vermis hypoplasia and bilateral basal ganglia calcification in maternally inherited diabetes and deafness.

Maternally inherited diabetes and deafness (MIDD) is a rare diabetic syndrome mainly caused by a point mutation in the mitochondrial DNA. It affects up to 1% of patients with diabetes but is often unrecognized by physicians. We report a case of MIDD in a 29-year-old man with coexisting imaging of cerebellar vermis hypoplasia and bilateral basal ganglia calcification.

[Case Report of Cerebellar Vermis Arteriovenous Malformation Presenting with Hydrocephalus due to Aqueductal Stenosis].

A 56-year-old man complained of gait disturbance and confused thinking. Magnetic resonance imaging(MRI)revealed an arteriovenous malformation(AVM)of the cerebellar vermis(Spetzler-Martin grade IV)causing hydrocephalus. One dilated precentral cerebellar vein was compressing the aqueduct. After feeder embolization over 3 sessions using N-butyl cyanoacrylate(NBCA), the nidus was reduced to one-third in size. However, symptoms remained unimproved, and endoscopic third ventriculostomy(ETV)was performed. The third ventricle showed thinning of the floor, with a fenestration in part of the floor. Radi

Positional nystagmus of benign paroxysmal type (BPPN) due to cerebellar vermis lesions. Pseudo-BPPN.

Positioning nystagmus accompanied by severe vertigo had been reported in patients with partial lesions of the inner ear, especially otolith lesions. Typically this type of nystagmus represents a latent period and subsequent fatiguability. We concur with this finding and have constantly emphasized the significance of this phenomenon in clinical diagnosis. Since we started to use CT-scanning, this type of nystagmus has been noted in 20 patients, all of whom had cerebellar vermis lesions. Attention should be focused on this association. A simple coincidence could not be excluded if such a combina

Reactivity to visual signals and the cerebellar vermis: Evidence from a rare case with rhombencephalosynapsis.

The modulatory role of the cerebellum was investigated in a case with rhombencephalosynapsis (RS), a rare dysplasia characterized by the absence of the cerebellar vermis. The visual psychophysical task involved localizing a target and ignoring a distractor appearing either before, at the same time as, or after the target. It allowed us to assess reactivity to warning signals, distraction, and reactivity to signals appearing during attentive processing. Compared with a control sample, the patient exhibited greater reactivity to warning signals and difficulties interrupting attentive processing,

Muscarinic receptors in the cerebellar vermis modulate the gain of the vestibulospinal reflexes in decerebrate cats.

1. The Purkinje (P)-cells of the cerebellar vermis, which exert a prominent influence on posture as well as on the gain of vestibulospinal (VS) reflexes, are under the control not only of the classic mossy fibers and climbing fibers which liberate excitatory amino acids as neurotransmitter, but also of cholinergic afferents. The role of these afferents was investigated in precollicular decerebrate cats by using the method of local microinjection of cholinergic agents into appropriate areas of the cerebellar cortex. 2. Unilateral injection into the vermal cortex of the culmen of the non-selecti

The influence of lobule IX of the cerebellar posterior vermis on the baroreceptor reflex in the decerebrate rabbit.

Stimulation of lobule IX (the uvula) of the cerebellar posterior vermis, either electrically or chemically, has been shown to evoke marked cardiovascular effects in both rabbit and cat. Recent experiments suggest that this stimulation can also reduce the responsiveness of neurons in the nucleus tractus solitarii to baroreceptor inputs. The aim of the present study was thus to determine whether the uvula exerts an influence on the baroreceptor reflex. In order to evoke a reflex bradycardia in decerebrate rabbits, blood pressure was increased by inflating a Swan-Ganz catheter positioned in the d

Regional distributions of choline acetyltransferase and acetylcholinesterase activities in layers of rat cerebellar vermis.

Entire sagittal sections of rat cerebellar vermis were dissected into microgram-sized samples of molecular, granular, and white matter layers. Assayed activities of choline acetyltransferase and acetylcholinesterase were plotted back onto sectional maps of exact sample locations. On the average, the activities of choline acetyltransferase and acetylcholinesterase in the granular layer were about four and five times, respectively, those in the molecular layer. The highest activity of both enzymes was in the granular layer of the vestibulocerebellum, the nodulus and ventral uvula (lobules X and

[Quantitative analysis of cerebellar vermis in fetuses with Dandy-Walker syndrome with three-dimensional ultrasonography].

OBJECTIVE: To evaluate the third plane image of the three-dimensional sonography (3D US) for the quantitative analysis of the cerebellar vermis in normal and Dandy-Walker syndrome (DWS) fetuses. METHODS: The cerebellar vermis was scanned with trans-abdominal 3D US in the second and third trimesters in 571 normal fetuses and 39 fetuses with Dandy-Walker syndrome. The surface area of the vermis in the mid-sagittal view was measured and calculated. The correlation between the vermian area and the pregnant week was analyzed. The data of vermian area between normal and DWS fetuses was compared. R

Hypoplasia of the cerebellar vermis and corpus callosum in thrombocytopenia with absent radius syndrome on MRI studies.

Thrombocytopenia with absent radius (TAR) syndrome is infrequently (7%) associated with mental retardation. In those cases, the mental deficiency is presumed to be a consequence of intracranial hemorrhage due to the thrombocytopenia. We report on 2 infants with TAR syndrome. One had developmental delay with evidence of cerebral dysgenesis by magnetic resonance imaging (MRI). Such findings have not been noted in the literature, but may not have been investigated in most cases. The other infant with TAR syndrome, who has had normal psychomotor development, has a normal brain on MRI scan. Detaile

Muscle phosphofructokinase deficiency in a myopathic child with severe mental retardation and aplasia of cerebellar vermis.

Muscle phosphofructokinase (PFK) deficiency in man is responsible for at least two forms of myopathy; one is characterized by painful contractures of muscles and typically occurs in adults, whereas the other is often disabling and typically occurs in childhood, with psychomotor and growth retardation. In this investigation, a young myopathic patient with severe mental retardation and aplasia of the cerebellar vermis presented with muscular hypotrophy of the limbs, generalized hypotonia, convergent strabismus and marked pain during passive movement. Biopsy of quadriceps femoris muscle showed va

Cerebellar vermis: a vulnerable location of remote brain haemorrhages after thrombolysis for ischaemic stroke.

Extra-ischaemic (remote) brain heamorrhages after thrombolysis for ischaemic stroke occur in less than 3 % of treated patients, but it worsens prognosis. Little attention has been paid to the location of the haematomas. Among 12 patients with remote brain haemorrhage after thrombolysis, we report three patients with haemorrhage in the cerebellar vermis (25 %), with poor outcome. Previous hypertensive vasculopathy is deemed to be the most plausible cause.

Non epileptic absence seizures and cognitive outcomes after cerebellar stroke in vermis and posterior lobe.

OBJECTIVE: Cerebellar Cognitive Affective Syndrome (CCAS) is traditionally characterized by impairments in executive functioning, visuospatial processing, language, and affective regulation. Classic descriptions emphasize symptoms such as irritability, apathy, depression, and disinhibited behaviors, frequently associated with vermian or hemispheric cerebellar lesions. We describe a case that diverges from these classical profiles. METHODS: A case report shows that patient demonstrated preserved overall cognition and absence seizures with context-dependent emotional dysregulation, while mainta

Cerebellar vermis hypoplasia in CHARGE syndrome: clinical and molecular characterization of 18 unrelated Korean patients.

CHARGE syndrome (OMIM 214800) is a rare autosomal-dominant congenital malformation syndrome that results from haploinsufficiency of the chromodomain helicase DNA-binding protein 7 (CHD7). We performed a phenotypic characterization and genetic analysis of CHD7 in 18 Korean patients with CHARGE syndrome. Eighteen unrelated Korean patients (10 females and 8 males; age range 0.0-19.6 years) with CHARGE syndrome were enrolled. Clinical data were collected by retrospective review of medical records. A serial analysis via sequencing and multiple ligation-dependent probe amplification of CHD7 was perf

Verbal short-term memory and cerebellum: evidence from a patient with congenital cerebellar vermis hypoplasia.

The impairment of phonological short-term memory has been reported in adults with cerebellar lesions. At the same time, a role of the cerebellum in speech production has been hypothesized. Cerebellar malformations have been related to developmental problems and language acquisition in children. We describe a 5-year-old male child with cerebellar vermis hypoplasia who presented a severe linguistic deficit. On language testing, verbal production was almost absent, while comprehension was partially spared. Digit span was markedly reduced. An extensive examination of phonological short-term memory

Novel Genetic Variation in the KIF1A Gene Associated With Cerebellar Vermis Hypoplasia: A Case Report.

KIF1A-associated neuronal disorder (KAND) is a spectrum of disease caused by a genetic mutation in the KIF1A gene. Genetic variants in this gene have been associated with cerebellar vermis hypoplasia, as well as mild to severe symptoms, including ataxia, motor and global developmental delay, and intention tremors. Given the limited number of reported cases, ample research is necessary to effectively manage this condition. A two-year-old girl presents with global developmental, motor, and speech delay, as well as jerky movements, possible athetosis, hand tremors, ataxia, vertical nystagmus, and

A homozygous variant in ARHGAP39 is associated with lethal cerebellar vermis hypoplasia in a consanguineous Saudi family.

Cerebellar vermis hypoplasia refers to a varying degree of incomplete development of the cerebellum and vermis. A Saudi family with four affected individuals with cerebellar vermis hypoplasia, facial dysmorphology, visual impairment, skeletal, and cardiac abnormalities was ascertained in this study. Three out of four patients could not survive longer and had died in early infancy. Genetic analysis of the youngest affected was performed by genome-wide homozygosity mapping coupled with whole exome sequencing (WES), followed by Sanger validation. Genome-wide genotyping analysis mapped the phenoty

Diagnostic utility of whole exome sequencing in patients showing cerebellar and/or vermis atrophy in childhood.

Cerebellar and/or vermis atrophy is recognized in various types of childhood disorders with clinical and genetic heterogeneity. Although careful evaluation of clinical features and neuroimaging can lead to correct diagnosis of disorders, their diagnosis is sometimes difficult because clinical features can overlap with each other. In this study, we performed family-based whole exome sequencing of 23 families including 25 patients with cerebellar and/or vermis atrophy in childhood, who were unable to be diagnosed solely by clinical examination. Pathological mutations of seven genes were found in

Congenital rubella with agenesis of the inferior cerebellar vermis and total anomalous pulmonary venous drainage.

Congenital rubella infection has been associated with a number of abnormalities including cardiac, central nervous system and placental complications. We present a case with multiple fetal abnormalities detected on prenatal ultrasound, and confirmed postnatally, that included a single umbilical artery, severe tricuspid regurgitation, micrognathia and agenesis of the inferior cerebellar vermis. Postnatal echocardiography additionally revealed unobstructed total anomalous pulmonary venous drainage (TAPVD) into the coronary sinus. Placental examination showed signs of placentitis, and polymerase
