Neisseria meningitidis endogenous endophthalmitis with meningitis in an immunocompetent child.

Neisseria meningitidis is a major cause of childhood morbidity and mortality worldwide. We describe an exceptional case of an immunocompetent 15-month-old child presenting with a unilateral anterior uveitis, hypopyon, and sepsis. Anterior chamber aspirate demonstrated gram-negative cocci before Neisseria meningitidis was identified in blood and cerebrospinal fluid. Meningococcal endophthalmitis presents variably with sepsis, meningitis, or isolated ocular symptoms. Diagnosis is a clinical challenge, requiring diagnostic sampling and treatment from both pediatricians and ophthalmologists. Delay

Unusual extra-podal fungal mycetoma with black grains in a Senegalese child.

Mycetoma remains endemic in the tropical and subtropical regions of the "mycetoma belt" including Senegal. It affects more commonly young men in the age group of 20 to 40 years. The foot represents the most commonly affected site. The most common extra-podal localizations are leg, knee, buttocks, hand and arm. We report an exceptional case of cervical fungal mycetoma that occurred in a 13-year-old Senegalese child. He consulted for a cervico-submandibular tumefaction with multiple sinuses and black grains discharge evolving since 6 years, associated to laryngeal dyspnoea. Mycological examinati

[Bi-symptomatic CINCA syndrome with inaugural urticaria and major articular lesions].

BACKGROUND: CINCA syndrome (chronic infantile neurological cutaneous and articular syndrome) observed in young children associates chronic urticaria and rheumatological disorders which may cause deformation of the larger peripheral joints. Neurological signs develop later leading to severe prognosis: chronic meningitis, deafness, mental retardation. We present a bisymptomatic case which was distinctive by the absence of central nervous system involvement after 11 years of course. CASE REPORT: Since the age of six months a female child experienced chronic urticaria with nearly daily episodes.

The human brain age 7-11 years: a volumetric analysis based on magnetic resonance images.

Volumetric magnetic resonance image (MRI)-based morphometry was performed on the brains of 30 normal children (15 males and 15 males) with a mean age of 9 years (range 7-11 years). This age range lies in a late but critical phase of brain growth where not volumetric increment will be small but when the details of brain circuity are being fine-tuned to support the operations of the adult brain. The brain at this age is 95% the volume of the adult brain. The brain of the female child is 93% the volume of the male child. For more than 95% of brain structures, the volumetric differences in male an

Ophthalmic Shingles in Children: A Rare Form and a Danger to Be Aware of.

Shingles in children is rare, especially the ophthalmic form. The occurrence of shingles in children is exceptional but often benign, requiring only symptomatic treatment. Antivirals are exceptionally used for complicated forms, which are mainly seen in immunocompromised individuals or in ophthalmic locations that can lead to severe ocular complications. Various studies agree on the benign nature of this condition in immunocompetent children and an excellent prognosis. We report a case of ophthalmic shingles in an immunocompetent child aged two years and seven months. The purpose of this clini

[Cryptorchidism associated with ectopic adrenal cortical tissue in the spermatic cord in a 51-year-old male].

This case report describes an exceptional case of ectopic adrenal cortex tissue (EACT) in the spermatic cord associated with ipsilateral cryptorchidism in an adult. While both EACT and cryptorchidism are fairly common congenital anomalies in boys, adult cases are uncommon. Although the spermatic cord is a known site of EACT, the reports of its association with cryptorchidism have been limited to child cases. During surgery, undescended testis was discovered and incidentally ectopic adrenal tissue along the spermatic cord was also identified. This combination of developmental aberrations in the

[Cauda equina syndrome revealing neuroblastoma].

INTRODUCTION: Neuroblastoma is the most common solid tumor of childhood. Neurological involvement is rare resulting from cord or nerve compression. CASE REPORT: A 7-year-old child was hospitalized for cauda equina syndrome. MRI showed retroperitoneal lesion with dumbbell intradural spreading. Blood and urinary catecholamine level were elevated confirming the diagnosis of neuroblastoma. She was treated with chemotherapy alone with partial neurological recovery. DISCUSSION AND CONCLUSION: Cauda equina compression revealing neuroblastoma is exceptional especially at an early phase in the younge

[Congenital dermatofibrosarcoma of Darier and Ferrand: a pediatric case].

INTRODUCTION: Dermatofibrosarcoma show an extremely aggressive tendency to invade surrounding tissue. It was first described in 1924. It usually occurs in young men. This type of tumor is exceptional in childhood. The authors report a case of congenital dermatofibrosarcoma diagnosed in a child. REPORT OF CASE: A two-year old female patient presented with a tumor of the vertex scalp since her birth. Biopsy revealed a dermatofibrosarcoma. The tumor was removed surgically with 3cm margins. The primary reconstruction was performed using a double temporoparietal flap (H). There was no recurrence a

Encephalocraniocutaneous lipomatosis with a mutation in the NF1 gene.

Encephalocraniocutaneous lipomatosis (ECCL) is a congenital hamartomatous disorder characterised by unilateral skin lesions, lipomas, and ipsilateral ophthamological and cerebral malformations. The disorder is thought to represent a localised form of Proteus syndrome. In this report, a child is described with ECCL and a de novo nonsense mutation in exon 29 (S1745X) of the neurofibromatosis type 1 (NF1) gene. Although it is possible that both ECCL and NF1 occur coincidentally in this patient, we favour the hypothesis that in exceptional cases a mutation in the NF1 gene might give rise to severe

The parental fitness of mothers with multiple personality disorder: a preliminary study.

Recent studies show that nearly all individuals who develop multiple personality disorder (MPD) were abused as children. The majority of identified MPD patients are women in the age range associated with child-rearing responsibilities. A review of the parenting patterns of 75 mothers with MPD yielded findings suggesting that 38.7% were competent or exceptional mothers, 16% were grossly abusive to the extent of injuring, molesting, or placing their children at risk, and 45.3% were compromised or impaired as parents. The compromised/impaired mothers were a mixed group, including psychologically

[Ophthalmic Zoster In Children: About An Observation And Literature Review].

OBJECTIVE: the goal of this clinical case is to report a case of ophthalmic zoster in children and to insist on the relevance of early antiviral treatment so as to minimize corneal affection and preserve visual function. OBSERVATION: we report an eleven-years-old-girl case, with a history of varicella infection at 5 years old, who came for consultation for a painful eruption affecting the forehead, the upper eyelid, the nose, and the upper lips. The clinical examination showed many vesicles erythematous affecting the left hemi face. The diagnosis of ophthalmic zoster has been retained. The bi

[Mother-infant dengue transmission during the 2009-2010 dengue epidemic: report of four cases].

The risks related to dengue virus infection during pregnancy have been increasingly better described over the past 10 years. The possibility of maternal-fetal transmission is now recognized, but the diagnosis is still too late because of a wide range of clinical signs that the infected newborn child can present. From December 2009 to October 2010, Guadeloupe Island underwent an exceptional dengue epidemic. During this epidemic, at least four cases of vertical virus transmission were biologically proved. The purpose of this article is to describe the clinical aspects of these cases, some of whi

Progressive multifocal leukoencephalopathy in a child with hyperimmunoglobulin E recurrent infection syndrome and review of the literature.

Progressive multifocal leukoencephalopathy (PML) is a fatal demyelinating disease due to infection with polyomavirus JC (JCV). PML occurs almost exclusively in immunocompromised patients, and although it has increased markedly in relation to AIDS, remains exceptional in children. We present the case of an immunocompromised child with hyperimmunoglobulin E recurrent infection syndrome (HIES) and pathologically-proven PML. HIES is a rare congenital immunodeficiency that to our knowledge has never before been reported in association with neurological complications. Following a recurrence of bronc

Disseminated cysticercosis in a child: whole-body MR diagnosis with the use of parallel imaging.

Cysticercosis is a parasitic disease caused by infestation with the encysted larval stage of the pork tapeworm, Taenia solium. Disseminated cysticercosis is an exceptional expression of this disease characterized by high morbidity due to massive symptomatic parasite burden in the central nervous system, striated muscles, subcutaneous tissues and other organs. Less than 50 such cases have been reported worldwide, and fewer than 10 children. We report on the whole-body MR diagnosis of extensively disseminated cysticercosis in a child. The critical role of whole-body MR as a stand-alone modality

[Case report. Therapeutic intervention using a "particular interest" of an autistic child with visual agnosia].

Although special education under its diverse forms is consensually recognized as the privileged area of pervasive developmental delay, there is little studies on re-education intervention for autistic persons of normal intelligence. This article presents a psychoeducative intervention with a three year old autistic boy in a pedopsychiatric day care centre. The interest of this observation is double. On the one hand, this child, aside from autism, is carrier of an exceptional ailment: visual agnosia. On the other hand, his "restrictive autistic interest", namely perceptive attraction and his qu

[Aneurysmal cysts located in the maxilla or mandible (author's transl)].

Aneurysmal cysts of the mandible and maxilla are rare conditions, though not exceptional, which are seen in particular in the child and adolescent. A part from facial asymmetry and tumefaction, it is rare to find other clinical manifestations. No neurological signs were seen in the present study. An aneurysmal cyst gives a radiological appearance of non-characteristic lysis compatible with a large range of diagnoses. Histological study permits early diagnosis but must always involve the whole specimen with careful examination to detect any other associated lesion. The pathogenesis of the condi

Nasopharyngeal hairy polyp with recurrence in the middle ear.

A 6-year-old girl was referred to The Johns Hopkins Hospital for a suspected congenital cholesteatoma. The lesion was determined to be a recurrence of a nasopharyngeal hairy polyp removed in the postnatal period. Simple excision is normally regarded as curative: malignant transformation and local recurrence do not occur. Our case is exceptional in that incomplete excision of the nasopharyngeal polyp was followed by re-growth and extension into the middle ear over a 6-year period. Thus, not all hairy polyps are static lesions. Indeed, this case documents progressive growth of a hairy polyp duri

Hydrops fetalis and fibrosarcoma: case report of an uncommon association.

UNLABELLED: Fetal hydrops associated with neonatal tumours is an uncommon occurrence. The diagnosis can be established prenatally by ultrasound examination. The treatment of choice is surgery which may be curative. We report the case of a male born at 32 weeks gestation who presented with severe hydrops fetalis and a thoracic mass. The child could not be operated upon because of rapid clinical deterioration. The autopsy findings confirmed the diagnosis of congenital fibrosarcoma. This is, to our knowledge, the first case of hydrops fetalis associated with fibrosarcoma. CONCLUSION: The associa

Tracheal rupture in a newborn during a complicated delivery. Diagnosis and surgical repair.

Rupture of the trachea is an exceptional obstetrical lesion. The infant reported in this paper, at 1 hour of age, developed respiratory distress with pneumomediastinum, bilateral pneumothorax and subcutaneous emphysema. This resulted from the fact that the trachea had ruptured, within 1 cm of the carina, during the difficult delivery. When the child was 23 days old, operation proved necessary because extubation was not feasible. The stenotic portion of the trachea was resected and continuity restored by end-to-end anastomosis. The tracheal lumen at the site of the anastomosis proved normal by
