Cardiac arrhythmias in Chagas' heart disease.

Chagas' disease is a chronic parasitosis affecting most Latin American countries. Its most important clinical manifestation is a late developing chronic myocarditis and, much less frequently, an early acute myocarditis. Chagasic myocardial damage is microfocal and disseminated throughout the heart. In most cases, the coexistence of areas of myocytic degeneration, inflammatory infiltration, and fibrosis suggests a permanent evolving process. Commonly, chronic chagasic myocarditis resembles a dilated cardiomyopathy, with characteristic ECG abnormalities (atrial and ventricular extrasystoles, int

Chagas' disease in Latin American immigrants.

Chagas' disease is caused by Trypanosoma cruzi, a protozoan parasite that infects an estimated 10 to 12 million Latin Americans. Because infection is often lifelong, individuals from endemic areas may develop symptomatic chronic Chagas' disease years after emigrating to the United States. We report three immigrant patients from Latin America who illustrate the indeterminate phase and the symptomatic cardiac and gastrointestinal syndromes of this illness.

Chagas disease: An increasing clinical concern.

Chagas disease (CD), or American trypanosomiasis, is a parasitic infection caused by Trypanosoma cruzi and transmitted primarily by triatomine "kissing bugs." Once limited to Latin America, CD is now considered endemic in the United States (US), affecting an estimated 280 000 individuals. This article aims to highlight the growing clinical relevance of CD through a case example and review of its epidemiology, clinical features, and management. Globally, 6 to 8 million people are infected, resulting in more than 50 000 deaths annually. Globalization, migration, and climate change have expanded …

Brain-Thymus Connections in Chagas Disease.

BACKGROUND: The brain and the immune systems represent the two primary adaptive systems within the body. Both are involved in a dynamic proc … ides that can be altered in situations of infectious stress or chronic inflammation. In particular, Chagas disease, caused by the protozoan parasite Trypanosoma cruzi (T. cruzi), impacts upon immunoneuroendocrine circuits disrupting thymus physiology. Here, we discuss the most relevant findings reported in relation to brain-thymic connections during T. cruzi inf …

WHF IASC Roadmap on Chagas Disease.

BACKGROUND: Chagas Disease is a neglected tropical disease caused by the protozoan Trypanosoma cruzi, with some of the most serious manifestations affecting the cardiovascular system. It is a chronic, stigmatizing condition, closely associated with poverty and affecting close to 6 million people globally. Although historically the disease was limited to endemic areas of Latin America recent years have seen an increasing global spread. In addition to the morbidity and mortality associated with the disease, the social and economic burdens on individuals and society are substantial. Often called …

Chagas Disease-induced Sudden Cardiac Arrest.

Sudden cardiac death (SCD) is the most common cause of death in patients with Chagas disease (ChD). There are over 300,000 ChD-infected individuals living in the United States, of whom 10-15% have undiagnosed Chagas cardiomyopathy (CCM). CCM patients have a higher risk of SCD compared to non-CCM patients, although early and appropriate treatment of CCM patients can result in a 95% relative risk reduction of SCD. Emergency physicians have a unique opportunity to improve outcomes among these patients by becoming more vigilant in recognizing the signs and symptoms of CCM in patients who present i…

Current status of Chagas disease chemotherapy.

Chagas disease affects 7.7 million people and 28 million people are at risk of acquiring the disease in 15 endemic countries of Latin America. Benznidazole and nifurtimox are drugs that have been used to treat the disease. However, both drugs induce severe side effects. Treatment with benznidazole has been recommended for the acute phase (0-4 months after infection), recent chronic phase (children 0-14 years of age, treated 4 months after infection) and congenital infection. Average cure rates for Chagas disease patients obtained from clinical trials were 97.9% (congenital infection, treatment…

Epidemiology of American trypanosomiasis (Chagas disease).

Trypanosoma cruzi, the cause of American trypanosomiasis, or Chagas disease, is a protozoan parasite that is enzootic and endemic in much of the Americas, where it infects a wide variety of wild and domestic mammals as well as many species of triatomine vectors, in addition to humans. Historically, vector-borne transmission of T. cruzi has been the most important mechanism through which humans have become infected with the parasite, but transmission by blood transfusion and congenital transmission also have been important. In many of the endemic countries transmission of T. cruzi has improved …

Natural Chagas disease in four baboons.

BACKGROUND: Chagas disease is common in Central and South America and the southern United States. The causative agent is Trypanosoma cruzi (order Kinetoplastida, family Trypanosomatidae), a kinetoplastid protozoan parasite of humans and other vertebrates. It is a serious public health issue and the leading cause of heart disease and cardiovascular death in Central and South America. In 1984, a colony baboon was discovered to be infected with T. cruzi. METHODS: As the initial diagnosis was made by microscopic observation of the amastigote forms of T. cruzi in myocardial fibers, T. cruzi amasti…

[Chronotropic-metabolic index in Chagas' disease].

Chronotropic incompetence is a common feature in Chagas' disease patients. New methodologies are now available to evaluate the chronotropic response in different subsets. The chronotropic-metabolic index (CMI) is one of these new indexes and quantifies the relationship between the increment of heart rate and the maximal oxygen consumption (VO2 max) during exercise testing. In normal subjects there is linear response and the index is round 1.0. The aim of the study was to evaluate the chronotropic response in healthy controls and Chagas' disease patients with and without left ventricular dysfun

Serum IgE levels in Chagas' disease.

Elevated levels of serum immunoglobulin E are found in many parasitic diseases caused by helminths. We have previously demonstrated that giardiasis was not associated with hyperimmunoglobulinaemia E. In order to determine the influence of Trypanosoma cruzi on IgE levels of clinically symptomatic patients, a controlled study was performed. There was no difference between the serum IgE values from Brazilian patients with Chagas' disease and those from matched healthy Brazilian controls. We suggest that protozoan parasitic infections do not increase serum IgE levels.

Chagas disease immunogenetics: elusive markers of disease progression.

Introduction: Chagas disease (CD) is caused by a parasitic infection. The disease usually occurs after decades of the primary infection and can involve the myocardium or the digestive system. Of note, around 30% of T. cruzi infected patients develop CD while the other 70% may remain asymptomatic for their entire life. CD is usually observed as familial clustered phenomena. Moreover, individuals with chronic Chagas heart disease (CCHD) usually present a strong, deregulated immune response, which strongly suggests an immunogenetic effect.Areas covered: In this article we review and discuss the i…
