Regional cerebral blood flow using quantitative MR angiography.

BACKGROUND AND PURPOSE: We sought to derive regional cerebral blood flow using vessel flows from quantitative MR angiography (qMRA). MATERIALS AND METHODS: Flow rates in the 15 major cerebral arteries were measured on retrospectively gated fast 2D phase-contrast MR angiography obtained in 83 healthy adult volunteers (age range, 24-74 years; mean, 42 years). The arterial network of the brain was partitioned into 12 different regions, in which flows were calculated from the measured flows of the 15 cerebral arteries. RESULTS: The mean flows of the 15 arteries and the 12 regions were calculated

Anterior cerebral artery variations detected by MR angiography.

INTRODUCTION: From anatomical and angiographic studies, it is well known that there are several variations of the anterior cerebral artery (ACA). However, ACA variations have rarely been studied by magnetic resonance (MR) angiography. The purpose of this study was to investigate not only the type, location, configuration, and incidence of ACA variations, but also coexisting arterial pathology such as aneurysms detected by cranial MR angiography. METHODS: We retrospectively reviewed cranial MR angiography images of 891 patients at our institution. All images were obtained with one of two 1.5-T

Transradial approach for selective cerebral angiography: technical note.

Selective cerebral angiography is currently being performed using transfemoral and transbrachial approaches. However, these techniques require patients to tolerate a prolonged focal compression and sometimes cause serious complications such as pulmonary embolism. The authors describe a technique of transradial approach as a safer selective cerebral angiography. Between July 1997 and November 1998, 70 patients underwent selective cerebral angiography with a transradial approach using a 4-F catheter. The collateral blood supply to the hand from the ulnar artery was confirmed using Allen's test p

Three-dimensional computed tomographic angiography of cerebral aneurysms.

We describe our technique and results of dynamic, reconstructed, three-dimensional (3-D) computed tomographic (CT) angiography in our first 18 patients with either complex aneurysms, producing subarachnoid hemorrhage or the compression of neighboring structures, or small asymptomatic aneurysms, and assess the role of this technique in treatment planning. A dynamic, infused CT scan producing 1.5-mm cuts of the area of interest was performed, and the two-dimensional images were reconstructed in three dimensions with the ISG Allegro system (ISG Technologies, Toronto, Ontario, Canada). Results wer

Magnetic resonance angiography (MRA) of ruptured cerebral aneurysm.

The purpose of this study was to evaluate the high resolution magnetic resonance angiography (MRA) as pre-operative angiography for the detection of ruptured cerebral aneurysms. MRA was performed on 1.5 tesla system using the 3-dimensional time of flight (3D-TOF) method. The field of view was 16 cm or 20 cm and matrix size was 192 x 256 or 256 x 512. Twenty patients with ruptured cerebral aneurysms and 35 cases of non-ruptured aneurysms (incidental aneurysm) examined by both conventional angiography and MRA were included in this study. All the ruptured aneurysms were operated on based on the i

Transient global amnesia after cerebral angiography with iohexol.

Transient global amnesia is an unusual neurologic phenomenon that occasionally occurs after angiography, usually of the cerebral vessels. It represents a benign event, and normal function is ultimately restored, but the amnesia may be frightening for both the patient and the physician until its nature is recognized. The authors describe two cases of transient global amnesia occurring after cerebral angiography with iohexol, outline the criteria for the diagnosis and discuss the proposed mechanism of the phenomenon.

Central retinal artery occlusion following transfemoral cerebral angiography.

Whereas most ocular complications following cerebral angiography are benign and transient, central retinal artery occlusion following cerebral angiography produces severe permanent visual deficits in nearly 40% of patients. Physicians ordering and performing cerebral angiography should be aware of this complication, since immediate attention to patients with visual disturbance from central retinal artery occlusion may save useful vision. All previously reported cases of central retinal artery occlusion following cerebral angiography have occurred after direct percutaneous carotid angiography.

[More safety in catheter cerebral angiography (author's transl)].

Hazards and difficulties of catheter carotid and vertebral angiography are reported. Our experience is based on 257 persons who had undergone 338 carotid and 33 vertebral arteriographies. In 31 patients, bilateral carotid as well as aortic arch and abdominal aortic, iliac and lower limb arteriography were carried out in one session. Our material consists of consecutive cases. The characteristics of the "sidewinder-technique" are emphasized. Several safety measures considered necessary during the examination are described.

Investigation of childhood central nervous system vasculitis: magnetic resonance angiography versus catheter cerebral angiography.

AIM: We compared the clinical utility of magnetic resonance angiography (MRA) to catheter cerebral angiography (CA) in the investigation of children with suspected central nervous system (CNS) vasculitis. METHOD: Single-centre retrospective review of children with a suspected diagnosis of CNS vasculitis studied with both MRA and CA. MRA and CA-detected abnormalities (location, multiplicity, and morphology) were compared; sensitivity and specificity were calculated on a per lesion and per patient basis for MRA, with CA as the reference standard. RESULTS: Findings in fourteen patients (median

A comparative study of low voltage, low contrast cerebral computed tomography angiography with iterative reconstruction and conventional cerebral computed tomography angiography.

BACKGROUND: Cerebral computed tomography angiography (CTA) has revolutionized the diagnosis of neurovascular emergencies. Strategies to reduce radiation, a concern for cancer, involve tube voltage and current reduction but with increased noise and inferior image quality. Hence, the objective of the study was to evaluate the quality of images obtained through low-dose radiation and low-contrast volume CTA with an iterative reconstruction (IR) technique versus standard CTA without IR. METHODS: This prospective trial involved 100 adults requiring cerebral CTA for cerebrovascular diseases. They w

Perfusion-derived cerebral CT angiography with artificial intelligence iterative reconstruction as a replacement for routine cerebral CT angiography in acute ischemic stroke.

OBJECTIVES: To investigate the feasibility of replacing routine cerebral CT angiography (CTA) with CT perfusion (CTP)-derived cerebral CTA by using artificial intelligence iterative reconstruction (AIIR) for acute ischemic stroke (AIS) patients. METHODS: Patients with AIS undergoing cerebral CTP and CTA were prospectively collected. CTA images derived from CTP at arterial phase (100kVp/150mAs) were reconstructed with hybrid iterative reconstruction (HIR, Group A1) and AIIR (Group A2), whereas routine CTA images (100kVp/ref. 200mAs) were obtained with HIR (Group B). Cerebral infarction in young people. A study of 148 patients with early cerebral angiography.

The aetiology of strokes was studied in a hospital based series of patients aged up to 40 years with precise clinical and radiological criteria. One hundred and forty five patients (75 males and 73 females) aged five to 40 years with cerebral ischaemia were evaluated. Aetiology was heterogeneous and could be classified into seven groups. Cerebral arteriograms were performed in all cases and indicated the aetiological diagnosis in most patients. Embolism was the most frequent recognised abnormality (38.4%). There were no complications of arteriography. Arterial dissections discovered by arterio

Intravenous 3-Dimensional Digital Subtraction Angiography: A Noninvasive, High-Quality Alternative to Cerebral Catheter Angiography.

BACKGROUND AND OBJECTIVES: Arterial angiography remains the gold standard for the assessment of cerebrovascular anatomy and pathology. However, when arterial access is higher risk (patients with extreme tortuosity, type 4 Ehlers Danlos Syndrome, or the young pediatric population), and for long-term follow-up or intraoperative scenarios, intravenous 3-dimensional digital subtraction angiography (IV 3D-DSA) is an acceptable substitute. We assessed our experience in consecutive IV 3D-DSA procedures. METHODS: Five hundred twenty-eight adult and 21 pediatric IV 3D-DSAs were performed at our instit

Digital subtraction cerebral angiography after negative computed tomography angiography findings in non-traumatic subarachnoid hemorrhage.

BACKGROUND: CT angiography (CTA) is widely used for the detection of vascular lesions in patients with non-traumatic subarachnoid hemorrhage (ntSAH); however, digital subtraction angiography (DSA) remains the gold standard for diagnosis. Our aim was to analyze the diagnostic yield of DSA after negative high-resolution CTA findings. METHODS: Records of patients with a CTA-negative ntSAH at a single institution from 2014 to 2018 were retrospectively analyzed. ntSAH patterns were categorized as cortical, perimesencephalic, or diffuse. Subsequent DSA findings were compared across the three cohort

Variability of magnetic resonance angiography and computed tomography angiography in grading middle cerebral artery stenosis.

BACKGROUND AND PURPOSE: Magnetic resonance angiography and computed tomography angiography are new, noninvasive methods to provide images of the cerebral vasculature. The reliability of magnetic resonance angiography and computed tomography angiography when used to grade middle cerebral artery stenosis remains to be established. We sought to study the interobserver and intraobserver variabilities of magnetic resonance angiography and computed tomography angiography in grading middle cerebral artery stenosis. METHODS: A total of 50 middle cerebral arteries in 25 patients were studied with magn

Transradial cerebral angiography: predicting left ICA selective angiography success using pre-diagnostic aortic arch factors.

BACKGROUND: Transradial cerebral angiography (TRA) is a convenient but challenging procedure, particularly for selecting the left internal carotid artery (ICA) and vertebral artery. OBJECTIVE: To predict the selection of the left ICA using CT and MR images acquired before TRA. METHODS: Overall, 306 patients with TRA were enrolled and divided into either the group with success (264 patients) or the failure (42 patients) group. The following anatomical factors were measured: A1 (subclavian artery angle), A2 (right subclavian-innominate artery angle), A3 (innominate-left common carotid artery a
