Charles Bonnet's syndrome: not only a condition of the elderly.

PURPOSE: To investigate whether the experience of visual hallucinations, namely Charles Bonnet's syndrome, in psychologically healthy people is a phenomenon solely of elderly, visually impaired people. METHODS: In a prospective controlled study, four groups of subjects (total 324) were formed: age ≤40 years, no visual impairment; age ≤40 years, visually impaired; age >40 years, no visual impairment; age >40 years, visually impaired. Visual impairment was defined as best-corrected visual acuity ≤0.3 (Snellen) in the better-seeing eye. Each group consisted of 81 subjects. Visual hallucinations

[Paradoxical cessation in a case of Charles Bonnet syndrome].

CASE REPORT: 78-year-old male patient diagnosed with Charles Bonnet syndrome (CBS) showing secondary visual deficit toward end-stage glaucoma. He progressed to amaurosis, with an abrupt disappearance of hallucinations in parallel to the loss of residual vision. DISCUSSION: The paradoxical cessation of CBS occurs when the patient loses residual vision and progresses to amaurosis. The lack of stimulation, both in the corresponding retina and the cortex, lead to the disappearance of hallucinations because the desafferented and hyper-excited neurons lose the necessary stimulus that triggers CBS.

Charles Bonnet syndrome in patients referred for cataract surgery.

PURPOSE: To investigate the prevalence and clinical characteristics of Charles Bonnet Syndrome (CBS) in patients referred for bilateral cataract surgery. METHODS: This prospective, cross-sectional clinical trial included 391 patients attending cataract assessment at a single-center tertiary cataract clinic. After clinical examination, participants were screened for presence of visual hallucinations (VHs). Participants experiencing complex VHs were further questioned regarding characteristics of their hallucinatory experiences to properly determine if they were attributable to CBS. CBS was def

Charles Bonnet syndrome versus Occipital Epilepsy, a diagnostic challenge.

Charles Bonnet Syndrome (CBS) is a disorder with visual deficit and complex recurrent visual hallucination in conscious patients, described for the first time by Charles Bonnet. It has been found in association with variable pathologic conditions of the eyes, central visual pathways and occipital lobe. Occipital lobe lesion is an important cause of visual field deficit associated with elementary simple hallucinations, whereas complex hallucinations are related to occipitotemporal and occipitoparietal visual association neocortex damage.

Cognitive impairment and Charles Bonnet syndrome: a prospective study.

OBJECTIVES: Charles Bonnet syndrome (CBS) is a common cause of visual hallucinations in older people. The relationship between CBS and cognitive impairment is unclear, but anecdotal reports exist of dementia emerging in patients diagnosed with CBS. This work set out to determine if there is an increased incidence of dementia, and increased severity of cognitive impairment, in people with CBS compared to controls from the same clinical setting. METHOD: People over 65 attending low-vision and glaucoma clinics, and a cohort of age-matched controls, underwent a psychiatric assessment. The cohorts

Charles Bonnet syndrome in glaucoma patients with low vision.

PURPOSE: To characterize the nature and frequency of Charles Bonnet syndrome in glaucoma patients with low vision. PATIENTS AND METHODS: All patients attending the glaucoma clinic during a period of 10 months who had visual acuity of 20/80 or less in both eyes were included in this study. Each patient was questioned about the occurrence of visual hallucinations. Those who responded positively had a thorough interview relating to the characteristics of the hallucinations. Medical history and social history were taken, followed by a complete ocular examination. RESULTS: Eighty-nine patients me

Charles Bonnet syndrome with auditory hallucinations: a diagnostic dilemma.

Charles Bonnet syndrome (CBS) appears to be commoner in elderly people than was previously thought. Nonetheless, as yet, no firm diagnostic criteria have been established for CBS, leaving the need for well-defined diagnostic criteria describing this syndrome unfulfilled. Two recently proposed diagnostic criteria for CBS insist on an absence of hallucinations in modalities other than visual. However, we experienced 2 cases suffering from probable CBS who experienced auditory hallucinations. In this report, we recommend that even if CBS coexists with other hallucinations, we should not exclude C

Charles Bonnet syndrome in leprosy; prevalence and clinical characteristics.

Charles Bonnet syndrome (CBS) is diagnosed when a visually impaired patient without any mental disorder develops visual hallucinations. A survey of patients in a National Leprosarium revealed that the point prevalence of CBS in leprosy was 0.4%. This prevalence appears to be high, as few cases with CBS have been reported. The semeiology of visual hallucinations was typical of CBS. However, the clinical features were different from previous reported cases because of the history of leprosy and associated multi-sensory loss. Patients with leprosy appear to be at increased risk for CBS, due to fre

Charles Bonnet Syndrome Following a Mild Traumatic Brain Injury.

Charles Bonnet syndrome (CBS) describes the presence of hallucinations in patients with poor or deteriorating vision. The physician awareness of CBS is low, despite reporting of CBS hallucinations occurring in a range of ocular and central nervous system conditions. Following mild traumatic brain injury (mTBI), patients can experience visual or oculomotor dysfunction. As such, it is possible that CBS might present following mTBI. An adult male suffered an mTBI and whiplash injury following a motor vehicle accident. He developed persistent post-concussion symptoms (PPCS) including headaches and

Investigation of structural brain changes in Charles Bonnet Syndrome.

BACKGROUND AND OBJECTIVES: In Charles Bonnet Syndrome (CBS), visual hallucinations (VH) are experienced by people with sight loss due to eye disease or lesional damage to early visual pathways. The aim of this cross-sectional study was to investigate structural brain changes using magnetic resonance imaging (MRI) in CBS. METHODS: Sixteen CBS patients, 17 with eye disease but no VH, and 19 normally sighted people took part. Participants were imaged on a 3T scanner, with 1 mm resolution T1 weighted structural imaging, and diffusion tensor imaging with 64 diffusion directions. RESULTS: The thre

Charles Bonnet syndrome as first manifestation of occipital infarction.

PURPOSE: To describe a case of Charles Bonnet syndrome as the first manifestation of occipital infarction in a patient with preserved visual acuity. OBSERVATIONS: We report a 78-year-old man followed in our department with a two-month-long history of visual hallucinations based on the vision of flowers and fruits intermittently, being perceived as unreal images. Best-corrected visual acuity was stable in the follow-up time being 20/20 in the right eye and 20/25 in the left eye. Charles Bonnet Syndrome as Another Cause of Visual Hallucinations.

Charles Bonnet syndrome (CBS) presents as gradual vision loss and associated visual hallucinations in a patient who is otherwise neurologically and psychiatrically intact. This syndrome presents primarily as ophthalmologic disease, however, may be secondary to an ischemic stroke or tumor in the occipital lobe. Patients present with a complaint of vivid visual hallucinations ranging from spots and geometric shapes to seeing people, distorted figures and landscapes.First noted in the 1760s, CBS did not reach the western scientific community until the early 1980s. Our patient reported seeing her

Nonpsychotic Hallucinations and Impaired Vision: The Charles Bonnet Syndrome.

Charles Bonnet syndrome is a clinical entity that presents with visual hallucinations in patients with worsening visual acuity and no underlying neuropsychiatric disorder. A 93-year-old male presented to the emergency department complaining of complex visual hallucinations. He had been without his prescription glasses as they were being repaired. After work-up and medication review, no underlying drug or neuropsychiatric disease was found. Moreover, the hallucinations resolved after his vision improved. This case illustrates a frequently missed clinical entity in our practice. Clinical suspici

Reversible Charles Bonnet syndrome secondary to upper lid ptosis.

Complex visual hallucinations can occur in visually impaired individuals with no underlying psychiatric disorder. This phenomenon is known as Charles Bonnet syndrome (CBS). It is more common in elderly patients who are suffering from impaired vision due to ocular or neurological disease processes, resulting in sensory deprivation. We report a case of CBS in an elderly female with marked ptosis, which was exacerbated following a knee replacement surgery under general anaesthesia. Her CBS symptoms persisted until surgical correction of the ptosis, with a rapid and dramatic resolution of her hall

Charles Bonnet syndrome and cognitive impairment: a systematic review.

BACKGROUND: Charles Bonnet syndrome (CBS) is defined as complex persistent visual hallucinations in the absence of mental disorder. It is common in conditions causing significant visual impairment. Many authors advise reassurance, considering the condition benign. However, others have suggested that CBS may in some patients represent the early stages of dementia. This review seeks to systematically examine the evidence for any link between CBS and cognitive impairment. METHODS: Literature search using OVID Medline, PsychINFO, and Embase. RESULTS: Three studies where cognitive functioning was

Acute Reversible Charles Bonnet Syndrome Following Eye Patch Placement.

Charles Bonnet syndrome (CBS) is characterised by recurrent vivid visual hallucinations in the presence of normal cognition. We present a case of CBS secondary to eye patching following Pars Plana Vitrectomy with an unusually acute onset in a 48-year-old woman. She presented with formed visual hallucinations that started less than 30 min after patching of her left eye. The patch was removed after 2 d, and these hallucinations persisted 2 d following eye patch removal. It is important that the ophthalmic surgeon be aware of the potential for development of CBS and offer appropriate referral and

Delirium superimposed on Charles Bonnet syndrome: a case study.

Older adults with visual impairment may experience visual hallucinations in the setting of normal cognition and absence of psychiatric illness. This phenomenon is referred to as Charles Bonnet syndrome. Information concerning Charles Bonnet syndrome predominantly comes from case studies. Reassuring the person experiencing the hallucinations they are not suffering from psychosis constitutes the mainstay of treatment. What follows is the case of a vision impaired, older adult male with known Charles Bonnet syndrome, who, following emergency surgery and associated delirium while in the intensive

Charles Bonnet syndrome in cranio-maxillofacial surgery: case report.

BACKGROUND: Complex visual hallucinations in the presence of a clear mental state and in the absence of underlying neurological disorders have received increased recognition since Charles Bonnet reported visual hallucinations secondary to mature cataracts in 1760. The prevalence of Charles Bonnet syndrome (CBS) varies widely in the literature and might be underestimated in most settings. The current paper presents a case of acute-onset CBS due to a unilateral Frost suture after revision of an orbit floor fracture. CASE REPORT: A 68-year-old male patient underwent an operation to repair a unil

Charles Bonnet syndrome: three cases in the emergency department.

BACKGROUND: Charles Bonnet Syndrome (CBS) is a cause of visual hallucinations in elderly patients that is often unrecognized by emergency physicians and has a relatively benign course. As the population ages, it is likely that the number of cases of CBS will increase (and thus, the numbers of those who present to an Emergency Department [ED] will be increasing). OBJECTIVES: The case reports presented in this article will facilitate the recognition of CBS by the emergency physician. CASE REPORT: We describe 3 patients who presented to one ED for visual disturbances and were diagnosed with CBS
