Dermatomal Contact Heat Evoked Potentials for the Detection of Subclinical Cervical Spinal Cord Injury in Asymptomatic Degenerative Cervical Cord Compression.

BACKGROUND: Asymptomatic degenerative cervical cord compression (ADCC) represents a premyelopathic stage of degenerative cervical myelopathy (DCM), with high prevalence in older age and unclear management. Contact heat-evoked potentials (CHEPs) assess the integrity of the thermo-algesic somatosensory pathway and have shown the highest sensitivity among neurophysiological methods in detecting dysfunction in the centromedial and anterior spinal cord in DCM. METHODS: This cross-sectional cohort study evaluated the utility of upper extremity dermatomal C4, C6 and C8 CHEPs (dCHEPs) compared to sta

Optimizing Surgical Cord Decompression in Traumatic Spinal Cord Injury and Degenerative Cervical Myelopathy.

Preoperative assessment of intramedullary lesion length has become part of a planning algorithm for decompression surgery. In degenerative cervical myelopathy, intramedullary signal abnormalities on MRI often reflect ongoing biological activity rather than fixed tissue loss. As compression persists, reduced perfusion and altered cerebrospinal fluid dynamics may create a compartment-like intradural environment in which the spinal cord has limited reserve capacity. The goal of surgery remains restoration of a permissive intradural environment for the spinal cord. Intraoperative adjuncts such as

Could spinal cord oscillation contribute to spinal cord injury in degenerative cervical myelopathy?

INTRODUCTION: Degenerative Cervical Myelopathy [DCM] is a slow-motion spinal cord injury. Compression and dynamic compression have been considered disease hallmarks. However, this is likely an oversimplification, as compression is more commonly incidental and has only modest correlation to disease severity. MRI studies have recently suggested spinal cord oscillation could play a role. RESEARCH QUESTION: To determine if spinal cord oscillation could contribute to spinal cord injury in degenerative cervical myelopathy. MATERIAL AND METHODS: A computational model of an oscillating spinal cord w

Significance of spinal cord swelling in the prognosis of acute cervical spinal cord injury.

Seven cases of acute cervical cord injury examined by CT-myelography are reported. Cord swelling was observed in 2 patients who developed complete lesions. Another 5 cases had an incomplete lesions. Spinal cord swelling indicated severe cord damage. External decompression seemed to have little effect on cord swelling. When cord swelling is absent, neurological improvement was expected though the initial neurological state showed severe deficits. A further 4 patients who had an acute cervical cord injury who were treated by myelotomy at the injury site are also reported. Two patients obtained s

Charting Cervical Spinal Cord Morphometry Across the Lifespan.

Spinal cord morphometry provides essential biomarkers of neurological health, but clinical interpretations are confounded by inter-subject variability and a lack of normative references across the full human lifespan. We address this gap by generating the first comprehensive lifespan charts for cervical spinal cord morphometry. We leveraged 30 population-based brain MRI datasets, aggregating 78,269 scans from 41,042 individuals (ages 0-100) whose imaging protocols included cervical cord coverage. To overcome contrast variability, we employed a state-of-the-art contrast-agnostic deep learning s

Spinal cord herniation following cervical laminoplasty: illustrative case.

BACKGROUND: Postoperative spinal cord herniation (SCH) is an exceptionally rare but serious delayed complication of cervical decompressive surgery, resulting from a dorsal dural defect associated with pseudomeningocele formation. The authors report the case of a 73-year-old man who developed quadriparesis 6 years after C3-6 laminoplasty for ossification of the posterior longitudinal ligament. OBSERVATIONS: MRI demonstrated dorsal herniation of the spinal cord through a dural defect into a cystic lesion consistent with a postoperative pseudomeningocele at C4. Reexploration via a posterior midl

Spinal Cord Tract Integrity in Degenerative Cervical Myelopathy.

BACKGROUND AND OBJECTIVES: Degenerative cervical myelopathy (DCM) is the most common cause of spinal dysfunction globally. Despite surgical intervention, motor dysfunction may persist in many patients. The purpose of this study was to comprehensively examine specific spinal cord tract changes in patients with DCM, to better understand potential substrates for compensatory recovery of function. METHODS: Cervical spinal cord MRI scans with diffusion tensor imaging were performed in patients with DCM and in healthy volunteers. Spinal Cord Toolbox was used to register the PAM50 template, which in

Cervical cord injury complicated by acute mesenteric ischemia.

Acute mesenteric ischemia is a rare, life-threatening complication of cervical cord injury. This case report emphasized the importance of prompt diagnosis and treatment of this complication. A 60-year old Japanese man with no co-morbidities was diagnosed as a C2 cervical cord injury and traumatic brain injury after a fall-down trauma. He was admitted to the intensive care unit after cord decompression surgery. Nine days later, he presented tachycardia and hypotension. Ultrasound showed dilated intestines and ascites. Intestinal ischemia was suspected based on the contrast-enhanced computed tom

Pure Tethered Cervical Cord and Review of Literature.

Tethering of the spinal cord in the lumbosacral region with myelomeningocele is a well-known phenomenon. Only sporadic cases of tethering along the rest of the neuraxis, including the hindbrain, cervical, and thoracic spinal cord have been documented, always along with some associated congenital malformations (hydrocephalus, Chiari malformation, myelomeningocele, meningocele, hamartomatous stalk, spina bifida occulta, intramedullary lipoma, intradural fibrous adhesions, the fusion of the sixth and seventh cervical vertebrae, split cord malformation, or low-lying cord). Increased spinal cord movements in cervical spondylotic myelopathy.

BACKGROUND CONTEXT: Magnetic resonance imaging (MRI) is a very useful diagnostic test for cervical spondylotic myelopathy (CSM) because it can identify degenerative changes within the spinal cord (SC), disclose the extent, localization, and the kind of SC compression, and help rule out other SC disorders. However, the relationships between changes in cerebrospinal fluid (CSF) flow, cord motion, the extent and severity of spinal canal stenosis, and the development of CSM symptoms are not well understood. PURPOSE: To evaluate if changes in the velocity of CSF and SC movements provide additional

Respiratory muscle training for cervical spinal cord injury.

BACKGROUND: Cervical spinal cord injury (SCI) severely comprises respiratory function due to paralysis and impairment of the respiratory muscles. Various types of respiratory muscle training (RMT) to improve respiratory function for people with cervical SCI have been described in the literature. A systematic review of this literature is needed to determine the effectiveness of RMT (either inspiratory or expiratory muscle training) on pulmonary function, dyspnoea, respiratory complications, respiratory muscle strength, and quality of life for people with cervical SCI. OBJECTIVES: To evaluate t

Rapid diaphragm atrophy following cervical spinal cord hemisection.

A cervical (C2) hemilesion (C2Hx), which disrupts ipsilateral bulbospinal inputs to the phrenic nucleus, was used to study diaphragm plasticity after acute spinal cord injury. We hypothesized that C2Hx would result in rapid atrophy of the ipsilateral hemidiaphragm and increases in mRNA expression of proteolytic biomarkers. Diaphragm tissue was harvested from male Sprague-Dawley rats at 1 or 7 days following C2Hx. Histological analysis demonstrated reduction in cross-sectional area (CSA) of type I and IIa fibers in the ipsilateral hemidiaphragm at 1 but not 7 days. Type IIb/x fibers, however, h

Reverse Lhermitte's phenomenon provoked by cervical cord compression.

PURPOSE: Lhermitte's phenomenon (LP) is a rare manifestation, which is defined when a sudden electric-shock sensation transmitted down the spine induced by neck flexion; however, the reverse LP is defined when symptoms are induced by neck extension, not flexion. Because reports of LP are limited in the Taiwan literature, we report this case. CASE REPORT: A 74-year-old woman presented to our emergency department with sudden onset of right neck pain when extending the neck. The pain mimicked an electric shock and radiated to the left shoulder. Imaging showed spondylosis and spondylolisthesis wi

Split cervical spinal cord malformation and vertebral dysgenesis.

We report a case of vertebral malformation associated with diplomyelia believed to be a type II split cord malformation. Cervicothoracic level cases are exceptional. This article reports the case of an 11-year-old boy with no neurological symptoms who had not undergone surgery. The diagnosis was made during pregnancy by prenatal screening with ultrasound and MRI. Several embryological theories have been offered to provide an explanation for this syndrome. Close follow-up is mandatory. Surgery must only be considered if neurological deterioration occurs.

Exophytic intramedullary meningioma of the cervical spinal cord.

Intramedullary spinal cord neoplasms are relatively uncommon. The most common intramedullary tumors are astrocytomas and ependymomas. Meningiomas can occur as an intradural tumor; however, they are typically in the extramedullary compartment. A 42-year-old male presented with progressive sensory loss in the upper extremities and lower extremity weakness. Pre-operative imaging suggested an intramedullary cervical lesion. To treat the progressive neurological abnormality, surgical resection was planned. At surgery, it was noted that the tumor originated in the cervical spinal cord and extended i

Surgical treatment of cervical intramedullary spinal cord tumors.

The aim of this study is to determine if radical microsurgical removal of cervical intramedullary spinal cord tumors (CIMSCT) is achievable without causing respiratory, motor and sphincter deficits. Sixteen consecutive patients, who underwent surgical removal of a cervical intramedullary spinal cord tumor between 1988 and 1997, are presented. Surgical findings and results are analyzed. Patients' pre- and postoperative neurological conditions were evaluated using the clinical/functional scale of McCormick et al. The tumor was totally removed in 15 patients (93%). An improvement in sensory and m
