Temporary threshold shift following ear canal microsuction.

Objectives: To investigate a temporary threshold shift (TTS) of hearing and pain/discomfort caused by the microsuction procedure. Hearing loss induced by impacted cerumen was also investigated.Design: Impacted cerumen was removed from patients using microsuction. Hearing assessments were carried out before the procedure, immediately after and 1-week later. Hearing thresholds measured in different sessions were compared to determine the TTS caused by the microsuction procedure and hearing loss induced by impacted cerumen. A questionnaire was used to evaluate the pain/discomfort experienced by p

Ceruminal diffusion activities and ceruminolytic characteristics of otic preparations - an in-vitro study.

BACKGROUND: An in-vitro setup was established in order to determine a) the diffusion activities of eight otic preparations (Aurizon®, Eas Otic®, Epi Otic®, Otifree®, Otomax®, Panolog®, Posatex®, Surolan®) through synthetic cerumen, and b) the ceruminolytic capacity and impregnation effects of these products. The main lipid classes of canine cerumen produced with moderate, non-purulent otitis externa were determined by thin layer chromatography and were subsequently used to produce a standardised synthetic cerumen (SCC). SCC was filled into capillary tubes, all of which were loaded with six com

Comparison of an antifungal agent with a mixture of antifungal, antibiotic and corticosteroid agents for the treatment of Malassezia species otitis in dogs.

Twenty dogs with otitis externa in both ears and numerous Malassezia species yeasts on cytological examination were treated in one ear with a combination product containing clotrimazole, marbofloxacin and dexamethasone, and in the other ear with a topical antifungal containing miconazole. The effects of the treatments were analysed on the basis of the scores for pruritus, erythema and amount of cerumen, and the number of yeasts on cytological smears. There were reductions in the counts of Malassezia species after both treatments, but the combination product gave significantly greater reduction

Misdiagnosis complicating acupuncture.

A 33-year-old man, profoundly deaf since the age of 5 from meningitis, received 25 acupuncture treatments at a center in western Massachusetts. Subsequent hearing evaluation at the Yale New Haven Medical Center again confirmed his profound deafness. He also was found to have cerumen (rock-hard and long standing) in both ear canals. Final removal of the cerumen one week later permitted inspection of a normal eardrum on the right and a debris-filled retraction pocket on the left. The diagnosis of cholesteatoma was confirmed at surgery. Representatives of the medical profession should accept resp

Catastrophic otologic injury from oral jet irrigation of the external auditory canal.

Jet irrigation is an accepted method for removal of cerumen from the external auditory canal. Even at a submaximal power setting, oral jet irrigators can generate enough force to rupture the tympanic membrane. Parameters for safe use of these irrigators have never been established. Three cases are reported in which, in addition to tympanic membrane rupture, oral jet irrigators caused ossicular disruption, round and oval window fistulae, and subluxation of the stapedial footplate. A prospective study performed on 25 fresh cadavers demonstrated a 6% incidence of tympanic membrane perforation whe

The efficacy of ceruminolytics: everything old is new again.

Impacted cerumen frequently poses a problem to both patients and physicians. Over the years, many topical ceruminolytics have been advocated, many of which are currently used with no scientific proof of their effectiveness. The effects of a variety of ceruminolytics were tested in vitro on standard spheres of normal human cerumen. The only truly effective ceruminolytics had an aqueous base, and the most effective ceruminolytic was a 10% solution of sodium bicarbonate. In contrast, those ceruminolytics which had an organic base, including most of the commonly available commercial preparations,

An optimization protocol of the volatile organic compounds analysis in earwax samples for untargeted volatilomics.

Recent studies have highlighted the potential of earwax or cerumen, a non-conventional biomatrix, in volatilomics research as a valuable matrix for disease biomarker discovery. Despite that, there are still gaps in using non-conventional biomatrices in metabolomics research. In this sense, this study aimed to elucidate the main analytical factors involved in the extraction and analysis of volatile organic compounds (VOCs) in cerumen by headspace/gas chromatography-mass spectrometry (HS/GC-MS) using Design of Experiments (DoE) approaches. Furthermore, we present a repeatability study for the pr

The role of the microbiome in allergic dermatitis-related otitis externa: a multi-species comparative review.

The external ear canal, characterized by species-specific structural and physiological differences, maintains a hostile environment that prevents microbial overgrowth and foreign body entry, supported by factors such as temperature, pH, humidity, and cerumen with antimicrobial properties. This review combines several studies on the healthy ear canal's structure and physiology with a critical approach to the potential existence of an ear microbiome. We use a comparative multi-species approach to explore how allergic conditions alter the ear canal microenvironment and cerumen in different mammal

Acute otitis media: making an accurate diagnosis.

The diagnosis of acute suppurative otitis media is not as easy and straightforward as it may seem. Many of the signs and symptoms in children with acute otitis media are also observed in children without it. Furthermore, several of the "classic" findings of acute otitis media, such as fever and earache, are often absent, even in cases confirmed by myringotomy. An otoscope with a fresh bulb and a good power source, as well as a view of the tympanic membrane that is not obstructed by cerumen, are essential to making the diagnosis of acute otitis media. A bulging, cloudy, immobile tympanic membra

Ear wax management.

BACKGROUND: Ear syringing is a very common practice among general practitioners (GPs). It is used by many as the treatment of choice for cerumen (ear wax), and is usually effective and safe. However, complications from syringing are an increasingly common reason for presentation to ear, nose and throat (ENT) specialists and medico-legal complaints against GPs. OBJECTIVE: The aim of this article is to provide GPs with the knowledge to safely manage cerumen. DISCUSSION: Ear wax is an important part of a normal functioning ear. It is mostly asymptomatic and requires no treatment. Softening ear
